Five Most Common Prognostically Important Fusion Oncogenes are Detected in the Majority of Pakistani Pediatric Acute Lymphoblastic Leukemia Patients and are Strongly Associated with Disease Biology and Treatment Outcome

  • Background and Objectives: Acute lymphoblastic leukemia (ALL) is a complex genetic disease involving many fusion oncogenes (FO) having prognostic significance. The frequency of various FO can vary in different ethnic groups, with important implications for prognosis, drug selection and treatment outcome. Method: We studied fusion oncogenes in 101 pediatric ALL patients using interphase FISH and RT-PCR, and their associations with clinical features and treatment outcome. Results: Five most common fusion genes i.e. BCR-ABL t (22; 9), TCF3-PBX1 (t 1; 19), ETV6-RUNX1 (t 12; 21), MLL-AF4 (t 4; 11) and SIL-TAL1 (del 1p32) were found in 89/101 (88.1%) patients. Frequency of BCR-ABL was 44.5% (45/101). BCR-ABL positive patients had a significantly lower survival ($43.7{\pm}4.24$ weeks) and higher white cell count as compared to others, except patients with MLL-AF4. The highest relapse-free survival was documented with ETV6-RUNX1 (14.2 months) followed closely by those cases in which no gene was detected (13.100). RFS with BCR-ABL, MLL-AF4, TCF3-PBX1 and SIL-TAL1 was less than 10 months (8.0, 3.6, 5.5 and 8.1 months, respectively). Conclusions: This is the first study from Pakistan correlating molecular markers with disease biology and treatment outcome in pediatric ALL. It revealed the highest reported frequency of BCR-ABL FO in pediatric ALL, associated with poor overall survival. Our data indicate an immediate need for incorporation of tyrosine kinase inhibitors in the treatment of BCR-ABL+ pediatric ALL in this population and the development of facilities for stem cell transplantation.

<Field action report> Development and Application of Participatory Action Oriented Training(PAOT) for Improvement of Agricultural Working Environment in Korea (<사례보고> 농작업 환경개선을 위한 한국형 참여형 개선활동 교육(PAOT)의 개발과 실제 적용 사례)

  • Objectives: This study was conducted to develop a Korean version of the Participatory Action Oriented Training (PAOT) program and training materials for improvement of agricultural working environments. Methods: The PAOT manual and checklist were developed on the basis of the original English version of the training materials, a questionnaire survey of agricultural health and safety professionals, and a brainstorming conference. Good example pictures were collected through visits to agricultural workers' houses, and an easily understandable picture was drawn by an animation specialist. A PAOT action manual and five types of action checklists were developed for use in different agricultural environments. Each action checklist contained six categories and 38 items of agricultural health and safety principles; the six categories were material storage and handling, work stations and tools, machine safety, physical environment, working schedule and organization, and basic health and safety management. Incorporating these training materials, a one-day Korean PAOT program was developed. Results: Among 307 candidate agricultural workers from four rural Korean villages, 94 workers (59 males, 35 females) participated in a PAOT from July 2007 to Oct 2008. The PAOT program was successful and the mean self-estimated satisfaction score of the participants was greater than 90%. Conclusions: A Korean PAOT program and training materials were successfully developed and applied to Korean agricultural workers. Although more studies are needed, it is expected that PAOT will greatly contribute to the improvement of agricultural working conditions and health and safety through the use of agricultural workers' self initiatives.

Utility of B-type Natriuretic Peptide in Patients with Acute Respiratory Distress Syndrome (급성호흡곤란증후군 환자에 있어서 B-type Natriuretic Peptide의 유용성)

  • Background B-type natriuretic peptide (BNP) has been shown to be strong mortality predictors in a wide variety of cardiovascular syndromes. Little is known about BNP in patients with acute respiratory distress syndrome (ARDS). We studied whether BNP can predict mortality in patients with ARDS. Method Echocardiographic study was done to all patients with ARDS, and we excluded patient with low ejection fraction (less than 50%) or showing any features of diastolic dysfunction. 47 patients were enrolled between December, 2003 and February, 2006. Parameters including BNP were obtained within 24h hours at the time of enrollment. Result Mean BNP concentrations and APACHE II scores differed between the survivors and nonsurvivors (BNP, $219.5{\pm}57.7pg/mL$ vs $492.3{\pm}88.8pg/mL$; p=0.013, APACHE II score, $17.4{\pm}1.6$ vs $23.1{\pm}1.3$, p=0.009, respectively). With the use of the threshold value for BNP of 585 pg/mL, the specificity for the prediction of mortality was 94%. The threshold value for APACHE II of 15.5 showed sensitivity of 87%. 'APACHE II + $11{\times}logBNP$' showed sensitivity 63%, and specificity 82%, using threshold value for 46.14. Conclusion BNP concentrations and APCHE II scores were more elevated in nonsurvivors than survivors in patients with ARDS who have normal ejection fraction. BNP can predict mortality. Further study should be done.

The Development of Imitated Cheese Using Whole Milk Powder and Fermented Milk (전지분유와 발효유를 이용한 치즈 유사품개발)

  • Imitated cheese was prepared from whole milk powder and fermented milk and the moisture content, general components, noncasein nitrogen, nonprotein nitrogen and free amino acids were analyzed to determine the optimal ripening conditions needed to produce imitated cheese that was similar to natural cheese. The moisture content of the imitated cheese was 40.27% one day after being produced. The cheese was ripened using two different methods; at $12^{\circ}C$ with vacuum sealing and at $12^{\circ}C$ and 95% RH with a spray of Penicillium camemberti. The lactose content decreased rapidly from 24.64 to 5.43% at the $4^{th}$ wk of ripening when it was ripened with Penicillium camemberti. The degradation of protein by mold ripening in the imitated cheese was more rapid than that of vacuum sealing. The flavor and body texture were optimal at the $4^{th}$ wk ripening. The noncasein nitrogen and nonprotein nitrogen content increased from 28.10 to 54.05, and from 6.58 to 23.06 mg/mL, respectively, when ripened with P. camemberti. When the cheese was ripened at $12^{\circ}C$, 95% R.H with P. camemberti after 4 wks, all free amino acids increased significantly except asparagines. The total free amino acid and bitter amino acid concentrations increased from 8.40 to 34.87, and from 1.53 to 10.02 nmol/mg, respectively. When the imitated cheese was prepared, the protein degradation and flavor of the cheese was better when ripened with P. camemberti.

Effects of Polyethylene Mulch and Levels and Placements of Nitrogen on Soil Properties and Sweet Corn Growth (비닐 피복, 질소시비량 및 시비방법이 토양의 이화학적 특성과 단옥수수의 생육에 미치는 영향)

  • A sweet corn hybrid, Honey Bantam, was planted on 24 May, 1984 in a silty clay loam soil to investigate the effects of polyethylene(P.E.) mulch and different levels and placements of Nitrogen(N) on soil properties and sweet corn growth. A split-split plot design with three replications was employed; P.E. mulch and bare soil were main plot, N levele of 8, 12, 16, and 20 kg/10a were subplot, and band and broadcast of fertilizers were sub-subplots. At early growth stage soil temperature under P.E. mulch was higher than that in bare soil by 5-10$^{\circ}C$, but the differences decreased as plant growth advanced. Soil hardness increased with soil depth while P.E. mulch reduced soil hardness probably by holding high soil moisture. Soil pH decreased up to the 6th week after planting and then increased in bare soil, but it contineously decreased up to the 8th weeks under P.E. mulch regardless N levels and placements. Electrical conductivity(EC) of soil increased up to the 6th weeks after planting and then decreased in all treatments except broadcast of fertilizers under P.E. mulch where EC increased contineously. Generally, soil EC under P.E. mulch was higher than that in broadcast. Broadcast of fertilizers did not affect emergence of seedlings in all N levels under P.E. mulch and bare soil, but band of fertilizers at all N levels under P.E. mulch and higher levels of N in bare soil reduced emergence rate significantly. Percent stand was possitively correlated with soil EC and it strongly influenced the number of marketable ears. Plant growth was enhanced and silking date was earlier by 14-19 days under P.E. mulch compared to bare soil probably due to increased soil moisture, reduced soil hardness and higher soil temperature.

A Survey on the Perception of Food Sanitation Officers Toward the Genetically Modified Foods (유전자재조합식품에 대한 관련 식품위생공무원의 인지도 조사)

  • A survey was conducted to investigate the perception of food sanitation officers toward the Genetically Modified Foods. They were mainly from Regional Agencies of KFDA, City/Province office, and National quarantine station. Some of them were professors of university and researchers of research institute. Most of respondents had experiences of hearing or reading GM foods (over $95\%$) and over $90\%$ of respondents much needed the label of GM foods. Although some of officers of city/province office and national quarantine station showed less knowledge than other respondent groups, most of respondents had basic knowledge about biology. The frequency of respondents worked over 20 years and worked in the general administration was higher than that of other groups in the question of unsafe of GM foods. The answer frequency of careless treatment of foods was highest in the question of risk factor, and the frequency of GM foods was lowest ($4.4\%$). It was concluded that food sanitation officers had positive opinion about GM foods, but there were some differences in the knowledge among agencies. Therefore, it is necessary more educations and informations are needed for food sanitation officers.

Effect of Bambusae Caulis in Liquamen on Lipid Metabolism in Rats Fed High Fat Diet (죽력(Bambusae Caulis in Liquamen)이 고지방식이를 급여한 흰쥐의 체내 지질대사에 미치는 영향)

  • The purposes of this study were to investigate effects of Bambusae Caulis in Liquamen(BCL) on antioxidant activities and inhibitory activities of HMG-CoA reductase of in vitro, and lipid metabolism in rats fed the high cholesterol diet in vivo. Sprague-Dawley male rats weighing l00$\pm$10 g were devided into five groups ; normal group(NOR), the high cholesterol diet administered group(1 $\%$ cholesterol and 0.25$\%$ sodium cholate)(CON), 5$\%$ BCL administered group (5BL), the high cholesterol diet and 5$\%$ BCL administered group (5BCB) and the high cholesterol diet and 10\$\%$ BCL administered group (10BCB), respectively. In antioxidative activities of BCL using Rancimat in vitro, 1.25 diulent and original solution were more excellent activities than the control group, and in inhibiting activities of HMG-CoA reductase, BCL was shown inhibitory effects compared with the control, in dose dependent manners, especially 57.9$\%$ in original solution and 36.0$\%$ in 1.25 diulent. The growth rate of the control group was higher than the normal group, wheras the group given 5$\%$ BCL and 10$\%$ BCL were gradually decreased, especially the most excellent effect in 10$\%$ BCL. Serum levels of total cholesterol, LDL-cholesterol, triglyceride and free cholesterol were significantly decreased, whereas levels of HDL-cholesterol and phospholipid were increased, but not significantly. BCL administered group was increased in HDL-cholesterol/total cholesterol ratio and lowered antherogenic index. The activities of AST in serum were rather lowered in the BCL administration group than the cholesterol diet group, but not in ALT and ALP. The hepatic contents of total cholesterol were lowered significantly than control group, but not in triglyceride. Therefore, it might be expected that BCL is believed to be a possible protective or curative effects for fatty livers and hyperlipidemia-induced by a hi~h cholesterol diet.

Identification and Measurement of Hospital-Related Fears in Hospitalized School-Aged Children (학령기 입원아동의 병원관련 공포에 관한 탐색연구)

  • When children are admitted to hospital, they have to adapt to new and unfamiliar stimuli. Children may respond with fear to stimuli such as pain or unfamiliar experiences. One goal of nursing is to help hospitalized children to adapt effectively to their hospital experience. Accordingly, nurses need to assess childrens' fears of their hospital experience to contribute to the planning of care to alleviate these fears. The problem addressed by this study was to identify and measure hospital-related fears(hereafter called HRF) in hospitalized school-aged children. The study was conceptualized with Roy's model. A descriptive qualitative approach was used first, followed by a quantitative approach. This study was conducted from November 30, 1989 to January 12, 1991. The sample consisted of 395 hospitalized school-aged children selected through an allocated sampling technique in nine general hospitals. The HRF questionnaire (three point likert scale ) was developed by a delphi technique. The data were analyzed by an SAS program. Factor analysis was used for the examination of component factors. Differences in the HRF related to demographic variables were examined by t-test, analysis of variance and the Scheffe test. The crude scores of the HRF scale were transformed into T- scores to calculate the standard scores. The results included the following : 1. Forty-four items were derived from 188 statements identifying the childrens' hospital-re-lated fears. These items clustered into 14 factors, fear of injections, operations, bodily harm others' pain, medical rounds, physical examinations, medical staff, disease process, blood and X-rays, drugs and cockroaches, tests, harsh discipline from parents or staff, being absent from school, and separation from family. The 14 factors was classified into four categories,'pain','the unfamiliar','the un-known' and 'separation'. 2. The reliability of the HRF instruments was .92(Cronbach's alpha). In the factor analysis, Cronbach's alpha coefficients for the 14 factors ranged from .84 to .86 and Cronbach's alpha coefficients for the four categories ranged from .70 to .84. Pearson correlation coefficient scores for relationships among the 14 factors ranged from ,11 to .50, and among the four categories, from ,44 to ,63, indicating their relative independence. 3. The total group HRF score ranged from 45 to 130 in a possible range of H to 132, with a mean of 74.51. The fears identified by the children were, in order, injections, harsh discipline by parents or staff, bodily harm, operations, medical staff, disease process, and medical rounds ; the least feared was others' pain. The fear item with the highest mean score was surgery and the lowest was examination by a doctor. HRF scores were higher for girls than for boys, and for grade 1 students than for grade 6 students. HRF scores were lower for children whose fathers were over 40 than for those whose fathers were in the 30 to 39 age group, and whose mothers were over 35 than for those whose mothers were in the 20 to 34 age group. HRF scores were lower when the mother rather than any other person stayed with the child. The expressed fear of pain, the unfamiliar, the un-known and of separation directs nurses' concern to the threat felt by hospitalized children to their concept of self. This study contributes to the assessment of fears of hospitalized children and of stimuli impinging on those fears. Accordingly, nursing practice will be directed to the alleviation of pain, pre-admission orientation to the hospital setting and routines, initiation of information about procedures and experiences and arrangments for mothers to stay with their children. Recommendations were made for further research in different settings and for development and testing of the instrument.

Quality Characteristics of Fresh Noodles With Perilla Leaves (들깻잎을 첨가한 생면의 품질특성)

  • This study investigates the optimal application of perilla leaves to fresh noodles in terms of a variety of beneficial effects and functions such as the anti-microbial function of the leaves. First, we measured the water contents of the fresh noodles and found that the ones added with 7% lyophilized perilla leaf extract showed the maximum water contents whereas the control noodles without perilla leaf extract the minimum. Increasing amount of raw perilla leaf extract or lysophilzed perilla leaves in the fresh noodles elevated the L value and significantly reduced the A value. The mass and volume of the fresh noodles were gradually decreased with raw perilla leaves added. Addition of raw perilla leaf extract and lyophilized perilla leaves decreased the turbidity of the fresh noodles. Hardness of the fresh noodles was increased by the addition of lyophilized Perilla leaves. Next, we tested the effect of the addition of sesame leaves on microbial growth. No microbes were found in the fresh noodles in the absence or presence of sesame leaves at day 0. When the noodles were stored for 3 days, the greatest number of bacteria was detected in the noodles without perilla leaves while addition of perilla leaves lowered the amount of bacteria in the noodles. We then performed the sensory test. For the raw perilla leaf extract addition, the noodles with 9% of extract exhibited the highest in appearance, flavor, color (6.47), texture (6.60), and overall acceptability (7.67). Texture was the highest in the ones with 3% (6.87) and 5% (6.20) of extracts added. Overall acceptability (7.07) was the best when 3% perilla leaves were added. Overall, 9% addition of raw perilla leaf extract or 3% addition of lysophilized perilla leaves showed optimal tastes.

Total Folate Contents of 15 Edible Plants Consumed in Korea Using Trienzyme Extraction Method (국내 소비되는 봄나물의 Trienzyme 추출법을 적용한 엽산 함량 분석)

  • Trienzyme digestion (AOAC Official Method 2004.05) procedure using protease, ${\alpha}$-amylase, and chicken pancreas conjugase was evaluated to determine its usefulness in the microbiological quantitation of total folate in foods. Folate values obtained by alkali hydrolysis (Korean Food Standards Codex) were compared to those obtained by the trienzyme method for four certified reference materials (CRM) representing diverse matrixes. Trienzyme treatment increased measurable folate from most CRM compared to levels found after alkali hydrolysis. The largest increases were observed with CRM 487 (pig liver, 5.8-fold) and CRM 121 (whole meal flour, 3.1-fold) after trienzyme digestion. Using trienzyme digestion method, total folate contents of raw and blanched edible plants were determined. Eleutherococcus senticosus ($146.9{\mu}g/100g$) showed the highest total folate content, followed by Aster glehni F. Schmidt ($142.8{\mu}g/100g$) and Ledebouriella seseloides H. Wolff ($140.4{\mu}g/100g$) on a wet weight basis. Blanching of samples resulted in an insignificant decrease in folate content for five samples and 11~63% reduction for nine samples. Our finding suggests that trienzyme digestion method is accurate for the determination of food folate in leafy vegetables.
