장기간의 생식 섭취가 성장기 흰쥐의 성장, 골밀도 및 혈중 IGF-1의 농도에 미치는 영향 (Effects of Long-Term Administration of Saengshik on Growth Increment, BMD and Blood IGF-1 Concentration in Growing Rats)

  • 생식의 섭취가 실험동물의 길이성장 및 균형성장에 미치는 효능을 평가하기 위하여 12주간 생식을 1g/kg/day(1xJS), 2g/kg/day(2xJS) 및 4g/kg/day/(4xJS)의 용량으로 경구투여한 후 처치시점 4주, 8주 및 12주에 각각 희생시켜 성장에 관련된 요인들을 측정하였다 실험동물의 식이섭취량, 체중증가량 및 장기무게는 생식을 경구투여한 실험군과 대조군 간에 유의적인 차이가 없었으며, 공복 시 혈중 AST와 ALT의 농도 역시 군 간에 유의적인 차이를 보이지 않았고 모두 정상수치를 기록하였다. 실험동물의 신장 및 대퇴골 길이의 변화는 12주간의 실험기간 중 생식을 경구투여한 실험군이 대조군에 비하여 평균적으로 증가하였으며 특히, 처치시점 12주에서는 신장과 대퇴골 길이의 변화가 생식을 경구투여한 모든 실험군이 대조군에 비하여 유의하게 증가한 것으로 관찰되었다. BMD와 BMC의 결과에서도 BMD의 경우 4xJS군, BMC의 경우 1xJS군과 4xJS군이 대조군에 대하여 유의한 증가를 나타내었으나, 생식 투여량에 대한 농도의존적인 경향은 나타나지 않았다. 또한, 성장인자의 하나인 혈중 IGF-1의 경우 처치시점 4주에서 생식을 경구투여 한 모든 실험군이 유의적인 증가를 나타내었으며, 8주 이상에서는 4xJS군에서만 유의적인 증가가 나타났다. 이상의 결과에서 성장기에 장기적인 생식의 보충 섭취는 실험동물의 골성장을 통한 성장 발육에 도움을 준 수 있을 것으로 사료된다.

퇴직한 납 취급 근로자들에서 골밀도 저하와 경골납량이 혈중납량에 미치는 영향 (Effect of Bone Demineralization and Tibia Lead on Blood Lead in Retired Lead Workers)

  • This study was designed to investigate the effect of bone demineralization and tibia lead on blood lead in retired lead workers. Two hundred thirty five(126 females and 109 males) retired lead workers who worked in 4 different lead factories and 101 non-occupationally lead exposed subjects(51 females and 51 males) were recruited from March 2004 to October 2004. Bone mineral density(BMD) was measured at left calcaneous bone area by broadband ultrasound attenuation(BUA) method with QUS-2(Metra Biosystems Inc, USA). The BUA value transformed into T-score by WHO standard conversion criteria. Tibia bone lead was measured for skeletal bone lead with K-xray fluorescence(K-XRF) and blood lead was analyzed with flameless atomic spectrophotometer. Hemoglobin, hematocrit, serum calcium and iron were also analyzed. In addition, information for smoking and drinking status and basic personal data such as age, gender and lead exposure were also collected using questionnaire inquiry. Blood lead was correlated with tibia lead (r=0.664) and these two variables were negatively correlated with BMD in bivariate analysis. BMD showed significant main effect on the change of blood lead independent to tibia lead without any effect modification of age or gender; the one T-score unit decrease of mineral bone density made $0.43{\mu}g/dl$ increase of blood lead. On the other hand, tibia lead showed effect modification with gender on blood lead; the slope of tibia lead on blood lead in male was steeper than in female and crossed at around zero of tibia lead. In the multiple regression analysis of blood lead and tibia lead on BMD after adjustment of related covariates, only blood lead showed statistically significant effect on BMD. This study confirmed that BMD and blood lead were significantly associated. To verify the causal association of BMD on blood lead and vice versa, further longitudinal studies are needed.

Simultaneous Estimation of the Fat Fraction and R2* Via T2*-Corrected 6-Echo Dixon Volumetric Interpolated Breath-hold Examination Imaging for Osteopenia and Osteoporosis Detection: Correlations with Sex, Age, and Menopause

  • Objective: To investigate the relationships of T2*-corrected 6-echo Dixon volumetric interpolated breath-hold examination (VIBE) imaging-based fat fraction (FF) and R2* values with bone mineral density (BMD); determine their associations with sex, age, and menopause; and evaluate the diagnostic performance of the FF and R2* for predicting osteopenia and osteoporosis. Materials and Methods: This study included 153 subjects who had undergone magnetic resonance (MR) imaging, including MR spectroscopy (MRS) and T2*-corrected 6-echo Dixon VIBE imaging. The FF and R2* were measured at the L4 vertebra. The male and female groups were divided into two subgroups according to age or menopause. Lin's concordance and Pearson's correlation coefficients, Bland-Altman 95% limits of agreement, and the area under the curve (AUC) were calculated. Results: The correlation between the spectroscopic and 6-echo Dixon VIBE imaging-based FF values was statistically significant for both readers (pc = 0.940 [reader 1], 0.908 [reader 2]; both p < 0.001). A small measurement bias was observed for the MRS-based FF for both readers (mean difference = -0.3% [reader 1], 0.1% [reader 2]). We found a moderate negative correlation between BMD and the FF (r = -0.411 [reader 1], -0.436 [reader 2]; both p <0.001) with younger men and premenopausal women showing higher correlations. R2* and BMD were more significantly correlated in women than in men, and the highest correlation was observed in postmenopausal women (r = 0.626 [reader 1], 0.644 [reader 2]; both p < 0.001). For predicting osteopenia and osteoporosis, the FF had a higher AUC in men and R2* had a higher AUC in women. The AUC for predicting osteoporosis was highest with a combination of the FF and R2* in postmenopausal women (AUC = 0.872 [reader 1], 0.867 [reader 2]; both p < 0.001). Conclusion: The FF and R2* measured using T2*-corrected 6-echo Dixon VIBE imaging can serve as predictors of osteopenia and osteoporosis. R2* might be useful for predicting osteoporosis, especially in postmenopausal women.

일 대학 여대생의 골 (骨) 건강 관련 지식, 자기효능감 및 건강행위 (Female College Students' Knowledge, Self-efficacy and Health Behaviors related to Bone Health)

  • Purpose: This study described female college students' knowledge, self-efficacy and health behaviors related to bone health. Methods: A descriptive cross-sectional survey design was used. A total of 214 students were conveniently recruited from a school of nursing in Korea. Participants (mean age=21.2, SD=1.9 years) bone mineral density (BMD) was measured using dual energy x-ray absorptionmetry. Participants also completed a questionnaire consisting of knowledge test, self-efficacy scale and health behaviors related to bone health. Data were analyzed using SPSS/WIN 17.0 program. Results: The mean BMD at left femoral neck site was $0.86g/cm^2$ and 6.1~12.6%. Results showed that participants were below the expected range of BMD. Level of knowledge (correct rate=54%) and self-efficacy were moderate (mean 54.8 out of 84). More than 84% of subjects drank alcohol more than once a month and 58.4% did participate in regular exercise. Only 7.9% participants took calcium and vitamin-D rich foods every day. There were significant positive correlations between knowledge and self-efficacy (r=.17), health behaviors (r=.14) and self-efficacy and health behaviors (r=.28, all p<.05). Conclusion: These results suggest that health care professionals need to provide effective interventions for young women in order to enhance knowledge, self-efficacy and health behaviors related to bone health.

생체 내 경골의 피질골에서 시간역전 램파의 군속도와 피질골 특성 사이의 상관관계 (Correlations between the group velocity of time-reversed Lamb waves and cortical bone properties in tibial cortical bone in vivo)

  • 노화 및 골다공증으로 인한 경골 또는 요골과 같은 긴 뼈의 외부 껍질을 구성하는 피질골의 골강도 변화는 골절의 위험인자로 알려져 있다. 본 연구에서는 시간역전법을 이용하여 생체 내 경골의 피질골에서 발생된 시간역전 램파의 군속도를 측정하고, 군속도가 피질골의 골강도와 밀접한 관련이 있는 cortical bone thickness(cTh) 및 cortical bone mineral density(cBMD)와 갖는 상관관계를 조사하였다. 7명의 실험대상자의 우측 경골에서 측정된 시간역전 램파의 군속도는 cTh와 r = 0.90(p < 0.0001)의 매우 높은 상관관계를 보이며, cBMD와는 r = 0.69(p < 0.0001)의 상대적으로 낮은 상관관계를 보이는 것으로 나타났다. 본 생체 내 연구의 한계점은 단지 7명의 건강한 성인으로 구성된 정상군에 대하여 시간역전 램파의 군속도가 측정되었다는 것이다. 향후 다수의 건강한 성인 및 골다공증 환자로 각각 구성된 정상군 및 골다공증군에 대한 후속연구를 통하여 시간역전 램파의 임상적 유용성이 입증된다면 골다공증 진단을 위한 정량적 초음파 기술의 신뢰도를 향상시킬 수 있을 것으로 기대된다. 또한 시간역전 램파의 군속도를 측정하기 위한 골격 부위를 경골뿐만 아니라 대퇴골 또는 요골까지 확장시킬 필요가 있다.

The effect modification by the level of BMD (low: lower than the median of BUA and high: higher than the median of BUA) was observed between the association of tibia lead and blood lead after adjustment of covariates. The subjects who had higher BMD seemed to have lower blood lead by the increase of tibia lead than those of lower BMD. In the multiple regression analysis of blood lead and tibia lead on BUA after adjustment of covariates, only blood lead showed statistically significant effect on BUA. This study confirmed that BMD and blood lead were significantly associated. To verify the causal association of BMD on blood lead and vice versa, further longitudinal studies are needed.

농촌 성인 여성들의 골밀도에 영향을 미치는 요인 분석 연구 (Some Factors Affecting Bone Mineral Density of Korean Rural Women)

  • The factors affecting bone mineral density (BMD) of 103 rural women aged 30 to 76 years were investigated. Data for food and nutrient intake was obtained by 24-hour recall method. BMD of lumbar spine(L1-L4), femoral neck, ward's triangle and trochanter was measured by XR-series x-ray bone densitometer. Serum was collected and analyzed for total Ca, P and ionized Ca (Ca++)content. Relationship between the factors and BMDs was analyzed by Pearson's correlation coefficient(r) and multiple regression analysis. The results are summarized as follows. 50% of the subjects under 50 years of age($\leq$49 yr group) and 86.4% of the subjects from 50 up($\geq$50yr group) were classified as osteopenia or osteoporosis. Mean body weight, height and BMI were 153.1cm, 59.1kg and 25.0kg/$m^2$ in $\leq$49 yr group and 151.9cm, 55.9kg, and 24.2kg/$m^2$ in $\geq$50 yr group. BMDs of lumbar spines and femurs ranged from 0.84 to 1.05g/$m^2$ and from 0.67 to 1.16g/$m^2$ in $\leq$49 yr group, and ranged from 0.67 to 0.85kg/$m^2$ and from 0.68 to 0.44g/$m^2$ in $\geq$50 yr group, respectively. On the whole, the BMDs were reduced to 83.8 to 94.2% of peck bone mass in $\leq$49 yr group and 55.2 to 78.9% of those in $\geq$50 yr group. Mean daily intake of Ca was much less than the Korean RDA, \67.2% in $\leq$49 yr group and 62.3% in $\geq$50 yr group. The average concentration of total Ca, P and Ca++ in serum were within normal range in both age group. Both age and height were significantly related with BMD in both age group but the relationship tended to be stronger in $\geq$50 yr group than in $\leq$49 yr group. Body weight was also a potent determinant of BMD only in 50 yr group. In $\leq$49 yr group, total food intake was positively related with BMDs of ward's triangle, L1 and L2 and intake of cereals and grain products, sugars and sweets, milk and dairy products was positively related with BMDs measured in this study. On the contrary, intake of eggs, oil and fats were positively related with a few BMDs in 50 yr group. The BMDs were positively affected by intake of energy, protein, carbohydrate, Ca, P and Fe in $\leq$49 yr group and those of protein, fat Ca, P, vitamin B1, vitamin B2 and vitamin C in $\geq$50 yr group. It was noteworthy that serum Ca++ concentration was positively related with BMDs of lumbar spine in boty age groups. According to multiple regression analysis, the four factors, age, body weight, height and BMI additionally accounted for 21% of the variance in BMD of trochanter in $\leq$49 yr group and only two factors, age and C a intake accounted for 38% of that of femoral neck in $\geq$50 yr group. Further investigation is necessary to make sure of the relations between BMD and serum Ca++ level.

Effect of Titanium Prosthesis on Computed Tomography Measurements of Bone Mineral Density

  • 본 연구에서는 체내의 티타늄, 보철물, malalignment, 측정범위, 그리고 보철물 distal flare등이 전산화단층(CT) 촬영으로 골밀도를 축정할때에 미치는 영향을 조사하였다. 8구의 남성 사체로부터 대퇴골과 경골을 각각 8개씩 얻었으며 경골의 상부에 위치해있는 해면골 부분에서 15개의 평행사변형 시편을 만들었다. 평행사변형 시편과 대퇴골을 CT 스캔받침대의 중앙에 바로 놓았을때와 조금 떨어져 놓았을 때 그리고 티타늄 보철물을 체내에 넣었을 경우와 그렇지 않았을 경우 모두에 대해서 각각 스캔하였다. 이때 얻어진 CT 영상자료는 측정범위를 10cm와 30cm로 해서 재구성 하였다. 그리고 골밀도는 영상분석 소프트웨어을 사용하여 측정하였다. 그 결과 시편을 CT 스캔받침의 중앙에 놓았을 경우에는 그렇지 않았을 경우에 비해 골밀도 평균값의 상대차이가 피질골과 해면골 모두에 대해서 1% 미만이었다. 그러나 보철물의 malalignment와 distal flare는 골밀도의 측정값에 상당히 영향을 미쳤다. 체내에 티타늄 보철물의 존재여부는 시편을 CT 스캔받침대의 중앙에 놓았을 경우에는 피질골 뿐만 아니라 해면골 골밀도 측정값에 영향을 미치지 않았다. 그러나 체내에 티타늄 보철물의 존재가 malalignment 효과와 합쳐졌을때에는 골밀도 측정값이 많은 차이를 드러내었다.

The Change of Bone Mineral Density by Bisphosphonates Therapy with Calcium-Antagonists in Osteoporosis

  • Imbalance in calcium and phosphorous metabolism due to aging or menopause leads to osteoporosis. In contrast to patients with normal blood pressure, hypertensive patients have a higher loss of calcium in the urine with its attendant risk of osteoporosis. The high blood pressure is associated with the risk of bone loss and abnormalities in calcium metabolism leading to calcium loss. So we retrospectively investigated the changes of bone mineral density (BMD) which drugs can have clinical influences over osteoporosis treatments of patients with calcium-antagonists as common antihypertensive drugs and with bisphosphonates which causes a most effective inhibition of osteoclasts resorption. As a result over 70 years of age group and within bisphosphonates group, alendronate 70 mg once-weekly group showed significant increase of BMD in lumbar area. Combination group of cilnidipine and $maxmarvil^{(R)}$ showed very significant decrease of BMD. In conclusion, it is desirable that combination therapy with calcium-antagonists is used carefully in the treatment of osteoporosis with high blood pressure.

Fe 오염에 따른 Si내의 deep level거동에 관한 연구 (The Study of Deep Level Behaviors in Si Contaminated by Iron)

  • Fe 강제오염된 p-Si에서 여러 가지 quenching 조건에 기인한 에너지 준위들을 deep level transient spectroscopy(DLTS)를 이용하여 측정하였으며, 또한 선택 에칭방법/Optical microscope을 이용한 BMD(bulk micro-defeat)측정을 통하여 Fe 침전물 형서에, Fe 확산을 위한 어닐링 후 Cooling 조건이 미치는 영향을 분석하였다. Cooling 조건들이 여러 종류의 hole trap과 bulk micro-defeat(BMD)형성에 영햐을 주는 것으로 나타났으며, normal cooling의 경우 $\textrm{Fe}_{i}$, 또는 Fe-O complex 와 관계있는 $\textrm{T}_{1},\;\textrm{T}_{2},\;\textrm{T}_{3},\;\textrm{T}_{4}$ trap이 나타났으며, Slow Cooling 의 영향으로 인하여 활성화 에너지가 0.4eV에 해당하는 trap들이 관찰되었다. 또한 $\textrm{Fe}^{+}\textrm{}^{-}$ pair(H4: 0.56eV)는 $\textrm{LN}_{2}$ quenching한 경우에서만 나타났다.
