The Inhibitory Effects of Cheonsagunja-tang and Leech on the IL-6, IL-16, GM-CSF mRNA Level by Human Epithelial Cells (천사군자탕(喘四君子湯)과 수질(水蛭)(마황초(麻黃炒))가 인간 기관지상피세포의 IL-6, IL-16, GM-CSF 발현에 미치는 영향)

  • Objectives : We aimed to identify the dose-dependent inhibitory effects of Cheonsagunja-tang(喘四君子湯), leech(Hirudo medicinalis/水蛭) roasted with Ephedrae Herba(麻黃) on the mRNA expression of IL-6, IL-16, GM-CSF involved in the asthma model. Methods : In the study BEAS-2B cell lines, human epithelial cells were used. These cells were stimulated with tumor necrosis factor(TNF)-${\alpha}$ for artificial inflammatory expression. ${\beta}$-actin messenger RNA(mRNA) was used by internal standard. After 24 hours of the Cheonsagunja-tang, leech-treatment, total cellular RNAs were collected treating RNA zol directly on the living cells. Then the transcriptional activities of IL-6, 16 and GM-CSF were measured by RT-PCR with electrophoresis. Results: In the Cheonsagunja-tang study, the mRNA expression of IL-6 showed 30% transcriptional inhibitory effect compared to the control group in the $100{\mu}l/ml$ category(p<0.005). In the IL-16, there was 26%, 31% and 31% transcriptional inhibitory effect compared to the control groups in the $4{\mu}l/ml$, $20{\mu}l/ml$ and $100{\mu}l/ml$ categories, respectively(p<0.05). In the GM-CSF, the experimental group had 56% transcriptional inhibitory effect compared to the control group in the $100{\mu}l/ml$ category(p<0.001). In other concentrations, there was no inhibitory effect. In the leech study, the mRNA expression of IL-6 showed 37% transcriptional inhibitory effect compared to the control group in the $100{\mu}l/ml$ category(p<0.001). In the IL-16, there was 63% and 67% transcriptional inhibitory effect compared to the control groups in the $20{\mu}/ml$ and $100{\mu}/ml$ categories, respectively(p<0.001). In the GM-CSF, there was 64% and 68% transcriptional inhibitory effect compared to the control groups in the $20{\mu}l/ml$ and $100{\mu}l/ml$ categories, respectively(p<0.001). In other concentrations, there was no inhibitory effect. Conclusions : This study shows that Cheonsagunja-tang and leech roasted with Ephedrae Herba have dose-dependent inhibitory effects on the mRNA expression of IL-6, IL-16 and GM-CSF in BEAS-2B cell lines, human epithelial cells.

The impact of Health Risk Perception on Health Risk Behavior in Middle and High School Students (중고등학생의 건강위험지각이 건강위험행위에 미치는 영향)

  • Adolescence is vulnerable to various Health Risk Behaviors (HRB). These behaviors can affect his remaining life as well as adolescence, thus prevention of HRB is a critical issue in health education. This study is aimed to provide basic information for prevention of HRB. Thus, this study was conducted to analyze the impact of peer group's health risk behaviors on health risk perception (HRP) and that of health risk perception on health risk behaviors based on 832 respondents. The 852 subjects were selected in six middle and high schools in Seoul through random sampling. Data were collected from September, 18-October, 21, 1998, and the 832 data were analyzed after excluding the 20 incomplete and inaccurate data. Questionnaire items and measures are based on an instrument to measure Perceived Health Risk Perception, which Hodge B.C. developed in 1992. Cronbach alpha is used to test the reliability. The reliability of HRP and HRB is 0.9473, 0.8768 in this study, Statistical analysis divided into four phases. First, the impact of socio-demographic characteristics on HRP is analyzed by oneway ANOV A. Male students have lower HRP than female students. As grade goes up, HRP is getting lower. Perceived higher concern of parents and HRP are correlated. And the experience of school health education and HRP are correlated. Second, the impact of peer group's HRB on the HRP is analyzed by linear regression. Peer group's HRB and HRP are negatively correlated, Third, the impact of HRP on HRB is analyzed by linear regression. There is a correlation between high HRP and low HRB. Fourth, Powerful impact factors on HRB are analyzed by stepwise multiple regression. Grade, gender, peer group's HRB, and related HRP is entered as independent variables. Because of correlation between entered variables, three interaction variables between grade, gender, peer group's HRB and related HRP also entered, In general, peer group's HRB is the most accountable factor to HRB. And Interaction variable between HRP and peer group's HRB and HRB are negatively correlated. These results indicate that HRP may reduce the impact of peer group's HRB on HRB. Some recommendations are as follows: First, health educational programs suitable for gender and grade are required. Second, a systematic cooperation between school and home is necessary for effective prevention of HRB. Third, the educational effect for decreasing HRB by increasing HRP is statistically assisted. However, peer group has much stronger impact on HRB than subjective HRP, thus special consideration and management are necessary for peer group which does HRB more frequently.

Nutritional Status of Antioxidant Vitamins in the Elderly Living in Ulsan Metropolitan City

  • Body antioxidant status is an important factor for the prevention of many chronic diseases in the elderly. This study was done to evaluate the nutritional status of antioxidant vitamins of the elderly by determining their intakes and plasma levels. It was also aimed to compare daily intakes and plasma levels of antioxidant vitamins by sex of age. Subjects were 225 elderly persons aged over 60 years old (63 males, 162 females) living in Ulsan area. Subjects were divided by groups according to age(< 65, 65-74, 75) and sex. Dietary Intakes were assessed by semi-quantitative food frequency questionnaires(FFQ). Plasma Vitamin C level was measured by 2,4-dinitrophenylhydrazine method and plasma levels of vitamin E, A and ${\beta}$-carotene were measured by HFLC. The average intakes of vitamin C were 104.9g(150% of RDA) and 104.4g(149% of RDA) in the elder1y males and females, respectively. Vitamin C intake of the elderly was significantly decreased by aging but not different by sex. The average intakes of vitamin A were 678$\mu\textrm{g}$RE(96.9% of RDA) and 604$\mu\textrm{g}$:RE(86.2% of RDA) in elderly males and females, respectively. The average levels of Plasma vitamin C were 6.22mg/L and 11.45mg/L in the elderly males and females, respectively. Those levels are within normal range(6-20mg/L). However the percentage of the elderly males with deficiency(< 2mg/L) and marginal level(2-4mg/L) of vitamin C were 27.4% and 16.1%, respectively. Plasma retinaol levels were 0.39mg/L for the elderly males and 0.37mg/L for the elderly females, which were within normal range. But the percentage of subjects with marginal level were 28% ill both males and females. Plasma ${\alpha}$-tocopherol levels of the elderly were lower than normal range(5-12mg/L). Plasma levels of vitamin C, E and ${\beta}$-carotene, except retinol, were significantly higher in the elderly females compared to males and showed significant decrease by aging.

A Study of Variation Characteristics of the Phytoplankton Community by UPLC Located in the Jinju Bay, Korea (UPLC를 이용한 남해 진주만 식물플랑크톤 군집 변동특성 연구)

  • In order to provide important information for the efficient management of the identified farm ecosystem in Jinju Bay, we investigated the spatial and temporal distribution of the phytoplankton community using a UPLC pigment analysis and a CHEMTAX program from the timeframe of February 2013 to January 2014. In addition, we measured the available physical and chemical parameters controlling the distribution of the phytoplankton communities. As a result of this comprehensive pigment analysis, it was noted that the Diatoms were the predominant species with an average of 77.1% as noted located in Jinju Bay. It was discovered that during the summer season, the phytoplankton community composition was changed by a reduction of diatoms and noted increases of the Cryptophytes, Prasinophytes, and Dinoflagellates. Especially, it was noted that the Cryptophytes and Prasinophytes were shown with an average of 18.8% and 17.8% in June, respectively. However, it was revealed that the Cryptophytes and Prasinophytes were not shown by a microscopic observation. The phytoplankton community composition was correlated with the temperature and salinity variations as noticed in the Jinju Bay. Therefore, the water temperature and freshwater inputs in the Jinju Bay were important environmental factors for controlling the phytoplankton community composition and the varying Cryptophytes and the noted amounts of Prasinophytes as well.

Development of an evaluation tool of quality of nursing care for gastrointestinal surgery patient (위.장관계 수술 환자간호의 질평가를 위한 도구개발)

  • Background : Quality of professional nursing care is the most essential factor for survival and growth of nursing profession. Then, nursing professionals have responsibility for the evaluation of quality of professional nursing care. The purpose of this study was to develope an evaluation tool of nursing care for patients received gastrointestinal surgery with general anesthesia. This study was a primary work for the developement of a computer program for the evaluation of nursing care. Methods : This study was done through some consecutive steps. They were (1) Developement of items for the tool (2) Developement of an evaluation tool of nursing care quality for the G-I surgery patient (3) Test of reliability and validity of the tool. Two groups of experts and expert pannels who had much experience of the QA and the care of G-I surgery patients participated for developement of the items. 85 nursing records were used for the test of reliability and validity of the developed tool. The evaluation tools were developed with two types of scoring, norm-referenced tool and criterion-referenced tool. Results The system of items for tool was evaluation area evaluation item-indicator. There were 7evaluation areas which contained 32evaluation items which contained 7lindicators. Evaluation areas 1, 2, 3, 4 were for the evaluation of process and 5, 6, 7 were for the evaluation of outcome of nursing care for G-I surgery patient. For the test of interrator reliability, correlation coefficients of each scores of items and intragroup correlation coefficients were calculated. The average correlation coefficients between two rators were 0.65, 0.54 and the intragroup correlation coefficient were 0.99 and 1.00 by the types of scoring. The Cronbach alpha coefficients of the tools were 0.54 and 0.46 by the types of scoring. The average content validity index of the items was 0.95 from 4 pairs of experts. Because there were significant differences between some scores of quality of nursing care of 3 general hospitals regardless of the types of scoring, the tools could be thought to have some construct validity. And also, there were significant correlations between some scores of quality of nursing care and admission days and admission days after surgery regardless of the types of scoring, the tools could be thought to have predictive validity. Conclusion In this study, the evaluation tool of nursing care was developed for the very specified group of patient, G-I surgery patient. And the items were developed and tested by the experts of nursing practice. Because of these reasons, it was supposed that the tool could be used effectively in nursing pratice. And the procedures for the development and the test of the evaluation tool of nursing care in this study were supposed to be used for the developement of other tools.

A study on awareness change after infection control education by dental hygiene students in Some areas (일부 치위생과 학생의 감염관리 교육 후 인식 변화에 관한 연구)

  • This study examined the change in the students' perception of infection after school education. A self-reported questionnaire was filled out by 300 dental hygiene students in Kangwon from December 3 to December 4, 2016. The contents of the questionnaire included the general characteristics (5 items), infection-related selection (2 items), infection control recognition (5 items), and infection control behavior (10 items). The data were analyzed by frequency analysis, t-test, one way ANOVA, Chi-square test, and hierarchical multiple regression analysis using SPSS 24.0. The Cronbach's Alpha was 0.941 and 0.658, respectively. The school year and hospital practice experience had some influence on the change in infection recognition. A total of 127 students (62.6%) who had experience in practicing in hospitals knew more about antibodies. Hand washing was the highest in personal protection, followed by masks. The selection of the infectious control education method showed that the practical training class was high irrespective of the hospital practice experience, and the perceptions of patient-related infections were the greatest explanatory factor in perception after education.

Inhibitory Effects and Molecular Mechanism of Adipocyte Differentiation by Rosae laevigata Fructus Ethanol Extracs (금앵자 에탄올 추출물에 의한 3T3-L1 지방세포의 분화억제 효과와 그 메커니즘 규명)

  • Obesity is caused by excess accumulation of body fat and contributes to various pathological disorders such as diabetes, hypertension, cardiovascular disease, and cancer. In this study, we investigated the effect of a 30% ethanol extract of Fructus Rosae laevigata (RLE) on adipogenesis in 3T3-L1 adipocytes, measured by triglyceride accumulation and expression of adipogenesis-related transcription factors during differentiation of pre-adipocytes into adipocytes. RLE decreased the intracellular triglyceride contents (assessed by Oil Red-O staining) in a dose-dependent manner. It also downregulated the expression of adipogenic transcription factors and inhibited cell proliferation during the mitotic clonal expansion phase of adipocyte differentiation by inducing G1 phase arrest. We investigated the alterations in the levels of G1 phase arrest-related proteins. The expression of p21 protein significantly increased, while the levels of Cyclin E, Cdk2, and phospho-Rb decreased in a dose-dependent manner in 3T3-L1 cells treated with RLE. These results suggest that RLE inhibits the differentiation of 3T3-L1 adipocytes by suppressing the expression of adipogenic transcription factors and inducing G1 phase arrest in the early stages of adipocyte differentiation.

Effect of Dietary Fat Sources and L-arginine Supplementation on Endothelial function and Lipid Metabolism in Streptozotocin-Induced Diabetic Rats (식이지방의 종류와 L-arginine 보충이 당뇨쥐의 ENdothelium 기능 및 체내 지질대사에 미치는 영향)

  • Alteration in the syntesis or enhanced inactivation of nitric oxide(NO) can induce impairment of endothelial cell function. Insulin dependent diabetes mellitus(IDDM) is characterized by impaired endothelial function and vascular disease. NO is produced through L-arginine pathway To elucidate the hypothesis that the decreased production on NO in IDDM reflects vascular damage and the NO production can be manipulated by either dietary fat(7% of kg diet) or the oral supplementation with L-arginine(2g/kg bw), plasma markers for vascular endothelial damage and plasma lipid profiles were measured in streptozotocin(STZ)-induced diabetic rats. Diabetic or normal Sprague-Dawley rats were fed 6 different experimental diets for 4 weeks(SO : soybean oil, SOA: soybean oil + L-arginine supplementation, BT : beef tallow, BTA_ beef tallow + L-arginine supplementation, OV olive oil, OVA : olive oil + L-arginine supplementation). Plasma glucose, total cholesterel, HDL-cholesterol, LDL-cholesterol and triglyceride were measured. Endothelial markers, plasma von Willebrand factor(vWf), thromboxane B$_2$, and 6-keto PGF1$\alpha$ of aorta were measured by ELISA. Plasma NO production was evaluated through the measurement of nitrite by EIA. Feeding saturated fatty acid(SFA, BT) increased relative liver size(RLS) in diabetic rats compared to either polyunsatunted fatty acid(PUFA, SO) or monounsaturated fatty acid(MUFA, OV) The supplementation of L-arginine inhibited the liver and kidney enlargement in olive oil find diabetic rats. Plasma glucose was lower in diabetic animal find the olive oil compared to fed beef tallow and the supplementation L-arginine decreased it in diabetic rats find beef tallow significantly(p < 0.05). Plasma TXB$_2$ levels were increased due to diabetes and the value of beef tallow group showed highest value. Plasma vWf concentration of beef tallow group was higher value in normal rats and was elevated more in diabetes. In diabetic groups, the vWf concentration of olive oil group was lower than beef tallow or soybean oil group. The supplementation of L-arginine in diabetic rats decreased plasma TXB$_2$ and vWf levels significantly(p < 0.05). NO production was higher in normal olive oil fed rats and was tend to be decreased in diabetic rats and the supplementation of L-arginine recovered to normal value(p < 0.05), Olive oil supplemented with L-arginine tended to lower plasma total cholesterol and LDL-cholesterol after 4 week treatment. These results suggest that generalized vascular endothelial changes based on plasma TXB$_2$and vWf occurs in diabetic rats. and olive oil with L-arginine supplementation contributes to a better control of the hyperglycemia, endothelial changes and hypercholesterolemia accompanying diabetes as compared with beef tallow or soy bean oil in this rat model.

Enterococcus faecium FC-K Derived from Kimchi Is a Probiotic Strain That Shows Anti-Allergic Activity

  • A rise in the occurrence of allergic diseases is attributed to the dysregulated balance of type 1/type 2 immunity, where type 2 T-helper (Th2) cells predominate over type 1 T-helper (Th1) cells, leading to an abnormally increased production of IgE in response to unharmful antigens. Kimchi, a traditional Korean fermented food, is a rich source of beneficial lactic acid bacteria. In this study, we investigated the ability of Enterococcus faecium FC-K derived from kimchi to induce type I immunity in the presence of Th2 polarizing conditions in vitro and in vivo. Stimulation of mouse peritoneal macrophages with E. faecium FC-K induced the production of tumor necrosis factor alpha, interleukin (IL)-6, and IL-12. Under the in vitro Th2 conditions in which splenic T cells were activated in the presence of IL-4, E. faecium FC-K enhanced the ability of T cells to produce interferon $(IFN)-{\gamma}$. Using the ovalbumin (OVA)-induced allergy model, male BALB/c mice receiving E. faecium FC-K reduced the serum level of total IgE, but not that of OVA-specific IgE. Furthermore, the population of activated splenic B cells during OVA immunization was decreased in E. faecium FC-K-treated mice, accounting for a reduction of total IgE in the serum. Restimulating splenocytes from OVA-immunized mice with OVA ex vivo resulted in an increased production of $IFN-{\gamma}$, with no effect on IL-4, in E. faecium FC-K-treated mice. These observations provide the evidence that E. faecium FC-K can be a beneficial probiotic strain that can modulate the Th2-mediated pathologic response.

Prognostic factors in children with extracranial germ cell tumors treated with cisplatin-based chemotherapy

  • Purpose: To evaluate the outcomes and prognostic factors in children with extracranial germ cell tumors (GCTs) treated at a single institution. Methods: Sixty-six children diagnosed with extracranial GCTs between 1996 and 2012 were included in the study. Primary treatment was surgical excision, followed by six cycles of cisplatin-based chemotherapy. The survival rates were compared according to the International Germ Cell Cancer Cooperative Group classification used for GCTs in adults to validate the classification guidelines for GCTs in children. Results: The median patient age was 4.4 years. In 34 patients (51.5%), the primary tumor site was the gonad. Extragonadal GCTs were detected in 32 patients. The 5-year overall survival and event-free survival (EFS) were $92.0%{\pm}3.5%$ and $90.4%{\pm}3.7%$, respectively. In univariate analysis, tumor histology, metastasis, and elevated alpha-fetoprotein were not prognostic factors in children with extracranial GCTs. However, EFS was poorer in patients with mediastinal disease (n=12, $66.7%{\pm}13.6%$) than in those with nonmediastinal disease (n=54, $96.0%{\pm}2.8%$) (P=0.001). The 5-year EFS was lower in patients older than 10 years, (n=21, $80.0%{\pm}8.9%$) compared with those younger than 10 years (n=45, $95.2%{\pm}3.3%$) (P=0.04). Multivariate analysis identified the mediastinal tumor site as the only independent prognostic factor. Conclusion: The prognosis of children with extracranial GCTs was favorable. However, nongerminomatous mediastinal tumors were associated with poor survival in children. Further research is needed to improve the prognosis of children with malignant mediastinal GCTs.
