A Clinical case study about the Effects of Oryeong-san Gamibang on Case of Functional diarrhea (오령산가미방(五苓散加味方)으로 호전시킨 기능성 설사(泄瀉)환자에 대한 임상적 고찰)

  • Objectives : The purpose of this study is to investigate the effect of Leejung-tang plus Oryung-san Gamibang on functional diarrhea.Method : We treated a 72-year-old male who had severe watery diarrhea and bowel sounds. The patient was treated with Korean medicine including Oryung-san Gamibang from March 31, 2016 to May 23, 2016. Defecation frequency, Bristol stool scale, Bowel sound, Patient's condition were measured.Result : After oriental medical treatment, clinical symptoms of functional diarrhea (defecation frequency, stool form, bowel sounds) were recoverd normally.Conclusions : Leejung-tang plus Oryeong-san Gamibang effects on the functional diarrhea of a patient diagnosed as Deficiency Cold (虛寒). This case study shows that Oryeong-san Gamibang is clinically effective for functional diarrhea. So that, it is helpful in decreasing the symptoms of a patient and in improving quality of life.

Study on the Use of 'Saengsukron' in 'Tangaekpyeon of Donguibogam' (『동의보감(東醫寶鑑)·탕액편(湯液編)』에서의 생숙론(生熟論) 활용에 관한 고찰)

  • Objectives : Systematize concept of 'Saengsukron(生熟論)', providing theoretical basis for clinical and research purpose. Methods : Herbal medicines listed in 'Tangaekpyeon of Dongeuibogam' were studied in the following order ; (i) Search for herbal medicines that has applied heat. (ii) Choose herbal medicine with reference of change before and after heating. And choose herbal medicine with reference on purpose of heating. (iii) Classify herbal medicines with similar patterns. (iv) Catagorize herbal medicine by its effects, side effects, nature of herbal medicines, tastes of herbal medicines, meridian tropism of the herbal medicines, and others(color of herbal medicines, formation of herbal medicines, storage of herbal medicines) (v) Systematize concept of 'Saengsukron'. Results : We were able to obtain systematized concept of 'Saengsukron(生熟論)'. They are classified by 'Saengsasukbo(生瀉熟補)', 'Saenghwalsukji(生活熟止)', 'Saengmusukyu(生無熟有)', 'Sukjeukhyojeung(熟則效增)', 'Sukjeukhyogam(熟則效減)', 'Sukjeukyudok(熟則有毒)', 'Saengdoksukgam(生毒熟減)', 'Saengjunsukwan(生峻熟緩)', 'Saengchangsukso(生脹熟消)', 'Sukjeukwiseong(熟則爲升)', 'Sukjeukwigang(熟則爲降)', 'Sukjeukwion(熟則爲溫)', 'Sukjeukwiryang(熟則爲凉)', 'Saengjosukgam(生燥熟減)', 'Sukjeukbyeonmi(熟則變味)', 'Sukjeukbyeonchwi(熟則變臭)', 'Sukjeukbui(熟則部異)', 'Sukjeukjongsa(熟則從邪)', 'Sukjeukbyeonsaek(熟則變色)', 'Sukjeukbyeonhyeng(熟則變形)' and 'Sukjeukbojang(熟則保長)'. Conclusions : In this study, heat processing of herbal medicines confirmed that certain regular changes occur. Based on this, concept of 'Saengsukron' could be systematized. Research on 'Saengsukron' will help practitioners and researchers.

Review of Research for Herbal Medicine on Systemic Sclerosis (전신경화증의 한약치료에 대한 국내외 연구 동향)

  • Objectives : Systemic sclerosis(SSc) is an autoimmune disease characterized by fibrosis of the skin and internal organs and vasculopathy. The purpose of this study was to investigate the trend in the research on SSc using herbal medicine. Methods : We searched for papers which had both systemic sclerosis and herbal medicine from Pubmed, KCI and NDSL. After searching papers, we classified according to the study design and analyzed selected studies. Results : 18 studies were searched. The types and numbers of study were as follows: 11 were in vitro or in vivo studies using herbal medicine or active components, and 7 were clinical research including case reports. 1. Herbal medicines include the therapeutic effects of "tonifying qi(補氣)" or "active blood(活血)" improved systemic sclerosis in vivo and in vitro studies. 2. Active components isolated from Herbal Medicine such as Astragalus membranaceus(黃芪), Zhizi(梔子), Salvia miltiorrhiza(丹蔘) have anti-fibrotic effects. 3. Clinical trials showed that herbal medicine can improve the symptom of systemic sclerosis including skin fibrosis, Raynaud's phenomenon, pain and gastric dysmotility. Conclusions : This study showed that herbal medicine can be effective for treating SSc. However, further studies are needed to develop novel medicine for SSc.

A Study on the Safety of Bangpungtongsung-san in Rats (랫드에서 방풍통성산의 안전성 연구)

  • Objectives : This study was conducted to investigate the safety of Bangpungtongsung-san in rats. Methods : The safety of this prescription on acute toxicity was evaluated by single dose toxicity study. Rats were orally administrated in a single dose of 0 and 2,000 mg/kg(limited dose) Bangpungtongsung-san. There were 7 rats in each groups. All animals were sacrificed after 14 days of treatment. After single administration, mortality, clinical signs, and body weight changes were observed for 14 days. Three parameters(autopsy finding, clinical chemistry, and hematology) were tested on the last day. Results : In this study with rats, Bangpungtongsung-san treatment did not show any acute toxicity. No mortality was noted for 14 days of treatment. There were no adverse effects on clinical signs, body weight changes, and autopsy finding at all treatment groups. The clinical chemistry parameters attesting to liver and kidney functions as well as the hematological parameters were within the normal ranges. Conclusions : It is considered that $LD_{50}$ of Bangpungtongsung-san is over 2,000 mg/kg in oral administration by rats. This finding of the safety of Bangpungtongsung-san is expected to strengthen the position of this prescription as nontoxic medicine.

Case report of The liver transplant patient's cerebral infarction treated with Korean Medicine (간이식 환자의 뇌경색에 대한 한의학적 치료 증례보고)

  • Objectives : Liver cancer usually appears in the setting of underlying liver disease. Liver transplantation has potentials to improve survival for patients with liver cancer. This case report was designed to assess the clinical effect of Korean medicine on liver transplant patient's cerebral infarction.Methods : We provided Chiganpung Granule(Chodeungsan) to a patient who was 58 year old man complained of cognitive impairment and dysarthria. We checked the change of symptoms(K-MMSE, Aphasia Rapid Test) and liver function test and kidney function test.Results : We observed that Chiganpung Granule(Chodeungsan) decreased cognitive impairment and dysarthria. Liver function test and Kidney function test was improved.Conclusions : From the above results, it is considered that Korean medicine treatments is effective on liver transplant patient's cerebral infarction.

  • About the case of the patient who has diagnosed Systemic Lupus Erythematosus and admissed to our hospital from 27. January. 2005 to 28. February 2005, we could controll by herb medications userd differently patients-to-patients. SLE does not exactly correspond to any specific category of oriental medicine. But, according to previous reports, it can be controlled by oriental differential diagnosis of symptoms and signs. Because we diagnosed and treated her as at the point of Oriental Medicine and got the improvement, so we report. We think that the patient must get the screen test exactly and treat the disease proper ly, and if we apply this result to clinical cases at the point of Oriental Medical base from gathering and researching more cases, it will arouse sympathy-the excellence of Oriental Medicine and make the necessity of the further research and report from now on.

A Study on the system in the Theory of 'Syndrome Differentiation' from the Viewpoint of Yoon Gilyeong (윤길영의 변증체계 고찰)

  • Objectives Syndrome differentiation and treatment (辨證論治) was one of the core theories in Korean medicine and syndrome differentiation (辨證) constitutes a branch of disease diagnosis in Korean medicine. Yoon Gil-Young, one of the modern outstanding scholar of basic medical science in Korean medicine, wrote on basic theories of Korean medicine such as physiology, pathology, formula science, etc. Hereby we will analyze and discuss his works to understand his recognition of historical changes in the syndrome differentiation. Methods We conducted researches into the two works of Yoon Gil-Young's, which are "The Clinical Formula Science of Eastern Medicine (東醫臨床方劑學)" and "The theory of Four-Constitution Medicine (四象體質醫學論)". From Yoon's academic standpoint which connects the basic medical science with the clinical medicine, we analyzed his opinion about the system in the Theory of 'Syndrome Differentiation'. Results According to Yoon's research work on the Theory of 'Syndrome Differentiation', the system of syndrome differentiation, which had its deep root in the theory of Yin and Yang (陰陽) & the theory of abbreviation of the five circuit phases (五運) and the six atomspheric influences (六氣) of the "Huangdi's Internal Classic (黃帝內經)". Conclusions Yoon Gil-Young's theory of differentiation of syndromes and treatment is widespread so much that he studied on the learning field of Traditional Korean Mediciine and ingenious as well. He explain on the main principles of differentiation of syndromes based on "Huang Di Nei Jing" and the system of differentiation of syndromes is composed of Traditional Korean Medical Physiology.

Research Trends of Animal Experiment on the Herbal Medicine Treatment for Endometriosis (자궁내막증의 한약 치료에 대한 동물 실험 연구 동향)

  • Objectives: The aim of this study is to investigate the research trends of animal experiment on the herbal medicine treatment for endometriosis over the past 10 years. Methods: We searched the databases (Pubmed, Embase, Science-on) with terms as endometriosis, Korean medicine, herbal medicine and Chinese medicine. Among the results, we reviewed studies published from 2013 to 2022. Results: 26 studies were selected and analyzed. 8 single herbs and 18 complex formula were used in these studies. The most commonly used herbal medicine for endometriosis was Paeoniae Radix. Each studies showed that herbal medicine had significant effect on treating endometriosis. Conclusions: These results suggest that treating endometriosis with herbal material is effective in inhibiting ectopic lesion and angiogenesis, increasing pain threshold and apoptosis. Further research should be conducted to verify its therapeutic mechanisms and effectiveness.

  • ElM (Eighteen Incompatible Medicaments) is an important component in Oriental pharmacology and is directly related to clinical prescriptions. Medical practitioners argued that the definite cause and meaning of ElM was ambiguous and therefore debated the issue of clinical application of the ElM. This study conducted an in-depth literary research on the origin, meaning and contents of ElM with the purpose to contribute in its efforts to be used clinically. Even after thousands of years have past since establishment of Oriental medicine. ElM is still tabooed and was an obstacle that hindered ideologies. Modern herbal medicine texts claim that the use of ElM can reduce treatment effects and promote poisoning and side effects. However, since long ago, there has been medical practitioners who reject this as false. Recently, poisoning caused by ElM has been claimed to be from the toxicity of the drug itself, rather than the result of interaction between the drugs. and therefore they suggest that ElM is not a forbidden domain. In addition. ElM showed a difference in number depending on the era. However, this can be understood not as a definite number. but instead as a warning to be careful during combination of drugs for use as clinical medicine. Historically. there were very few cases in which ElM was used for clinical tests and thus, the clinical value is not yet clear. However. some medical practitioners clinically applied it and found clear results for it. while others applied ElM directly to their bodies. which showed signs for the usefulness and potential of ElM for us. A more concrete and in-depth study must be made on ELM.

  • The irritable bowel syndrome(IBS) is the most common gastrointestinal disorder in clinical practice and is characterized by abdominal pain associated with a chronic disturbance of defecation. The subject is a-69-year-old man who has abdominal pain, chronic diarrhea. anorexia. general weakness and has been diagnosed as irritable bowel syndrome. We diagnosed this patient as the Gastrointestinal Phlegm(食痰) and prescribed Jengjengamiyijin-tang (Zhengchuanjiaweierchen-tang). In the result, we had improvement of his symptoms.
