측두두정근막 자유피판술을 이용한 수부재건의 장기추적조사 (Long-term Follow-up of Reconstruction of the Hand with a Temporoparietal Fascial Free Flap)

  • Purpose: Soft tissue defect of the hand, which cannot be covered with skin graft or local flap, is usually reconstructed using a free flap. Temporoparietal fascial free flap is one of the best alternatives for functional reconstruction of the hand with exposed tendons, bones, and joints. Materials and Methods: We have experienced four cases of reconstruction using a temporoparietal fascial flap with a skin graft and followed up for 20 years. We conducted a retrospective review of the patients' clinical charts and photos. Results: At the time of initial injury, the average age of patients was 50.3 (39~62) years. The radial artery was used for reconstruction of the dorsal side of the hand, whereas the ulnar artery was used for that of the volar side of the wrist. Short term complication such as skin graft loss and donor site alopecia occurred. However, during the long term follow-up period, no change of flap volume was noted, and full range of motion in the adjacent joint was maintained. In addition, hyperpigmentation of the grafted skin on the flap disappeared gradually. Conclusion: Selection of the optimal flap is important for reconstruction of the hand without functional limitation. We obtained satisfactory soft tissue coverage and functional outcomes using a temporoparietal fascial free flap and followed up for 20 years.

아이언 페이드샷의 운동학적 특성 분석 (A Kinematical Characteristic Analysis of a Iron fade-shot with a Golf Swills)

  • 본 연구는 골프 기술 가운데 하나인 페이드샷에 대하여 3차원 영상분석을 시도하여 비교분석한 연구이다. 연구 대상은 남자 프로 골프선수 3명을 대상으로 하였으며 7번 아이언으로 국한하여 실험을 실시하였다. 이때 운동학적 주요 변인들을 산출하여 그 변인들에 대한 특성을 파악하고 지도자와 선수들에게 자료를 제공함으로써 효과적인 지도 방법 및 원리를 제공하는데 목적을 두었다. 그 결과 클럽헤드의 궤도는 아웃사이드에서 인사이드의 형태로 나타나 클럽헤드의 궤적이 이상적인 패이드 샷에 영향을 미치는 것으로 판단된다. 신체중심은 후방으로의 움직임이 나타날 때 실패의 가능성이 높은 것으로 나타나 전방으로의 중심이동이 성공요인에 긍정적인 영향을 미치는 것으로 판단된다. 코킹은 임팩트 후 자연스럽게 팔로스로우를 하여 손목을 풀어 주었으며 움직임의 순서에서 어깨 동작이 먼저 이루어지고 허리가 따라가는 형태로 나타났다. 고관절 각속도는 지연히팅으로 인한 자연스러운 페이드샷이 이루어지는 형태와 일치되었으며 클럽헤드의 속도 또한 임팩트시 코킹이 풀리면서 빠른 것으로 나타났다.

관성 측정 장치의 융합연구 형태와 방법에 관한 체계적 고찰 (Systematic Review on the Type and Method of Convergence Study of Inertial Measurement Unit)

  • 본 연구의 목적은 관성 측정 장치(Inertial Measurement Unit, IMU)의 융합연구 형태와 방법에 대한 연구를 체계적 문헌고찰 방법으로 분석하여 IMU의 형태와 방법에 대한 경향을 파악하고자 한다. 연구수행은 PRISMA(Preferred Reporting Items for Systematic Reviews and Meta-Analyses)가이드라인을 이용하여 수행되었다. 3개의 데이터베이스에서 검색된 630편 중 최종적으로 선정기준에 부합하는 23편을 선정하였다. 본 연구결과 전 세계적으로 IMU를 사용한 다양한 연구를 진행하고 있음을 알 수 있었고, IMU의 형태는 스트랩, 전신 슈트, 벨트, 손목시계, 신발, 장갑이 있었다. 이 중, 스트랩 형태의 IMU가 11편으로 가장 많았다. IMU의 장점인 간소화와 실시간 데이터 수집, 적용의 쉬움으로 작업 활동, 보행, 관절 가동 범위 등의 측정 방법으로 사용되었다. 본 연구의 결과는 IMU의 연구를 진행하는 의료 및 재활 분야의 전문가들에게 기초 자료로 활용될 수 있으리라 기대된다.

휠체어 새천년 건강체조의 팔꿈치 운동속도에 따른 상지협응과 근활성도 변화 (Wheelchair-based New Millennium Health Gymnastics: Muscle Activity and Upper Limbs Coordination by Elbow Exercise Velocity)

  • PURPOSE: This study concerns the wheelchair-based rehabilitation of elderly people, investigating muscle activity and coordination of upper limbs during wheelchair-based new millennium health gymnastics with varying elbow exercise velocity. METHODS: Twelve elderly people participated in new millennium gymnastics twice per week during 12-weeks. The group was separated into 0.4, 1.0, and 1.6 Hz groups (controlled by the metronome speed). Range of motion was measured by electrogoniometer, electromyography signals used root mean square values. The data application was normalized using reference voluntary contraction (%RVC). Upper limb (wrist and elbow joint) data gathered while standing up after the "falling on hips" was investigated in terms of coordination of angle-angle plots. One-way ANOVA, paired t-test and Scheffe's post hoc comparisons, were used for statistical analyses. RESULTS: There were results taken before and after the experiments. The results demonstrated a significant improvement in the triceps brachii and flexor carpi radialis of the 0.4 Hz group (p<.05). There was significant difference in the triceps brachi of the 1 Hz group. No significant differences were found in all muscles of the 1.6 Hz group. Muscle co-activation indexes of the 0.4 Hz group were larger than the others. The 0.4 Hz graph was turning point synchronized clockwise. The 1 Hz graph was out of phase with the negative slope. The 1.6 Hz graph was turning point synchronized counterclockwise, and uncontrolled factor phase was offset on angle-angle plots. CONCLUSION: It is found that improvement of muscle activity and upper limbs coordination of elderly people using wheelchair-based new millennium gymnastics is optimal with elbow exercise velocity with a frequency of 0.4 Hz.

가상현실환경에서 정상성인의 거울보행이 보행특성에 미치는 영향 (The Effect of Gait Exercise Using a Mirror on Gait for Normal Adult in Virtual Reality Environment: Gait Characteristics Analysis)

  • Purpose : The study aims to determine the effects of virtual and non-virtual realities in a normal person's mirror walk on gait characteristics. Methods : Twenty male adults (Age: 27.8 ± 5.8 years) participated in the study. Reflection markers were attached to the subjects for motion analysis, and they walked in virtual reality environments with mirrors by wearing goggles that showed them the virtual environments. After walking in virtual environments, the subjects walked in non-virtual environments with mirrors a certain distance away after taking a 5 min break. To prevent the order effect caused by the experiential difference of gait order, the subjects were randomly classified into groups of 10 and the order was differentiated. During each walk, an infrared camera was used to detect motion and the marker positions were saved in real time. Results : Comparison between the virtual and non-virtual reality mirror walks showed that the movable range of the leg joints (ankle, knee, and hip joints), body joints (sacroiliac and atlantoaxial joints), and arm joints (shoulder and wrist joints) significantly differed. Temporal characteristics showed that compared to non-virtual gaits, the virtual gaits were slower and the cycle time and double limb support time of virtual gaits were longer. Furthermore, spacial characteristics showed that compared to non-virtual gaits, virtual gaits had shorter steps and stride lengths and longer stride width and horizontally longer center of movement. Conclusion : The reduction in the joint movement in virtual reality compared to that in non-virtual reality is due to adverse effects on balance and efficiency during walking. Moreover, the spatiotemporal characteristics change based on the gait mechanisms for balance, exhibiting that virtual walks are more demanding than non-virtual walks. However, note that the subject group is a normal group with no abnormalities in gait and balance and it is unclear whether the decrease in performance is due to the environment or fear. Therefore, the effects of the subject group's improvement and fear on the results need to be analyzed in future studies.

Constant score in asymptomatic shoulders varies with different demographic populations: derivation of adjusted score equation

  • Background: In the present study, the age- and sex-adjusted Constant score (CS) in a normal Indian population was calculated and any differences with other population cohorts assessed. Methods: The study participants were patients who visited the outpatient department for problems other than shoulder and healthy volunteers from the local population. Patients without shoulder pain/discomfort during activity were included in the study. Subjects with any problem that might affect shoulder function (e.g., cervical, thoracic spine, rib cage deformity, inflammatory arthritis) were excluded. Constant scoring of all participants was performed by trained senior residents under the supervision of the senior faculty. Shoulder range of movement and strength were measured following recommendations given by the research and Development Committee of the European Society for Shoulder and Elbow Surgery (2008). A fixed spring balance was used for strength measurement; one end was fixed on the floor and the other end tied with a strap to the wrist of the participant, arm in 90° abduction in scapular plane with palm facing down. Results: Among the 248 subjects (496 shoulders), the average age was 37 years (range, 18-78 years), 65.7% were males (326 shoulders) and 34.3% females (170 shoulders). The mean CS was 84.6±2.9 (males, 86.1±3.0; females, 81.8±2.9). CS decreased significantly after 50 years of age in males and 40 years of age in females (p<0.05). The mean CS was lower than in previous studies for both males and females. Heavy occupation workers had higher mean CS (p<0.05). A linear standardized equation was estimated for calculating the adjusted CS for any age. Conclusions: Mean CS and its change with age differed from previous studies among various population cohorts.

Effects on Mechanical Properties, Joint Range of Motion, and Grip Strength of Forearm Muscles Depending on Wrapping Direction of the Floss Band

  • Objective: The purpose was to investigate changes in mechanical properties, range of motion, and grip strength of the forearm muscle in 13 students depending on the wrapping direction of the floss band. Design: A single-blind, crossover study Methods: Subjects were randomly assigned to the wrapping direction of the flossing band and then performed a concentric exercise of the wrist flexors using a Flex-Bar. Intervention A applied the flossing band in a right spiral direction, Intervention B applied the flossing band in a left spiral direction, and Intervention C performed the exercise alone. All subjects used their dominant right hand, and pre- and post-assessments were conducted between interventions. To analyze differences in changes between groups pre- and post-, all results were subjected to one-way ANOVA, followed by Scheffe's test as a post-hoc analysis. The paired samples t-test was used to analyze the difference between pre- and post-change within groups. Results: First, in the mechanical properties of the Flexor carpi ulnaris (FCU) muscle, interventions A and B significantly improved muscle tone and stiffness than intervention C (p<0.05), and intervention A showed a significant decrease in decrement (muscle elasticity) than intervention B (p<0.05). Second, interventions A and B showed significant improvement in grip strength than intervention C (p<0.05). Conclusions: The right spiral direction of the flossing band tended to increase the elasticity of the muscles compared to the left spiral direction. Therefore, in future studies, it is necessary to choose the direction of the flossing band to increase the elasticity of the muscles.

테니스 플랫 서브 동작의 운동학적 분석 (The Kinematic Analysis of the Tennis Flat Serve Motion)

By the comparison and the analysis of the different factors during the tennis flat serve motion such as the required time per section, the movement displacement of the racket, the velocity of the upper limbs joints, the physical center of gravity, and the angle and the angular velocity of the upper limbs joints between an ace player and a mediocre player, these following results were drawn. First, the experiment result of the total time required per section in a tennis flat serve motion showed that an ace player was faster than a mediocre player by 0.4 seconds. This result suggested that it was required to increase the speed of the racket head by a swift swing to perform an effective flat serve motion. Second, the experiment result of the movement displacement of the racket in the tennis flat serve motion showed that an ace player greatly moved toward the left side on an x-axis. But both an ace and a mediocre player were shown to be at the similar points on a y-axis at the moment of the impact of the racket. An ace player was also shown to be located at a higher position on a z-axis by 0.23m. Third, the velocity of the center of gravity of an ace player was faster in every phase than that of a mediocre player in a tennis flat serve motion. Fourth, the velocity of the upper limb joints of an ace player was faster in every phase than that of a mediocre player in a tennis flat serve motion. Fifth, the experiment result of the speed of the racket head in tennis flat serve motion showed that a mediocre player was faster than an ace player in the first phase, but the latter was faster than the former in the second, third, and the fourth phases. Sixth, at the moment of impact of a tennis flat serve, an ace player had greater flexion of the angle of the wrist joints by an 11.8 degree than a mediocre player. An ace player also had greater extension of the angle of the elbow joint and the shoulder joint respectively by a 5.2 degree and a 1.4 degree with a mediocre player. Seventh, an ace player had greater angular velocity of the upper limb joints and the hip joints than a mediocre player at the moment of the impact of tennis flat serve. Eighth, an ace player was shown to have a greater change of the forward and the backward inclination (or the anterior and posterior inclination) of the upper body

골프 드라이브 스윙시 구질 변화에 따른 운동학적 분석 (Biomechanics analysis by golf drive swing pattern)

  • 본 연구에서는 볼 구질의 변화에 따른 스윙 동작의 운동학적인 변인을 3차원적으로 비교 분석하여 스윙 동작의 운동역학적인 원리를 제시하며, 볼 구질이 다른 스윙 동작의 차이점을 밝히는 데 목적이 있으며, 프로골퍼로 경력 3년 이상의 남자 선수 7명을 대상으로 스트레이트 성공, 페이드 성공과 실패로 구분하여 골프 스윙의 운동학적 변인들을 과학적으로 제시하기 위해 비교 분석한 결과 다음과 같은 결론을 얻었다. 페이드 성공시에는 임팩트를 지나서도 계속 손목의 각도가 근소한 차이로 줄어드는 것으로 나타났고 실패시에는 각 프레임의 각도가 규칙적으로 변하지 않게 나타났다. 이는 스트레이트 성공과 페이드 성공에 대한 손목각도의 차이가 영향을 미치는 것으로 나타나 다른 요인과 손목각도와의 관계를 분석하는 것이 필요하다고 하겠다. 페이드 실패의 경우는 임팩트시의 몸통 비틀림각도가 성공때 보다 작아지면서 클럽헤드가 오픈되지 못한 것으로 나타났다. 골프스윙의 형태의 변화로 인하여 어깨관절의 회전력을 더 많이 사용하여 파워을 증진시키기 위한 동작이며, 골프 스윙에서 어깨회전 운동은 축을 중심으로 회전하기 때문에 어깨회전각도의 차이는 없는 것으로 나타났다. 스트레이트 성공시 보다 페이드 성공시에 고관절의 회전각도가 점진적으로 일정하게 각이 커졌다. 이는 고관절의 회전을 더 크게 하여 지연히팅을 보다 자연스럽게 한 것으로 나타났다. 페이드 볼 구사시 임팩트 존에서 클럽의 페이스가 닫히는 것과 팔의 자연스러운 롤링 동작을 억제시키는 것으로 나타났다. 본 연구에서는 구질 변화에 따른 골프 스윙 동작을 스트레이트 성공, 페이드 성공과 실패시로 구분하여 운동학적 변인들을 3차원 동작 분석을 통하여 경기력 향사에 도움이 되고자 하였으며 앞으로 많은 연구자들이 각 종목에 있어서 경기력 향상을 위한 역학적인 분석이 계속 진행되어져야 할 것이다.

경호원의 삼단봉 머리치기 동작의 운동학적 분석 (Kinemetic analysis of a thumping security motion with an expandable barton)

  • 본 연구에서는 경호, 경비현장에서 이루어지는 대치상황에서 경호, 경비원의 장비 중 삼단봉을 선택하여 머리치기 동작의 운동학적 분석을 실시하였다. H대학교 경호학과 학생 출신10명을 대상으로 연구를 실시하였다. 숙련자 그룹에서 삼단봉 교육 프로그램 실시하였고, 숙련자그룹과 비숙련자 그룹으로 연구 하였다. 구간별 소요시간은 숙련자 (e2) 0.428sec 비숙련자 0.435 sec, (e3) 에서는 숙련지 0.230 sec 비숙련자 0.232 sec 소요되었으며, 비숙련자 그룹에서 소요시간이 많이 소요된 것으로 나타났다. 신체중심 이동 변위는 좌, 우 숙련자$2.16{\pm}0.95cm$, $3.78{\pm}1.42cm$, 전체적으로 $5.94{\pm}2.03cm$ 변화를 보였다. 비숙련자 $2.97{\pm}1.01cm$, $4.56{\pm}1.57cm$, 전체적으로 $7.53{\pm}2.13cm$ 변화를 보였고, 전, 후 이동에서는 숙련자$32.48{\pm}3.86cm$, $35.21{\pm}4.64cm$, 전체적으로 $69.36{\pm}5.72cm$ 변화를 보였다. 비숙련자 $34.50{\pm}6.12cm$, $37.04{\pm}3.70cm$, 전체적으로 $71.46{\pm}7.17cm$ 변화를 보였다. 그리고 상, 하 이동변위에서는 숙련자 $5.62{\pm}2.49cm$, $4.54{\pm}1.87cm$, 전체적으로 $10.11{\pm}1.57cm$ 변화를 보였다. 비숙련자 $6.33{\pm}1.78cm$, $4.86{\pm}1.85cm$, 전체적으로 $10.68{\pm}1.81cm$ 변화를 보였다. 손목관절의 각도 변화에서 손목각도이며, 숙련자e1, $114.62{\pm}7.13$, e2 $68.27{\pm}6.37$, e3 $131.64{\pm}6.27$, 비숙련자 e1, $112.62{\pm}6.13$, e2 $66.28{\pm}7.38$, e3 $137.42{\pm}4.28$ 팔꿉치 관절의 각도 변화이며, 숙련자 e1, $132.31{\pm}6.55$, e2 $117.92{\pm}8.42$, e3 $144.41{\pm}6.32$, 비숙련자 e1, $133.58{\pm}8.56$, e2 $114.45{\pm}8.21$, e3 $139.89{\pm}4.38$ 어깨관절의 각도 변화이며, 숙련자 e1, $13.55{\pm}3.85$, e2 $131.42{\pm}11.24$, e3 $78.32{\pm}6.28$, 비숙련자 e1, $9.45{\pm}1.23$, e2 $136.74{\pm}13.21$, e3 $79.75{\pm}4.24$, 각 변화를 보였다.
