수피상처치료를 위한 식물생장조절물질 기반 상처유합제 개발 (Development of Wound-treatment Formulation Using Plant Growth Regulators for Wound Healing of Some Tree Species)

  • 상처로부터 수목을 보호할 상처유합제를 개발하기 위하여 생장조절물질을 첨가한 MS 배지에 느티나무, 대추나무, 소나무, 은행나무 및 칠엽수의 가지 절편을 배양하고 유합조직의 형성률을 조사하였다. 기내시험 결과를 바탕으로 2,4-D, NAA, IBA 등을 후보물질로 선발하고 증량제인 잔탄검과 혼합하여 상처유합제를 조제하였다. $2{\times}10cm$ 크기로 수피를 완전히 제거한 절단면에 각각의 상처유합제를 충분히 바르고 12주 뒤에 유합조직의 크기를 조사하였다. 유합조직 형성량은 수종에 따라, 상처유합제의 유효성분에 따라 다르게 나타났다. 느티나무와 단풍나무에서는 2,4-D 2 mg/L가 가장 좋은 효과를 보였으며, 잣나무에서는 NAA 8 mg/L이, 그리고 은행나무에서는 IBA 1 mg/L이 가장 좋은 효과를 보였다. 은행나무는 다른 수종보다 유합조직 형성량이 현저히 적었다. 티오파네이트메틸 도포제는 느티나무에서만 효과가 뛰어났으며, 다른 수종에서는 모든 생장조절제제를 능가하지는 못했다. 수피의 상처처리제로 널리 사용되고 있는 바셀린은 4 수종 모두에서 상처부위에 오래 잔존하며 부후를 촉진시키는 역효과를 보였다. 따라서 활엽수의 상처유합제로는 2,3-D 제제가, 송백류에는 NAA 제제가, 그리고 은행나무에는 IBA 제제가 적당한 것으로 밝혀졌다.

Intraoperative Cerebrospinal Fluid Leak in Extradural Spinal Tumor Surgery

  • Objective: Patients with extradural spine tumors are at an increased risk for intraoperative cerebrospinal fluid (CSF) leaks and postoperative wound dehiscence due to radiotherapy and other comorbidities related to systemic cancer treatment. In this case series, we discuss our experience with the management of intraoperative durotomies and wound closure strategies for this complex surgical patient population. Methods: We reviewed our recent single-center experience with spine surgery for primarily extradural tumors, with attention to intraoperative durotomy occurrence and postoperative wound-related complications. Results: A total of 105 patients underwent tumor resection and spinal reconstruction with instrumented fusion for a multitude of pathologies. Twelve of the 105 patients (11.4%) reviewed had intraoperative durotomies. Of these, 3 underwent reoperation for a delayed complication, including 1 epidural hematoma, 1 retained drain, and 1 wound infection. Of the 93 uncomplicated index operations, there were a total of 9 reoperations: 2 for epidural hematoma, 3 for wound infection, 2 for wound dehiscence, and 2 for recurrent primary disease. One patient was readmitted for a delayed spinal fluid leak. The average length of stay for patients with and without intraoperative durotomy was 7.3 and 5.9 days, respectively, with a nonsignificant trend for an increased length of stay in the durotomy cases (p=0.098). Conclusion: Surgery for extradural tumor resections can be complicated by CSF leaks due to the proximity of the tumor to the dura. When encountered, a variety of strategies may be employed to minimize subsequent morbidity.

천공분지에 기저를 둔 도서형 회전 근막 피부 피판을 이용한 압박궤양의 치료 (Perforator-Based Fasciocutaneous Island Rotation Flap in Treatment of Pressure sore)

  • The reconstruction of deep soft tissue defect such as pressure sore has difficult problems. Myocutaneous flaps have been used commonly as the best coverage method for pressure sore. But, they have several drawbacks such as sacrifice of functional muscle, high morbidity of the donor sites and bulkiness at the recipient site. The concepts of perforator flap has recently developed and widely used to overcome these disadvantages. Between March 2005 to July 2006, we have treated 9 patients who had pressure sore using perforator based fasciocutaneous island rotation flap. Preoperative unidirectional Doppler was used in all cases. Mean number of perforator vessels was 3.8 and flap sizes were from $7{\times}5\;cm$ to $14{\times}13\;cm$. Rotation angles of flap were from 90 degree to 180 degree. In all cases, donor sites were closed primarily. All flap survived completely and postoperative complications were wound dehiscence in 1 case, wound infection in 3 cases. The mean postoperative follow up period was 15.7 months and recurrence was not reported. We could decrease donor site morbidity and cover wound sites easily by using flap rotation and get robust blood supply without sacrifice of functional muscle. Fasciocutaneous perforator island rotation flap would be very useful for various pressure sore treatment.

악안면 연조직 외상치료의 최신지견 (Current Concepts in the Treatment of Maxillofacial Soft Tissue Trauma)

  • The maxillofacial soft tissue trauma is one of the major causes to visit the emergency room. For the past few decades, however, the basic concept of the repairing the soft tissue wound have not been changed. Therefore, it could be worthwhile to remind the fundamental concepts and practical information belong to the soft tissue injury management. Among the many types of soft tissue trauma, laceration wound which is most frequently met in the clinic will be discussed in this review.

영아 지루성(脂漏性) 피부염(皮膚炎)에 대한 증례(證例) 1례(例) (A Case Report of Seborrheic Dermatitis in an Infant)

  • 심한 두부(頭部) 발진(發疹), 소양(搔痒) 및 화농(化膿)을 호소하여 동국대학교(東國大學校) 분당한방병원(盆塘韓方病院) 한방소아과(韓方小兒科)에 내원(來院)한 영아 지루성(脂漏性) 피부염(皮膚炎) 환자(患者) 1예(例)에 대하여 초진(初診) 진료(診療) 후 Aroma Lotion과 Aroma oil을 혼합한 외용제(外用劑)를 사용한 도포치료(塗布治療)와 carbon 광선치료(光線治療)를 병행하여 37일간 치료(治療)한 결과 두부(頭部) 발진(發疹), 소양(搔痒) 및 화농(化膿)이 완전 소실되어 유효한 결과를 얻었기에 이에 치료경과(治療經過)를 보고하는 바이다.

고압맥동 평류자극이 가토 상처치유에 미치는 영향 (Effect of High Voltage Pulsed Galvanic Current on Wound Healing in Rabbits)

  • This study was performed to assess the efficacy of high voltage pulsed galvanic current for the healing of wounds in rabbits. Skin wounds were created laterally on the flank of 12 domestic rabbits($3{\times}3cm$). The wounds of each group were treated with an intensity of 170 V at a frequency of 70 pulses per second, which was applied for 30 minutes a day for 10 days. The experimental groups were randomly assigned to either EXP I (n=3), EXP II(n=3), EXP III(n=3) or control(n=3). Each group was stimulated under the following conditions : 1) EXP I (Negative polarity), 2) EXP II (Change in polarity, negative electrode stimulation during the first 3 days and then positive electrode stimulation from 4 to 10 days), 3) EXP III(Positive polarity), 4) control(No stimulation). An active electrode was placed over the wound and a dispersive electrode on the buttock. The rate of wound closure was compared with the original wound size, evaluated by a tracing film in each measurement period. Finally, on the wound in each group, skin tissue was excised for histological evaluation after treatment for 10 days. The results obtained are as follows : 1) It was found that the control group did not show a complete remodeling of epitherial layer and had a chronic inflammatory response. Judging from the irregularity of intercellular space and the loose alignment of connective tissue, these findings show that wound healing was delayed. 2) EXP I showed a significant bactericidal effect, but a moderate response of vasodilation. The rate of wound closure was slower when compared with EXP II, III. 3) EXP II showed a complete remodeling of epitherial layer and a positive repair of connective tissue. Its rate of wound closure was best when compared with the others. 4) EXP III had a slower rate of wound closure than EXP II, but judging from the greater proliferation of collagen fibers and the dense alignment of connective tissue, this positive electrode was very effective in the formation of neo - connective tissue.

Hairless mouse의 피부상처 후 실크단백질 처리가 표피성장인자의 발현에 미치는 영향 (The Effects of Bombyx mori Silk Protein for the Expression of Epithelial Growth factor in the Wound Healing Process of the Hairless Mouse)

  • 피부 섬유아세포에 대한 증식을 촉진하는 가잠의 실크 단백질 분획물, BM-1이 상처 치유 과정에 미치는 효과를 검증하기 위하여 hairless mouse의 등쪽에 인위적인 창상을 유발한 후 표피성장인자의 발현 정도를 관찰하였다. 실험은 창상 유발 후 단순 음수만을 실시한 대조군과 매일 BM-1($100{\sim}116 ug/day$)을 포함한 음수를 식이하도록 한 처리군으로 구분하여 7일간 진행하였다. 1) 대조군과 처리군간에 체중 및 사료와 음수 소비량에 유의할 만한 차이는 없었다. 2) 상처 부위의 재생율은 3일째 대조군은 22.6%, 처리군은 38.1%의 회복율을 보였으며, 7일째에는 각각 68,1%, 90%의 회복율을 보여 처리군의 재생 속도가 유의하게 빠름을 알 수 있었다. 3) 대조군에 비하여 처리군의 피부 상처 조직에서의 표피성장인자의 단백질 발현과 mRNA의 발현이 높게 나타나는 양상을 보였다. 따라서, 실크 단백질, BM-1은 피부 창상의 치유 과정에서 표피성장인자를 활성화시킴으로서 상처 치유시간을 단축시키고 치유를 촉진하는 것으로 사료된다.

  • Neck trauma is a relatively uncommon but can be a life-threatening injury. Several guidelines for neck trauma is established to recommend a proper management such as no clamping of bleeding vessels, no probing of wounds, Trendelenberg position for preventing venous air embolism. Here, we present a regretful case of 49-year-old man with neck trauma presenting undesired bleeding after probing of wound, and then discuss about treatment guildeline for neck trauma with a review.

Application of a Silicone Sheet in Negative-Pressure Wound Therapy to Treat an Abdominal Wall Defect after Necrotizing Fasciitis

  • Necrotizing fasciitis (NF) is an aggressive soft-tissue infection involving the deep fascia and is characterized by extensive deterioration of the surrounding tissue. Immediate diagnosis and intensive treatment, including debridement and systemic antibiotics, represent the most important factors influencing the survival of NF patients. In this report, we present a case of NF in the abdomen due to an infection caused by a perforated small bowel after abdominal liposuction. It was successfully treated using negative-pressure wound therapy, in which a silicone sheet functioned as a barrier between the sponge and internal organs to protect the small bowel.

Limited Incisional Drainage and Negative-Pressure Wound Therapy in an Acute Morel-Lavallée Lesion

  • A Morel-Lavallée lesion is a post-traumatic closed degloving soft tissue injury after blunt trauma. Infection and skin necrosis frequently occur if it is not treated properly in the early stages. However, there is no clearly established treatment algorithm. In the acute stage, it is mainly treated with aspiration, simple compression, and incisional drainage. In the chronic stage, sclerotherapy is usually performed. If skin necrosis develops, the necrotic tissue is resected and a skin graft is needed. We describe a case of acute Morel-Lavallée lesion in the buttock region that was treated with limited incisional drainage and negative-pressure wound therapy, and also present a review of the literature.
