Association Between Initiation of Rehabilitation and Length of Hospital Stay for Workers with Moderate to Severe Work-Related Traumatic Brain Injury

  • Background: In workers with moderate to severe work-related traumatic brain injury (wrTBI), this study aimed to investigate the effect of the timing of rehabilitation therapy initiation on the length of hospital stay and the factors that can influence this timing. Methods: We used data obtained from the Republic of Korea's nationwide Workers' Compensation Insurance. In the Republic of Korea, between the years 2010 and 2019, a total of 26,324 workers filed a claim for compensation for moderate to severe wrTBI. Multiple regression modeling was performed to compare the length of hospital stay according to the timing of rehabilitation therapy initiation following wrTBI. According to the timing of the initiation of rehabilitation therapy following TBI, the proportions of healthcare institutions that provided medical care during each admission step were compared. Results: The length of hospital stay for workers who started rehabilitation therapy within 90 days was significantly shorter than that for workers who started rehabilitationment were first admitted to tertiary hospitals. Approximately 39% of patients who received delayed rehabilitation treatment were first admitted to general hospitals, and 28.5% were first admitted to primary hospitals. Conclusions: Our findings demonstrate the importance of early rehabilitation initiation and that the type of healthcare institution that the patient is first admitted to after wrTBI may influence the timing of rehabilitation initiation. The results of this study also emphasize the need to establish a Worker's Compensation Insuranceespecialized rehabilitation healthcare delivery system.

작업관련성 손상·질병 발생과 손실비용에 따른 예방정책 우선순위 연구 (An Analysis of Policy Priority for Work-Related Injury & illness and Investment Losses of Workplace Safety)

  • As workplace incidents has been being declining in Korea, there is criticism of the effectiveness of occupational safety policy implementation. It is unknown that which policy target group needs to be targeted to yield effective injuries prevention. The purpose of this paper is to analyze and reveal the policy intervention group with a high priority in terms of industrial incident prevention and the related investment cost. A Policy Priority Model(PPM) is composed of 6 indicators regarding influences of both the incident reduction and the cost reduction. Z-score analyses are used to confirm the high policy priority area or policy target group. Overall, workplace with worker below 50 persons, construction site with the sales of more than a hundred million won, workplace with relatively small percentage of female employees and relatively higher percentage of older worker should be prioritized to reduce workplace injuries. This paper provides an analytic way that can be used to decide the policy priority workplace in order not only to reduce work-related injuries&illnesses and the related investment cost but to further lessen the related societal costs.

여성근로자의 노동시장 및 작업환경 특성 연구 - 성인지적 비교분석으로 통해 본 - (A Study on Characteristics of Labor Force Participation Rate and Work Environment of Female Workers)

  • The purpose of this study is to review the labor force partitpation rate and work environment characteristics of female workers to provide basic information for establishment and implementation of effective policies related to accident prevention and workers health protection for female workers. It was analyzed employment status, work environment and injuries and illnesses based on economic activity census results, compensation of industrial injury and the second working condition survey. According to economic activity census results, female labor force participation rate has been steadily increased, whereas male labor force participation rate has been decreased since 1970. Industrial accident rate has been declined in male workers but that in female workers has been steadily increased even though male workers were higher accident rate than female workers. It was evaluated that female workers are vulnerable to industrial injuries and illnesses in the aspect of their working environment and employment status. Also, Substantial differences between female and male workers in occupational exposure patterns, occupational disease and occupational environments were observed. Therefore it was recommended that special care programs for female workers such as a specialized monitoring and management program should be introduced in the near future.

Manual Handling in Aged Care: Impact of Environment-related Interventions on Mobility

  • The manual handling of people (MHP) is known to be associated with high incidence of musculoskeletal disorders for aged care staff. Environment-related MHP interventions, such as appropriate seated heights to aid sit-to-stand transfers, can reduce staff injury while improving the patient's mobility. Promoting patient mobility within the manual handling interaction is an endorsed MHP risk control intervention strategy. This article provides a narrative review of the types of MHP environmental controls that can improve mobility, as well as the extent to which these environmental controls are considered in MHP risk management and assessment tools. Although a range of possible environmental interventions exist, current tools only consider these in a limited manner. Development of an assessment tool that more comprehensively covers environmental strategies in MHP risk management could help reduce staff injury and improve resident mobility through auditing existing practices and guiding the design of new and refurbished aged care facilities.

탈북여성들이 경험하는 도덕적 손상에 관한 탐색적 연구: 탈북여성을 대상으로 한 질적 선행연구물 내용분석 (Exploratory study on the Moral Injury among Female North Korean Refugees: Qualitative Content Analysis of Qualitative Studies on Female North Korean Refugees)

  • 본 연구는 심리적 트라우마를 경험한 후 일상에 적응하지 못하는 현상에 대한 비정신병리적 접근으로의 도덕적 손상이라는 개념을 탈북여성의 경험을 재해석하고 이해하는데 적용시키고자 실시되었다. 이를 위해 데이터베이스 KISS와 DBpia를 이용하여 2017년 8월 13일까지 국내 학술지에 출판된 질적 연구물 중 '탈북여성' '새터민 여성' '북한이탈 여성'이라는 주제어를 포함하는 연구물을 검색 했다. 최종 선정된 총 51개의 질적 연구물의 내용분석을 실시한 결과는 다음과 같다. 첫째, 연구주제로는 적응경험, 어머니로서의 경험, 북한 체제와 전쟁에 대한 경험, 일/직업/고용 경험, 정신적인 충격에 대한 극복 및 성장과 회복탄력성의 경험, 중국에서의 삶에 관한 경험, 트라우마나 심리적인 경험, 심리적 불안함을 치료하는 과정과 효과에 대한 순으로 많이 나타났다. 둘째, 도덕적 손상으로 인해 나타날 수 있는 부정적 감정에 관한 단어는 총 260회, 존재론적 믿음에 미치는 영향과 관련된 단어는 총 82회, 인지의 변화와 관련 단어들이 총 71회, 그리고 행동의 변화와 관련 표현이 총 21회 나타났다. 셋째, 도덕적 손상의 현상을 일으키는 맥락으로 크게 '정체성 부정'과 '가족 해체'가 드러났다. 그리고 도덕적 손상을 일으킬 만한 맥락을 경험했음에도 불구하고 도덕적 손상의 흔적이 발견되지 않았던 정 반대되는 사례들 속에서 '정체성 재구성'이라는 맥락을 공통적으로 발견했다. 이러한 연구결과를 통해 탈북여성이 도덕적 손상으로부터 예방 되는, 혹은 도덕적 손상으로부터의 회복에 필요한 실천적 함의를 제언하였다.

소규모 섬유업종의 산업재해 특성 및 예방 (Characteristics and Prevention of Occupational Accidents in the Small-Sized Textile Industry)

  • In this study 1,079 occupational accident reports were used to examine the characteristics and causes of occupational injuries in the small-sized textile companies with less than 50 employees. These data were analyzed in terms of age of injured person, work experience, accident type, injury type, and agency of accident. The results show that there are some patterns: (1) injuries occur more frequently in the 40~49yr age group; (2) about half of all accident occurred during the first year of employment; (3) there is a higher percentage of sick people leaving in the 29~90day range; (4) 'caught in and between objects' represents the leading accident type; (5) the most common type of incidence is related to the machinery; (6) 'lower back injuries' is the leading type of occupational disorder. These results can be used to develop more effective accidental occupational injury prevention programs for small-sized textile industries.

Work-related Musculoskeletal Disorders among Agricultural Workers

  • This paper describes the current status of musculoskeletal disorders(MSDs) and its ergonomic risk factors, including the preventive intervention in terms of the working conditions and health promotion among agricultural workers. Korean agricultural workers have been exposed to the poor working condition. They have suffered from a variety of work-related injury and diseases more than the general population or the other occupational group. MSDs are one of the most prevalent problems threatening the agricultural worker's health and quality of life. Low back pain, osteoarthritis of hip and knee joint and carpal tunnel syndrome are well known MSDs that was found easily and frequently among agricultural workers. MSDs are worsened by poor ergonomic working conditions such as heavy weight lifting, repetitive movement and awkward posture which involves squatting with long sitting work on the ground, long working hours and heavy workload which are inherent in the greater part of agricultural works. Even though Korean agricultural workers are very weak population in terms of occupational safety and health, no many studies were conducted in the past and no many social and institutional supports were provided sufficiently. We need more interests and efforts in agricultural fields to improve its working condition and to promote health status of agricultural workers.

The Efficacy of Scalene Injection in Thoracic Outlet Syndrome

  • Objective : To evaluate the efficacy of scalene injection in patients with thoracic outlet syndrome. Methods : We selected 142 patients diagnosed with thoracic outlet syndrome between January 2005 and October 2009. We performed a series of scalene injection with conservative treatment in all cases. Patients rated their pain degrees using a visual analogue scale. We also evaluated the time to return to everyday life and work, and patients' functional capacity. Results : There were no complications or instances of inadvertent somatic or sympathetic ganglionic blockade after scalene injection. Overall, 111 patients (76.5%) experienced improved symptoms after the first set of scalene injection and 128 patients (88.2%) improved after scalene injection followed by conservative treatment. Of the 68 patients who returned to work during the study period, 54 returned within 1 week, and 62 within 2 weeks. Of those who returned to work, 61 reported nearly full functional capacity. We found that scalene injection was more effective in cases of thoracic outlet syndrome related to trauma than in those related to work-related repetitive stress. Conclusion : In patients with thoracic outlet syndrome, scalene injection effectively reduces pain. We recommend scalene injection as an adjunct to conservative treatment.

The effect of neuropathic pain on quality of life, depression levels, and sleep quality in patients with combat-related extremity injuries

  • Purpose: There is limited research on the effects of neuropathic pain (NP) on quality of life, depression levels, and sleep quality in patients with combat-related extremity injuries. This study evaluated whether patients with combat-related extremity injuries with and without NP had differences in quality of life, sleep quality, and depression levels. Methods: A total of 98 patients with combat-related extremity injuries, 52 with NP and 46 without, were included in this cross-sectional study. The presence of NP was determined using the Leeds Assessment of Neuropathic Symptoms and Signs questionnaire. The outcome measures were a visual analogue scale (VAS), the 36-Item Short Form Survey, the Beck Depression Inventory, and the Pittsburgh Sleep Quality Index (PSQI). Results: The VAS subparameter scores for pain (all P<0.05), PSQI sleep dur ation subscale scores (P=0.025), PSQI sleep disturbance subscale scores (P=0.016), and PSQI total scores (P=0.020) were significantly higher in patients with NP than those without. Logistic regression analysis showed that VAS scores of 5 and above for average pain during the previous 4 weeks contributed independently to the prediction of NP. Conclusions: Patients with combat-related extremity injuries with NP had more pain and poorer sleep quality than those without NP. Sleep quality should be evaluated as part of the diagnostic work-up in patients with combat-related extremity injury with NP, and interventions to improve sleep quality may help manage NP in this patient group.

초소규모 제조업 사업장의 안전보건활동과 정부지원사업 효과 (The Industrial Safety and Health Activities and Effectiveness of Government Support Program in Very Small Manufacturing Workplaces)

  • Last five years, Occupational injury rate is declining but still a standstill as 2%, if the rate of deaths all people from 2% to 1% in 2013, but decreased as, occupational safety and health in terms industrial accidents yet has a very weak structure in very small Manufacturing workplace. The purpose of this study was to survey a 1:1 directly person interviews with a structured questionnaire intended for 150 a very small manufacturing workplace. In the results, working with non-regular(informal) workers is one in 10 persons 11.7% and female workers is about three people on 10 persons with 31.0%, and that 75% of non-regular workers who are mainly engaged in the production line appeared. And the work-related injury and accident experience was 4.0% and the occupational injury rate was 1.24%, especially occupational injury rate of older workers was 3.65%, a female workers 1.72%. Workplace risk assessment carried out in response that it was very low as 9.3% of the total. In the technology and funding programs that are supported by the government, was highest with 62.7% of the clean support program awareness, awareness of the rest of the programs is about 10-14% lower. And the satisfaction of supporting a clean support program were the highest 81.3%.
