Current Treatment and Clinical Outcomes of Community Acquired Pneumonia According to Pneumonia Severity Index (Pneumonia Severity Index에 따른 원외획득폐렴 환자의 치료 현황 및 성과)

  • Purpose: There is considerable variability in rates of hospitalization for patients with community-acquired pneumonia (CAP) in part because of physicians' uncertainty in assessing the severity of illness at presentation. The purpose of the study was to examine the current treatment patterns and factors influencing the Pneumonia Severity Index (PSI) and clinical outcomes in the patient with CAP. Method: The retrospective data collection of the patients with CAP was conducted and the data were reviewed. The collected data included demographic, clinical, laboratory and microbiological medical information. All patients were stratified into three risk groups according to PSI: low risk (PSI score I-II), moderate risk (III) and high risk (IV-V) groups. The examined treatment patterns were the appropriateness of admissions, category of antibiotics used. The prognostic factors associated with PSI and clinical outcomes were examined. Results: One hundred and six patients' medical data were reviewed. The overall appropriateness of admissions was low presenting many of patients were admitted or intensely treated in the hospital despite of lower risk of prognosis and treated with intravenous antibiotics instead of oral fluoroquinolones. Primary pneumonia pathogens were Klebsiella pneumoniae (27%) and Streptococcus pneumoniae (21.6%). Mean LOS was 8.5 days and was significantly longer (10.0days) (p<0.001) in high risk group. The patients with age >65 (p<0.001), diabetes mellitus (p<0.001), mental alteration (p<0.001), and/or $PaO_2$ <60 mmHg (p<0.001) had a tendency to have higher PSI. The prognostic factors associated with longer LOS were age >65 years (p=0.008), mental status alteration (p<0.001), dyspnea (p=0.002) and PSI score (p=0.001). The prognostic factors associated with mortality were congestive heart failure (p=0.038), systolic blood pressure <90 mmHg (p=0.002) and arterial pH <7.35 (p=0.013). Conclusion: Most of patients were found to over-utilize medical service according to appropriateness of admissions. The elderly, mentally altered patients with low $PaO_2$ had higher PSI score with increased risk of LOS. The mortality could be increased in the patient with disease state of congestive heart failure, high blood pressure, and/or acidosis.

Development and Validation of a Novel Generic Health-related Quality of Life Instrument With 20 Items (HINT-20)

  • Objectives: Few attempts have been made to develop a generic health-related quality of life (HRQoL) instrument and to examine its validity and reliability in Korea. We aimed to do this in our present study. Methods: After a literature review of existing generic HRQoL instruments, a focus group discussion, in-depth interviews, and expert consultations, we selected 30 tentative items for a new HRQoL measure. These items were evaluated by assessing their ceiling effects, difficulty, and redundancy in the first survey. To validate the HRQoL instrument that was developed, known-groups validity and convergent/discriminant validity were evaluated and its test-retest reliability was examined in the second survey. Results: Of the 30 items originally assessed for the HRQoL instrument, four were excluded due to high ceiling effects and six were removed due to redundancy. We ultimately developed a HRQoL instrument with a reduced number of 20 items, known as the Health-related Quality of Life Instrument with 20 items (HINT-20), incorporating physical, mental, social, and positive health dimensions. The results of the HINT-20 for known-groups validity were poorer in women, the elderly, and those with a low income. For convergent/discriminant validity, the correlation coefficients of items (except vitality) in the physical health dimension with the physical component summary of the Short Form 36 version 2 (SF-36v2) were generally higher than the correlations of those items with the mental component summary of the SF-36v2, and vice versa. Regarding test-retest reliability, the intraclass correlation coefficient of the total HINT-20 score was 0.813 (p<0.001). Conclusions: A novel generic HRQoL instrument, the HINT-20, was developed for the Korean general population and showed acceptable validity and reliability.

The Effect of Balance Task-Related Circuit Training on Chronic Stroke Patients (뇌졸중 환자의 균형 향상 과제 중심 순환 훈련의 효과)

  • Purpose: This study investigated the efficacy of task-related circuit training aimed at improving balance in individuals discharged from rehabilitation facilities following a stroke. Methods: We recruited 12 stroke patients (34-66 years of age) to participate in a task-related circuit training program. Baseline assessment included a history of stroke and an assessment using the Mini-Mental State Examination Korea (MMSE-K). After a baseline assessment, follow-up assessments were administered pre- and post-training. These included Berg Balance Scale (BBS), Functional Reach Test (FRT), and the Time Up & Go Test (TUG). Physiotherapists trained study subjects under the one-to-one supervision of students from the department of physical therapy. Circuit class study participants attended 90-minute treatment sessions, one day a week for 12 weeks (from September to December 2008). The program consisted of a light warm-up period (10 min), physical exercises for improving balance (20 min), tasks focused on improving balance (50 min) and a cool-down period (10 min). Results: Scores for the BBS assessment increased significantly (from 43.2 to 49.7) after the training (p<0.05). Reach distance on the FRT increased substantially (from 27.7 cm to 47.0 cm), although the improvement was not significant (p>0.05). The average time on the TUG test decreased significantly (from 23.7 sec to 19.5 sec) after the training (p<0.05). Conclusion: The task-related circuit training program improved the balance and mobility of subjects, indicating that such a group program is useful for stroke patients who are discharged from the hospital. More such task-related programs set in a community environment should be developed.

A study on the negative factors reflected in the will and the factors of well-aging as an alternative (유서에 반영된 부정적 요인과 대안으로서의 웰에이징 요소 연구)

  • The purpose of this article is to study the negative factors reflected in the will and the factors of well aging as an alternative. The survey data was 36 wills published in the media such as newspapers and broadcasting between 2008 and 2020. As a result, various aspects of negative factors were found in youth wills between the ages of 13 and 34. In middle-aged wills between the ages of 35 and 49, female was not found. The negative factors in the socio-economic aspects were remarkable in the wills of adulthood between the ages of 50 and 64. All the writers of wills over the age of 65 were women, and their writings were strongly linked to the spiritual side. In view of these results, the will explored in this study can paradoxically become a proposal for a complete life. The will is a record with the potential of well aging. Sources of the suicide note included daily newspaper, broadcasting and local media. This study analyse the age and gender and the negative factors reflected in the will, by using the physical aspect, the mental aspect, and the socio-economic aspect as the methodology. In addition, the frequency of words and expressions exposed in the will were analyzed and keywords were created in word cloud.

Sleep Patterns of Pregnant Women (임부의 수면양상)

  • Objectives : The change of sleep patterns commonly occurs in association with the pregnancy. This study was to investigate sleep habits during the course of normal pregnancy. Methods : Sleep habits questionnaire was administered to healthy women in their first trimester(TR1) of pregnancy and then the same questionnaire was repeatedly administered during their second(TR2) and third(TR3) trimesters. The following aspects were assessed : patterns of night sleep, daytime status, sleep posture, reasons for sleep alteration, and the experience of any particular parasomnias, as well as sleep problem-related treatment or medication. Data analysis was based on 26 women who maintaind good health throughout their pregnancy and completed the questionnaire three times. Results : In comparisons between each trimester and non-pregnant state, total night sleep time, daytime tiredness, and sleepiness were significantly increased in all trimesters. Sleep latency was significantly decreased in TR1 and TR2, but not in TR3. In addition, refreshed feeling on waking the following day was significantly decreased and the number of awakenings during night sleep was significantly increased in TR3, but not in TR1 and TR2. In comparisons between trimesters, there was a significant increase in sleep latency, daytime sleepiness and the number of awakenings during night sleep and a significant decrease in refreshed feeling on waking the following day in TR3 compared to TR1 and TR2. Over the course of pregnancy, the rate of lateral position during sleep was gradually increased and all the pregnant women took the lateral sleeping posture in TR3. The major reasons for sleep pattern alteration were nausea, vomiting and heartburn in TR1, urinary frequency, fetal movement and ache in hips in TR2, and urinary frequency, fetal movement, cramp in legs and backache in TR3. Conclusion : These findings are expected to be useful for educating pregnant women about sleep hygiene. In future studies, the underlying factors and mechanisms regarding sleep patterns during pregnancy will need to be clarified.

Study for correlation between MMPI results and Sasang constitutions, in out-patients of Oriental Neuropsychiatry (내원 환자의 MMPI 군집분석과 사상체질과의 상관성 연구)

  • Objective : For investigating correlation among diagnostic types, MMPI results and Sasang constitutions, in out-patients of Oriental Neuropsychiatry. Method: We got the following result by comparing the out-patients' MMPI and QSCCII from 2000/12/1 to 2001/12/30 at Sanbon Hospital of Oriental Medicine, Wonkwang University. Result and Conclusion: 1. A specific character of population for study's objects was that female (128, 68.4%) was twice bigger that the male(59, 31.6%). And their education levels were 52 of college graduate (27.8%), 40 of high school graduate (21.4%), and 14 of middle school graduate (14 %). A distribution of the occupation, there are 62 of labors (33.2%), 62 of white collars (33.2%), 30 of sales (16%), 16 of students (8.6%). Their marrige statuses were 144 of married (77%), 31 of single (16.6%), 4 of divorced (2.1%), and 1 of bereavement (0.5%). 2. In the physical constitutions there were 70 of Soeumin(37.4%), 56 of Soyangin(29.9%) 55 of Taeumin (29.4%), 6 of others (3.2%). A differentiation with their diagnosis, there were 55 of palpitations (29.4%), 45 of depression (24.1%), 18 of women's diseases (9.6%), 14 of insomnia (7.5%), 13 of headache (7%), 8 of pain (4.3%), 8 of hwabyung (4.3%), 7 of psychosis (3.7%), 4 of dizziness (2.1%), and 15 of others(8%). 3. A result of groupling MMPI, we could classify into 4 different groups. Group 1 is a profile of trasition in a neurosis. Group 2 is a proflie of a mental disease. Group 3 is a proflie of $'{\wedge}'$ type neurosis. And group 4 has normal profile. 4. Regarding each group with physical constitution and a distribution of diagnosis, Group 1 is found out 36.4 % of Soeumin, 34.10 % of palpitations. And group 2 showed 43.6% of Soyangin, 25.6% of depression. Group 3 showed 46.4% of Soyangin, 31.90% of palpitations. Group 4 showed the most in 34.3% of Soyangin, and 31.40% of palpitations 5. A result of the study using Chi-square test is that there is no correlation between the 4 different groups and the Sasang physical constitution.

The Effects of Computer-Based Cognitive Rehabilitation Program(CoTras) for Visual Perception and ADL in Stroke (한국형 전산화 인지재활프로그램(CoTras)이 뇌졸중 환자의 시지각 기능 및 일상생활동작에 미치는 효과)

  • Objective : The purpose of study was to verify the clinical effect of a Korean Computer-based cognitive rehabilitation program(called CoTras) for recovering the visual perception function and ADL in stroke. Methods : A CBCRT was applied to 14 Stoke patients who rehabilitation professional medical treatment hospital. All participant were evaluated with four standardized assessment tolls(Motor-Free Visual Perception Test; MVPT, Korean version of Mini-Mental State Examination; MMSE-K, Assesment of Motor and Process Skills: AMPS) before and after the planned computer based cognitive rehabilitation sessions. Results : A significant effect was confirmed (p<.05) from the CBCRT which visual perception function. By each entry comparative result, visual memory, figure ground, visual close, spatial relation, visual discrimination, were the order of treatment. Neither was found any significant effect in improving process skills from AMPS. Conclusion : These results indicate that CoTras have effects on improving visual perception and ADL performance in stroke patients. Will be able to present with the fundamental data CoTras will be able to contribute to increase visual perception function & ADL performance to the stroke patient who has visual perception dysfunction.

Research on Change of Heart Rate Variability and Psychological Scale by Sasang Constitution according to before and after of the Meditation Programs (α version) (명상프로그램(α version) 시행 전후의 사상체질별 심리척도 및 HRV 변화 연구)

  • Objectives: In this study, the meditation programs (${\alpha}$ version), which are properly coordinated according to the motion, breathing, and relaxation, are evaluated and researched upon to have positive effects on stress and in the area of psychology. Methods: Approved by the Clinical Trials Deliberation Committee in Oriental Medicine, Dongguk University, Ilsan Hospital, this study collected data according to the applicant's consents, demographic information and anthropometry for the Sasang Constitutional diagnosis. Sasang Constitutional diagnosis measured the beta tools by Institute of Oriental Medicine and a decision tree was made for the Sasang Constitutional questionnaires. The STAI, STAXI, BDI, and HRV were measured before and after the meditation in order to compare the effects of meditation according to Sasang Constitution. The HRV was used as a ProComP KM Tech (co). Results: 1) The positive changes available in the Time-domain analysis of heart rate variability assessment showed that the peace of mind is increased. By analyzing the Sasang constitution, So-eum In's peace of mind included a physical stability of the autonomic nervous system. 2) According to the psychological scale evaluation, each depression scale, trait anger, anger-in, state anxiety and trait anxiety index proved significantly positive effects. By analyzing the Sasang constitution, Eun-In which involved So-eum In and Tae-eum In, had positive effects. 3) The psychological scale changed the group of diagnosed depression or anxiety, it did not mean that the psychological scale changes in the depression group, but the index of the anxiety group had been significantly reduced. This program had clinical effects for anxious patients and Eum-In which involved Tae-eum In and So-eum In according to the analysis of Sasang constitution. 4) Correlations between the gender of each psychological scale showed that women have overall low correlations, but, there were no significant changes. Conclusions: The meditation program developed by adequately mixing Action, relaxation and breathing shows that it is effective for overall Eum-in physical and mental relaxation and concentration. In the future, It will have to be developed Meditation program to show the same effect for all people.

The Survey on the Drug use status, Knowledge and Attitude of College Students (Seoul and Northern Part of the Kyunggi-do) (대학생의 약물 사용 실태.지식 및 태도(서울 및 경기 북부지역))

  • The Purpose of this study was to grasp the status of drug uses in college students, and to investigate the knowledge and attitude when they use it. We collected through questionnaires from Sep. 1 to Oct. 30 in 1999. The subjects were 490 college students in Seoul and northern of the Kyunggi-do. The data analysis was performed using SPSS (version 7.0) and ANOVA. The results of this study were as follows. 1. There was no correlation for all factors in the relationship between general characteristics of subjects and knowledge for drugs. On the contrary, in the relation of the general characteristics and attitude for drug of subjects, some factors are statistically significant e.g., department (F=3.049, p<.010), satisfaction for school life (F=6.167,p<.002), body shape(F=8.092, p<.000), and the relationship with ones parents (F=3.728, p<.005). 2. In the analysis of drug use status and knowledge, only in the factor of administration control was statistically significant(F=4.304, p<.014) and in the relation of attitude for drug uses, drug administration was statistically significant (F=4.521, p<.004). 3. In the mean scores for the drug knowledge analysis, the highest factor was 'A narcotic make deformed persons through poisoning of physical and mental' (M=4.14), the lowest factor was 'If catch the flu during the pregnancy, should be take drug as possible as quickly to reduce negative effect for fetal' (M=1.94). 4. In the analysis for drug attitude, the highest factor was 'A drug is alike a poison' (M=3.48), 'Should be keep the usage and dosage of drug' was the lowest (M=1.48). 5. From the investigation for status of drug use, it was revealed that the most subjects (73.6%) were purchase drugs after explain their symptoms to pharmacist. And they take drugs only when they felt painful in 43.1%. The most students (70.4%) were experienced control of drug administration. It was inquired that subjects were mainly obtained information about drugs from pharmacist and television (or radio) advertisement, 33.5% and 33.1%, respectively. In the examination for existence of long-term administrators in their family, 'none' and 'only parents' are 49.6% and 37.3%, respectively. When their parents have illness, the persons go to drugstore and hospital for heath-care, 47.8% and 44.3%, respectively. On the basis of results of this study, we suggest as follows. 1. This study was analyzed data from questionnaires for college students in a part of local areas, so we suggest that the next research should be perform for national-wide students as subjects to generalize the results. 2. It is need more intensive research methodologies such as interview and observation. 3. Additional research is required for knowledge and behaviors of drug uses that will how impact on ones health behavior.

A Validation of The Korean Version of Eating Attitude Test-26 (한국판 식사태도검사-26(The Eating Attitude Test-26 : KEAT-26) 의 타당화)

  • This study was attempted to investigate the discriminant validity of Korean version of Eating Attitude Test-26(KEAT-26) and to provide the sensitivity, specificity and efficiency according to cutting score, which may be useful to determine the optimal cutoff point on various purposes. The KEAT-26 was administered to 108 female patients with eating disorders, 179 female participants in body slimming center, 120 female athletic college students, 227 female college students, and 183 healthy normal women. Validity was tested by ANOVA and ROC curve analysis. The results revealed that the total score of the KEAT-26 showed a statistically significance between groups and that the score of the KEAT-26 of eating disorders group was significantly higher than that of the other groups in post hoc test. In comparison of the 4 subfactor score of the KEAT-26 between groups, significant differences in main effect within groups were found in all subfactors except factor IV. ROC curve analysis showed 80% of efficiency to discriminate eating disorders group from normal control group using cutoff score on maximum discriminant efficiency and 69% of efficiency to discriminate eating disorders group from high risk groups for eating disorders. Each cutoff score on maximum in efficiency was as follows ; 25 between eating disorders group and participants in body slimming center, 19 between eating disorders group and healthy normal woman, 23 between eating disorders group and athletic college students, 21 between eating disorders group and college students. Using 22(T score 65) of the KEAT-26 as the cutoff score, sensitivity was 54%, specificity was 84%, and overall efficiency was 80%. These results indicate that the KEAT-26 has a good discriminant validity in Korean population and also suggest that the KEAT-26 may be useful assessment tool to screen the disordered eating problems on clinical and epidemiological purposes.
