Review on Cases Applying Hwangryunhaedok-tang External Agent in Korea (황련해독탕(黃連解毒湯) 외용제를 적용한 국내 치험례 고찰)

  • Objectives : The purpose of this review study is to identify clinical indications of Hwangryunhaedok-tang(HHT) external application and to suggest directions for future research and treatment by analyzing domestic cases applying HHT external treatment except for pharmacopuncture research. Methods : We searched 235 studies published in Korean since 2000 from four domestic databases: Oriental Medicine Advanced Searching Integrated System(OASIS), Research Information Sharing Service(RISS), Korean Traditional Knowledge Portal(KTKP), and Korean studies Information Service Systems(KISS) using keywords such as 'HTT and external use', 'HHT and external agent' 'HHT and wet dressing', or 'HHT and case'. We excluded duplicate papers, papers related to acupuncture including pharmacopuncture, or papers not related to the topic. Results : We selected and analyzed seven case reports. HHT external agent was used alone in one case, as main treatment in two cases, and as adjuvant treatment in four cases. HHT external application was applied to various skin diseases accompanied by inflammation mainly in the form of wet dressings. Conclusion : HHT external agent may be effective on various skin diseases accompanied by inflammation when applied alone or in combination with other agents according to the treatment stage. Based on this results, further studies will be needed to establish the primary clinical indication of HHT external agent.

The Effects of the Ulmus Root-bark Dressing in Tissue Regeneration of Induced Pressure Ulcers in Rats (느릅나무 근피 드레싱이 흰쥐에 유발된 욕창의 조직재생에 미치는 효과)

  • Purpose: The purpose of this study was to examine the effects of the ulmus root-bark dressing on tissue regeneration in experimentally-induced pressure ulcers in rats. Method: A randomized pretest/post-test control group time-series study design was used. Thirty-three male Sprague-Dawley rats were used in this study. The rats were anesthetized with 100mg/kg of ketamine. Pressure ulcers were induced at 140mmHg for three hours using a personally-designed pressing apparatus. For four weeks, the ulmus root-bark dressing was applied every other day in the experimental group (n=18) and a wet gauze dressing in the control group (n=15). For data analysis, the statistical program SPSS WIN 12 was used. The wounds were examined by light microscopy andelectron microscopy. Result: There were significant statistical differences in the size of the pressure ulcers as time went by(p=0.006). It should be noted that there were no significant statistical differences in the number of capillaries. Using light microscopy the inflammatory infiltration and neovascularization in the dermis in the experimental group emerged densely in the early stages, but recovered rapidly at the latter stages. In addition, the reepithelization of the epidermis occurred earlier than in the control group. By electron microscopy, the cell organelles of the capillary endothelial cells and the basal lamina of capillaries in the experimental group showed a more rapid maturation during the latter stages, compared with the control group. Conclusion: According to this study, it can be concluded that the ulmus root-bark dressing is effective regarding the healing of pressure ulcers.

  • A field experiment was conducted to investigate changes of $NH_4-N$, N uptake, N use efficiency and rice yield by the Latex coated urea(LCU) application on wet-seeding rice from 1994 to 1997. Nitrogen deficiency symptom appeared when LCU was applied as basal dressing continuously until the ripening stage during normal temperature year, but was completed at heading stage during year at high temperature. Percentage recovery of fertilizer N was higher in LCU than in urea but decreased in high temperature. Turn over of N applied as LCU to grain increased more during years high temperature than the normal year. Rice yield with LCU treatment compared to conventional treatment was similar during the normal year but decreased by 6% during year of high temperature. Top dressing of urea at panicle initiation stage in addition to basal dressing of LCU increased rice yield to 12%.

Three cases of acne (좌창 치험 3례)

  • Acne, which intrucdes the pilosebaceous unit, is a chronic inflammatory disease. The symptom of comedones, papules, pustules, cysts, nodules and scar mainly raises on the face, upper back, shoulder and neck. The cause of acne is the secreting growth of sebum, abnormal follicular keratiniziation, the form of group of propionibacterium acnes, and the respones of inflammation etc. But the original mechanism of the cytobiologic level is exactly not known yet. We treated three patients suffered from acne with acupuncture, venesection, Chungsangbangpung-tang, wet dressing with Gamihwangryunhaedok-tang. After treatments we observed the improvement. Based on this study, it is considered that acupuncture, venesection, Chungsangbangpung-tang wet drssing with Gamihwangryunhaedok-tang can be applied to the management of acne.

  • Houttuynia cordata Thunb. grown in wet earth in China, Japan and Korea, has often been cited in medical literature for its medicinal effects. In this paper, the pharmaceutical effects of Houttuynia cordata Thunb., featured in the East Asian literature were studied. It was revealed that it has a wide variety of uses, including remedial treat-ments for pneumonia, antidote, inflammation, syphilis, abscess, paralysis and gynecological diseases. Its application methods also are numerous: drinking as tea or squeezed juice, application to wounds, wet dressing, chewing raw roots and mixing with other plants, liquor and food materials. The major pharmaceutical effects are as follows: antibiosis, immunity enhancement, urination, pain relieving, vasodilator and cough lozenge.

Pain Control for Hydrofluoric Acid Chemical Burn Using EMLA® creams (EMLA® 연고를 이용한 불산 화학 화상의 통증 조절)

  • Purpose: Although Hydrofluoric acid(HF) is not a strong acid when compared to other hydrogen halides, it is a feared corrosive and is particularly dangerous at higher concentrations. HF burns are characterized by symptoms, often delayed and localized with diluted HF solutions, to include erythema, edema and severe pain. Pain, a well known symptom following exposure to calcium binding. And, EMLA$^{(R)}$ cream is a topical formulation based upon the eutectic mixture of lidocaine and prilocaine and is used in clinical settings to provide pain relief undergoing superficial surgical procedures. The aim of this study is to evaluate effects of EMLA$^{(R)}$ cream, pain - control dressing on the treatment for HF injury wound. Methods: From June 2007 to June 2008, this study was carried out with 10 patients who had HF partial thickness burns. We were applied topically EMLA$^{(R)}$ cream to injured wound with vaseline gauze and 10% calcium gluconate wet gauze dressings. As a principle, in the emergency treatment, partial or complete removal of the bullae along with copious washing with normal saline was done, depending on the degree of HF invasion of the distal digital extremities. The effect of dressing was investigated by visual analogue pain scale. Results: We therefore reviewed 10 cases of HF - induced pain and pain relief treatment principle. The 10 cases who came to the hospital nearly immediately after the injury healed completely without sequelae and EMLA$^{(R)}$ related complications. Conclusion: Proper initial treatment of HF burns are important, if not promptly recognized and properly treated, for produce serious injury. Topical EMLA$^{(R)}$ cream remain a powerful, new advancement for minimizing HF - related pain during dressing procedures. When used appropriately, topical EMLA$^{(R)}$ cream can provide a safe and effective alternative to other forms of HF - pain control treatment.

  • Drug eruption refers to an adverse effect which is unintentionally given rise to by dosage, injection, inhalation, suppository and ointment, etc. Maculopapular, urticarial, morbilliform, papulosquamous, pustular, and bullous morphologies may be encountered. Adverse drug reactions also can cause pruritus or dysesthesia unaccompanied by rash. We treated a patient suffered from maculopapular rash and pruritus with acupuncture, herb medication and wet dressing. After treatment we observed the improvement. Based on this study, it is considered that oriental medical treatment can be applied to the management of drug eruption.

치료용 레이저를 이용한 대상포진 (Herpes Zoster)치료 19례에 대한 보고

  • 문헌에 의하면 대상포진의 원인은 바이러스에 대한 세포성 면역이 저하될 때, 또는 악성종양 등으로 면역억제제 치료를 받아서 2차적으로 면역이 저하될 때 체내에 잠재해 있던 바이러스가 재활되어 발병하는 것으로 추정하고 있다. 본 저자도 이에 대하여 의견을 같이하고 있는데 노령층의 $60\~70$대 여자환자수가 압도적으로 많았다는 점과, $20\~39$세 사이의 청${\cdot}$장년층에서는 단1명의 대상자가 없다는 점뿐 아니라, 본 대상자중 11세의 소녀는 7세때 뇌종양 수술을 2차례에 걸쳐 실시한 후 면역제제를 계속적으로 사용하는 있는 점 등은 문헌의 발병원인과 상당히 일치하였다. <전파 양식> 비말 감염 (droplet infection)으로 전파되며, 수두와 달리 전염성이 높지 않다. <잠복기> 잠복해 있던 바이러스가 언제 재활할지 알 수 없으므로 불명이다. 그러나 본조사에서는 대상포진에 대한 병력을 가진 사람이 없었고, 발생 원인을 본인 자신도 모르고 있었다. <합병증> Ramsay Hunt의 증후에 의하면 합병증은 외이도의 수포, 안면신경마비, 혀의 2/3부분의 미각상실, 간혹 청각 및 평행장애를 일으킨다고 한다. <치료> 대상포진의 대증요법으로 calamine lotion Burrow solution의 wet dressing 진통제 및 cytosine arabinoside나 adenosine arabinoside와 같은 항히스타인제, 최근에는 Acyclovir가 많이 사용되고 있는 것으로 알려졌다.

Case Study on Removing Lentigo Using a Cauterization (낙법(烙法)을 이용한 흑색점 제거 증례)

  • Objectives : This report is intended to suggest that lentigo can be removed using the cauterization. Methods : Handle of the needle, which is distal part to the tip and shaft, was heated with an alcohol lamp to cauterize the lentigo one by one to form a crust, and wet dressing was applied to the treatment sites by using Hwangryunhaedok-tang pharmacopuncture solution. Results : Ten days after the cauterization, the lentigo was cleared from the normal skin without adverse reactions. Conclusions : The cauterization is a useful technique for removing lentigo, but the conventional method is somewhat inconvenient, so a radiofrequency technique or a laser-based cauterization may be more useful Instead of the traditional cauterization.

Two Cases of Taeeum Person's Nipple Eczema Improved by Korean Medicine (태음인(太陰人) 유두 습진 환자 증례보고 2례)

  • Objectives: The purpose of this study is to report the effectiveness of Korean Medicine treatment on nipple eczema. Methods: Two patients were treated with herbal medicine, acupuncture, wet dressing with herbal medicine and pharmaco-acupuncture. Photographs of lesions were taken to evaluate the changes in symptoms. Results: Two patients' pruritus, discharge, crusting, excoriation, erythema, and condition of lesions were improved after treatment. Conclusions: This study shows that Korean Medicine was effective to treat nipple eczema.
