The New Health Promotion Strategy in Japan-focusing on life-style related diseases (일본의 건강증진 정책의 방향 -생활습관병 대책을 중심으로-)

  • The prevention of life-style related diseases is an increasingly important issue in Japan, because not only have the number of patients with life-style related diseases increased but also medical care costs. This paper gives recent strategies for the prevention of cardiovascular diseases through life-style modification. Health objectives for the year 2010, called "Healthy Japan 21", were established in 2000 by the Ministry of Health, Labour and Welfare and the Health Promotion Act was enacted in 2002 to promote this health policy. However, the prevention efforts for life-style related diseases have not been effective in regard to the evaluation of the strategy objectives. The reform of the medical care system which included a new nationwide prevention strategy for life-style related diseases was presented in 2006. The new strategy starting from April 2008 included a "specific health checkup" and "specific health education" for those with metabolic syndrome. The specific health checkup is used to screen people according to criteria of the metabolic syndrome and divide them into 3 groups. These groups will receive specific health education. The purpose of this strategy is the early detection of those who have cardiovascular risk factors, and the early management of the clustering of cardiovascular risk factors of obese people aged 40-74 years old. It is mandatory for every insurer to conduct a specific health checkup and specific health education under the new Act. The implementation rate of the specific health checkup and the specific health education, and a reduction rate of individuals with metabolic syndrome among insured people will be evaluated every year. The national objective is to increase the rate of those undergoing the specific health checkup to 80% and the rate of those receiving the specific health education to 60% by the year 2015. The national objective also targeted a reduction rate of 25% for those with metabolic syndrome. This new strategy will be the biggest intervention trial in the world, and it will produce a big health care market in Japan. Not only public administrative institutions but also private institutions are now preparing to take part in this new strategy. However, various tasks remain, such as training more professionals in health education, developing more evidence based practices, and encouraging cooperation with various sectors, to enforce this new strategy.

Determinants of Pastors' Economic Preparations for Old Age (목회자의 경제적 노후 준비 결정요인)

  • This study is intended to figure out determinants affecting the economic preparations for old age of pastors from major four protestant associations having a church ministerial pension system: the General Assembly of Presbyterian Church in Korea (GAPCK), the Presbyterian Church of Korea (PCK), the Presbyterian Church in the Republic of Korea (PROK), and the Korean Methodist Church (KMC). Thereby, it conducted a survey to 452 pastors from the four religious associations in Korea. The followings sum up the results of this research. First, it was shown that the conventional idea that there is no need for pastors to prepare for their old age or their preparations for old age imply disbelief had been diminished. Second, there were few pastors who had made economic preparations for their old age privately. Third, associations adopting a compulsory church ministerial pension system were PCK, PROK, and KMC indicating a high occupation of more than 75% whereas GAPCK with an optional system showed a very low percentage of 11.5%. Fourth, with regard to the national pension system, the associations except for PROK (61.8%) had more non-enrolled pastors (53.3%), and they responded economic difficulty as the biggest reason for the unenrollment. Fourth, determinants affecting their economic preparations for old age were academic career, total income, church independency, church reserving, and national pension enrollment. Fifth, among the pastors from the four associations, the ones of PROK prepared for their old age best in consideration of GAPCK. According to the results of this paper, the most critical determinant affecting pastors' economic preparations for old age was total income. Pastors with higher income can make church ministerial pension, national pension, and private preparations while the ones with lower income cannot afford for public as well as private preparations; that is, there exists a polarization phenomenon in pastors' economic preparations for old age. Therefore, it is necessary to make devices to narrow the income gap between pastors in religious associations. Second, even in the associations having a compulsory church ministerial pension system, many pastors were not insured, so it is needed to reform or improve the pension system. Third, it is also required to better the national pension system and change the recognition on it.

Evolution of the National Pension Scheme in Korea: Uniqueness and Sustainability of the Korean Model (국민연금제도 전개의 한국적 특징과 지속가능성)

  • The goal of this paper is to define the distinguishing characteristics of Korea's National Pension Scheme compared to the National Pension types of other countries and sees if those characteristics are significant enough in order to warrant calling these the "Korean Model". Also, another point to consider is, if this "Korean Model" does indeed exist, whether it is a 'sustainable' model or not. The National Pension Scheme, which was implemented in 1988, is similar to the public pension system formerly used in Japan. The National Pension Scheme broke away from this 'Japanese Model' in 1995 with implementation of the Farmers and Fishermen Pension, and the unique "Korean Model National Pension" was completed in 1998 with revision of the National Pension Law. The characteristics of the Korean National Pension can be defined as being balanced equally on ability and equality, possessing strong intergenerational income redistribution, having a nationally integrated structure, an incomplete funded method financial neutralism of the government and also as being a Monroe-oriented pension system. There are several limits to the sustainable development of this Korean Model National Pension, though. Even though the precondition of "the income determination problem of self-employed persons", which has strong intra-generational income redistribution. in actuality there are still many policy issues to be confronted such as the structure which 'transfers the burden to the future generation', the 'inter-generational inequity' of the incomplete funded system, persons excluded from coverage under the national integrated structure, 'compulsory loaning of the public sector by the National Pension Fund' under the government's principle of finance neutralism, the separate existence of the 'Monroe-oriented National Pension' from other pensions, etc.,. Therefore, it need to reform of NPS once again to sustainable development of KMNP.

The Excluded from Public Pension : Problem, Cause and Policy Measures (공적연금의 사각지대 : 실태, 원인과 정책방안)

  • As National Pension Scheme for all nation complete in 1999 through expanding application in cities, the public pension including Public Occupational Pension became main axis of old-age income maintenance. After 4years since then, now, it is only half of total National Pension insured persons who have been qualified to receive pension through participate and contribution. The other half of National Pension insured is left the excluded from public pension. This paper is intended to identify scale and characteristics of the excluded from public pension and to analysis its cause, and to explore policy measures for solving the excluded's problem. for current recipients over 60 years old generation, the its excluded's scale is no less than 86% of the old over 60 years. The probability of getting in the excluded is high in case of old elderly and female for current elderly generation. For future recipients 18-59 years working generation, the its excluded's scale is no less than 61% of the 18-59 years total population. The probability of getting in the excluded is high in case of 18-29 years and female for current working generation. As logistic regression analysis determinant factor of paying or not pension contribution for future recipients, it appear that probability of getting in the excluded for current working generation is high in case of younger old, lower education attainment, irregular employee, working at agriculture forestry fishery sector, construction sector, wholesale retail trade restaurants hotels sector, financial institution and insurance real estate renting and leasing sector in comparison with manufacturing sector, occpaying at elementary occupation, professionals technicians and associate professionals, sale and service workers, plant machine operators and assemblers, legislators senior officials and managers in comparison with clerks. The Policy measures for the current recipient old generation have need to reinforce supplemental role of Senior's pension(non-contribution pension) until maturing of public pension, because of no having chance of public pension participants for them. And the Policy measures for the future recipient working generation have need to restructure social security fundamentally corresponding with social-economic change as labour market and family structure etc. The pension system has need to change from one earner one pension to one citizen one pension with citizenship rights. At this point, public pension have need to manage with combining insurance's contribution principle and citizenship principle financing by taxes. Then public pension will become substantially universal social network for old-age income maintenance and we can find real solution for the excluded from.

A Comparative Study of the Function of Nursing Management According to the Position of Nursing Department in Hospital Organization (병원조직내 간호부서의 위치에 따른 간호관리 기능 비교 연구)

  • These days our nation is standing on tiptoe of welfare nation. so hospital in authority have done reformation to provide quality medical services. This effective nursing work to provide quality nursing by keeping pace with the reformation of management of hospital for quality medical function might be well said to be depending on the degree of function of nursing management of nursing department. This essay has been aimed to prepare the data to build the position of nursing organization which can elevate the quality of nursing by clarifying the difference of the degree of the function of nursing management according to the position of nursing department in hospital organization. 135 nurses of 1 general hospital in Seoul which has independent nursing organization of the hospitals of over 600 beds and 155 nurses of 1 general hospital in Seoul which has the nursing organization under the management of medical department have been the objects of this study. The tool of Yoon. Young Ae(1988) has been used by having it amended and complemented and the degree of reliability of the tool was resulted in Cronbach's Alpha .9155. The collected data have been analysed by SPSS program as mean. frequency, chi-square. t-test. F-test(ANOVA) and the results are as the follows; 1. Compared result of general characteristics of the two groups by $x^2$-test showed statistically significant difference between religion ($x^2$=10.375, p=.015) and educational background($x^2$=51.222. p=.000) 2. The t-test aimed to compare the degree of function of nursing management according to the position of nursing department is as the below: Compared result of the degree of the function of nursing management according to the areas has shown higher point in independent type(M=3.22) than in the type under the management of medical department(M=2.85) in the personnel and administrative activities of nursing department. the standard and regulation of nursing duty also showed higher point in independent than in the type under the management of medical department (M=3.37) and the education of nursing showed higher point in independent type(M=3.53) than in the type under the management of medical department(M=3.19) and the evaluation of nursing quality has shown higher point in independent type(M=3.33) than in the type under the management of medical department(M=3.05), The area which showed the highest difference of the degree of the function of nursing management between the two organizations was in the activities of personnel and administration management of nursing department (independent type M=3.22. the type under the management of medical department(M=2.85). The degree of function of general nursing management showed higher in independent type nursing organization than in the type under the management of medical department by 3.41 in independent type and 3.11 in the type under the management of medical department. The items which showed the lowest degree of function of nursing management of both organizations have been suitable nursing man power and distribution and the reflection of the opinion of nursing department. In conclusion. the degree of function of nursing management to be able to provide quality nursing for the hospital goal of quality medical works showed visibly higher in independent nursing organization than in the nursing organization under the management of medical department. Therefore it is desirable for the hospital to operate the nursing department in hospital organization by independent type rather than the type under the management of medical department and the chief of nursing department of the nursing organization of the type under the management of medical department should make effort to reform its structure to be able to establish the position of performing independent nursing management. And also the chief of the nursing organization of independent type should endeavor to build substantial independent type organization more than ever under the viewpoint of securing suitable nursing man power and the low degree of management in the reflection of the opinion of nursing department.

Regulatory Reform Proposals for the Korean Deep Sea Fishing Industry (원양어업(遠洋漁業)에 대한 정부규제(政府規制)의 개선방안(改善方案))

  • The basic purpose behind the Korean government's policy toward the Korean deep sea fishing industry is to limit growth of the industry. Therefore, the regulations on the industry are generally restrictive and interventionist. The policy is intended to maintain high domestic fish prices in order to protect the domestic coastal fishing industry. Some regulations have also been introduced to maintain "industrial order." Each fishing vessel must obtain a government permit for operation. The permit specifies the kind of fish it can catch, the area of sea in which it can operate, and the port at which it can unload its catches. The number of permits government issues each year is based on the estimates of the demand increase calculated by government officials, and the government traditionally has been fairly conservative in its estimation, reflecting its concern for fish price stabilization, which actually implies a gradual increase of the prices. There is also a restriction on importing vessels from abroad. This regulation is intended to protect the domestic shipbuilding industry. However, this regulation has resulted in an unusually high average age of Korean fishing vessels, causing fishing costs to rise. These regulations and the inflexible response of the regulators to changing circumstances have resulted in many problems: i) high domestic fish prices, which are, to some extent deliberately, inflated to three or four times the level of international prices, resulting in huge consumer welfare losses; ii) over-exploitation of coastal fish resources; iii) provision of a hospitable environment for inefficient firms to survive, which is especially evident from the fact that, despite the high fish prices in Korea, most of the firms in the industry do not enjoy high profitability. It also must be pointed out that the actual beneficiaries of the high fish prices are the large operators, who are protected from competition and provide most of the fish for domestic consumption, rather than the low-income fishing households and small coastal operators whom the policy was originally designed to help. This study proposes a set of regulatory reforms and policy changes which could Promote competition and equity within the industry and allow firms to reduce costs and increase productivity. Such changes can make the industry more efficient and internationally competitive. Major proposals are, among others: minimization of bureaucratic discretion in issuing fishing permits and maintaining transparency in the governments' decision-making processes; reduction of the government permit specifications and simplification of the operational categories within the industry; and removal of the restrictions on importing foreign fishing vessels.

학교교육과정의 인구교육내용분석

  • This study focused on the anding of Inchon's identity The empirical method and ethnomethodological approaches were used to collect the data. Among members ofcitizen movement groups,government workers,and students who are living inInchon were selected as 613 samples using a purposive sampling method. MultipleClassification Analysis (MCA) and cross-tabulation methods were used in theanalysisThe study of identity in an area is important in terms of providing the solution ofthe problem in a region and social integration of the citizens. The scores of the indexabout Inchon's identity are quite low and more than half of the respondents to allthree groups showed the middle position of the scores from the identity index. By thecharacteristics of the respondents,female,unmarried single,30 years or more,lowerincome groups showed relatively higher identity index scores than other counter-parts . And professional,administrative,clerical workers'identity index scores werehigher than people who work at sales,service,and agricultural sectors. Respondentswith 2 years of college or more,with intentions to donate special monies for cultural, social welfare, environmental reform,persons who want to live in Inchon for along period of time equipped with a stronger identity index.For the character of Inchon's identity,there are no identity,making it fromnow on,capacity or broad-minded city,vanguard pioneer,displeased, Oiversity/multiplicity of the city,defense spirit from foreign invasion,entrancecity from the world in that order. Therefore,it is hard to say what exactly Inchon'simage is in a single word. However,Inchon can be characterized as a diverse citywith capacity to live together without any serious conflicts among citizens who come from Seoul,Kyunggi-Do,Chungchung-Do,Chunla-Do,Kyungsang-Do,and foreign countries including North Korea.
