The effects of composit grafts of allogenic decalcified freeze Dried bone and calcium sulfate on the healing of 1-wall intrabony defects in dogs (성견의 1면 골내낭에서 탈회 냉동 건조골과 calcium sulfate 혼합이식이 치주조직 치유에 미치는 영향)

  • The present study evaluates the effects of calcium sulfate and DFDB on alveolar bone regeneration and cementum formation and connective tissue adhesion in intrabony angulated 1 wall defects of dogs. Four millimeter-deep angulated one-wall intrabony defects were surgically created in the mesial & distal aspects of premolars and with flap operaion alone(control group), with calcium sulfate(experimental group 1), with composit graft of 50% calcium sulfate and 50% DFDB(experimental group 2), with DFDB alone(experimental group 3). Histologic analysis following 8 weeks of healing revealed the following results: 1. The lengths of connective tissue adhesion was $1.05{\pm}0.48mm$ in the control, $1.30{\pm}0.67mm$ in the test group I, $0.97{\pm}0.22mm$ in the test group II and $0.93{\pm}0.15mm$ in the test group III. There was no statistical significance between control and all experimental groups. 2. Changes in alveolar bone level was $0.97{\pm}0.27mm$ in the control group, $1.45{\pm}0.42mm$ in the test group I, $2.00{\pm}0.33mm$ in the test group II, $1.88{\pm}0.34mm$ in the test group III. There was no statistically significant difference between control and experimental group I. There was a statistically significant difference between the control and experimental group II,III.(p<0.05). There was no statistically significant difference between all experimental group. 3. Cementum formation was $1.13{\pm}0.17mm$ in the control, $1.78{\pm}0.31mm$ in the test group I, $2.17{\pm}0.38mm$ in the test group II, $2.15{\pm}0.47mm$ in the test group III with statistically significant differences between control group and all experimental group(P<0.05). There was no statistically significant differences between all experimental group. These results suggest that the use of composit graft of 50% calcium sulfate and 50% DFDB and DFDB alone in angulated 1 wall intrabony defects has little effects on connective tissue adhesion, but has significant effects on new bone and new cementum formations.

Surgery for Partial Anomalous Pulmonary Venous Connections: Modification of the Warden Procedure with a Right Atrial Appendage Flap

  • Background: Surgical repair of a partial anomalous pulmonary venous connection (PAPVC) to the superior vena cava (SVC) may be complicated by sinus node dysfunction or SVC obstruction. We modified the Warden procedure by using a right atrial auricular flap to decrease the occurrence of these complications. Methods: Between February 2005 and July 2012, 10 consecutive patients underwent a modified Warden procedure to correct PAPVC. The median patient age was 5.7 years. Eight patients (80%) had an atrial septal defect. To surgically correct the PAPVC, we made a U-shaped incision on the right atrial appendage and sutured the flap to the posterior wall of the SVC. The anterior wall was reconstructed with various patch materials. Results: No early or late deaths occurred, nor did any patient require early or late reoperation for SVC or pulmonary venous obstruction. No new arrhythmias appeared during follow-up, which was complete in all patients (mean, 29.5 months). Conclusion: Our modification of the Warden procedure seems to be effective and safe. This technique may lower the risk of SVC obstruction, pulmonary venous obstruction, and sinus dysfunction.

  • This study was carried out to investigate the effects of moisture content, temperature, C/N ratio and pH of the sewage sludge mixed with hulle and sawdusts for making compost under aerobic condition and to improve the defect of the structure of experimental equipment heat lose and handling method. and obtained results were as follows 1.The temperature was reached 73$^{\circ}$ C around 50 hours fermentation in the condition of 0.8 L/min. of air and 60.4% of moisture content. and favorable moisture content of initial condition ranged from 50 to 65% 2.The temperature near bottom of the batch composter was decreased due to evaporate water vapor and lose the heat produced during aeration. and it is required to be improved. 3.The temperature in the batch composter from the center to the inside wall surface was gradually decreased. the temperatures of the points located in r=9cm and the wall surface were 4$^{\circ}$ C and 6$^{\circ}$ C respectively. and therefore it is required to be insulated. 4. The maximum C02 production was obtained as 7.3% per volume in the temperature of 63$^{\circ}$C at the moisture content of 60% 5.The temperature range of active microbes growth was found to be as 20$^{\circ}$C to 40$^{\circ}$C in the case of mesophiles and 50$^{\circ}$C to 65$^{\circ}$C in the case of thermophiles due to increase and decrease C02 production. 6.C/N ratio after decomposition was 1.3 to 2.6 smaller than that of initial one due to increase the amount of nitrogen. The more C/N ratio increased. the less the reaction velocity decresed. The optimum of it as found to be 30. 7.pH values after decomposition were slightly increased than that of initial ones. The reaction velocity was decreased at acid and alkall condition. Therefore it is neseseary to neutralize the medium to improve the reaction.

Malignant Fibrous Histiocytoma Arising in Old Burn Scar on the Anterior Chest (흉벽의 오래된 화상 흉터에서 발행한 악성 섬유성 조직구종)

  • Purpose: Malignant changes of Marjolin's ulcer arising from chronic burn scar are rare. The majority of them are squamous cell carcinoma and basal cell carcinoma. Malignant fibrous histiocytoma is a deep seated pleomorphic sarcoma, which occurs principally as a mass of the extremities, abdominal cavity, or retroperitoneum in adults. Methods: We report a 58-year-old male patient who was admitted due to $3.5{\times}5cm$ chronic ulceration of anterior chest wall on the center of old burn scar. His scar had been occurred by boiling oil and treated with conservative treatment 45 years ago. Preoperative punch biopsy showed suspicious malignant changes and contrast enhanced chest CT showed well-defined, irregular shape enhancing lesion on anterior chest wall without intrathoracic metastasis. Results: The tumor was widely excised and defect was covered with skin graft without infection, necrosis and any other complication. The pathologic findings are compatible with malignant fibrous histiocytoma(storiform - pleomorphic type). The patient underwent 3 cycles of chemotheraphy. Although distant metastasis to the lung developed 6 months later and the patient died 9 month later, there was no local reoccurrence. Conclusion: Aggressive and early excision is needed because malignant fibrous histiocytoma has characteristics of high malignancy with a propensity for early and distant spread. Furthermore, the patient's education about disease entity and postoperative regular follow-up for local recurrence or metastasis is very important. To prevent malignancy from secondly healing burn scar, early skin graft is recommended for patients with deep second degree burn.

Rupture and Intrapleural Migration of a Cohesive Silicone Gel Implant after Augmentation Mammoplasty: A Case Report (코헤시브 실리콘 젤 유방삽입물을 이용한 유방확대술 후 발생한 유방삽입물의 흉강내로의 이탈 및 파열 증례보고)

  • Purpose: Breast implant ruptures and displacement are problematic complications after augmentation mammoplasty. The authors report a patient whose cohesive silicone gel implant ruptured and migrated into the pleural cavity after augmentation mammoplasty. Methods: A 23-year-old female had received augmentation mammoplasty at a local clinic a week before visiting our hospital. When the patient's doctor performed a breast massage on the sixth postoperative day, the left breast became flattened. The doctor suspected a breast implant rupture and performed revision surgery. The implant, however, was not found in the submuscular pocket and no definite chest wall defect was found in the operative field. The doctor suspected implant migration into the pleural cavity, and after inserting a new breast implant, the doctor referred the patient to our hospital for further evaluation. The patient's vital signs were stable and she showed no specific symptoms except mild, intermittent pain in the left chest. A CT scan revealed the ruptured implant in the left pleural cavity and passive atelectasis. Results: The intrapleurally migrated ruptured implant was removed by video-assisted thoracic surgery (VATS). There were no adhesions but there was mild inflammation of the pleura. No definite laceration of the pleura was found. The patient was discharged on the first day after the operation without any complications. Conclusion: Surgeons should be aware that breast implants can rupture anytime and the injury to the chest wall, which may displace the breast implant into the pleural cavity, can happen during submuscular pocket dissection and implant insertion.

Effects of Geometry and Imperfection of a Small-sized Groove on Stress Distributions in the Vicinity of the Joined Region of an ABS Part with a Thin Wall (기저부에 생성된 작은 홈 형상과 결함이 박벽이 포함된 ABS 재료로 제작된 제품의 결합 영역 응력 분포에 미치는 영향)

  • The geometry and the defect of the groove of the part provoke a sudden change of stress in a local region. The objective of this paper is to investigate the effects of the geometry and the imperfection of a small groove on stress distributions in the vicinity of the joined region for the ABS part with a thin wall using a three-dimensional finite element analysis (FEA). Several types of groove are designed to improve joining characteristics in the vicinity joined region. The imperfection model of the small-sized groove is obtained from observation of deposition characteristics of a fused deposition modeling process. Local stress distributions in the vicinity of the joined region are predicted by the FE model with refined meshes. The influence of the angle and the imperfection of the groove on appearance regions of the maximum stress and distributions of the defined principal stress for different loading conditions is examined using the results of FEAs. Finally, a proper design of the groove is proposed to improve joining characteristics between the substrate and the ABS part.

  • The Crack, occurred in the concrete structure, is a major cause of function deterioration such as strength, durability and waterproof. Therefore the need for crack reduction will continue as long as there is concrete structure. But, it is not easy to reduce or prevent crack cause of impact factor of it. There are so many factors of crack such as material, construction and maintenance environment, and structure, and these are acting in combination. So, this study derives a correlation between the cracks and members through reviewing member crack occurred. And member's attribute, related crack, is analyzed by it. It consider that the basic attribute of member is similar when its requirements are similar. Through this study, therefore, foundation of countermeasure of crack will be provided by prediction of crack shape through member's attribute.

Mismatch Limit Load Analyses for V-groove Welded Pipe with Through-wall Circumferential Defect in Centre of Weld (원주방향 관통균열이 용접부 중앙에 존재하는 V-그루브 맞대기 용접배관의 한계하중 해석)

  • The present work reports the mismatch limit loads for a V-groove welded pipe for a circumferential crack using finite element (FE) analyses. To integrate the effect of groove angles on mismatch limit loads, one geometry-related slenderness parameter was modified by relevant geometric parameters including the groove angle, crack depth, and root opening based on plastic deformation patterns in the theory of plasticity. Circumferential through-wall cracks are located at the centre of the weldments with two different groove angles ($45^{\circ}$, $90^{\circ}$). With regard to the loading conditions, axial (longitudinal) tension and bending are applied for all cases. For the parent and weld metal, elastic-perfectly plastic materials are considered to simulate and analyze under- and over-matching conditions in plasticity. The overall results from the proposed solutions are found to be similar to the FE results.

An Analysis on the Correlation between Crack Condition and Safety Grades in Masonry Buildings (조적조 건축물의 균열양태 및 등급판정과의 상관성에 관한 연구)

  • In Seoul, structural deterioration is severe in the private masonry buildings which have been built since 1906s. But most of these structures remain without any repair works. As a result, the rate of deterioration is getting faster and these dangerous structures may cause hazardous circumstances to the adjacent structures and neighborhood. The purpose of this study is to investigate the actual conditions of wall cracks among the defect types which occur in private masonry buildings and to analyze the correlation between safety grades and wall cracks for offering the fundamental data. Using these date we can establish basic criteria for safety grades of structures and improve the quality of masonry buildings. The result of this study indicate that there are high correlations between safety grades and the width of crack but much less so with the length. Furthermore, with regard to crack patterns, vertical cracks much more negatively effected the safety grades.

  • This paper introduces developed prototype intelligent pig which detects corrosion on pipeline by using Magnetic Flux Leakage technology. The 8 inch developed MFL(Magnetic Flux Leakage) pig is composed of 5 yokes which magnetize pipeline wall and 45 Hall sensors which detect MFL signal. The designed MFL modules are analyzed by using magnetic circuit method in order to confirm whether pipeline wall is fully saturated. A variety of artificial defects are manufactured on 8 inch diameter steel pipeline in order to acquire MFL signals. So leakage flux of the axial, radial and circumferential component was acquired as defects. The results of this paper show that design technique for 8 inch MFL pig can be applied to large diameter MFL pig and 0.5mm defect depth can be detected.
