발 내재근을 동반한 발목 스트레칭이 만성 뇌졸중 환자의 발목 관절가동범위, 정적 및 동적 균형과 보행속도에 미치는 영향 (Effects of Ankle Stretching with Intrinsic Muscles on Ankle Range of Motion, Static and Dynamic Balance, and Gait Speed in Chronic Stroke Patients)

  • Purpose: This study aimed to investigate the effects of ankle stretching with intrinsic muscles on ankle range of motion, static and dynamic balance, and gait speed in chronic stroke patients. Methods: The participants were 20 chronic stroke patients, divided into two groups, 10 in the ankle stretching with intrinsic muscles group and 10 in the slant board exercise group. Both groups performed their respective interventions once daily, with three sets per session, five times a week for four weeks. Ankle range of motion, dynamic and static balance, and gait speed were measured before and after the intervention. An independent t-test was used to compare the results between the two groups before and after the intervention. Results: Both groups showed significant improvements in ankle range of motion, dynamic and static balance, and gait speed after the intervention (p < .05). The ankle stretching with intrinsic muscles group showed more significant improvements in ankle range of motion, dynamic and static balance, and gait speed after the intervention compared to the slant board group (p < .05). These results suggest that ankle stretching with intrinsic muscles may be more effective than the slant board for certain outcomes. Conclusion: Ankle stretching with intrinsic muscles is effective in improving ankle range of motion, dynamic and static balance, and walking speed.

도시지역 노인들의 도구적 일상생활 수행능력(IADL)에 영향을 미치는 요인 (Factors Affecting to the Instrumental Ability of Daily Living(IADL) in the Urban Elderly)

  • This study was intended to find out the factors affecting to the IADL, allowing objective assessment of physical function status of increasing elderly populations. The subjects of 635 elderly persons aged over 65 years old who live in Taejon metropolitan city were interviewed during the two-month period from June to July of 2000. The IADL of Older American Resources and Services(OARS), developed in Duke medical college of USA, was interpreted for use. The study results were statistically processed using SPSSWIN(ver 10.0) and conferred the following results: 1. Among the seven items of IADL, the women showed higher rates of 'yes' in items about using the telephone, getting to the places out of walking distance, going shopping for groceries, taking their own medicine while outnumbered by the men only in the item about managing their own money, without significant differences between them in items about preparing their own meals and doing their own housework. 2. In terms of IADL scores, $82.0\%$ of subjects showed the normal range of scores and $18.0\%$ were under the normal range, meaning dysfunction IADL. Specifications of general characteristics revealed that more dysfunctional were the male subjects, the higher age groups who are more than 75 years old, the higher educated groups, the groups who live in nursing homes, the groups without a spouse. 3. Based on daily activities, lower scores of IADL were found in the subjects who don't go out, who don't have recreational activities, who don't attend elderly gatherings in their neighborhood, who don't hold social meetings. Specifications of psychological traits revealed that more dysfunctional were IADL in the subjects who don't feel satisfied with their lives of the past or the present, who have a deep feeling of isolation, and who don't have the will to live. 4. Among 7 items of health habits, only the subjects who don't regular exercise had lower scores of IADL than those who do. According to HPI, the lower HPI, the more dysfunctional. 5. Based on the factors associated with IADL, the odds ratio of the subjects who don't live together with their families were 1.53 times that of the ones who do, who are educated 3.22 times that of the ones who are not, who don't have spouses 2.09 times that of the ones who do, who don't go out 4.35 times that of the ones who frequently go out, who don't recreational activities for an interest 2.64 times that of the ones who do, who don't attend elderly gatherings in their neighborhood 1.47 times that of the ones who do, who don't hold social meetings 2.23 times that of the ones who do, who don't feel satisfied with their present living 1.43 times that of the ones who do, who have a feeling of isolation 1.53times that of the ones who don'1, who have the weak will to live 3.21 times that of the ones who have the strong one, and who don't regular exercise 2.45 times that of the ones who do. 6. Logistic regression analysis of the study results found that such factors are significantly related as the degree of education, with/without spouse, social meetings, the will to live, and regular exercise, and that higher rates of dysfunctional subjects were in the more educated group, in the group without spouse, in the group who don't frequently go out, who don't have social meetings, who have the weak will to live, and who don't exercise.

체중조절 내담자의 영양섭취 및 식행동 실태 분석 (Analysis on Nutrient Intakes and Eating Behaviors of Female Students Visiting Nutrition Counselling Office)

  • The purpose of this study was to analyze health-related habits, weight control experience and body composition of 344 female students visiting nutrition counselling office. Dietary habits of the subjects were assessed by means of interview with questionnaire and nutrient intakes were evaluated by the simple dietary assessment method. Weight and height were measured to get body mass index(BMI) and waist-hip ratio and their body composition were measured by Inbody 3.0. The obtained results were as follows: 1) Among students, 14.5% were exercising on a regularly basis, of which the exercises were walking and gymnastic exercise(22.1%), jogging(14.1%), and rope-skipping(4.7%), 48.4% of students were little drinking and 30.5% were once a week and smokers were 4.1%. 2) 28.3% of students were little having breakfast and 15.1% were once or twice a week. Two third of total students were not having breakfast regularly. Also only 59.6% of students were having dinner everyday, which means many of them were even skipping dinner. The reasons why they were not having breakfast were because they don't have enough time to eat(66.0%) and for a diet(2.8%) and the reasons for skipping dinner were because they were not hungry(23.0%), for the weight loss(18.2%). It was shown that they would skip dinner rather than breakfast for a weight control. 51.6% of students were taking snack 1~2 times a day and 5.3% were having little snack. 55.4% of students were dining out once a day and 15.4% more am 2 times a day. 3) 46.7% of students were already experiencing weight control before visiting the counselling office. 78.5% of students tried on one kind of weight control method, 11.4% on two kinds, and 10.1 % on more than three kinds. The method they tried for a weight control most was the one food diet using egg, fruits and beans, which is the most popular among them, and the next were an aerobic exercise(23.6%). a diet tried by a famous entertainer (15.5%), and the fasting(14.5%). 4) The average BMI was 21.2 and the body fat rate was 28.1%. As a result of grouping BMI, 12.8% were underweight 67.6% normal weight 11.6% overweight and 8% obesity. Less than standard for the body fat rate were 0.6%, 50.6% standard, 48.5% more than standard. 49.2% of students as normal weight on a BMI were assessed the so-called "skinny obesity. 50% of consulted students situated fat intra-abdominally at the umbilical level(WHR>0.8). 5) The energy intake was 76.6$\pm$17.8% of RDA and constituent ratio of carbohydrate, fat and protein were 64.2$\pm$5.2%, 21.6$\pm$3.7% and 14.3$\pm$2.3%, respectively, which is little over of 20% of recommended ratio of fat. Protein and niacin intake were more than 90% of RDA and riboflavin and vit. C were taken more than 100% of RDA. But Fe intake was 69.4$\pm$19.3%, Ca 76.6$\pm$23.6%, which were the least constituents. There was not remarkable significance between energy intake and nutrient density based on the groups of underweight, normal weight, overweight and obesity of BMI. 6) It was shown that body fat rate had remarkably significant correlation(p<0.000) with BMI(r=0.760) and WHR(r=0.817) respectively, but body fat rate was more correlated with WHR than with BMI. There was not much significant difference between body fat rate and WHR whether they exercised or not. However. BMI was significantly higher in the exercise group because one who showed higher BMI started to exercise since they looked fat in appearance and perceived as they were fat. fat.

근린공원 환경의 만족도가 신체활동과 건강에 미치는 영향 - 창원시 성산구와 의창구를 대상으로 - (Effect of Satisfaction in Neighborhood Park Environments on Physical Activity and Health - The Case of Seongsan-gu and Uichang-gu in Changwon City -)

  • 본 연구의 목적은 경상남도 창원시를 대상으로 근린공원 환경에 대한 만족도가 신체활동과 건강증진 목적의 공원이용횟수와 체질량지수(BMI)에 의한 건강수준에 미치는 영향을 분석하는 것이다. 설문조사는 창원시 성산구와 의창구 도심지역에 분포하는 8개 근린공원의 이용자와 그 주변에 거주하는 429명의 주민들을 대상으로 실시하였고, 설문분석은 근린공원 환경에 대한 만족도가 신체활동과 건강에 미치는 영향을 분석하기 위해 일대일대응 선형회귀분석을 활용하였다. 연구결과를 요약하면, 공원을 이용하지 않는 주된 이유는 시간적 여유의 부족, 공원까지의 이동거리와 소요시간, 공원 내 이용시설의 부족 등이고, 공원을 이용하는 주된 이유는 공원까지의 접근성, 걷기 또는 산책, 휴식시설, 수목이나 그늘, 운동공간의 다양성 등으로 나타났다. 공원 이용자의 경우, 공원에서 걷기나 산책을 가장 많이 하고, 주 2회 이상 평균 10분 정도 걸어가서 1시간 정도 운동을 하는 것으로 나타났다. 공원과 그 주변의 물리적 환경 중에서 집에서 공원까지의 거리와 이동하는 가로 환경에 대한 만족도가 높았고, 공원 내 수공간이나 볼거리 등에 대한 만족도가 낮게 나타났다. 다음으로, 운동목적의 공원이용 여부, 공원이용시간, 공원이용의 만족도와 건강증진 효과, 공원까지의 거리, 따른 건강증진 효과만이 유의수준 10% 이내에서 상호관련성이 있는 것으로 나타났다. 향후에도 공원의 신체활동과 건강 증진 효과에 대한 실증적 사례노약자들의 공원이용 편이성 등이 유의수준 5% 이내에서 신체활동과 건강증진 목적의 공원이용횟수에 긍정적인 영향을 보였고, 체질량지수와는 공원이용에 를 지속적으로 축적해 나갈 필요가 있으며, 이를 통해서 각종 개발용도로 전환되거나 그 면적이 축소되고 있는 도시지역의 공원 및 녹지공간을 보전 확대시켜 나가고, 신체활동 및 건강증진 장소로서의 기능을 제고하기 위한 공원계획 설계 요소를 도출하는데 근거 자료를 제공하는 것이 가능할 것으로 판단된다.

Ensemble of Nested Dichotomies 기법을 이용한 스마트폰 가속도 센서 데이터 기반의 동작 인지 (Ensemble of Nested Dichotomies for Activity Recognition Using Accelerometer Data on Smartphone)

  • 최근 스마트 폰에 다양한 센서를 내장할 수 있게 되었고 스마트폰에 내장된 센서를 이용항 동작 인지에 관한 연구가 활발히 진행되고 있다. 스마트폰을 이용한 동작 인지는 노인 복지 지원이나 운동량 측정. 생활 패턴 분석, 운동 패턴 분석 등 다양한 분야에 활용될 수 있다. 하지만 스마트 폰에 내장된 센서를 이용하여 동작 인지를 하는 방법은 사용되는 센서의 수에 따라 단일 센서를 이용한 동작인지와 다중 센서를 이용한 동작인지로 나눌 수 있다. 단일 센서를 이용하는 경우 대부분 가속도 센서를 이용하기 때문에 배터리 부담은 줄지만 다양한 동작을 인지할 때에 특징(feature) 추출의 어려움과 동작 인지 정확도가 낮다는 문제점이 있다. 그리고 다중 센서를 이용하는 경우 대부분 가속도 센서와 중력센서를 사용하고 필요에 따라 다른 센서를 추가하여 동작인지를 수행하며 다양한 동작을 보다 높은 정확도로 인지할 수 있지만 다수의 센서를 사용하기 때문에 배터리 부담이 증가한다는 문제점이 있다. 따라서 본 논문에서는 이러한 문제를 해결하기 위해 스마트 폰에 내장된 가속도 센서를 이용하여 다양한 동작을 높은 정확도로 인지하는 방법을 제안한다. 서로 다른 10가지의 동작을 높을 정확도로 인지하기 위해 원시 데이터로부터 17가지 특징을 추출하고 각 동작을 분류하기 위해 Ensemble of Nested Dichotomies 분류기를 사용하였다. Ensemble of Nested Dichotomies 분류기는 다중 클래스 문제를 다수의 이진 분류 문제로 변형하여 다중 클래스 문제를 해결하는 방법으로 서로 다른 Nested Dichotomy 분류기의 분류 결과를 통해 다중 클래스 문제를 해결하는 기법이다. Nested Dichotomy 분류기 학습에는 Random Forest 분류기를 사용하였다. 성능 평가를 위해 Decision Tree, k-Nearest Neighbors, Support Vector Machine과 비교 실험을 한 결과 Ensemble of Nested Dichotomies 분류기를 사용하여 동작 인지를 수행하는 것이 가장 높은 정확도를 보였다.

저체중 독거노인의 질병과 건강행태 : 2014년 지역사회건강조사 자료를 중심으로 (Disease and Health Behavior of Low-Weight Elderly Living Alone : Focusing on the Community Health Survey 2014)

  • 본 연구는 독거노인의 질병과 건강행태의 파악을 통해 독거노인의 저체중에 영향을 미치는 요인을 규명하기 위한 조사연구이다. 2014년 지역사회건강조사 원시자료를 활용하였으며, 65세 이상 저체중 독거노인 922명의 자료를 최종분석에 사용하였다. 자료의 분석은 SPSS/WIN 22.0 프로그램의 복합표본분석 모듈을 이용하여, 기술통계, Rao Scott $x^2$ test, 로지스틱 회귀분석을 통해 분석하였다. 본 연구의 결과, 독거노인들은 만성질환을 가지고 있는 비율이 높았으며, 흡연, 음주, 운동량 부족, 짜게 먹는 식습관, 틀니사용에 의한 저작 불편, 나쁜 주관적 건강상태와 같은 좋지 않은 건강행태를 가지고 있는 것으로 나타났다. 로지스틱 회귀분석 결과 독거노인의 성별에 따른 저체중 발생 위험확률은 다음과 같다. 흡연으로 인한 저체중 위험도는 여자노인에서 가끔 피우는 경우 3.004배, 걷기량으로 인한 저체중 위험도는 3일미만 걷는 사람이 저체중일 가능성은 1.420배로 유의하게 높았다. 주관적 스트레스를 많이 느끼는 경우 저체중일 가능성은 남자노인에서 2.220배, 여자노인에서 1.282배로 유의하게 높았다. 주관적 건강수준이 나쁜 사람이 저체중일 가능성은 남자노인에서 3.633배, 여자노인에서 1.590배로 유의하게 높았다. 본 연구의 결과를 토대로 저체중 독거노인의 건강행태 개선을 위한 적절한 간호중재의 수립과 관리방안의 마련이 필요하다. 또한, 저체중 독거노인에 대한 신체적, 심리적, 사회적 특성 등의 다양한 변인을 고려한 반복연구가 필요할 것이다.

후지마비견(後肢痲痺犬)에 대한 봉독(蜂毒) 약침(藥鍼) 및 봉독(蜂毒) 약침(藥鍼)과 한약제(漢藥劑)의 병용치료(倂用治療) : 증례보고(症例報告) (Treatment by Injection-Acupuncture with Apitoxin and Apitoxin Combined by Chinese Herbal Medicine in Patients with Canine Bind Limb Paralysis : Case Report)

  • 개 추간판 디스크 2 증례(症例)를 대상으로 봉독약침 및 봉독약침과 한약제로 각각 치료하였다. 증례(症例) 1은 흉-요추부 추간판탈출증(T11-T12, T12-T13, L3-L4 및 L4-L5)으로 진단(診斷)된 증례이었으며. 증례 2는 흉추부 추간판탈출증(T10-T11 및 T12-T13)으로 진단(診斷)된 증례이었다. 이들 증례에 대하여 봉독약침(0.1 ml/혈위, 총$200{\mu}g$)을 실시(實施)하였으며, 운동요법과 수영요법도 병용하였다. 봉독약침에 사용한 혈위는 GV20-Bai Hui, GB30-Huan Tiao, ST36-Zu San Li, GB34-Yang Ling Quan, ST40-Feng Long, ST41-Jie Xi 및 BL40-Wei Zhong, 병변부 및 압통점이었다. 또한 증례 2에 대하여는 봉독약침과 더불어 Zheng Gu Zi Jin Dan(정골자금단(正骨紫金丹) : 1 g), Shiuh Duann(속단(續斷) .0.2 g), Du Zhong(두중(杜仲) : 0.2 g), Mo Yao(몰약(沒藥 : 0.2 g), Ru Xiang(유향(乳香) : 0.2 g) 및 Pyrite(자연동(自然銅) : 0.2 g)를 각각 총 9일간 경구투여(2회/1일)하였다. 증례 1은 총 4주간 11회 치료, 증례(症例) 2는 총 2주간 6회 치료 후 각각 보행이 가능하였다.

1개 종합병원 환자의 낙상에 관한 조사 (A Study on Fall Accident)

  • The study was conducted from November 1995 to May 1996 at the one general hospital in Seoul. The total subjects of this study were 412 patients who have the experience of fall accident, among them 31 was who have fallen during hospitalization and 381 was who visited emergency room and out patient clinic. The purposes of this study were to determine the characteristics, risk factors and results of fall accident and to suggest the nursing strategies for prevention of fall. Data were collected by reviewing the medical records and interviewing with the fallers and their family members. For data analysis, spss/pc+ program was utilized for descriptive statistics, adjusted standardized $X^2$-test. The results of this study were as follows: 1) Total subjects were 412 fallers, of which 245(59.5%) were men and 167(40.5%) were women. Age were 0-14 years 79(19.2%), 15-44 years 125(30.4%), 45-64 years 104(25.2%), over 65 years 104(25.2%). 2) There was significant association between age and the sexes ($X^2$=39.17, P=0.00). 3) There was significant association between age and history of falls ($X^2$=44.41. P= .00). And history of falls in the elderly was significantly associated with falls. 4) There was significant association with age and medical diagnosis ($X^2$=140.66, P= .00), chief medical diagnosis were hypertension(34), diabetis mellitus(22), arthritis(11), stroke(8), fracture(7), pulmonary tuberculosis(6), dementia(5) and cataract(5), 5) There was significant association between age and intrinsic factors: cognitive impairment, mobility impairment, insomnia, emotional problems, urinary difficulty, visual impairments, hearing impairments, use of drugs (sedatives , antihypertensive drugs, diuretics, antidepressants) (P < 0.05). But there was no significant association between age and dizziness ($X^2$=2.87, P=.41). 6) 15.3% of total fallers were drunken state when they were fallen. 7) Environmental factors of fall accident were unusual posture (50.9%), slips(35.2%), trips (9.5%) and collision(4.4%). 8) Most of falls occurred during the day time, peak frequencies of falls occurred from 1pm to 6pm and 7am to 12am. 9) The places of fall accident were roads(22.6%), house-stairs 06.7%), rooms, floors, kitchen (11.2%), the roof-top, veranda, windows(10.9%), hospital(7.5%), ice or snowy ways(5.8%), bathroom(4.9%), playground, park(4.9%), subway-stairs(4.4%) and public-bathrooms (2.2%). 10) Activities at the time of fall accident were walking(37.6%), turning around or reaching for something(20.9%), going up or down stairs09.2%), exereise, working07.4%), up or down from a bed(2.7%), using wheelchair or walking aids, standing up or down from a chair(2.2%) and standing still(2.2%). 11) Anatomical locations of injuries by falls were head, face, neck(31.3%), lower extremities (29.9%). upper extremities(20.6%), spine, thorax, abdomen or pelvic contents(l1.4%) and unspecified(2.9%). 12) Types of injures were fracture(47.6%), bruises03.8%), laceration (13.3%), sprains(9.0%), headache(6.6%), abrasions(2.9%), intracranial hemorrage(2.4%) and burns(0.5%). 13) 41.5% of the fallers were hospitalized and average of hospitalization was 22.3 days. 14) The six fallers(1.46%) died from fall injuries. The two fallers died from intracranial hemorrage and the four fallers died of secondary infection; pneumonia(2), sepsis(1) and cell lulitis(1). It is suggested that 1) Further study is needed with larger sample size to identify the fall risk factors. 2) After the fall accident, comprehensive nursing care and regular physical exercise should be emphasized for the elderly person. 3) Safety education and safety facilities of the public place and home is necessary for fall prevention.

Diabetes Mellitus and Risk of Colorectal Cancer Mortality in Japan: the Japan Collaborative Cohort Study

  • Objective: Our aim was to estimate whether diabetes mellitus (DM) may be associated with an increased risk of colorectal cancer (CRC) mortality in Japan. Methods: The Japan Collaborative Cohort (JACC) Study is a nationwide prospective study, initiated in 1988, which involves 110,585 subjects (age range: from 40 to 79 years; 46,395 males and 64,190 females). Our present analysis population comprised 96,081 (40,510 men and 55,571 women) who provided details on DM history. The questionnaire also included age, sex, weight, height, family history of CRC, smoking, drinking and exercise habits, and education. Cox proportional-hazard regression was used to estimate the hazard ratio (HR). We used SPSS 21 software to analyze all data. Results: Among the participants with DM, we followed up for 71,174 person-years and 640. deaths from CRC were confirmed; and, among the non-diabetic participants, 785 CRC deaths were identified during 1,499,324 person-years. After adjusting for multivariate confounding factors, such as age, sex, body mass index (BMI), family history of colorectal cancer, smoking habit, drinking habit, physical activity (sports and walking) and education, DM was associated with an increased risk of CRC death (HR 1.4, 95% confidence interval [CI] 1.0-2.0). Diabetic women, but not diabetic men, experienced increased mortality from CRC (HR 1.7, 95% CI 1.0-3.0). Conclusion: The risk of CRC mortality is significantly increased in both sexes and women with diabetes, but no significant increase was seen for diabetic men among Japanese.

Despite leakage through the contact area between the mask and the face has various causes such as the wearer's activity, sweat accumulation, facial shape, etc., There is a lack of relevant research and regulation compared to developed countries that have already institutionalized the law 30 years ago and give the right to sell through a test Method: The work intensity was adjusted by walking or running at 6km/h and 11km/h on the treadmill, and tasks were defined with reference to the test procedure and the exercise sequence applied in the face leakage test of the dustproof mask. And fit factor was measured objectively using 'Respirator Fit Tester 8038' which measures fit factor calculated by dividing the number of dust present outside the mask by inside the mask.
