High early pregnancy serum 25-hydroxy vitamin D level, within a sub-optimal range, is associated with gestational diabetes mellitus: a prospective cohort study

  • BACKGROUND/OBJECTIVES: Low early pregnancy serum 25-hydroxy vitamin D (25[OH]D) levels can increase gestational diabetes mellitus (GDM) risk, although inconsistent findings related to that association have been reported. This study examined the association of serum vitamin D with GDM and the possible influencers on this association. SUBJECTS/METHODS: This study included 259 pregnant women within the Seremban Cohort Study (SECOST). Blood samples at < 14 weeks of gestation were drawn to determine serum 25(OH)D levels. GDM diagnosis was made at 24 to 32 weeks of gestation using a standard procedure. Association between serum vitamin D and GDM was tested using binary logistic regression. RESULTS: Nearly all women (90%) had mild (68.3%) or severe (32.2%) vitamin D deficiency (VDD). Non-GDM women with mild VDD had a significantly higher mean vitamin D intake than GDM women with mild VDD (t = 2.04, p < 0.05). Women with higher early pregnancy serum vitamin D levels had a greater risk of GDM. However, this significant association was only identified among those with a family history of type 2 diabetes mellitus (T2DM) and in women with a body mass index indicating overweight or obese status. CONCLUSIONS: The high prevalence of VDD in this sample of pregnant women underscores the need for effective preventive public health strategies. Further investigation of this unexpected association between serum vitamin D level and GDM risk in predominantly VDD pregnant women and the potential effects of adiposity and family history of T2DM on that association is warranted.

Relation among Mineral (Ca, P, Fe, Na, K,Zn) Intakes, Blood Pressure, and Blood Lipids in Korean Adults (일부 성인남녀의 무기질 (Ca, P, Fe, Na, K, Zn) 섭취와 혈압 및 혈중 지질과의 관련성)

  • The purpose of this study was to investigate the relation among mineral intakes, blood pressure, and blood lipids for health management of Korean adults over 20 years old. Subjects were recruited from the district where they lived, which included rural (Yoju, n = 137) , coastal (Taean, n = 100) , and urban district (Suwon, n : 117) . hnthropornekical measurement, blood pressure measurement, dietary intake assessment using 24-hour recall method, and blood collection and analysis of blood lipids were conducted. The average age, height, weight, and BMI were 54.1 years, 168.5 cm, 67.3 kg, and 24.5 kg/$m^{2}$ for men and 53.9 years,153.8 cm,59.2 kg, and 25.0 kg/$m^{2}$ for women, respectively. The mean daily intakes of total food and energy were 1219.1 g and 1740.9 kcal for men and 1071.3 g and 1432.6 kcal for women. The mineral intakes of men and women were 448.5 mg and 400.4 mg for calcium,955.3 mg and 803.7 mg for phosphorus, 13.3 mg and 11.1 mg for iron, 4302.8 mg and 3393.3 mg for sodium,2588.0 mg and 2264.9 mg for potassium, and 8.9 mg and 7.3 mg for zinc. The daily intakes of energy, calcium, zinc, vitamin $B_{2}$, and vitamin I were $76.1\%$, $60.1\%$, $73.1\%$, $68.6\%$, and $80.4\%$ of RDAs, respectively. The intakes of energy, calcium, zinc, vitamin $B_{2}$ and I were not meet Korean RDAs. Especially, calcium, zinc, and vitamin $B_{2}$ were less than $75\%$ of RDAs. Blood pressure of the subjects was 128.3/75.5 mmHg for men and 124.1/73.7 mmHg for women. Serum lipids of men and women were 180.2 mg/dL and 184.9 mg/dL for total cholesterol, 160.8 mg/dL and 137.6 mg/dL for triglyceride,41.5 mg/dL and 44.7 mg/dL for HDL-cholesterol,106.6 mg/dL and 112.7 mg/dL for LDL-cholesterol, and 3.5 and 3.3 for atherogenic index. While serum triglyceride and AI of men was significantly higher, HDL-cholesterol was lower than those of women. In the relation among mineral intakes, blood pressure, and serum lipids, we found a significantly negative correlation of intakes of calcium, calcium/phosphorus, calcium density with systolic blood pressure. There were significantly negative correlation between intake of calcium/phosphorus and serum cholesterol, calcium density and serum cholesterol, sodium intake and serum HDL-cholesterol. Based on these results, we concluded that high intakes of calcium, calcium/phosphorus and low sodium intake were associated with favorable blood pressure and serum lipid profiles in adult men and women. The Literature Review on the Effectiveness of Fall-related Hip Fracture Prevention Programs (노인의 넘어짐으로 인한 고관절 골절 예방프로그램의 효과: 문헌 고찰)

  • While efforts have been made to address fall-related injuries in older adults, the problem is unsolved to date. The purpose of this review is to provide a guideline for fall and injury prevention programs in older adults, based on evidence generated over the past 30 years. Research articles published between 1990 and 2020 have been searched on PubMed, using keywords, including but not limited to, falls, hip fracture, injuries, intervention, older adults, prevention, hip protector, vitamin D, safe landing strategy, and exercise. Total of 98 articles have been found and categorized into five intervention areas: exercise program, hip protector, safe landing strategy, vitamin D intake, and compliant flooring. Furthermore, the articles have been rated based on their study design: class 1, randomized controlled trials; class 2, non-randomized controlled trials; class 3, experimental studies; class 4, all other studies. Exercise programs have shown to decrease the risk of fall, and associated injuries. Hip protectors, safe landing strategy, and vitamin D intake were effective in reducing a risk and incidence of hip fracture during a fall. Furthermore, compliant flooring has also decreased hip fracture risk without affecting balance. An integrated approach combining exercise program, wearing a hip protector, teaching safe landing strategies, scheduled vitamin D intake, and compliant flooring installation, is suggested to address fall-related injuries in older adults.

Effects of Aloe vera on the Cholesterol and Vitamin $D_2$-induced Atherosclerosis in Rats (Aloe vera가 콜레스테롤 및 비타민 $D_2$ 식이로 유발한 흰쥐의 동맥경화증에 미치는 영향)

  • This study was performed to investigate the induction of experimental atherosclerosis in rats and inhibitory effects of aloe vera on progression of atherosclerosis in rats. Adose range finding study of cholesterol and vitamin D2 for the induction of atherosclerosis and studies on the subchronic effect of aloe vera and on the chronic effect of aloe vera were carried out. A total of 3-week old 125 male rats of Sprague-Dawley were divided into 25 groups and fed with the diet containing cholesterol (0.1, 0.2, 0.3, 0.5, 1.0 and 2.0%) and vitamin D2 (500, 5000, 50000 and 500, 000 IU/100 g) for 4 weeks. 35 male rats were divided into 7 groups and fed with the diet containing aloe vera with 1.0% of cholesterol and 50, 000 IU/100 g of vitamin D2 for 4 weeks. 200 male rats were divided into 5 groups and fed with cholesterol and vitamin D2 for 6 and 12 months. Growth, clinical and pathological changes of rats in the three experiments were observed. The results were as follows: 1. In the dose-range finding study, feed intake, feed efficiency ratio and weight to body weight were increased in all of the feed groups containing 500, 000 IU/100 g of vitamin D2. Serum biochemical values of total cholesterol, high-density lipiprotein cholesterol (HDL-cholesterol), triglyceride, calcium, inorganic phosphorous and chloride of male rats in treated groups. The aorta and coronary artery of rats in all of the diet group containing 500, 000 IU/100 g of vitamin D2 showed typical atherosclerotic lesions. 2. Male rats fed with the diet containing aloe vera with 1.0% cholesterol and 50, 000 IU/100 g of vitamin D2 for 6 and 12 months did not show significant difference of diet intake and weight gain, and relative organ weight. The level of serum HDL-cholesterol and triglyceride recovered to the normal range by the aloe vera ingestation. 3. The aorta showed irregular appearence in the tunica intima with swelling, necrosis and calcification. The aorta of rat fed aloe vera diet showed no pathological lesions such as atherosclerosis of aorta. Aloe vera could have a helpful effect of vitamin D2 and cholesterol induced atherosclerosis in rats. Long-term supplementation of aloe vera may slow down the process of experimental atherosclerosis in rats have effects on the development of atherosclerosis.

Factors Affecting Bone Mineral Density of the Femur in Parkinson's Disease Patients (파킨슨병 환자의 골밀도 영향요인)

  • Purpose: The purpose of this study is to explore the level of bone mineral densities of the femur neck and to identify factors affecting bone mineral density of the femur in Parkinson's disease (PD) patients. Methods: Participants were 121 PD patients visiting the outpatient clinic of D University Hospital in B City. Bone mineral density was measured at the femur neck by using a dual-energy x-ray absorptiometry. The serum vitamin D level, the amounts of milk intake, caffeine intake, cigaret smoking, and alcohol consumption, and the number of steps taken daily were measured. Past and present disease history, the medication history, and duration of the disease were also collected. The level of disability was obtained by neurologists using the Hoehn and Yahr stage and the Unified Parkinson's Disease Rating Scale. Results: Among PD patients, 57% had osteopenia and 15.7% had osteoporosis. The bone mineral density of the femur was associated with weight, serum vitamin D level, age, and working status. Conclusion: The identification of weight and serum vitamin D level as important predictive factors emphasizes the importance of balanced nutrition for PD patients.

Seasonal Variation of Nutritional Intake and Quality in Adults in Longevity Areas (고령인구 비율이 높은 지역 성인 및 노인의 계절별 영양소 섭취실태)

  • The purpose of this study was to estimate seasonal variation of nutritional intake and quality in adults in longevity areas. Dietary survey was given to 469 subjects over age 20 living Bukjeju-gun, Yecheon-gun, and Sunchang-gun of Korea, using 24-hour recall method every 4 seasons over one-year period. The mean daily intakes (%RDA) of 4 seasons were 1313.3 ㎉ (72.0%) for energy, 47.3 g (82.2%) for protein, 20.2 g for fat, 228.0 g for carbohydrate, 12.8 g for dietary fiber. The differences were hardly significant among the seasons. Mean daily intakes of most of the vitamins and minerals were lower than RDA except vitamin C and folate, especially %RDAs of Ca, vitamin D, vitamin B$_2$, vitamin E were less than 60% of RDA. In general, nutrient intake were high in spring compared to other seasons except vitamin C which was high in fall and winter. Mean daily intakes of cholesterol were 151.7 mg and 124.3 mg in males and females, respectively. The differences was significant between the two sexes but not among the seasons. PUFA : MUFA : SFA ratio of the subject was 1.0 : 1.3 : 1.2. Average CPF ratio of energy intake was 72.7 : 14.4 : 12.9, and energy intake ratio from carbohydrate was low in spring, in contrast energy intake ratios from protein was significantly high in spring. Mean adequacy ratio (MAR), an index of overall nutritional quality was 0.64 for female and 0.71 for male. The indices of nutritional quality (INQ) were over 1 for most of nutrients except 0.73 of Ca, 0.87 of vitamin A, 0.69 of vitamin B$_2$, and 0.65 of vitamin E. Both MAR and INQ were significantly different among sex and seasons, values were higher in males than in females and were higher in spring with the exception of vitamin C. In conclusion, subjects in longevity areas did not consume enough nutrients quantitatively as well as qualitatively, especially Ca, vitamin A, vitamin B$_2$, and vitamin E. Also mean daily intakes of most of vitamins and minerals were insufficient in females, and were significantly different among season. Therefore we must consider a counterplan to augment nutrition intake for them. In addition, it seems to be essential to micro nutrients to the food composition database to estimate dietary intakes more accurately.

  • This study was designed to investigate the effect of nutrient intake on bone mineral density (BMD) of the lumbar spine(L2longrightarrowL4) in 41 postmenopausal women. The BMD of the lumbar spine was positively correlated with caiorie protein animal protein fat animal fat calcium animal calcium milk ad dairy calcium phosphorus iron animal iron vitamin A thiamin animal calcium milk and dairy calcium phosphorus iron animal iron vitamin A thiamin riboflavin niacin and ascorbic acid intake. Postmenopausal wmen of BMD$\geq$100% showed enhanced calorie protein fat calcium phosphrous niacin intake compared women of BMD<100% In the group of calorie protein riboflavin intake$\geq$recommended dietary allowances(RDA) BMD was significantly higher than BMD in the group of these nutrient intakes$\geq$700 mg/d is significantly higher than BMD that of examined using stepwise multiple regression analysis. From this analysis in subjects aged 50~59 years fat intake only in subjects aged 60~69 years niacin intake Ca/P in total subjects fat riboflavin intake were signifiant independent predictors of BMD In the group of menopausal significant independent predictor of BMD This study suggests that dietary calcium is a major constituent affecting lumbar spin BMD in postmenopausal women whose menopausal period is over than 5 years.

Evaluation of the Menus of Senior Welfare Centers for Home-bound Elderly (재가 노인들을 위한 노인복지관의 급식 식단 평가)

  • This study examined the characteristics of the foodservice menu items offered at senior welfare centers to provide information on Korean senior menu development. A total of 514 lunch menu items were collected from 27 senior welfare centers in April, July, October and January. The most frequently served staple foods, soups, and side dishes were multi-grain rice, seaweed soup, Bulgogi, Kimchi, and liquid yogurt. The proportions of carbohydrate, protein, and lipids of total energy serving of senior welfare centers were 59.8%: 16.7%: and 22.8%, respectively. The nutrients served at less than 40% of the Recommended Nutrient Intake (RNI) and Adequate Intake (AI) of Dietary Reference Intakes for Koreans (KDRIs) were chloride (1.0%), vitamin D (1.3%), biotin (1.7%), magnessium (4.5%), Iodine (7.5%), pantothenic acid (8.0%), vitamin E (12.5%), vitamin $B_6$ (20.0~21.4%), vitamin K (21.1~24.3%), and water (35.7~39.7%). The nutrients served in excess of the daily intake goal and RNI were iron (98.9~127.1%), sodium (104.9%), and copper (1,100.0%).

Association between Bone Mineral Density and Bone Nutrition Indicators in Elderly Residing in Low Income Area of the City (도시거주 저소득층 노인들의 골지표 및 영양소섭취와 골밀도와의 상관관계에 관한 연구)

  • This study explored the association between the bone nutrition indicators and the bone mineral density (BMD) in 138 apparently healthy elderly (male: 38, female: 100) dwelling in a low income area of the city. Dietary intakes were estimated from two meals (breakfast & dinner) and snack using 24 hr-recall method and lunch with weighing over 3 consecutive days. Female elderly showed significant lower intakes (p<0.001∼p<0-05) for most of the nutrients except calcium and vitamin C than the elderly male. Calcium and vitamin D intakes for both male and female were 331.0 mg, 1.89 $\mu\textrm{g}$ and 308.6 mg, 1.21 $\mu\textrm{g}$, respectively and they were below the 50% of the RDA. Both the BMDs at lumbar spine (LS) and femoral neck (FN) were positively correlated with the energy intake, calcium intake and vitamin D intake (p<0.05, respectively) for male. In female BMDs of the both sites were positively correlated with the intakes of carbohydrates, protein, lipid, calcium and vitamin D (p<0.01∼p<0.05). Female showed higher serum osteocalcin (p<0.01) and urinary deoxypyridinoline/creatinine (DPYR/CR) (p<0.001), meaning that female had elevated rate in bone turn over and bone resorption. The proportion of subjects with vitamin D deficiency assessed with serum 25(OH)VitD$_3$<10 mg/mL was 35.0% for female and 23.7% for male, respectively Both the BMDs at lumbar spine and trochanter were positively correlated with serum 25(OH)VitD$_3$ but BMDs in most of the sites were negatively associated with urinary DPYR/CR, phosphate/CR. Stepwise multiple regression showed physical activity, serum alkaline phosphatase, weight, vitamin D explained 47.6% of the variation of the LS BMD. The indicator variable for serum alkaline phosphatase was negatively associated with LS BMD. However, the indicator variable for weight and vitamin D intake were positive and significant (p=0.0087, p=0.0007, respectively). For FN BMD, the indicator variable for age and serum alkaline phosphatase were negative and significant (p<0.0075, p<0.0015, respectively) and the weight was positively associated with the FN BMD.

Vitamin B-6 Status of Mothers : Relation to Condition of the Newborn and the Neonate

  • Vitamin B-6 status parameters of mothers were assessed in relation to th condition of the infant at birth and during the neonatal period. Parameters were assessed at birth and then weekly in 18 mother-infant pairs during the neonatal period ; mothers were supplemented postnatally with 2 or 27 mg PN-HCI/d. Vitamin B-6 inadequacy in the 2mg supplemented group was suggested by the vitamin status parameters. Mothers whose infants had unsatisfactory Apgar scores at 5min after birth(<7) had lower vitamin B-6 status parameters than mothers whose infants were scored satisfactory. Also, infants who scored unsatisfactory at birth and whose mothers were supplemented with the low level of PN had significantly lower vitamin B-6 status parameters at 7 days of age than infants who scored satisfactory. Infants scored unsatisfactory showed some beneficial effects in both vitamin B-6 status and growth associated with the higher level of maternal postnatal vitamin B-6 supplement. In summary, the mother's prenatal and postnatal vitamin B-6 intake were significantly related to the condition of her infant at birth and during the neonatal period, respectively.
