Analysis to Performance of Prehospital emergency care for cardiac arrest patient showing shockable rhythm on EKG (제세동 가능 심정지 환자의 병원전 응급처치 수행에 대한 분석)

  • Purpose : Prehospital emergency care for shockable rhythm is one of major concerns of emergency medical services. But, in Korea, prehospital medical service systems are not yet well established. We tried to offer one of the fundamental data to develop of these system. Method : After application of exclusion criteria, 200 patients who had shockable rhythm from January to December, 2008 were included in this study. Restrospective review of Prehospital care Reports of these patients was done. Result : Total 200 cases of shockable rhythm and prehospital arrest were analyzed. The rates of assessment of vital signs were 89.0%, the rate of level of consiousness was 99.5%. Just 6.0% were communicated with medical director providing the prehospital care. The frequency of defibrillation was performed 58.5%. Conclusion : Survival rate was higher in defibrillation group than that of nondefibrillation group(20.5% vs 2.4%, p=0.000).

  • The use of a tidal-current power system is one source of renewable energy that can minimize the environmental impact of power production and offer many other advantages compared to conventional energy sources. Unlike other energy production approaches, rate of energy production can be precisely predicted and the operational rate is very high. The performance of the rotor, which has a vital role in energy production using tidal currents, is determined by various design factors, and it should be optimized for the specific ocean environment in the field. The horizontal-axis turbine is very sensitive to the direction of flow, and flow direction changes due to rise and fall of the tides. To investigate the performance of the rotor considering the interaction problems with incidence angle of flow, a series of experiments were conducted, and a 3D CFD model was designed and analyzed by ANSYS CFX. The results and findings are summarized in the paper.

  • The development of new materials that are light-weight, yet high in strength has become vital to the machinery, aircraft and auto industries. However, there are a lot of problems with developing such materials that require expensive tools, and a great deal of time and effort. Therefore, the improvement of fatigue strength and fatigue life are mainly focused on by adopting residual stress. The fatigue crack growth rate of the Shot-peened material was lower than that of the Un-peened material. And in stage I, threshold stress intensity factor of the shot-peen processed material is high in critical parts unlike the Un-peened material. Also, fatigue crack growth exponent and number of cycle of the Shot-peened material was higher than that of the Un-peened material. That is concluded from effect of da/dN. And Fatigue life shows more improvement in the Shot-peened material than in the Un-peened material. And compressive residual stress of surface on the Shot-peen processed operate resistance force of fatigue crack propagation.

A Survey on Satisfaction of Protective Clothing for Riot Policemen (남자경찰보호복 착용실태 및 만족도에 관한 연구)

  • This paper examines the protective clothing satisfaction rate of riot policemen. The protective clothing currently in use is the version that underwent enhancement and was distributed in 2009. This version underwent significant enhancements compared to the protective clothing introduced in 1996 in terms of safety and body fitness; however, there is room for further enhancement in terms of activity, thermal comfort and easiness. In this research, a questionnaire survey was conducted on 386 riot policemen working at the Seoul Riot Police Station to study their use of protective clothing and satisfaction rate. The survey questions included safety, activity and body fitness in terms of functionality and comfort as well as easiness and design in terms of wearing comfort. Activity scored the lowest (2.89) in terms of functionality as well as comfort (2.40) that scored the lowest in terms of wearing comfort. It is concluded that, in order to correct deficiencies, the weight of protective clothing must be reduced and design changes must be made to eliminate unnecessary areas in clothing and facilitate faster dressing/undressing. In addition, it is strongly recommended that shock absorption materials be inserted only in vital areas in order to enhance thermal comfort.

  • The production of chitosan from the mycelia of Absidia coerulea was studied to improve cell growth and chitosan productivity. Culture conditions were optimized in batch cultivation (pH 4.5, agitator speed of 250 rpm, and aeration rate of 2 vvm) and the maximum chitosan concentration achieved was 2.3g/L under optimized conditions. Continuous culture was carried out successfully by the formation of new growth spots under optimized conditions, with a chitosan productivity of 0.052g/L(sup)-1 h(sup)-1, which is the highest value to date, and was obtained at a dulution rate of 0.05h(sup)-1. Cell chitosan concentrations reached about 14% in the steady state, which is similar to that achieved in batch culture. This study shows that for the continuous culture of Absidia coerulea it is vital to control the medium composition.

  • Objectives : We tried to investigate the effects of Carthami Semen(CF) pharmacopuncture and Bovis Calculus Fei Ursi(BU) pharmacopuncture on the heart rate variability(HRV) in adult men. As well as we tried to observe how CF pharmacopuncture and BU pharmacopuncture effect on the balance of the autonomic nervous system. Methods : We investigated on 40 healthy volunteers consisted of 20 subjects in CF pharmacopuncture group and 20 subjects in BU pharmacopuncture group respectively. We ruled out subjects whose vital sign isn't in normal range, yet they had taken a rest. The study established by a randomized, single-blind clinical trial. CF pharmacopuncture and BU pharmacopuncture was applied on each group. We measured HRV 7 times : baseline measurement and every 5 minutes for 30 minutes after injection. The SPSS 15.0 for Windows was used to analyze the data by the paired t-test(in group) and Independent sample t-test(between the groups). Results 1. After injection of CF pharmacopuncture, SDNN, Ln(TP), Ln(VLF) and Ln(LF) increased significantly, and Complexity, pNN50 decreased significantly. 2. After injection of BU pharmacopuncture, RMSSD, SDSD and HRV-index increased significantly. Conclusions : We suggest that CF pharmacopuncture activate sympathetic nervous system and BU pharmacopuncture tend to activate the autonomic nervous system.

Development of On-Line Diagnostic Expert System Algorithmic Sensor Validation (진단 전문가시스템의 개발 : 연산적 센서검증)

  • This paper outlines a framework for performing intelligent sensor validation for a diagnostic expert system while reasoning under uncertainty. The emphasis is on the algorithmic preprocess technique. A companion paper focusses on heuristic post-processing. Sensor validation plays a vital role in the ability of the overall system to correctly detemine the state of a plant monitored by imperfect sensors. Especially, several theoretical developments were made in understanding uncertain sensory data in statistical aspect. Uncertain information in sensory values is represented through probability assignments on three discrete states, "high", "normal", and "low", and additional sensor confidence measures in Algorithmic Sv.Upper and lower warning limits are generated from the historical learning sets, which represents the borderlines for heat rate degradation generated in the Algorithmic SV initiates a historic data base for better reference in future use. All the information generated in the Algorithmic SV initiate a session to differentiate the sensor fault from the process fault and to make an inference on the system performance. This framework for a diagnostic expert system with sensor validation and reasonig under uncertainty applies in HEATXPRT$^{TM}$, a data-driven on-line expert system for diagnosing heat rate degradation problems in fossil power plants.

Research on the Bone Mineral Density Performed in Volunteer Medical Service (의료봉사활동에서 시행한 골밀도 검사의 유소견 연구)

  • Thanks to recent development of science and technology, economic standard has been improved and life expectation has been gradually increased. Accordingly, disease aspect and demand for health have been changed in the concept from conventional center of disease to health improvement As the health issue in middle and senior age has also been vital in the national health, osteoporosis, one of the physical maladies occuring in these age group, is on the rise as a matter of important concern so that the researches on the osteoporosis have being accomplished actively in several medical domains. Research has been earned out for 735 examined objects, who have received medical examination of bone mineral density in the volunteer medical service of Daegu and Kyungbuk province for recent 4 years. The result indicated that serious osteoporosis prevailed over osteoporosis for male while osteopenia was majority to osteoporosis and that abnormal bone mineral density was more prominent than normal bone mineral density for both male and female. This study is to present basic data which is indispensible to preparation of senior citizen dominated society afterward, investigating the prevalence rate of osteoporosis.

Successful TAE after DCS for Active Arterial Bleeding from Blunt Hepatic Injury in a Child: A Case Report

  • Transcatheter arterial embolization (TAE) for blunt hepatic injury in children is not common and is especially rare after damage control surgery (DCS). We report a successful TAE after DCS on a child for massive bleeding from the left hepatic artery due to a motor vehicle accident. The car (a sport utility vehicle) ran over the chest and abdomen of a 4-year-old boy. On arrival, initial vital signs were as follows: blood pressure, 70/40 mmHg; heart rate, 149/min; temperature, $36.7^{\circ}C$; respiratory rate, 38/min. After resuscitation, computed tomography was done, and a suspicious contrast leakage from a branch of the left hepatic artery and a spleen injury (grade V) were found. TAE was performed successfully after DCS for a liver injury.

Sarcocystis and Its Complications in Camels (Camelus dromedarius) of Eastern Provinces of Iran

  • The prevalence of Sarcocystis spp. was investigated by gross and histopathological examinations in 250 camels (Camelus dromedarius) slaughtered from 2002 to 2005 in the Mashhad Slaughterhouse, eastern Iran. Samples were taken from the diaphragm, heart, tongue, esophagus and masseter muscles for histopathological studies. No macroscopic sarcocysts were found in the samples at gross inspection. Sarcocysts were detected in 209 of 250 (83.6%) examined camels at histopathological level. The infection rate of the esophagus, heart, masseter muscles, diaphragm, and tongue was 58.8%, 48.0%, 46.8%, 41.6%, and 28.0%, respectively. There was no significant difference in the rate of infection between male (85.8%) and female (81.0%) camels. The tissue response to vital cysts was minimal; however, reaction to the degenerating cysts was severe and caused tissue damages resulting in hyperemia, hemorrhages, mononuclear cell infiltration, necrotic changes, and fibrosis. The wild and domestic carnivores especially dogs may be the final hosts of Sarcocystis spp. in this area.
