시각장애인의 정서 기반 음악-색채 연합에 대한 융복합적 연구 (A Convergence Study on Music-color Association Responses of People with Visual Impairment Mediated by Emotion)

  • 본 연구의 목적은 음악-정서 척도와 음악-색 척도를 사용하여 시각장애인의 음악-색 연합 반응을 살펴보는 것이다. 참여자는 S시와 B시에 소재한 장애인복지관의 서비스를 이용하고 있는 시각장애인 60명(선천성 30명, 후천성 30명)이다. 이를 위해 음악으로 유도될 수 있는 기본 정서 유형 4가지, 행복, 슬픔, 분노, 두려움을 선정하고, 해당 정서를 유도하는 음악에 대한 음악-색 연합 반응을 자기보고식 척도를 사용하여 분석하였다. 그 결과 첫째, 정서 유형에 따른 음악-색 연합 반응에서 행복과 슬픔은 분명한 대조를 보인 반면, 분노와 두려움은 유사한 것으로 확인되었다. 둘째, 음악 정서 변인(정서가, 정서각성, 정서강도)에 따른 음악-색 연합 반응에서 정서가는 정서 유형별 반응과 일치하였고, 정서각성은 부정적인 정서가 높았으며, 정서강도는 행복/슬픔이 분노/두려움에 비해 높았다. 셋째, 장애발생 시기별(선천성, 후천성) 음악-색 연합에는 유의한 차이가 없었다. 이에 본 연구는 시각장애인의 음악-색 연합 반응을 살펴봄으로써, 음악-색 연합 반응이 정서를 매개로 연결 될 수 있음을 밝혔다는 점에서 의의가 있다.

  • 공항 관련 법률에서는 공항 주변에 항공등화의 식별을 방해하거나 조종사와 관제사에게 눈부심을 일으키는 조명의 설치를 제한하도록 하고 있다. 그러나 조명이 조종사 또는 관제사에게 주는 눈부심에 대해서는 정성적인 판단에 기초하여 제한여부를 판단할 뿐, 정량적인 검토는 이루어지지 않고 있다. 이에 이 연구에서는 야간운항 중 공항 주변 대형 조명시설에 의해 조종사와 관제사에게 발생될 수 있는 시각장애현상을 불능글레어와 사물인지도 및 식별방해도를 통해 정량적으로 계산하고 확인하였다. 그리고 이를 바탕으로 조명시설에 의한 조종사와 관제사의 시각장애현상을 방지하기 위해 관련 제도의 보완 등을 제언하였다.

The Effects of Computerized Gaming Program on Cognition in Children with Mental Retardation: A Case Study

  • Purpose: The purpose of this study was to analysis of effectiveness between cognitive function assessment scores and gaming cognitive rehabilitation system in children with intellectual impairment. Methods: Five children (male=5, $age=10.00{\pm}0.80$) with intellectual impairment participated in this study and were randomly assigned to the experiment that played (received) gaming cognitive rehabilitation system (Neuroworld). The children were applied 2 times a week for 30 minutes during 3 months. The children were assessed K-WSIC-VI (Korean-Wechsler intelligence scale for children-fourth edition) and recorded that gained score in gaming cognitive rehabilitation system before and after intervention. K-WSIC-VI contained five primary index scores: verbal comprehension index, visual spatial index, fluid reasoning index, working memory index, and processing speed index. Gaming cognitive rehabilitation system scoring was composed visual recall, target recall, sequence recall, selective attention, continuous attention, and exploration. Results: In the intelligence quotient (IQ) of K-WSIC-VI, there were significant increased in all children. The visual recall item was highest effective in all children. However, sequential recall showed the lowest improvement in all children. The performance speed of selective attention item was decreased, this means that children's skills have improved. Also, their ability to explore has improved significantly. Conclusion: In conclusion, gaming cognitive rehabilitation system was significant effectiveness in cognitive function in some categories for children with intellectual impairment. However, the visual recall and performance speed don't represent of all cognitive function. Therefore, further studies will need to verify by applying more subject and longer duration.

저시력 노인의 시력보조기구 사용에 영향을 미치는 요인에 관한 연구 (A Study on the Factors Affecting the use of Optical Aids by the aged with Low Vision)

  • 본 연구는 저시력 노인의 시력보조기구 사용에 영향을 미치는 요인을 탐색하기 위해 2차년도 한국노동연구원의 고령화연구패널에서 시력으로 일상생활이 어려운 만 65세 이상의 노인 384명을 최종분석에 활용하였다. 분석은 Andersen-Newman의 서비스 이용모델을 기반으로 선행요인, 가능성요인, 욕구요인으로 구분하여 각각의 요인들이 저시력 노인의 시력보조기 사용에 미치는 영향력을 살펴보기 위해 로지스틱 회귀분석(logistic regression)을 실시하였다. 분석결과 선행요인에서는 교육수준이 높고 미취업상태에서, 가능성 요인에서는 주관적 계층의식이 낮을수록, 욕구요인에서는 인지기능이 높을수록 시력보조기구 사용수준이 유의미하게 높아지는 것으로 나타났다. 이러한 결과를 토대로 연구의 제한점과 함의를 제안하였다.

휴대용 전산화 인지훈련 시스템이 경도인지손상이 있는 뇌졸중 환자의 시지각에 미치는 효과 (Effects of a Portable Computerized Cognitive Training System on the Visual-perceptual Function of Stroke Patients with Mild Cognitive Impairment : A Pilot Study)

  • 본 연구는 휴대용 전산화 인지훈련 시스템이 경도 인지손상이 있는 뇌졸중 환자의 시지각에 미치는 효과를 알고자 하였다. 재활병원에 입원 중인 경도인지손상이 있는 12명의 뇌졸중 환자가 본 연구에 참여하였다. 모든 대상자들은 실험군과 대조군으로 6명씩 할당되었다. 모든 대상자들은 실험군 또는 대조군의 훈련을 하루 30분, 주 5회, 4주 동안 수행하였다. 실험군은 휴대용 전산화 인지훈련 시스템을 실시하였으며, 대조군은 보편적인 인지훈련을 수행하였다. 시지각을 측정하기 위하여, MVPT-3(Motor-Free Visual Perception-3)을 측정하였다. 연구 결과, 두 군은 중재 전과 후로 MVPT-3 total score에서 유의한 향상을 보였다(p<.05). 두 군 간 변화량 비교에서 실험군은 대조군보다 MVPT-3 total score와 visual short-term memory of sub-item of MVPT-3에서 유의하게 더 큰 향상을 보였다(p<.05). 이러한 결과는 휴대용 전산화 인지훈련 시스템이 경도인지손상이 있는 뇌졸중 환자의 시지각 능력 향상에 긍정적인 효과를 이끌어 낼 수 있는 중재방법이 될 수 있음을 시사한다.

한의 복합 치료를 통해 개선된 뇌졸중 후 시야 장애 환자 2례 증례보고 (Post-stroke Visual Impairment Treated with Korean Medical Treatment: A Case Series)

  • Objective: This study investigated two cases of Korean medical treatment for visual field impairment after stroke: Case 1, a 56-year-old male with a posterior cerebral artery infarction and right homonymous hemianopsia, and Case 2, a 46-year-old male with an intracerebral hemorrhage in the left parietal lobe and right homonymous hemianopsia. Methods: Case 1 was treated with acupuncture, electroacupuncture, and herbal medicine (Mangeum-tang) for two months, and Case 2 was treated with acupuncture, electroacupuncture, and herbal medicine (Oryeong-san) for 40 days. Results: Following treatment, for Case 1, the Humphrey visual field test showed improvement. The visual field indexes (VFIs) for the left and right eyes improved from 44% to 55% and 49% to 64% respectively, and the mean deviations (MDs) for the left and right eyes improved from -21.11 dB to -19.91 dB and -17.45 dB to -13.89 dB, respectively. The mean visual sensitivities (MVSs) of the left and right eyes also improved from 8.67 dB to 11.33 dB and 1.67 dB to 9.67 dB, respectively, with no side effects. For Case 2, the VFI for the left eye improved from 36% to 64% and that for the right eye remained unchanged. The MDs for the left and right eyes also improved from -22.02 dB to -14.47dB and -22.11 dB to -21.34 dB, respectively, with no side effects. Conclusions: This study suggests that Korean medical treatment may improve visual impairment after stroke, but further research is needed.

A Systematic Review on the Management of Cortical Visual Impairment

  • Objective: Cortical Visual Impairment (CVI) is a leading cause of pediatric blindness and the most common form of pediatric visual disability, particularly prevalent among children with cerebral palsy (CP). This systematic review synthesizes the latest research on various interventions for managing CVI, focusing on studies published in the last decade. Design: A systematic review Methods: A comprehensive search was conducted in March 2024 across several databases including MEDLINE, CINAHL, Embase, and Web of Science. Studies were selected based on inclusion criteria set under the PICOSD framework and were limited to those involving human subjects, published in English, and conducted within the past ten years. The selected studies included randomized controlled trials, observational studies, and case reports focusing on rehabilitation, therapy, and surgical interventions for CVI. Results: Out of 221 studies screened, 5 met the inclusion criteria and were reviewed in detail. These studies covered a range of interventions including physiotherapy, sensory integration training, visual training programs, neuromotor rehabilitation, and surgical procedures aimed at improving visual function and overall quality of life for CVI patients. Conclusions: The studies demonstrate the potential benefits of structured, early intervention programs that incorporate family involvement and are tailored to the unique needs of children with CVI. However, there remains a significant need for further research to establish evidence-based practices in this field.

Visual Outcome after Transsphenoidal Surgery in Patients with Pituitary Apoplexy

  • Objective : Pituitary apoplexy is one of the most serious life-threatening complications of pituitary adenoma. The purpose of this study is to investigate the visual outcome after early transsphenoidal surgery for the patients with pituitary apoplexy. Methods : We retrospectively reviewed the 31 patients with pituitary apoplexy who were admitted due to acute visual acuity or field impairment and treated by transsphenoidal surgery. Five patients were excluded because of the decreased conscious level. The visual acuity of each individual eye was evaluated by Snellen's chart Visual fields were also checked using automated perimetry. To compare the visual outcome according to the surgical timing, we divided the patients into 2 groups. The first group, 21 of the patients have been undertaken transsphenoidal approach (TSA) within at least 48 hours after admission. The second group included 8 patients who have been undertaken TSA beyond 48 hours. All patients were monitored at least 12 months after surgery. Results : Patients were 21 males and 8 females (M : F=2.6 : 1) with the mean age of 42.4 years. Among the enrolled 29 patients, 26 patients presented with decreased visual acuity and 23 patients revealed the defective visual field respectively. Postoperatively, improvement in the visual acuity was seen in 15 patients (83.3%) who underwent surgery within the first 48 hours of presentation, as compared to those in whom surgery was delayed beyond 48 hours (n=5; 62.5%) (p=0.014). Improvement in the visual field deficits was observed in 15 (88.2%) of patients who had been operated on within the first 48 hours of presentation, as compared to those in whom surgery was delayed beyond 48 hours (n=3; 50.0%) (p=0.037). Conclusion : This study suggests that rapid transsphenoidal surgery is effective to recover the visual impairment in patients with pituitary apoplexy. If there are associated abnormalities of visual acuity or visual fields in patients with hemorrhagic pituitary apoplexy, early neurosurgical intervention within 48 hours should be also required to recover visual impairment.
