Effect of Gocho(苦椒) Extract Mesotherapy on Regional Fat Loss in Obese Korean Women

  • Objectives: The purpose of this study was to determine whether gocho (苦椒) extract mesotherapy can effectively reduce weight and abdominal fat in obese patients. Methods: Forty obese participants were recruited and randomly assigned to two groups. Five participants dropped out voluntarily during the course of the study and four men were excluded from the analysis to avoid gender bias. Results are presented on a total of thirty-one women. Over an eight-week period, the treatment group (n=13) received gocho (苦椒) extract injections (100mcg/3cc) in the abdominal skin, twice per week; the control group (n=18) received normal saline injections (3cc). A mixed lidocaine and prilocaine cream (2.5%/2.5% EMLA cream) was applied before the injection. At baseline and 8 weeks, body weight, body-mass index (BMI), waist circumference (WC), waist-hip ratio (WHR), abdominal fat, energy expenditure, and questionnaires (eating attitudes and physical activity) were evaluated in both groups. Results: Reductions in body weight (p<0.005), BMI, and WC (p<0.05) were greater in the treatment group. WHR, total fat area, and visceral fat area decreased only in the treatment group (p<0.05). Resting metabolic rate (RMR) change was correlated with weight loss only in the control group (r=-0.498, p<0.05). Before and after the treatment, there were no significant differences between the two groups in questionnaire variables (p>0.05). Conclusion: his study suggests that abdominal gocho (苦椒) extract mesotherapy might be an effective way to promote weight and abdominal fat loss in obese Korean women.

Prediction of Suitable Site to Measure Abdominal Skin Fold Thickness and Correlation among Obesity Indicators in Patients with Type 2 Diabetes Mellitus (제2형 당뇨병 환자에서 피부두겹두께의 측정부위 예측 및 비만지표들 간의 관련성)

  • Purpose: The purpose of this study was to predict measuring site suited for abdominal skin fold thickness (ASFT) by measuring the distribution of abdominal subcutaneous fat thickness (AScFT) and ascertain the correlations among obesity indicators. Methods: The size of analysis materials was 124 secondary data measured by ultrasonic device, bioelectrical impedance analyzer and caliper. Data were analyzed using t-test, and Pearson's correlation. Results: The average of AScFT was 10.63± 6.79mm with its range 1.39-36.16 mm, and AScFT of female and of central parts were thicker than those of male and outer parts in the abdomen. The average of ASFT was 29.26±12.59 mm. Site 5 on Figure 1 was most similar to the average of AScFT in both sexes. Body mass index (BMI) and waist hip ratio (WHR) were 23.65±3.98 and 0.88±0.05 respectively. The body weight, BMI, WHR, visceral fat, ASFT vs AScFT revealed in significant correlation (r= .29, r= .55, r= .39, r= .33. r= .29). Conclusion: BMI and WHR seem more useful than other obesity indicators, when obesity control is necessary for Type 2 diabetes patients. Site 5 on Figure 1 is most suitable site to measure ASFT.

The Study of Relationships between tile Abdominal Fat Distribution and Sasangin (허리둘레로 평가한 복부 비만의 사상인별 차이 분석)

  • Objectives : Waist circumference (WC) is the best simple anthropometric index of abdominal visceral adipose tissue accumulation and obesity-related health risks. We measured WC and other anthropometric indices to assess relationships between the abdominal fat distribution and Sasang constitution. Methods : 196 participants who completed a QSCC II were measured some anthropometric indicies - height, weight and WC. Collected data were stratified by sex and analysed by oneway analysis of variances among constitutional groups. Results : In men, there were significant differences in weight, BMI and WC among Constitutional groups (p<0.001). In women, there were significant differences in weight and BMI, but there were no significant difference in WC among Constitutional groups. Conclusions : There was a significant relationship between taeumin men md abdominal chesty. But, in women, there was no clear relationship between abdominal obesity and participant's constitution.

The Relationship between Epicardial Fat Thickness and Dampness-Phlegm Pattern in the Patients with ischemic stroke

  • Objectives: Epicardial fat is true visceral fat that is known to be associated with metabolic syndrome, high abdominal fat, insulin resistance, coronary artery diseases, low coronary flow reserve and subclinical atherosclerosis. Dampness-Phlegm pattern is one of the pattern diagnosis of traditional Korean medicine. Previous studies showed that Dampness-Phlegm pattern is associated with hypertension, dyslipidemia, metabolic syndrome. This study is intended to find association between Dampness-Phlegm pattern and epicardial fat thickness. Methods: This study was a community-based single center trial. Ischemic stroke patients within 30 days after their ictus were enrolled. Epicardial fat thickness was measured using transthoracic echocardiography. Other measured and obtained variables are medical history, weight, height, body mass index, fasting blood glucose, cholesterol, triglycerol, high density lipoprotein, lipid and low density lipoprotein. Results: Three hundred sixty six were enlisted, and one hundred forty were diagnosed with the Dampness-Phlegm pattern. Dampness-Phlegm pattern group had significantly thicker epicardial fat. Binary logistic regression also showed statistically significant result. Conclusions: This study showed close association between epicardial fat and Dampness-Phlegm pattern. This result suggests a clue to standardization of pattern identification.

Effect of Gentro $F^{(R)}$ on Abdomen Fat (젠트로 $F^{(R)}$의 복부지방에 미치는 영향)

  • The participants were recruited 77 healthy adult persons aged between 20 and 50 who have BMI above $25kg/m^2$ in this study. All subjects were randomly assigned to the Gentro $F^{(R)}$ drinking group and non-drinking group. We were investigated about abdomen fat decreasing effect of Gentro $F^{(R)}$, distillate of pepper. Total fat area (TFA), subcutaeneous fat area (SFA) and visceral fat area (VFA) and subcutaneous fat area/visceral fat area ratio (SVR) has been assessed by obesity index (BMI, body fat percent, waist circumference), CT scan taken on the $L4{\sim}5$ position, umbilicus level and blood analysis evaluated during 3 month. The obesity indexes were a little decreased in two groups. However, the waist circumference (WC) was decreased about 5% in drinking group after 3 month and there was significant difference in the change 2 and 3 between two group. TFA and VFA were significantly decreased in the drinking group compare to the non-drinking group (p<0.05) and in umbilicus and $L4{\sim}5$ position, were $370.33{\pm}92.30,\;380.35{\pm}97.64\;and\;114.90{\pm}44.91,\;101.99{\pm}41.24$, respectively. These result means that Gentro $F^{(R)}$ is effective on abdomen fat decreasing. Total cholesterol were decreased without difference in both groups. Lipid factors (TG, HDL- and LDL-cholesterol) were decreased without significance. The mea surement of SVR taken on the $L4{\sim}5$ position were more significantly correlated with obesity index that BMI was 0.787, body fat percentage was 0.754 than on the umbilicus level. The TFA and VSA was correlated with ALP, ASP, Albumin, Insulin and the SVR was correlated with the indexes of liver function and lipid factor found in the blood. Therefore, it is conclude that Gentro $F^{(R)}$ drinking leads to a decrease in abdominal obesity by reducing waist circumference and visceral fat area.

  • The purpose of this study was to investigate the effects of modified fasting therapy using Gamrosu and herbal medicine on the changes of body compositions in women. We analyzed the medical records of 11 patients, who carried out modified fasting therapy at Dunsan Korean Medicine Hospital of Daejeon University. Body compositions (body weight, body mass index, body fat mass, skeletal muscle mass, basal metabolic rate, extracellular water ratio, abdominal circumference, thickness of abdominal subcutaneous fat, visceral fat area) were reviewed before and after modified fasting therapy. The body composition values (body weight, body mass index, body fat mass, skeletal muscle mass, basal metabolic rate, extracellular water ratio, abdominal circumference, thickness of abdominal subcutaneous fat, visceral fat area) decreased after modified fasting therapy. Results from this investigation showed that modified fasting therapy using Gamrosu and herbal medicine has positive effects on changes of body compositions.

  • This thesis estimates relationships between area and volume of visceral adipose tissue about health risk. Visceral adipose tissue quantity showed the quantitative relationship, where degree of area was similar to the volume.Threrfore, it is more useful to use the area which is simpler,than the volume when using visceral adipose tissue quantity for estimating the degree of danger factors related with fatness.

Efficacy of Electroacupuncture using an Insulated Needle in Adults with Abdominal Obesity: A Pilot Study

  • Objectives : This study evaluated the efficacy of electroacupuncture therapy using an insulated needle in adults with abdominal obesity. Methods : This study was a randomized, double-blind, parallel-designed pilot trial. Sixteen participants eligible according to the inclusion and exclusion criteria were randomly divided into an insulated needle group and a control group. Insulated or common needles were inserted at acupoints located on the abdomen (CV12, CV6, ST25, ST27, SP15) and were electrically stimulated for 30 minutes (16 Hz, within tolerable strength). A total of 10 sessions of treatment were performed twice per week for 5 weeks. All participants were requested to maintain their usual diet and lifestyle. The outcome measures were waist circumference (WC), waist-to-hip ratio (WHR), and abdominal computed tomography (CT) of the total fat area (TFA), subcutaneous fat area (SFA), and visceral fat area (VFA). Results : A total of 12 participants divided into the insulated needle group (n = 5) and the control group (n = 7; common needle) were treated for 10 sessions and analyzed per-protocol (PP). WC decreased significantly after 10 sessions in both groups. The WC, TFA, SFA, and VFA of abdominal CT in the insulated needle group decreased more than in the control group; however, there were no significant differences in any parameter between the insulated needle group and the control group. Patients in the insulated needle group were more strongly stimulated with electrical stimulation than patients in the control group. Conclusion : Electroacupuncture using insulated needles in adults with abdominal obesity might be a more effective treatment than common needles. Additional studies are required to compensate for the limitations of this pilot study and to verify the results and efficacy.

Effects of Nutrition and Exercise Education on Fat Mass and Blood Lipid Profile in Postmenopausal Obese Women (폐경 후 비만 여성의 식이 및 운동 교육이 체지방향 및 혈중지질농도에 미치는 효과)

  • Obesity is an independent risk factor fur coronary artery disease in the postmenopausal women, which may be mediated by alteration of blood lipid metabolism. We are aimed to evaluate the effects of low energy diet, restriction of high fat foods and exercise education on weight, fat mass and blood lipid profile. Fifteen postmenopausal obese women were studied. Subjects received detailed advice about how to achieve a reduction of weight by a low-energy diet (1,200kcal), restriction of high fat food and increased exercise in every 4 wks during 8 wk. To evaluate the effectiveness for education programs, dietary fat habit and daily nutrient intakes, exercise were tested before and after intervention. Anthropometry, computerized tomography, and blood lipid profile were assessed before and after intervention. According to the nutrition education, energy intake (from 1776.1 ${\pm}$ 28.2 kcal to 1268.7 ${\pm}$ 115.2 kcal, p < 0.001) and percent of energy from fat (21.9%, p < 0.01) were significantly decreased. However, the index of nutritional quality was over 1.0. On the contrary, exercise (from 341.3 ${\pm}$ 222.1 kcal to 569.4 ${\pm}$ 309.8 kcal, p < 0.05) was increased and dietary fat habit scores (from 30.8 ${\pm}$ 5.2 to 36.1 ${\pm}$ 3.0, p < 0.01) were improved. Also, body weight (4.2%, p < 0.001), BMI (4.5%, p < 0.001), body fat mass (3.4%, p < 0.05) and waist to hip ratio (1.1%, p < 0.05) were decreased. Among abdominal fat, visceral fat (26.1%, p < 0.05) and subcutaneous fat (14.8%, p < 0.01) were decreased. But there was no difference in visceral fat to subcutaneous fat ratio. We observed HDL-cholesterol increase (11.7%, p < 0.05), triglyceride reduction(14.8%, p < 0.05) and atherogenic index improvement (from 2.7 ${\pm}$ 0.7 to 2.3 ${\pm}$ 0.7, p < 0.05) However, there was no change in LDL-cholesterol and total cholesterol. Theses results showed that low energy diet, restriction of high fat foods and exercise education could result in reduction of body weight, fat mass, visceral and subcutaneous fat, and in improvement of blood lipid profile in the postmenopausal obese women.

Associations between 24-hour Urine Sodium Excretion Level and Obesity-related Metabolic Risk Factors (비만인과 정상인에서 24시간 소변 내 나트륨 배출량과 비만관련 대사위험지표의 관련성)

  • Objectives: Excess sodium intake has been linked to obesity and obesity-related indices. However, the scientific evidence for this association is inadequate. The purpose of this study was to investigate the association between urinary sodium excretion and obesity-related indices among Korean adults. Methods: A convenience sample of 120 subjects (60 obese and 60 non-obese subjects) were recruited applying frequency matching for sex and age between two groups. Sodium intake level was assessed through 24-hour urine collection. Obesity-related metabolic risk factors, including fasting blood lipid indices, subcutaneous and visceral fat through computed tomography (CT), insulin resistance indices, blood pressure and liver enzymes were measured in all subjects. These obesity-related metabolic risk factors were compared between obese and non-obese group according to sodium excretion levels (<110 mEq/day, 110~180 mEq/day, >180 mEq/day). Results: After adjusting for age, gender, health behaviors (smoking, exercise, drinking), and energy intake, several obesity-related metabolic risk factors, including abdominal circumference, body fat percentage, subcutaneous and visceral fat, triglyceride, and systolic blood pressure were found to be significantly deteriorated as the sodium excretion level increases. In addition, multivariate adjusted-odds ratios of abdominal obesity, high blood triglyceride, and high blood pressure were found significantly higher in the highest sodium excretion group compared to the lowest group. The mean number of metabolic syndrome risk factors was also significantly greater in the highest sodium excretion group than in the lowest group. Conclusions: The current study findings suggested that high sodium intake can affect obesity and metabolic syndrome risk negatively, implying the necessity of future research on low-sodium diet intervention in relation to obesity and related health problems.
