흉추에서의 경척추경 접근법을 통한 경피적 척추체성형술 및 척추체 후만변형 복원술에 관한 후향성 연구 (Retrograde Study on Thoracic Percutaneous Vertebroplasty and Kyphoplasty Done by Transpedicular Approach)

  • Background: Transpedicular percutaneous vertebroplasty, along with kyphoplasty of the thoracic vertebrae, is technically more difficult than those of the lumbar vertebrae due to the anatomical differences. During the last four years, all the percutaneous vertebroplasty and kyphoplasty of the thoracic vertebrae carried out at our institution were performed using a transpedicular approach; therefore, we tried to find if there were any problems or complications associated with the process. Methods: The medical records of all the patients who had undergone thoracic percutaneous vertebroplasty or kyphoplasty were retrospectively reviewed. The following were looked up: the procedure name, unipedicular or bipedicular, the level of the thoracic vertebrae treated, and the pre- and postoperative changes in the Visual Analog Scales (VAS), the volume of cement injected and complications. Results: In the last four years, 58 vertebral bodies in 58 patients were treated. Twelve and 46 vertebral bodies were treated by kyphoplasty and vertebroplasty, respectively. A total of 58 mid and lower thoracic levels were treated: T5 (n=1), T6 (n=1), T7 (n=3), T8 (n=4), T9 (n=1), T10 (n=4), T11 (n=14) and T12 (n=30). The mean preoperative and postoperative VAS scores were $8.1{\pm}1.4$ and $5.2{\pm}1.7$, respectively. The mean volume of cement injected was $4.01{\pm}1.85ml$; $3.18{\pm}0.60ml$ at T5-8 and $4.22{\pm}2.27ml$ at T9-12. There were no clinical complications, such as pedicular fracture or cement leakage. Conclusions: Although transpedicular vertebroplasty and kyphoplasty at the mid to lower thoracic vertebral bodies is technically difficult compared to that at the lumbar region, the procedures can be performed safely.

Preliminary Report of Three-Dimensional Reconstructive Intraoperative C-Arm in Percutaneous Vertebroplasty

  • Objective : Percutaneous vertebroplasty (PVP) is usually carried out under three-dimensional (2D) fluoroscopic guidance. However, operative complications or bone cement distribution might be difficult to assess on the basis of only 2D radiographic projection images. We evaluated the feasibility of performing an intraoperative and postoperative examination in patients undergoing PVP by using three-dimensional (3D) reconstructive C-arm. Methods : Standard PVP procedures were performed on 14 consecutive patients by using a Siremobil Iso-$C^{3D}$ and a multidetector computed tomography machine. Post-processing of acquired volumetric datasets included multiplanar reconstruction (MPR) and surface shaded display (SSD). We analyzed intraoperative and immediate postoperative evaluation of the needle trajectory and bone cement distribution. Results : The male : female ratio was 2 : 12; mean age of patients, 70 (range, 77-54) years; and mean T score, -3.4. The mean operation time was 52.14 min, but the time required to perform and post-process the rotational acquisitions was 7.76 min. The detection of bone cement distribution and leakage after PVP by using MPR and SSD was possible in all patients. However, detection of the safe trajectory for needle insertion was not possible. Conclusion : 3D rotational image acquisition can enable intra- or post-procedural assessment of vertebroplasty procedures for the detection of bone cement distribution and leakage. However, it is difficult to assess the safe trajectory for needle insertion.

  • 본 논문에서는 골다공증성 척추압박골절에 대한 척추성형술시 골강화제인 PMMA 주입방법을 제안하였다. 척추성형술의 정량적 분석을 위해서는 유한요소해석을 사용하였으며 1m간격의 척추체 CT를 바탕으로 보다 정교한 척추체 모델을 제시함으로써, 단순화된 척추의 기하학적 형상과 간단한 재료 물성치를 사용하는 기존의 연구의 한계를 극복하고자 하였다. 본 연구 결과를 토대로 추체내 망상골에 주입되는 PMMA의 주입형태, 주입위치를 정량적으로 평가하여 이상적인 수술방법을 제시하였다. PMMA의 구입위치는 망상골의 앞쪽-위쪽에 많은 양이 집중하여 주입되었을 때, 가장 효율적으로 추체를 보완해 주는 것으로 나타났다.

척추 전이암 및 골수종 환자를 대상으로 시행한 경피적 척추체 성형술 (Percutaneous Vertebroplasty in Spinal Metastasis and Myeloma : 25 Cases Experience)

  • Objectives : In spinal metastasis and myeloma, percutaneous vertebroplasty could be an effective treatment method to provide spinal stabilization and to relief pain for early rehabilitation. The authors report twenty-five cases the clinical results of percutaneous vertebroplasty for twenty-five cases of spinal metastasis and myeloma. Materials and Methods : From September 1998 to December 1999, seventy percutaneous vertebroplasties(PVP) were performed for spinal metastases and myeloma in 25 patients, sixteen women and nine men ranging in age from 34 to 74. The primary malignancies were 6 multiple myelomas, and in metastatic tumore from various origin. All patients complained of severe pain and had osteolytic vertebral body destructions without spinal cord compression. To evaluate clinical improvement, suObjective verbal analogue pain score(VAS) and Karnofsky performance scale(KPS) were used. Thin sliced(2mm-thickness) sectional computed tomography(CT) was performed before and after PVP. Plain X-ray film was followed up every 1 month to assess the vertebral column stability. Results : In 25 patients, a total of seventy PVPS were performed successfully : 6 cervical, 33 thoracic and 31 lumbar vertebrae. Most patients had clear improvement of pain after PVP ; mean as score was 8.1 and 2.9 before and after PVP, respectively. Improvement was maintained in most patients. No further collapse of treated vertebrae was observed(mean follow-up, 7 months). Leakage of PMMA was notod in the spinal canal(13 levels), neural foramen (2 levels), adjacent disk(15 levels), paravertebral soft tissue(14 levels) and vein(8 levels). Pulmonary embolism was detected in three patients after the procedure, but was not associated with clinical symptoms. Conclusion : These results indicate that percutaneous vertebroplasty can be valuable treatment method in osteolytic spinal metastasis and myeloma, providing immediate pain relief and spinal stabilization and contributing to early rehabilitation.

Vertebroplasty Utilizing Percutaneous Vertebral Body Access (PVBA) Technique for Osteoporotic Vertebral Compression Fractures in the Middle Thoracic Vertebrae

  • Objective : Percutaneous approach to the middle thoracic vertebra through the transpedicular route for the patients with osteoporotic vertebral compression fractures is difficult due to the small size of the pedicle and parasagittally oriented vertebra body anatomy. The percutaneous vertebral body access [PVBA] technique utilizing the posterolateral extrapedicular approach avoids the pedicle and provides direct access to the vertebral body. The objective of this study is to evaluate the efficacy of the vertebroplasty utilizing PVBA technique for osteoporotic vertebral compression fractures in the middle thoracic vertebrae. Methods : A retrospective review was done on 20 patients who underwent vertebroplasty utilizing PVBA technique performed for painful osteoporotic compression fracture in the middle thoracic vertebrae at 22 levels from May 2003 to June 2006. The average amount of the injected cement was 1.5-2.5ml. The postprocedural outcome was assessed using a visual analogue scale [VAS]. Results : The treated vertebrae were T5 [1 level], T6 [5 levels], 17 [7 levels], and T8 [9 levels]. The compression rate and kyphotic angle were improved after procedure from $18%{\pm}13.4$ to $16%{\pm}13.8$ [p > 0.05] and from $6.9^{\circ}{\pm}6.7$ to $6.6^{\circ}{\pm}6.2$ [p>005], respectively. Preprocedural VAS was $8.2{\pm}0.70$ and was decreased to $2.1{\pm}1.02$ [p < 0.01] after treatment. Postprocedural cement leakage was noted in 3 levels [13.7%]. There were no cases of leakage to epidural space or neural foramen, segmental artery injury, and pneumothorax. Conclusion : These results suggest that the complication rates are low and good results can be achieved with vertebroplasty utilizing PVBA technique for the osteoporotic vertebral compression fractures especially in the middle thoracic vertebrae.

The Effectiveness of Osteoplasty System (Vertebroplasty using Large Cannula-low Pressure Delivery System) in Compression Fracture and other Spinal Pathology

  • Objective : The vertebroplasty is an excellent procedure in spine compression fracture, but there are some problems such as cement leakage and difficulties in bone biopsy. Recently, the osteoplasty system is developed to solve these problems, so we will report the usefulness of the osteoplasty system. Methods : From January 2003 to November 2003, there are 9patients with simple osteoporotic spine compression fracture, 2 compression fracture patients combined with suspicious spinal metastasis, 1patient with suspicious primary bone tumor, and 2patient with infection on spine. All patients were treated using the osteoplasty system. Results : All 11 compression fracture patients were relived the back pain after the osteoplasty and there is no complication. The bone biopsies in 3 suspicious cancer patients were also effectively performed using the osteoplasty system; negative result in 2patients and positive result in 1patient. The culture result of spontaneous discitis was no growth for 48hours. The spine tuberculosis was confirm using the osteoplasty system. Conclusion : The osteoplasty system has distinguished advantages in comparison with the vertebroplasty. That is, the risk of cement leakage is lower than vertebroplasty because of low pressure delivery system. And we can obtain the specimen effectively in bone biopsy because of large cannula. In conclusion, we emphasize that the osteoplasty system is a more useful procedure in spine compression fracture especially in the patient needed bone biopsy for diagnosis.

  • 경피적 척추성형술은 PMMA를 붕괴된 척추체에 경피적으로 주사하여 통증을 완화시키고 척추체를 구조적으로 보강하는 시술이다. 척추성형술은 아주 효과적인 치료방법이기는 하나 합병증이 전혀 없지는 않다. 척추성형술의 많은 합병증은 주로 PMMA 누출과 관련되어 있다. 대부분의 누출은 증상이 없고 또한 폐색전증은 매우 드물지만 시술도중 PMMA의 혈관내 이동을 조기에 발견하지 못할 때 발생할 수 있다. 저자들은 천식으로 인한 기침으로 내원한 환자의 흉부X선에서 우연히 PMMA에 의한 폐색전증을 진단하였기에 문헌 고찰과 함께 보고하는 바이다.

경피적 척추성형술 후 발생한 경막외농양 -증례 보고- (Epidural Abscess after Percutaneous Vertebroplasty -A case report -)

  • Although the incidence of epidural abscess is rare, once it occurs, its high morbidity rate and high mortality rate create a great deal of serious sequalae for these patient, if this condition is not diagnosed in time. We experienced a case of epidural abscess after performing percutaneous vertebroplasty in a patient who had a lumbar spinal compression fracture. This case will remind the pain clinician of the possibility of epidural abscess after such a procedure.

  • The compressed fracture of spine caused by osteoporosis is one of the most frequent diseases in bone fracture. Recently the vertebroplasty has drawn much attention as a medical treatment for the compressed fracture of spine, which strengthens the vertebral body and corrects deformity, and relieves pain in patients by injecting bone cement. The purpose of this research is to provide the theoretical background for the vertebroplasty by employing scientific methods and thus to find out the improvements of the conventional surgical method. By applying the engineering analysis tools to clinical study, new application fields can also be obtained

골 시멘트 중합 비율 변경이 척추성형술 치료에 미치는 영향에 대한 비교 분석 (A Micro Finite Element Analysis on Effects of Altering Monomer-to-Powder ]Ratio of Bone Cement During Vertebroplasty)

  • 골다공증이란 골량의 감소에 의해 야기되며. 해면골 미세구조의 골밀도가 감소하는 질병이다. 약물치료(부갑상선 호르몬)법은 골소주의 두께 및 골의 강도를 어느 정도 증가시킬 수 있는 호르몬 치료법이다. 척추성형술은 골다공성 척추 압박 골절의 치료를 위하여 척추 해면골에 주사기를 통하여 골 시멘트를 주입하는 최소 침습적 수술법이다. 임상적으로 골 시멘트의 물성치에 영향을 미치더라도 점성 감소와 주입 시간 확보를 위해 중합비율을 변경하여 사용한다. 본 연구의 목적은 골 시멘트 중합비율 변경에 따른 척추 해면골의 역학적 특성을 알아보고자 한다. 본 논문에서 는 수정된 보로노이 도형을 이용하여 척추 해면골의 유한요소 모델을 생성하고, 골 시멘트의 중합비율(0.40~l.07$m\ell$/g)에 따른 척추 해면골의 강도회복 정도를 비교하였다. 또한 골다공증 치료에서 호르몬 치료와 골 시멘트 치료의 상대적인 영향을 비교 분석하였다. 분석결과 골 시멘트 치료모델의 탄성계수와 강도는 정상 상태의 약 50%로 회복되었으며, 이 값은 호르몬 치료 모델의 약 2배이다. 0.53$m\ell$/g의 중합비율에서 골 시멘트의 탄성계수와 강도가 최고이며 1.07$m\ell$/g의 중합비율에서 최소의 탄성계수와 강도(각각 42%와 49%)이지만 약물 치료보다는 더 효과적임을 알 수 있다. 척추성형술시 제작사에서 추천하는 중합비율과 다른 비율을 사용할 경우 골 시멘트의 물성치 감소뿐만 아니라 환자의 골 밀도에 따라서 해면골의 강도회복에 문제가 발생할 수 있다는 것을 알 수 있었다.
