  • 지네는 머리와 체절이라고 부르는 수많은 마디가 이어진 형태를 이루는 절지동물목, 지네강에 속하는 동물로, 전 세계적으로 3,000여종, 국내에는 44종(4종 7과)이 분포하는 것으로 알려져 있다. 그중 왕지네는 유효물질 탐색 및 약리활성 연구를 위한 주요 생물자원으로 인식되고 있어 향후 그 가치는 더욱 높아질 것으로 전망되고 있다. 하지만 현재까지 이에 대한 생리적·생태학적 연구는 미비하여 이에 대한 연구를 확대할 필요가 있다. 본 연구에서는 왕지네의생태학적·생리학적 특성 연구에 대한 기초 연구로 왕지네에 대한 형태적 분석을 통한 분류학적 위치를 정립하고, 암·수 구별법을 제시하였다. 이것은 향후 왕지네 인공사육을 위한 기초자료로 활용될 수 있을 것으로 기대되며, 이를 통해 왕지네 시장 확대에 기여할 수 있을 것으로 예상된다.

약물 중단에도 지속되는 약인성 파킨슨증후군 환자의 한의 치험 1례 (A Case Report of Persistent Drug-Induced Parkinsonism After Drug Discontinuation)

  • This study examined the effectiveness of Korean medical treatment in a patient with persistent drug-induced parkinsonism after drug discontinuation. The changes in symptoms were assessed using the unified parkinson's disease rating scale (UPDRS), postural instability-gait disturbance (PIGD) score, and the 20 m gait time and steps. After 22 days of hospitalization, the UPDRS, PIGD score, and 20 m gait time and steps showed clinically significant improvement. The improvement persisted after discharge. This study indicated that Korean medical treatment could be an effective alternative therapy for treating persistent drug-induced parkinsonism after drug discontinuation.

국내 응급 센터의 뱀교상 환자의 특징: 국가응급의료정보망 (Epidemiology of patients with snake bite or envenomation in emergency department: NEDIS (National Emergency Department Information System))

  • Purpose: This study utilizes the NEDIS (National Emergency Department Information System) database to suggest a predictive model for snakebite and envenomation in Korea by analyzing the geographical distribution and seasonal variation of snakebite patients visiting the ER. Methods: This was a retrospective study on snakebite patients visiting the ER using the NEDIS database from January 2014 to December 2019. The subjects include patients with the KCD (Korea Standard Classification of Disease) disease code T63.0 (Toxic effect of contact with snake venom). Geographical location, patient gender, patient age, date of ER visit, treatment during the ER stay, and disposition were recorded to analyze the geographical distribution and seasonal variation of snakebite patients in Korea. Results: A total of 12,521 patients were evaluated in this study (7,170 males, 54.9%; 5,351 females, 40.9%). The average age was 58.5±17.5 years. In all, 7,644 patients were admitted with an average admission time of 5.04±4.7 days, and 2 patients expired while admitted. The geographical distribution was Gyeongsang 3,370 (26.9%), Cheonra 2,692 (21.5%), Chungcheong 2,667 (21.3%), Seoul Capital area 1,999 (16.0%), Kangwon 1,457 (11.6%), and Jeju 336 (2.7%). The seasonal variation showed insignificant incidences in winter and higher severity in spring and summer than in fall: winter 27 (0.2%), spring 2,268 (18.1%), summer 6,847 (54.7%), and fall 3,380 (27.0%). Conclusion: Patients presenting with snakebites and envenomation in the emergency room were most common in the Gyeongsang area and during summer. The simple seasonal model predicted that 436 snakebites and 438 envenomation cases occurred in July and August. The results of this study can be applied to suitably distribute and stock antivenom. Appropriate policies can be formed to care for snakebite patients in Korea.

Research Trends of Pharmacopuncture: a bibliometric analysis using VOSviewer (2007-2023)

  • Objectives: Pharmacopuncture is considered a unique new type of acupuncture in Korean medicine (KM). This study was performed to investigate the research trends regarding pharmacopuncture through a bibliometric analysis using VOSviewer. Methods: The Web of Science Core Collection was searched for this study. The search terms were pharmacopuncture and pharmaco-puncture. The search was conducted on May 23, 2023, and 185 studies retrieved up to this date were included. Software VOSviewer version 1.6.19 was used to perform the bibliometric analysis. After data clearing, keyword co-occurrence and cooperation pattern analyses were performed. Results: The number of studies on pharmacopuncture have gradually increased since the mid-2000s, with the most published in 2020 (46 studies). The quality of the studies has also steadily increased, and recently, several randomized controlled trials of pharmacopuncture have been conducted. Moreover, this treatment has been investigated in the context of KM clinical practice guideline development in Korea. The scope of pharmacopuncture studies to date seems to be limited to musculoskeletal pain and bee venom pharmacopuncture. The researchers from Jaseng Medical Foundation have been the core researchers in the field of pharmacopuncture in Korea. They collaborated with researchers from universities or government-funded research institutes. However, small-scale research teams from KM clinics and university researchers were also identified as independent clusters. Conclusion: Pharmacopuncture research has grown both quantitatively and qualitatively, but the scope needs to be further expanded. There is also a gap between the use of pharmacopuncture by KM doctors in clinical settings and pharmacopuncture research. Based on these findings, some suggestions for future studies in this field are discussed. Furthermore, these findings may be used as data to expand and globalize pharmacopuncture research in the future.

A Web-based Survey Research on Clinical Practice Patterns of Korean Medicine Treatment for Degenerative arthritis of hip and finger joints

  • Objectives: This study aimed to understand the current status of clinical practice patterns of Korean medicine in the treatment of degenerative arthritis of the hip and finger joints using web-based survey. Methods: An e-mail questionnaire survey was conducted with the Korean Medicine Doctor(K.M.D) registered in the Korean Medicine Association. The survey data were analyzed using Microsoft Excel. Results: Although the response rate of the survey was low, the characteristics of the respondents generally reflected the characteristics of the study population. A total of 35.2% of the respondents had heard of the guidelines; however, they did not use them for medical treatment. The reason as answered by 32.8% of the respondents was "not enough treatment time to check the manual and apply it." More than half (78.80%) answered that they were treating patients with both degenerative hip and finger arthritis; however, the average monthly number of first-time patients was ≤5. A total of 379 (60.9%) respondents answered that "acupuncture is very important," which was the only one that exceeded the majority compared to other treatments. As a result of the safety investigation, "Acupuncture, herbal medicine, cupping" were considered high in safety, while "Pharmacopuncture and Bee venom acupuncture" were considered low in safety. Conclusions: This survey helped to determine the current clinical practice patterns of Korean medicine treatments, further providing basic data for CPGs for degenerative arthritis of the hip and finger joints.

Tolosa-Hunt Syndrome으로 인한 통증성 안구운동 장애 치험 1례 (A Case of Painful Ophthalmoplegia Patient Diagnosed as Tolosa-Hunt Syndrome)

  • ■ Objectives The purpose of this clinical study is to evaluate the effect of Traditional Korean Medicine(KM) on a patient with Tolosa-Hunt syndrome. ■ Methods We treated a 58 years old male patien t with Tolosa-Hunt syndrome who had several clinical symptoms such as ocular pain, limitaion of ocular movement and unilateral ptosis. We used herbal medication, acupuncture, electro-acupuncture(EA), cupping, moxibustion, bee-venom injection. Then we evaluated the improvement by Numerical Rating scale(NRS), Distance of eye opening and ratio of pupil movement. ■ Results We demonstrated the decrease of pain score and improvement of equal ocular movement(EOM) and ptosis after the KM treatment. ■ Conclusion This study proved the effect of KM treatment on Tolosa-Hunt syndrome. It could cooperative treatment would be more effective than mono treatment of western medicine.

Melittin induces autophagy to alleviate chronic renal failure in 5/6-nephrectomized rats and angiotensin II-induced damage in podocytes

  • BACKGROUND/OBJECTIVES: Chronic renal failure (CRF) is a complex pathological condition that lacks a cure. Certain Chinese medicines, such as melittin, a major component in bee venom, have shown efficacy in treating CRF patients. On the other hand, the mechanisms underlying the therapeutic effects of melittin are unclear. MATERIALS/METHODS: A 5/6 nephrectomy model (5/6 Nx) of renal failure was established on rats for in vivo assays, and mouse podocyte clone 5 (MPC5) mouse podocyte cells were treated with angiotensin II (AngII) to establish an in vitro podocyte damage model. The 24-h urine protein, serum creatinine, and blood urea nitrogen levels were evaluated after one, 2, and 4 weeks. Hematoxylin and eosin staining, Masson staining, and periodic acid-Schiff staining were used to examine the pathological changes in kidney tissues. A cell counting kit 8 assay was used to assess the cell viability. Reverse transcription polymerase chain reaction and Western blot were used to assess the mRNA and protein levels in the cells, respectively. RESULTS: In the rat 5/6 Nx, melittin reduced the 24-h urinary protein excretion and the serum creatinine and blood urea nitrogen levels. Furthermore, the renal pathology was improved in the melittin-treated 5/6 Nx rats. Melittin promoted podocin, nephrin, Beclin 1, and the LC3II/LC3I ratio and inhibited phosphorylated mammalian target of rapamycin (mTOR)/mTOR in 5/6 Nx-induced rats and AngII-induced MPC5 mouse podocyte cells. Moreover, inhibiting autophagy with 3-MA weakened the effects of melittin on podocin, nephrin, and the LC3II/LC3I ratio in podocytes. CONCLUSION: Melittin may offer protection against kidney injury, probably by regulating podocyte autophagy. These results provide the theoretical basis for applying melittin in CRF therapy.

베게너 육아종으로 진단된 좌안면신경마비 환자에 대한 한의복합치료 1례 (A Case Report of Korean Medicine Treatment for a Patient with Left Facial Palsy Diagnosed with Wegener's Granulomatosis)

  • Background: The study reports the effect of Korean medicine treatment on a patient with left facial palsy caused by Wegener's granulomatosis. Case information: The patient was receiving steroid treatment for facial palsy and was hospitalized for 21 days. He received acupuncture, electroacupuncture, bee venom pharmacopuncture, and moxibustion treatment along with Ssanghwa-tang and Cheongansohabwon herbal medicine treatment. During hospitalization, the Sunnybrook facial grading scale, House-Brackmann scale and length of time for the eye to completely close were evaluated at 11/16, 11/21, 11/27, 12/1, and 12/6. Post treatment, the degree of the movement in the left eyelid was improved, it took less time for the eye to close completely, the movement of the forehead wrinkle muscles improved, but that of the facial muscles under the eyes and left orbicularis oculi muscle showed little improvement. Conclusion: Korean medicine treatment for facial palsy caused by Wegener's Granulomatosis helps restore mobility of the forehead and orbicularis oculi muscles.

개 안면신경마비에 대한 봉독과 덱사메타손의 치료효과 (Therapeutic Effect of Bee-Venom and Dexamethasone in Dogs with Facial Nerve Paralysis)

  • 사람에서와 같이 개에서도 안면신경마비가 발생되지만, 특발성 원인에 의한 안면신경마비에 대한 유용한 서양의학적 치료 방법이 없는 실정이다. 따라서 개 안면신경마비에 대한 약침의 치료 효과를 알아보기 위하여 인공 유발된 개 안면신경마비에 대하여 약침의 효과를 실험하였다. 안면신경마비가 인공적으로 유발된 12두의 개는 대조군(4두), 덱사메타손 투여군(4두) 및 봉독 투여군(4두)로 나누어 공시하였다. 안면신경마비 유발 후 대조군은 식염수 1ml을 머리 근육에 일주일에 두 번 근육주사 하였다. 반면에 덱사메타손 투여군과 봉독 투여군은 합곡, 영향, 사백, 하관, 예풍, 권료, 상관 및 양릉천 혈위에 각각 덱사메타손과 봉독을 일주일에 두 번 약침하였다. 실험 기간 동안 안면신경마비에 대한 임상증상과 혈중 creatine kinase(CK) 활성의 변화를 각각 검토하였다. 임상증상은 치료 14일에 봉독 투여군과 덱사메타손 투여군이 대조군에 비하여 유의성 있는 저치를 보였으나 봉독 투여군과 덱사메타손 투여군 간에는 유의성이 인정되지 않았다. 혈중 CK 활성의 변화는 치료 7일 및 14일에 봉독 투여군과 덱사메타손 투여군이 대조군에 비하여 유의성 있는 저치를 보였으나, 봉독 투여군과 덱사메타손 투여군 간에는 유의성이 인정되지 않았다. 따라서 개 안면신경마비의 치료에 봉독과 덱사메타손 약침이 효과적이었으며, 그 효과는 유사한 것으로 판단되었다.

사독의 조해물질에 관한 연구 (Inhibitory Substance on the Snake Venoms Produced by Penicillium sp.)

  • 사독은 단백성물질이며 그 작용은 효소적이다. 동물에 대한 강력한 작용은 독사교상의 치료에 있어서 중요한 문제점을 가지고 있다. 독사에 대한 항혈청의 개발은 오래 전부터 이루어 졌으며 근년에 와서는 거의 완성된 상태라고 생각한다. 그러나 이러한 형편에서도 항혈청에 지니고 있는 몇가지의 결점으로 인하여 유효한 화학치료제의 개발이 요구되고 있다. 그러나 사독의 다양성을 고려할 때 획일적인 치료제를 얻기란 매우 곤란할 것으로 생각된다. 사독을 화학물질로써 불활성화 시키려는 시도는 많아서 chelate 성 물질 thio 화합물, 혹종의 Amino acid, Tannic acid 등 단백질 응고제 등 시험관내에서는 강력히 작용하는 물질이 많이 발견되었으나 이들의 작용은 사독에 대해서 비특이적이며 또한 생체내에서의 작용이 강하여 그의 이용에 많은 제한이 있다. 저자들은 미생물로부터 사독에 대한 조해물질의 검색을 시도했던바 Penicillium 속의 한균주로 부터 유효한 물질을 얻을 수 있었으며 이것을 ISV-33이라 명명하였다. 이 물질은Agkistro-don 및 Trimeresurus 류 사독의 Proteinase와 출혈인자 및 치사인자에 잘 작용하여 이것들을 불활성화시키나 일반효소에는 거의 작용하지 않았다. 이 물질은 사독에 대해서 $\frac{1}{4}$~$\frac{1}{8}$ 량으로서 충분히 작용하나 그 자체의 독성은 위를 대상으로 실험했을 때 거의 나타나지 않는다. 또 이 물질은 사독에 대해서 특이성이 강하며 in vivo에서도 작용한다. 이 물질은 결정성이며 열수에는 용이하게 용해하나 명수나 명종 유기용매에는 거의 용해하지 않으며 단지 Ethylacetate에는 약간 용해한다. mp는 222$^{\circ}C$에서 승화하며 분자량 약 392인 Tetra carboxylic acid로 동정되었다 분자식은(C$_{8}$ $H_{6}$N $O_{5}$)$_2$라고 인정되며 이 물질은 Polycarboxylic acid 이므로 chelate 작용이 있을 가능성이 있으나 금속효소에 대해서는 일률적으로 작용하지 않음으로 사독의 불활성화에 있어서 chelate 작용에 인한 것은 아니라고 생각된다. 그 구조식은 아직 확정하지는 못하였으나 지금까지의 실험결과로서 cyclo hexane을 골격으로 하고 여기에 Epoxy group와 caborxyl 기가 있는 것으로 추정된다. 또 이 물질은 산, Alkali, 가열등에 대해서 매우 안정한 것으로 나타났다. 이 연구의 일부는 문교부 연구조성비(1978년도)에 의해서 수행되었다.
