롤러 슈즈 착용 후 보행시 근피로 상태에서 하지근의 근전도 Power Spectrum 분석 (EMG Power Spectrum Analysis of Wearing Roller Shoes on Muscle Fatigue in the Lower Extremity during Walking)

  • The purpose of this study was to compare the electromyography signal's power spectrum mean and median tendencies appearing in the lower extremity during walking while wearing roller shoes. 9 male subjects volunteered who have no experience riding inline-skate or roller-skate, and have no record of musculoskeletal disorder. Subjects walked on treadmill twice for an hour (Once a week, one trial with the roller on and the other without roller, Walking velocity = 1.39 m/s). Electromyography was measured every 15 minute (0, 15, 30, 45, 60 minutes). Surface electrode sticked muscle at rectus femoris(R.F.), vastus lateralis(V.L.), vastus medialis(V.M.), biceps femoris(B.F.), tibialis anterior(T.A.), gastrocnemius lateralis(G.L.), gastrocnemius medialis(G.M.). At Rectus femoris, Vastus Lateralis, Vastus medialis, and Biceps femoris showed no statistically significant decrease of median frequency or mean edge frequency as time passes. Also, between two treatments (wearing the roller shoes vs not wearing the roller shoes), no statistically significant difference. After 60 minutes, mean edge frequency showed statistically significant decrease at tibalis anterior and after 45 minutes, mean edge frequency showed statistically significant decrease compared to wearing roller shoes without the wheels at gastrocnemius lateralis. At gastrocnemius medialis after 30 minutes, median frequency showed statistically significant decrease, and showed statistically significant difference compared to the control group. Wearing the roller shoes with wheels for a long time resulted in statistically significant decrease of mean edge frequency and median frequency in lower extremity, especially in shank muscles. Increase of wearing time of roller shoes and walking on a bumpy road wearing roller shoes with wheels result fatigue and thus, danger of injury.

대퇴근활성화에 대한 자전거 운동 시 근전도와 슬관절 각도와의 상관관계 연구 (A Study of Relationship Between EMG Activation of Thigh Muscle(Rectos Femoris, Vastus Iateralis Muscles) and Knee Angle During Bicycle Exercise)

  • 본 연구는 자전거운동에서 슬관절 각도와 대퇴근 활성도의 관계를 확인하고자 하였다. 총 16명의 20대 남성을 대상으로 실험을 진행하였으며 대퇴근의 대퇴직근과 외측광근에서 EMG 신호를 측정하였다. 실험은 안장거리에 따라 총 5단계로 나누었으며 단계별로 1분씩 60RPM, 200W의 부하로 운동을 하였고 슬관절 각도는 동작분석 장비를 이용하여 측정하였다. 실험결과는 슬관절 각도와 대퇴직근 외측광근 각각은 상관관계가 Step 1에서만 높게 나타났으며 그 외에서는 나타나지 않았다. 이러한 이유는 개인의 습관에 따라 두 근육의 활성도가 다르기 때문으로 사료된다. 그러나 대퇴근의 근전도와 슬관절 각도를 비교한 결과 높은 상관관계가 나타났다.

신발 밑창 형태가 무릎 및 발목 근육의 근활성화에 미치는 영향 (Effect of Shoes Sole Form on Knee and Ankle Muscle Activity)

  • PURPOSE: This study was to examine changes in muscle activity of lower extremity shoes sole form (high heels, shoes with curved out sole, house shoes). METHODS: The subjects of this study were 12 women in their twenties. They put three kinds of shoes (high heels, shoes with curved out sole, house shoes) and walked 5m. The muscles activities of lower extremity muscles (rectus femoris, vastus medialis, tibialis anterior, gastrocnemius medial part) were measured using a wireless electromyogram (EMG). Rectus femoris was attached to 1/2 position at the distance between ASIS and knee bone and vastus medials was attached to 2cm from upper inside of knee bone. Tibialis anterior was attached to 75% position above line connecting knee joint and ankle joint and gastrocnemius medial part was attached to 3 5% position above knee joint and ankle. RESULTS: It was found that there were significant differences in changes of muscles activities of lower extremity muscles (rectus femoris, vastus medialis, tibialis anterior, gastrocnemius medial part) on shoes sole forms (p<.05). All lower extremity muscles were showed high muscles activities, when high heels wear (p<.05). Wearing shoes with curved out sole was showed high muscle activity of tibialis anterior and lower muscle activity of gastrocnemius medial part compared with wearing house shoes (p<.05). CONCLUSION: Shoes sole form should be considered when patients with knee and ankle joint problems choose shoes because muscles were showed different activities according to shoes sole forms.

New insight into the vasto-adductor membrane for safer adductor canal blockade

  • Background: This study aimed to identify exact anatomical landmarks and ideal injection volumes for safe adductor canal blocks (ACB). Methods: Fifty thighs from 25 embalmed adult Korean cadavers were used. The measurement baseline was the line connecting the anterior superior iliac spine (ASIS) to the midpoint of the patellar base. All target points were measured perpendicular to the baseline. The relevant cadaveric structures were observed using ultrasound (US) and confirmed in living individuals. US-guided dye injection was performed to determine the ideal volume. Results: The apex of the femoral triangle was 25.3 ± 2.2 cm distal to the ASIS on the baseline and 5.3 ± 1.0 cm perpendicular to that point. The midpoint of the superior border of the vasto-adductor membrane (VAM) was 27.4 ± 2.0 cm distal to the ASIS on the baseline and 5.0 ± 1.1 cm perpendicular to that point. The VAM had a trapezoidal shape and was connected as an aponeurosis between the medial edge of the vastus medialis muscle and lateral edge of the adductor magnus muscle. The nerve to the vastus medialis penetrated the muscle proximal to the superior border of the VAM in 70% of specimens. The VAM appeared on US as a hyperechoic area connecting the vastus medialis and adductor magnus muscles between the sartorius muscle and femoral artery. Conclusions: Confirming the crucial landmark, the VAM, is beneficial when performing ACB. It is advisable to insert the needle obliquely below the superior VAM border, and a 5 mL injection is considered sufficient.

Arthroscopic Reduction of Irreducible Posterolateral Knee Dislocation with Interposition of the Vastus Medialis: A Case Report

  • Irreducible traumatic knee dislocation is rare. The knee dislocation is classified depending on the incarcerated structures. Complete reduction is achieved by extracting the incarcerated structure. Several reports introduce the reduction of irreducible traumatic knee dislocation by open surgery or arthroscopy. This case describes irreducible posterolateral knee dislocation with interposition of the vastus medialis. Closed reduction failed in the emergency room, and complete reduction was attained by arthroscopically sectioning the muscle and fascia of the vastus medialis in the intercondylar notch.

건강한 여성에 대한 근전도를 이용한 생체되먹이기 훈련이 안쪽빗넓은근과 가쪽넓은근의 근활성비와 개시시간에 미치는 영향 (The Effect of Electromyographic Biofeedback Training on the VMO/VL Electromyographic Activity Ratio and Onset Timing in Women without Knee Pathology)

  • Purpose : The purpose of this study was to investigate the VMO/VL ratio and onset timing using EMG biofeedback training over a 5-day period. Methods : Twenty-one healthy female college students with no known right knee musculoskeletal dysfunction were recruited this study. Muscle activity was measured by surface electromyography(Myosystem 1400A, Noraxon Inc., U.S.A). Statistical analysis was used two-way repeated ANOVA to know difference between the vastus lateralis and vastus medialis oblique onset timing differences, VMO/VL ratio. Results : Biofeedback training group significantly improved VMO/VL ratio and EMG activity of the vastus medialis oblique after intervention. Conclusion : These result indicate that biofeedback training on the vastus medialis oblique has effect on the VMO/VL ratio. EMG biofeedback can be recommended for the facilitation of VMO muscular recruitment.

Comparison of Lower Limb Muscle Activities by Various Angles of a Medio-lateral Ramp During Gait

  • PURPOSE: This study investigated the activities of lower limb muscles according to the angle of a medio-lateral ramp while walking to promote awareness of the risks associated with a medio-lateral ramp. METHODS: This study was conducted on 20 healthy male adults. The muscle activities of the vastus medialis oblique (VMO), vastus lateralis oblique (VLO), tibialis anterior (TA) and peroneus longus (PL) were measured while the subjects were walking on a 3 m medio-lateral ramp. Five angles (flat, $2^{\circ}$, $5^{\circ}$, $10^{\circ}$, and $15^{\circ}$) were selected for the angle conditions of the experiment on a medio-lateral ramp. The activities were measured during the stance phase only in the middle cycle of a three-cycle walking experiment. The mean value obtained from the three walking tests was used for the analysis. RESULTS: Results showed that walking on a mediolateral ramp required more muscle activities than walking on a flat surface, through which balanced walking was achieved. CONCLUSION: Walking on a medio-lateral ramp requires proper muscle activation and control, without which the risks of injury to the joints of the lower limbs and falls are likely to increase. Therefore, special attention should be given to older people and the disabled under the condition of traversing a ramp.

엉덩관절 모음 압력에 따른 스쿼트 운동이 다리 근육의 활성도에 미치는 영향 (Effects of Squat Exercise on Muscle Activity in Leg Muscles According to the Pressure of Hip Adduction)

  • Background: This study examined the changes in the leg muscle activities of 30 healthy subjects according to different pressures caused by isometric hip adduction during squat exercise. Methods: With their knee flexed at 60°, the subjects performed isometric hip adduction with a pressure of 0, 20, 40, and 60mmHg. Surface electromyography was used to measure their muscle activities. Changes in the activities of the muscles, such as the gluteus maximus (GMAX), Gluteus medius (GM), vastus medialis oblique (VMO), vastus lateralis oblique (VLO), biceps femoris (BF), and Tensor fasciae latae (TFL) muscles, were analyzed. Results: The activities of the VMO, VLO, GM, BF, and GMAX muscles were significantly different after exercise compared to that before exercise (p<.05). The activities of all the leg muscles measured were highest at a pressure of 60mmHg during isometric hip joint adduction. Conclusion: The pressure produced by isometric hip joint adduction during squat exercise increased the leg muscle activities of the subjects. These results will provide basic data on effective squat exercise to alleviate knee joint diseases.

등척성 운동시 운동강도에 따른 중앙주파수의 특성 (Characteristics of Median Frequency According to the Load During Fatiguing Isometric Exercise)

  • Median frequency can be regarded as a valid indicator of local muscle fatigue. As local muscle fatigue develops, the muscle fiber conduction velocity decreases, the fast twitch fibers are recruited less, and consequently the median frequency shifts toward the lower frequency area. The aim of this study was to test the characteristics of the median frequency according to exercise load (30% and 60% of MVC on the biceps brachii, 40% and 80% of MVC on the vastus lateralis) during the fatiguing isometric exercise. Thirteen healthy male volunteer students of Yonsei University were recruited. After the testing maximal voluntary isometric contraction, three variables (initial median frequency, regression slope, fatigue index) from the regression line of MDF data were measured in each exercise load. The results showed that the regression slope and fatigue index were significantly different for the biceps brachii, but not for the vastus lateralis initial MDF was not significant difference according to the exercise load on both muscles. The regression slope and fatigue index could monitor physiologic muscle change during fatiguing isometric exercise. The results showed that two MDF variables reflect the local muscle fatigue according to the exercise load.

불안정한 바닥에서의 닫힌 사슬운동이 전십자인대 재건술 환자의 슬관절 안정성에 미치는 영향 (The Effects of Closed kinetic chain Exercises of Unstable Floor on the Stability of the Knee Joints of Patients with Anterior Cruciate Ligament Reconstruction)

  • Purpose : This study was to comparison of EMG of an stable exercise group and unstable exercise group on patients who have had anterior cruciate ligament reconstruction(ACL reconstruction). Methods : The subjects of the study were patients who had no less than 4 weeks after ACL reconstruction and could stand on one leg, and they divided into a control group with 9 patients doing closed kinetic chain exercises on the stable floor and an experimental group with 10 patients doing closed kinetic chain exercises on the unstable floor and in order to compare degrees of the muscle activity of the thigh extensor and flexor was tested each before the exercise, in 3 weeks and 6 weeks after doing exercises by using surface electromyography (Surface EMG). The patients made 3 sets of exercises (10 times per set), each of which consisted of exercises using elastic bands and the squat. Results : There was statistic significance about the vastus medialis muscle. Conclusion : It is thought that the closed-chain exercise could be an exercise program through which patients could enhance the muscle activity of the vastus medialis muscle optionally among the quadriceps muscle and the hamstring muscles which should weaken after ACL reconstruction.
