The Effect of Support Group Intervention on Various Adaptations of Primary Family Caregivers Caring for Cerebro-Vascular Accident Patients (집단지지 중재가 뇌졸중 환자 가족원의 제 적응에 미치는 효과)

  • The purpose of this research was to examine the effect of support group intervention on the various adaptations of primary family caregivers caring for Cerebro- Vascular Accident patients. The nonequivalent control group pretest-posttest design within the framework of Lazarus & Folkman's stress-adaptation model was used for this experimental study. The subjects were 86 primary family caregivers caring for Cerebro- Vascular Accident patients at K hospital in Taegu, D herbal hospital in Kyung Ju, H hospital in Pohang from March, 1998 to July, 1998. Among 86 subjects, 43 were placed in an experimental group and 43 in a control group. The experimental group was treated by researcher who administered informational and emotional support group intervention once a week over a five weeks period. The data were collected through interviews. Collected data was analized by means of a chi-square test, t-test, ANCOVA, and Pearson correlation coefficient. The results of this research were as follows: 1. Physical, emotional, and social adaptation scores in the experimental group were revealed to be significantly higher than those of the control group. 2. There was significant positive correlation among physical health, subjective burden, depression and objective burden. Accordingly, it is concluded that informational and emotional support group intervention was a useful nursing intervention on the various adaptations of primary family caregivers caring for Cerebro-Vascular Accident patients.

  • The purpose of this case-control study is to investigate the relationship between storke and stroke risk factors, such as family history and past history. The case-control study over clinical data registered from Daejeon Oriental Medical Hospital in the period of November 2006 to July 2010. Study subjects consisted of 108 patients with acute stroke within 1 month as the case group (Cases) and 108 people who visited hospital for health care as the genreral control group (Controls). The participants had been interviewed by residents to find out their family history and past history. Their blood was taken to check the blood lipid level and liver function. 1. The people whose family history included cerebrovascular accident had more probability of stroke than the people who did not have cerebral vascular accident as their family history. 2. The people who suffered from hypertension had more probability of stroke than the people who did not suffer from hypertension. 3. The people who suffered from diabetes mellitus had more probability of stroke than the people who did not suffer from diabetes mellitus. Based on this study, the people who had cerebral vascular accident as their family history should thoroughly treat their blood pressure and blood sugar level to prevent the cerebral vascular accident.

  • The purpose of this study suggest that Park Golf for rehabilitation training of the patients with hemiplegia. The increase of the aged caused various problems. Among the problems, chronic illnesses the old people suffer from charge their family with mental, physical, social and economic burden as well as the person. Especially, the more old people are the more risks and attacks of cerebral vascular accident(CVA) exist. Therefore, the increase of the aged means the increase of cerebral vascular accident(CVA) patients. The cerebral vascular accident(CVA) patient get into difficulties to secure their livelihood due to troubles and disoders, so medical care facilities and rehabilitation training are really necessary to help them return to their normal life with mental, physical and social capability. The rehabilitation of patient with hemiplegia is ongoing process. It may possibly accomplish throughout the whole life because it takes a lot of time and effort. In addition, an ultimate aim of the rehabilitation is returning and adaptation to society. In this way, it is hardly to expect a satisfied results of the rehabilitation without motive. Therefore, patient with hemiplegia rehabilitation motive will is the most important. If interesting of park golf the patient with hemiplegia can be absorbed in easily are applied to the rehabilitation training, the patient are will to participate in the training without troubling.

  • The purpose of this study nun to obtain basic information about the state of the physically disabled due to neurological impairments in order to provide precise data for planning and evaluating the Community Based Rehabilitation in Uiwang. The results were 1) the total number of registered disabled adults with cerebral vascular accident in this city consulted was 80 and the Prevalence rate was 1.0575 of total disabled Persons, 2) under 100 of the score of activity of daily living(ADL) with the subjects was 41.245 as Functional Independence Measure(FIM) and 3) ender 20 of the score of motor ability with the subjects was $45.1\%$ as Motor Assessment Scale(MAS). This results show that rehabilitation program for community based rehabilitation(CBR) should set up for each level of motor ability and ADL.

Clinical Effects of Sayuktanggami-bang on Cerebral Vascular Accident Patients with Normocytic Normochromic Anemia (뇌졸중환자의 정구성정색소성 빈혈에 대한 사육탕가미방의 임상적 효과)

  • Objective : Normocytic Normochromic Anemia is one of the most common cases in clinics of all types of morphological Anemia. This study is aimed at examining significant curative effects of Sayuktanggami-bang on Cerebral vascular accident(CVA) patients with Normocytic Normochromic Anemia. Methods : For this study to examine the effects of Sayuktanggami(四六湯加味方), Patients were divided into 2 groups. Group A(experimental group) consisted of 19 cases prescribed Sayuktanggami-bang, Group B (control group) consisted of 15 cases prescribed other herbs. Before presciption, They were tested for CBC, AST, ALT, Total protein, Cholesterol, TG, HDL, BUN, Creatinine, Uric acid, Na, K, Cl while fasting, then were given the herbs over two weeks. They were checked again for changes over those two weeks. Results : In cases of group A, the statistical significance of CBC changes before and after treatment were recognizable : RBC(p<0.01), Hb(p<0.01), Hct(p<0.01). In comparing CBC changes between group A and group B which developed cerebral infarction group, a type of CVA, the significance of curative effects in group A is recognizable over group B before and after treatment : RBC(p<0.01), Hb(p<0.01), Hct(p<0.05). Cytotoxic reaction was nonexistant. Conclusion : Results of this study suggest that Sayuktanggami-bang has curative effects for CVA patients with normocytic normochromic anemia.

Management of Femoropopliteal Vascular Injuries after Trauma: Surgical Outcomes (외상 후 대퇴-오금 혈관손상의 치료: 수술 성적)

  • Purpose: Vascular injuries caused by traffic, industrial accidents and by outside activities have increased in Korea. Especially, vascular injuries to the extremities can lead to limb loss and even mortality if they are not appropriately treated. The aim of the study was to evaluate the surgical outcomes of femoropopliteal vascular management after trauma. Methods: The medical records of 12 patients with femoropopliteal vascular injuries who were treated at Dankook University Hospital from 2011 to 2013 were reviewed. Iatrogenic vascular injuries were excluded. The clinical data including the causes of injury, associated injuries and surgical outcomes were analyzed retrospectively. Results: All patients were male, with a mean age of $46.8{\pm}16.3years$ (range: 26~69 years). The causes of vascular injuries were four traffic accidents, three industrial accidents, two iron plates, one outside activity, one glass injury and one knife injury. The average transit time between the place of the accident place and the emergency department was $3.0{\pm}2.1$ (0.5~12.5) hours, and the average preparation time for surgery was $8.0{\pm}6.7$ (1.7~23.3) hours. The anatomic injuries included the popliteal vessel in seven cases and the femoral vessel in five cases. The average Injury Severity Score (ISS) was $12.0{\pm}5.0$ (5~17), and the average Mangled Extremity Severity Score (MESS) was $5.7{\pm}2.1$ (2~9). The operation methods were four interposition grafts, three end-to-end anastomoses, two direct repairs and three patch angioplasties. One case required amputation of the injured extremity. Conclusion: Early recognition and revascularization of the injured vessel are mandatory to reduce limb loss and to obtain satisfactory outcomes. Therefore, careful/rapid evaluation of the vascular injuries and timely/successful surgical treatment are the keys to salvaging an injured limb.

Treatment of Traumatic Vascular Injury (외상성 혈관 손상의 치료)

  • Vascular injury has been increased with popular outside activities. However there are only a few studies for vascular trauma. We intended to find out the relationships between the vascular trauma and its prognosis. Material and Method: Forty-four patients were diagnosed and operated on for vascular injury in the Gyeongsang National University Hospital from 1992. to 1999. We reviewed their chart and analysed their data retrospectively. We studied to causes, treatments, and prognoses of traumatic vascular injury. Result: The transfer time between accident place and emergency department was mean 3,5 hour, and the operation preparing time was 8.8 hours. Five cases required amputation of extremity. All amputation cases were combined with communited bony fractures. Thirty-eight cases had combined other injuries, and almost combined injuries were muscular and neurological damages, Conclusion: The optimal treatment plan for vascular trauma was prompt diagnosis and quick management. It may decrease amputation rate and post-traumatic complications. Therefore we must be carefully evaluated the multiple traumatic patients for early diagnosis of vascular injury and operate quickly in emergency status.

Examining the Relationship between Depression, Daily Hassles, and Family Support among Elderly Cerebral Vascular Accident Outpatients (통원치료 중인 뇌병변 장애노인의 생활스트레스, 가족지원, 우울간의 관계성 검증)

  • Depression has a direct relationship with lower quality of life for elderly with cerebral vascular accidents(CVA). Daily hassles worsen depression for elderly with CVA disability and thus an effective intervention is necessary. This research examined the direct influence of daily hassles on depression and the mediation effect of family support among 161 elderly with CVA disability living in Seoul using Structural Equation Modeling. The direct effect of daily hassles on depression was statistically significant (p < .001) while family support was shown to be a mediator between daily hassles and depression. The mediation model of family support was examined using $x^2$ difference and Soble test. As a result, family support was proven to be a partial mediator. Implications are discussed.

Combined Treatment of Stromal Vascular Fraction and Ablative Fractional CO2 Laser for Hypertrophic Foot Scar

  • The treatment of keloid and hypertrophic scars (HTSs) remains one of the most difficult challenges, with a high recurrence rate regardless of the method of treatment. The latest trend in scar management is a combined approach using multiple modalities that are individualized to the patient and that would provide successful results for keloid and HTSs. There are previous reports that stromal vascular fraction (SVF) is effective for scar remodeling. Based on these reports, we introduced the concept of a combination treatment using SVF injection and fractional ablative CO2 laser. In this report, we present a 21-year-old woman who was involved in a car accident. A defect on her foot was covered with a skin graft, but the scars became elevated, which turned out to be HTSs. She was treated with a fractional ablative CO2 laser for five sessions. A month later, SVF injection and fractional ablative CO2 laser were conducted simultaneously. The result of a year's follow-up showed a flattened scar with resolution of pigment deposition. In conclusion, the combination treatment for HTSs with SVF injection and ablative fractional CO2 laser is one of the modalities to achieve an excellent outcome for treating HTS.

An Experience of Inferior Vena Caval Ligation in Traumatic Injury (하공정맥 손상의 치험 1례)

  • Because of the. rise in the incidence of high speed automobile accident and various gun shot wound, complicated vascular injuries are becoming more frequent. Inferior vena caval injury seems to be also in high incidence, but reports in the literature were rare. because of potentially lethal. Recently we have experienced a case of inferior vena caval injury due to stab wound on the posterior aspect of the right abdomen. This was successfully treated with inferior caval ligation on the both, proximal and distal of the injured infrarenal vena cava.
