서해안 패류양식장 인근 육상오염원에서 분리한 장구균의 Vancomycin 내성 유전자 검출 및 항생제 내성 특성 (Detection of Vancomycin Resistance Genes and Antibiotic Resistance Characteristics of Enterococcus spp. Isolated from Inland Pollution Sources Near Shellfish Farms on the West Coast of South Korea)

  • In this study, 143 strains of Enterococcus spp. were isolated from inland pollution sources near shellfish farms on the west coast of South Korea. Not all isolated Enterococcus spp. strains possessed vancomycin resistance genes (VanA and VanB). However, since vancomycin-resistance Enterococcus (VRE) have been detected not only in the clinical field but also out in the world, it is possible that the VRE gene may be transferred to other bacterial strains commonly found in coastal waters where seafood is produced. It is important to monitor trends in the appearance of VRE. In addition, antimicrobial resistance patterns of isolates were examined in this study. Overall antimicrobial resistance rates were high: ciprofloxacin (32.2% of isolates resistant), chloramphenicol (30.8%), quinupristin/dalfopristin (19.6%), and tylosin (15.4%). Eight E. faecium strains (6.2%), out of the 129 strains assessed, showed multidrug resistance. All multidrug-resistant E. faecium showed resistance to erythromycin, quinupristin/dalfopristin, tetracycline, and tylosin, in all 14 strains. All multidrug-resistant E. faecalis showed resistance to erythromycin, quinupristin/dalfopristin, tetracycline, and tylosin. Both multidrug-resistant E. faecium and multidrug-resistant E. faecalis showed common resistance to erythromycin, quinupristin/dalfopristin, tetracycline, and tylosin.

  • Vancomycin-resistant enterococci[VRE] are rare cause of meningitis, occurring in immunocompromised patients, severely ill, hospitalized patient, and patients who have undergone neurosurgical procedures. Resistance to vancomycin has increased in frequency during the past few years. Limited therapeutic options are available for VRE infectionsandtheoptimumtherapy has not been established. We report a case of VRE meningitis that was successfully treated with administration of quinupristin-dalfopristin [Synercid] by both intravenous and intraventricular routes. A brief review of the literature is provided, which indicates that optimal management with Synercid should include daily intraventricular doses of at least 2mg and intravenous 0.5mg/kg every 8 hours. We also review the previously reported cases of VRE meningitis.

  • 복막염은 지속성 외래 복막투석 환자에게 있어 중요한 합병증 중 하나이다. 이러한 복막염은 잦은 재발과 최근 내성균의 출현으로 치료에 어려움을 겪고 있다. vancomycin 내성 장구균(VRE, Vancomycin-resistant Enterococcus)에 의한 복막염은 1999년에 국내에서 처음 보고된 이래로 계속 증가하고 있으나 아직까지 치료에 대해 명확한 기준이 없는 실정이다. 저자들은 복막투석 소아에서 리네졸리드(linezolid)로 VRE에 의한 복막염을 성공적으로 치료한 1례를 경험하였기에 문헌고찰과 함께 보고하는 바이다.

Simple and Rapid Detection of Vancomycin-Resistance Gene from Enterococci by Loop-Mediated Isothermal Amplification

  • We developed a simple and rapid method for detecting vancomycin resistance genes, such as vanA and vanB, using loop-mediated isothermal amplification (LAMP). To identify not only vancomycin resistance genes, but also the genus Enterococcus, primers were designed for vanA, vanB, and 16S rRNA. Screening for vancomycin susceptibility in Enterococcus was performed using Etest (bioMérieux Inc). The results of the LAMP assay were compared to those of real-time RT-PCR. The optimal conditions for the LAMP assay were 65℃ for 60 min. The detection limits of the LAMP assay for vanA, and vanB were 2 × 102 copies/reaction. Compared to RT-PCR, the sensitivities and specificities of LAMP for 16S rRNA, vanA, and vanB were 100/100%, 100/100%, and 100/100%, respectively. The vanA genotype-vanB phenotype accounted for 57.5% (46/80) of the vancomycin-resistant Enterococci samples collected from 2016 to 2019. In conclusion, the LAMP assay developed in this study showed high sensitivity and specificity for vancomycin-resistant genes. Moreover, due to the simplicity and rapidity of the LAMP assay, its use can be very useful in clinical microbiology laboratories.

DD1.5k, the Gene Preferentially Expressed in Bloodstream Isolates of Vancomycin-Resistant Enterococcus faecium

  • Vancomycin-resistant Enterococcus faecium (VREFM) is becoming a threatening pathogen. We identified a gene called DD1.5K by differential display-PCR, which was preferentially expressed in the bloodstream isolates of VREFM. Due to its amino acid similarity to transfer complex protein, trsE, and tissue-specific expression, this gene may be involved in virulence of VREFM.

  • VISA and VRE are the main causes of surgical infection, urinary tract infections and bacteremia in hospitals. In this study; we selected VISA (Vancomycin Intermediate resistant Staphylococcus aureus) and VRE (Vancomycin Resistant Enterococcus) isolated from the clinical isolates. One of the isolated strains indicated the high resistance to severel anti-biotics (Vancomycin, Teicoplanin, Mupirocin, Synercid, Ciprofloxacin, Gentamicin, Lincomycin, Cefotaxim, Meropenem). Antimicrobial activity of Bifidobacterium spp. against VISA and VRE were measured. About $10^4$ cells of VISA or VRE were mixed with 1,5 and 9 ml of Bifidobacterium and the final volume was adjusted to 10 ml with brain heart infusion (BHI) broth. The cell suspension was incubated for 3, 6, 9, and 24 hr, serially diluted and then plated on BHI agar plate. As numbers of Bifidobacterium were increased viable cell count of VISA and VRE decreased. The strongest antimicrobial activity of the Bifidobacterium was observed after 9hr incubation in any mixture, almost completely inhibiting the growth of VISA and VRE.

Prevalence and Characterization of Vancomycin-Resistant Enterococci in Chicken Intestinles and Humans of Korea

  • The prevalence, genotype for antibiotic resistance and antibiotic susceptibility of vancomycin resistant enterococci (VRE) were determined. And molecular typings of the Enterococcus faecium isolates were analyzed. Prevalence of VRE in chickens, healthy children and intensive care unit (ICU) patients was 41.6%,7.9%, and 20.4%, respectively. Forty out of 54 isolates from chicken intestines, and 9 out of 11 from ICU patients were identified as Enterococcus faecium. Eleven out of 13 isolates from non-hospitalized young children were E. gallinarium. Twelve strains of E. faecalis were isolated from chicken intestines. The gene for the antibiotic resistance in E. faecium, and E. faecalis was vanA, while that in E. gallinarium was vanC1. E. faecium isolates were resistant to most of antibiotics except ampicillin and gentamicin. Molecular typing of the E. faecium strains obtained by pulse field gel electrophoresis and repetitive sequence-based PCR suggest that VRE transmit horizontally from poultry to humans, especially young children, via the food chains in Korea.

Distribution of Vancomycin-resistant Enterococci Isolates Using a ChromID VRE Agar

  • Vancomycin-resistant enterococci (VRE) have emerged as important healthcare-associated infection since last two decades. ChromID VRE agar (cIDVA) is useful for VRE rectal swab screening. We investigated all VRE were isolated on the cIDVA. A total of 363 rectal swabs of 85 patients to test VRE screening were inoculated into bile-esculin (B-E) broth with $6{\mu}g/mL$ vancomycin. After 24 hours incubation, we subcultured B-E broths were changed to black onto cIDVA. All isolates were identified by the MICROSCAN and VITEK2. The vanA gene and vancomycin minimal inhibition concentration (MIC) were detected by PCR and E-test respectively. 277 E. faecium (84.7%), 16 E. faecalis (4.9%), 25 E. avium (7.6%), 8 E. gallinarum (2.4%) and 1 E. raffinosus (0.3%) were isolated. 10.3% of VRE detected on cIDVA were other than E. faecium and E. faecalis that presented various color from colorless to pale violet. All isolates contained vanA and vancomycin MIC were > $256{\mu}g/mL$. VRE isolates other than E. faecium and E. faecalis should be objective to the contact precautions for healthcare-associated infection control if they possess vanA gene. Due to emerging enterococci carrying vanA such as E. avium, E. gallinarum, and E. raffinosus, VRE surveillance should be expanded to all isolates on chromogenic agar.

최근 충북지역에서 분리된 Gram 양성 세균종과 항생물질의 감수성 양상 (Gram-Positive Bacterial Species and Antimicrobial Susceptibility Patterns Isolated from Chungbuk Area)

  • 임상검체에서 분리되는 병원균을 주기적으로 분석하고 이에 대한 항균제 감수성의 변화 양상을 파악하는 것은 임상에서의 적절한 항균제의 선택이나 내성균의 감독과 조절을 위해 필수 적이라 할 수 있다. 저자들은 충북대학병원에 1996년 1년간 내원 환자의 미생물 배양 검사에서 분리, 동정된 1,689균주중 Gram양성인 542균주의 분리 빈도 및 주요 균종에 대하여 항생제 감수성 양상을 조사하였다. Gram 양성 균종은 S. aureus, S. pneumoniae, S. epidermidis, S. hemolyticus, coagulase negative Staphylococcus (CNS), Enterococcus faecalis 순으로 분리되었다. 한편, 항생제 감수성 시험 결과 S. aureus의 항생제 감수성은 penicillin과 gentamicin에 대한 저항성이 현저히 높았으며, teicoplanin 과 vancomycin (MIC$_{50}$ $\mu\textrm{g}$/ml)에 감수성이 높은 것으로 나타났다. CNS의 경우도 S. aureus와 대동소이 한 결과를 보였으나, S. epidermidis의 경우에는 ciprofloxacin과 clindamycin에 감수성 이 높은 것으로 나타났다. Enterococcus spp.는 vancomycin, penicillin, tetracycline 등에 저항성이 매우 높았으며, 검사한 모든 항생제에 대하여 대부분의 감수성이 zig-zag pattern을 보였다.

High-Quality Whole Genome Sequence of a Linezolid-Resistant and Vancomycin-Susceptible Enterococcus faecalis Isolate ES-2-1 from a Pig Stool in South Korea

  • We report the whole genome sequence of a linezolid-resistant and vancomycin-susceptible Enterococcus faecalis strain, ES-2-1, which was isolated from a pig stool in South Korea. The assembled genome of ES-2-1 consists of a 2,648,168-bp circular chromosome containing the optrA gene (encoding the ABC-F type ribosomal protection protein), an 84,891-bp plasmid containing numerous antimicrobial resistance genes, and an 82,106-bp cryptic plasmid. The ES-2-1 strain belongs to sequence type 1024 (ST1024) and carries multidrug resistant genes including the optrA (oxazolidinone phenicol transferable resistance A) gene, which confers linezolid resistance.
