대량 객혈에 대한 기관지동맥 색전술 -치험 2례- (Bronchial Artery Embolization of Massive Hemoptysis -2 cases-)

  • Prolapse of the aortic valve is the main cause of insufficiency of the aortic valve as a complication of ventricular septal defect. Aortic insufficiency gets worse by the progress of prolapse of aortic valve due to lack of support of the valve and the hemodynamic effect of blood flow through the ventricular septal defect. This produces typical clinical picture, that may be serious and threatening when it is untreated. Type and timing for the surgical treatment of the ventricular septal defect with aortic insufficiency is considered. Among 113 ventricular septal defect, 9 patients of ventricular septal defect with associated aortic insufficiency were experienced from June. 1983 to June 1988 at the Department of Thoracic and Cardiovascular Surgery, Chon-Buk University Hospital. Male was 6 patients and female was 3 patients. Ages were from 7 years to 24years. 5 patients were from 10 to 19 years age. 3 patients were below 10 years age. The ratio of pulmonary blood flow to systemic f low [Qp/Qs] was 1.53 and in pulmonary vascular resistance, normal or slight increase was 7 patients, moderate 1 patient, and severe 1 patient. Ventricular septal defect was subpulmonic in 5 patients and infracristal in 4 patients. Prolapse of right coronary cusp was 7 patients, right and non coronary cusp 1 patient and non coronary cusp 1 patient. Teflon patch closure of ventricular septal defect was undertaken in 3 patients and primary closure in 1 patient. Among the 4 patients of defect closure alone, one patient performed valve replacement 7 months later due to progressive regurgitation and cardiac failure and the result was good. The other 3 patients were good result. Closure of ventricular septal defect and aortic valvuloplasty performed in 4 patients. 2 patients of these required valve replacement for the sudden intractable cardiac failure and died due to low cardiac output. The cause of intractable cardiac failure was tearing of repaired valve at the fixed site. The other 2 patients were good result. Closure of ventricular septal defect and valve replacement performed in 1 patient with good result.

대동맥판막, 승모판막, 삼첨판막과 심실중격을 침범한 심내막염 -1예 보고- (Infective Endocarditis Involving Aortic Valve, Mitral Valve, Tricuspid Valve, and luterventricular Septum -A Case Report)

  • 본 증례는 고유 대동맥 판막, 승모판막, 삼첨판막과 심실중격을 침 범한 심내막염환자의 성공적 치료에 대한 보고이다. 환자는 11세때 심실중격결손폐쇄와 대동맥판막성형술을 받은 16세 남아로서 심내막염으로 항생제치료에도 불구하고 난치성 심부전을 보였다. 수술소견상 이전 수술시 폐쇄한 심실중격폐쇄를 따라 커다란 결손을 보였으며 다발성 천공과 우종을 동반한 대동맥 판막역류, 우종을 동반한 승모판 역류, 대동맥 판륜주위농양, 심실중격의 심근농양, 천공과 우종을 동반한 삼첨 판역류 소견을 보였다. 수술은 모든 감염조직과 괴사조직을 근치적절제후 심실중격결손을 Dacron patch로 폐쇄하면서 대동맥판막륜부위까지 확장보강하였으며, 대동맥판막치환시 확장보강한 Patch에 인공판막을 거치시켜 안전하게 치환할 수 있었다. 승모판막은 치환하였고, 삼첨 판은 자가심낭편으로 재건하였다. 환자는 수술후 6주간 항생제치료후 다른 합병증 없이 건강한 상태로 퇴원하였다.

Surgical Outcomes of Congenital Atrial Septal Defect Using da VinciTM Surgical Robot System

  • Background: Minimally invasive cardiac surgery has emerged as an alternative to conventional open surgery. This report reviews our experience with atrial septal defect using the da VinciTM surgical robot system. Materials and Methods: This retrospective study included 50 consecutive patients who underwent atrial septal defect repair using the da VinciTM surgical robot system between October 2007 and May 2011. Among these, 13 patients (26%) were approached through a totally endoscopic approach and the others by mini-thoracotomy. Nineteen patients had concomitant procedures including tricuspid annuloplasty (n=10), mitral valvuloplasty (n=9), and maze procedure (n=4). The mean follow-up duration was $16.9{\pm}10.4$ months. Results: No remnant interatrial shunt was detected by intraoperative or postoperative echocardiography. The atrial septal defects were mainly repaired by Gore-Tex patch closure (80%). There was no operative mortality or serious surgical complications. The aortic cross clamping time and cardiopulmonary bypass time were $74.1{\pm}32.2$ and $157.6{\pm}49.7$ minutes, respectively. The postoperative hospital stay was $5.5{\pm}3.3$ days. Conclusion: The atrial septal defect repair with concomitant procedures like mitral valve repair or tricuspid valve repair using the da VinciTM system is a feasible method. In addition, in selected patients, complete port access can be helpful for better cosmetic results and less musculoskeletal injury.

감염성 심내막염 환자에서 동반된 화농성 견관절염 - 증례보고 - (Pyogenic Arthritis of the Shoulder in Patient with Infective Endocarditis -A Case Report-)

  • 목적: 감염성 심내막염과 동반된 견관절 화농성 관절염의 치료 경험을 보고하고자 한다. 대상 및 방법: 감염성 심내막염에 동반한 화농성 견관절염을 가진 70세 남자 환자에 대하여 심장 판막 치환술 및 관절경하 활막 절제술과 배농술을 시행 하였다. 결과: 감염에 대한 순조로운 치유와 기능의 회복을 보였다. 결론: 상대적으로 견관절에 화농성 관절염이 동반되는 경우는 매우 드문 것으로 보고 되고 있으나 감염성 심내막염에 동반하는 일반적인 근골격계 증상으로 간과한다면 심각한 합병증으로 발생할 수 있음으로 주의 깊게 판단하여야 한다고 생각한다.

  • 대동맥 축착증 및 심한 대동맥 협착을 가진 생후 5일의 환자가 대동맥 협착을 해결하려는 중재적 시술 도중 유도 카테터에 의한 우측 경동맥 손상으로 인한 출혈 및 심낭 내 탐폰으로 인한 심정지가 발생하여 심폐소생술 후 체외막성산화기 보조를 받았다. 환자의 체중은 2.4 kg이었다. 1일 후 수술적 완전 교정술을 시행하였으나, 수술 직후 심한 심기능 저하로 인하여 양심실 보조 장치로 순환 보조를 하였고, 3일 후 양심실 보조 장치 이탈에 성공하였다. 이후 환자는 일반적인 치료 후 특별한 문제없이 퇴원하였다.

  • 선천성 심기형 환아들의 장기생존이 향상되면서 소아연령에서의 대동맥판막의 문제는 드물지 않게 경험하게 되며 또한 점차 증가하고 있다. 따라서 소아연령에서도 대동맥판막수술의 필요성이 점차 증가하고 있다. 과거 심장외측통로폰탄술식을 받은 10세 환아가 점차 심해지는 청색증과 운동시 호흡곤란으로 내원하였다. 술전 시행한 심초음파 및 심도자검사에서 간내 측부순환에 의해 간정맥에서 폐 심방으로 체정맥혈류가 유입되고 있었으며 방실판막은 중등도의, 상행대동맥의 확장 및 대동맥륜의 확장으로 인한 대동맥판막의 심한 폐쇄부전을 동반하고 있었다. 수술은 먼저 폐 심방으로 연결되는 간정맥을 결찰하고 체외순환을 가동하고 심정지를 시킨 후 상행대동맥의 축소성형 및 대동맥등 축소화를 포함한 대동맥근 재건술및 방실판막에 대한 성형술을 시행하였다. 술후 환아의 호흡곤란 및 청색증은 상당히 개선되었다.

Clinical Application of an Image-Guided Intervention in Three Dogs

  • A 9-month-old, 11.3 kg, intact, male, mixed-breed dog was referred for treatment of cor triatriatum dexter (CTD); a 5-month-old, 1.9 kg, intact, male Maltese for pulmonic stenosis (PS); and a 3-year-old, 6.62 kg, intact, female West Highland white terrier for esophageal stricture with regurgitation. A balloon catheter intervention was performed in the dog with CTD, and subsequent color Doppler ultrasound and abdominal ultrasound showed normal blood flow across the perforated membrane dividing the right atrium and the disappearance of the severe ascites present before treatment. Balloon catheter intervention in the dog with PS reduced the blood flow through the stenosis from 5.82 m/s to 3.97 m/s. In the dog with esophageal stricture, balloon catheter intervention widened the esophagus and no subsequent regurgitation was observed. Balloon catheter intervention is an interventional radiology procedure that represents a definitive treatment option for various stenotic lesions in dogs, including CTD, PS, and esophageal stricture. Although interventional radiology procedures for these diseases have already been reported, details of procedures and successful outcome have not been reported in Korea.

쇄골하정맥을 이용한 J 형의 전극도자를 가진 심방 Pacemaker 이식치험 2예 (Atrial pacemaker implantation through left subclavian vein puncture)

  • The management of cardiac arrhythmias by cardiac pacing has increased greatly since the treatment of complete heart block with an external transcutaneous pacemaker in 1952, followed by the use of myocardial wires connected to an external pulse generation, by external transvenous pacing, and then by transvenous pacing with implantable components in thoracic wall.By now, the three bases of modern cardiac pacing for bradyarrhythmias had been established [1] an implantable device [2] the transvenous approach [3] the ability of the pacemaker to sense cardiac activity and modify its own function accordingly. In transvenous implantation of a pacemaker, any one of four vessels at the root of the neck is suitable for passage of the electrode - cephalic vein, external jugular vein, internal jugular vein, costo-axillary branch of the axillary vein. The new technique of direct puncture of the subclavian vein, either percutaneously or after skin incision only has been made, is invaluable & is used routinely. We have experienced one 25 years old patient who had rheumatic mitral stenosis & minimum aortic regurgitation with sinus bradycardia associated with premature atrial tachycardia & another 54 years old female patient who was suffered from sick sinus syndrome with sinus bradycardia & sinus arrest. The 1st patient was taken open mitral commissurotomy & aortic valvuloplasty and then was taken atrlal pace-maker implantation through If subclavian puncture method in post-op 14 days, and the second patient was taken atrial pacemaker implantation through If subclavian puncture method. Their postop course was in uneventful & were discharged, without complication. Their condition have been good to now.

개심술 180례에 대한 임상적 고찰 (Open Heart Surgery:Clinical Analysis of 180 Cases)

  • Between Feb. 1990 and Aug. 1993, 180 cases of the open heart surgery were performed under cardiopulmonary bypass in the Department of Thoracic & Cardiovascular surgery, Gil General Hospital. There were 83 cases with congenital heart diseases [CHD] and 97 cases with acquired heart diseases [AHD]. The CHD consisted of 78 acyanotic[mortality: 3.8 %] and 5 cyanotic cases with heart anomaly[mortality:l case]. The AHD were 97 cases, which contained 53 valvular, 27 ischemic heart diseases, 10 aortic diseases, 5 cases with myxoma, 1 case with post-infarct VSD, and 1 case with removal of infected pacing wire in right ventricle. In the 53 valvular heart diseases, there were 45 cases with valve replacement[MVR 27, AVR 9,MVR + AVR 9] and 8 cases with valvuloplasty. The number of the implanted prosthetic valves were 53. In MVR, 25 St. Jude, 6 Sorin, 3 Carpentier-Edward and 2 Intact medical valves were used. In aortic position, 13 St. Jude, 3 Sorin and 1 Intact medical valves were applied. The operative mortality was 5.6 % [3/53]. The annuloplasty applying artificial ring was performed in 17 patients[4 cases associated with MVR] and the number of the implanted ring was 19, which included 14 Duran ring[10 mitral, 4 tricuspid] and 5 Carpentier ring [3 mitral, 22 tricuspid]. In the 27 ischemic heart diseases, there were 9 cases with left main coronary artery lesions, 7 one vessel, 5 two vessels, and 6 three vessels. Average number of anastomosis was 2.8 per patient. The operative mortality was 14.3 % [4/27]. Among the 10 patients with aortic diseases, 7 cases were aortic dissection[type A: 5, type B: 2] and 3 cases were descending thoracic aortic aneurysm. The operative morality occurred in 3 cases. The overall mortality and the operative mortality of congenital and acquired heart disease was 7.8 %, 4.8% and 10.4%, respectively.

선천성 승모판막 페쇄부전증이 있는 소아에서 승모판막 성형술에 대한 임상적 고찰 (Mitral Valve Repair for Congenital Mitral Regurgitation in Children)

  • 배경: 소아 환자의 승모판막 질환에 대한 외과적 치료는 그 모양의 다양성과 동반된 선천성 심장 기형 등으로 인해 기술적, 임상적으로 어렵다. 본 연구는 선천성 승모판막 폐쇄부전증 환아에서 본원에서 시행한 승모판막 성형 수술의 결과를 평가하였다. 대상 및 방법: 1997년 4월부터 2007년 10월까지 승모판막 폐쇄부전증으로 승모판막 수술을 시행한 소아 환자 22명을 대상으로 후향적으로 분석하였다. 수술 당시 환아의 평균 연령은 5.4세였고, 이 중 12개월 미만의 영아는 4명이었다. 동반된 심기형으로는 심실 중격결손이 13예로 가장 많았고, 그 외에 심방 중격결손과 판막 상부 대동맥 판막 협착증(supravalvar aortic valve stenosis)이 각각 1예 있었으며, 승모판막 페쇄부전증만 단독으로 있는 경우는 7예가 있었다. 수술 전 승모판막 페쇄부전증의 정도는 II, III, IV가 각각 4예, 15예, 3예였고, 그 원인으로 판막 탈출증(leaflet prolapse)이 12예, 판막륜 확장(annular dilatation)이 4예, 제한된 판막 움직임(restrictive leaflet motion)이 5예 있었다. 수술 전후의 페쇄부전증 정도와 승모판막륜의 Z-value를 비교함으로써 그 결과를 평가하였다. 결과: 모든 환자에서 판막 성형술이 가능했고, 수술 후 조기 사망과 만기 사망 모두 얼었다. 19예에서 판막륜 축소술(reduction annuloplasty)을 시행하였고, 18예에서 건삭 축소술(shortening of chordae)이나 판막열 봉합술(cleft closure) 등의 판막 성형술(valvuloplasty)은 추가로 시행하였다. 수술 직후 승모판막 폐쇄부전증의 정도는 줄어들었고(0=10, I=5, II=5, III=2), 승모판막륜의 Z-value도 감소하였다($2.2{\pm}2.1$ vs $0.7{\pm}2.3$, p<0.01). 3.68년의 추적기간 동안 3명의 환아가 승모판막 폐쇄부전증으로 재수술을 받았고, 한명은 재성형술을, 나머지 2명은 승모판막 치환술을 받았으며, 퇴원 시 3명의 환아에서 수술 직 후 보다 폐쇄부전증이 좀 더 진행되었다. 결론: 소아의 승모판막 페쇄부전증에서 일차 수술방법으로 적극적인 승모판막 성형술은 효과적이고 믿을만한 방법이다. 좀 더 좋은 수술 결과를 얻기 위해서는 수술 전 환아의 승모판막의 모양과 기능을 면밀히 파악하고, 다양한 수술적 기법을 적용하는 노력이 필요하겠다.
