  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The strip was illuminated at 45˚c through optical fiber and the intensity of reflected light was determined at rectanguLat angle using a photodiode. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500, 550, 570 and 610nm) for blood glucose strip. For higher concentration than 300mg/41 about glucose, a saturation state of abforbance was observed at 500, 550 and 570nm. The mice were divided into two groups. One group was injected with Photofrin II, intravenously (2.5mg/kg body weight). The other group received no Photofrin II. Mice from both groups were irradialed for approximately 15 minutes at 100, 300, or 500 mW/cm2 with the argon (488nm/514.5 nm), dye(628nm) and gold vapor (pulsed 628 nm) laser light. A photosensitizer behaved as an added absorber. Under our experimental conditions, the presence of Photolfrin II increased surface temperature by at least 40% and the temperature rise due to 300 mW/cm2 irradiation exceeded values for hyperthermia. Light and temperature distributions with depth were estimated by a computer model. The model demonstrated the influence of wavelength on the thermal process and proved to be a valuable tool to investigate internal temperature rise. 30536 T00401030536 ^x We investigated the structural geometry of thirty-eight Korean femurs. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. The results suggested that the femoral component suitable for Asians should be different from the currently-used components designed and manufactured mostly by European or American companies. 30537 T00401030537 ^x It is well known that nonlinear propagation characteristics of the wave in the tissue may give very useful information for the medical diagnoisis. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. Our department has developed an information system for routine hematology using down-sized computer system. We have used an IBM 486 compatible PC with 16MB main memory, 210 MB hard disk drive, 9 RS-232C port and 24 pin dot printer. The operating system and database management system were SCO UNIX and SCO foxbase, respectively. For program development, we used Xbase language provided by SCO foxbase. The C language was used for interface purpose. To make the system use friendly, pull-down menu was used. The system connected to our hospital information system via application program interface (API), so the information related to patient and request details is automatically transmitted to our computer. Our system interfaced with fwd complete blood count analyzers(Sysmex NE-8000 and Coulter STKS) for unidirectional data tansmission from analyzer to computer. The frame and leaflet of the polymer valve were made from polyurethane, To evaluate the hemodynamic performance of the polymer valve a comparative study of flow dynamics past a polymer valve and a St. Jude Medical prosthetic valve under physiological pulsatile flow conditions in vitro was made. Comparisons between the valves were made on the transvalvular pressure drop, regurgitation volume and maximum valve opening area. The polymer valve showed smaller regurgitation volume and transvalvular pressure drop compared to the mechanical valve at higher heart rate. Because the point of maximun temperature of laser-irradiated gelatin exists below the surface due to surface cooling, evaporation at the boiling temperature is made explosively from below the surface. The important parameters of this process are the conduction loss to laser power absorption (defined as the conduction-to-laser power parameter, Nk), the convection heat transfer at the surface to conduction loss (defined as Bi), dimensionless extinction coefficient (defined as Br.), and dimensionless irradiation time (defined as Fo). Dependence of Fo on Nk and Bi has been observed by experiment, and the results have been compared with the numerical results obtained by solving a 2-dimensional conduction equation. Fo and explosion depth (from the surface to the point of maximun temperature) are increased when Nk and Bi are increased.To find out the minimum laser power for explosive evaporative removal process, steady state analysis has been also made. We obtained a two-dimensional current density distribution within a phantom by processing the real and imaginary MR images from a 4.77 NMR machine. We implemented a resistivity mage reconstruction algorithm using the finite element method and sensitivity matrix. Optimal stimulus parameters, which can minimize muscle fatigue and can achieve effective muscle contraction were determined in slow and fast muscles in Sprague-Dawley rats. Stimulus patterns of our stimulator were designed to simulate electromyographic activity monitored during locomotion of normal subjects. Muscle types of the lower extremity were classified according to their mechanical property of contraction, which are slow muscle (msoleus m.) and fast muscle (medial gastrocneminus m., rectus femoris m., vastus lateralis m.). Optimal parameters of electrical stimulation for slow muscles were 20 Hz, 0.2 ms square pulse. For fast muscle, 40 Hz, 0.3 ms square pulse was optimal to produce repeated contraction. Higher stimulus intensity was required when synergistic muscles were stimulated simultaneously than when they were stimulated individually. Electrical stimulation for each muscle was designed to generate bipedal locomotion, so that individual muscles alternate contraction and relaxation to simulate stance and swing phases. Portable electrical stimulator with 16 channels built in microprocessor was constructed and applied to paraplegic patients due to lumbar cord injury. We use combination of MLP and fuzzy filter for pattern recognition to estimate the direction of a muscle and Hogan's method for the force estimation. They include the modules for electrocardiograph, respiration, invasive blood pressure, noninvasive blood pressure, temperature, and SpO2 with Plethysmograph.SpO2 with Plethysmograph.

선형가속기의 10년간 가동률과 고장률에 관한 통계분석 (Statistical Analysis of Operating Efficiency and Failures of a Medical Linear Accelerator for Ten Years)

  • 목적: 방사선치료의 핵심 장비인 선형가속기의 10년간 고장 기록을 분석하여 효율적인 관리 지표로 활용하고자 한다. 대상 및 방법: 1994년 4월부터 2004년 12월까지 10년 8개월간 의공기술과에서 기록한 장비 수리일지와 사용자가 기록한 업무일지의 내용을 통계분석을 통해 관리실태를 분석했다. 장비 장애 요인을 다각적으로 분석하기 위해 고장 원인을 치료기 부위별로 세분했고, 고장 발생 시 치료업무에 미치는 영향에 따라 세 단계로 구분하여 조사했다. 또한, 장비 사용량이 고장에 미치는 영향을 분석하기 위해 연간 치료 환자 수와 고장건수, 중요 부속의 평균 수명, 수리비용 등을 분석했다. 결과: 10년간 발생한 전체 고장건수는 587건이였으며, 이중 조사헤드부의 고장이 전체 건수의 $20\%$ 차지해 가장 높게 나타났고, 고장이 미치는 영향에서는 일시적 장애에 해당하는 '조사능력 저하'상태의 고장이 $41\%$를 차지해 가장 높았다. 고장 부위에 따른 장애 영향 분석에서는 장애 등급이 가장 높은 '조사 불가능'상태의 고장이 가속부에서 전체 건수의 약 $10\%$를 차지해 가장 높게 나타났으며, 고장과 관련된 각종 지표 및 수리비용은 사용연수 및 치료건수가 증가함에 따라 함께 증가했다. 중요 부속의 평균 수명은 클라이스트론과 싸이라트론의 경우 치료건수가 증가함에 따라 교체 주기가 빨라져 각각 제조사 권고치의 $42\%,\;83\%$ 수준이었다. 결론; 안정적인 치료서비스 제공을 위해서는 사용 연수가 증가함에 따라 장비 관리의 필요성이 더욱 중요시되어야하며, 10년간 장비 관리기록을 통해 얻은 각종 지표가 향후 예방적 관리의 좋은 지침이 될 것으로 생각된다.방사선으로 인한 폐손상 시에 항섬유증 약물로의 사용가능성을 확인하였다.방향으로 2.1 mm, 그리고 SI 방향으로 1.7 mm이다. 결론: 연구 결과 SSD, CLD, 클립의 움직임 및 isocenter의 위치변화 측면에서 분석될 경우 그다지 큰 오차는 발생하지 않았음을 보여준다. 그러므로 본 연구결과 유방암 환자의 경우 진단용 CT를 사용한다 하더라도 준비오차는 모의 CT를 사용하는 경우와 비교하여 차이가 없음을 알 수 있다. 그러나 모의치료와 CT스캔 사이의 준비오차를 감소하기 위해서는 CT 영상 획득 시 환자 위치고정에 특별한 주의를 기울여야 한다.EX>$\alpha/\beta$=10인 경우 $62.0\~121.9\;Gy_{10}$ (중앙값: $93.0\;Gy_{10}$)의 분포를, ${\alpha/\beta}=3$인 경우 $93.6\~187.3\;Gy_3$ (중앙값=$137.6\;Gy_3$ )의 분포를 보였다. 통상성 간질성 폐섬유증의 임상경과 (Clinical Course of Usual Interstitial Pneumonia)

  • 연구배경 : 통상성 간질성 폐섬유증(usual interstitial pneumonia : UIP)은 특발성 간질성 폐렴(Idiopathic interstitial pneumonia : I.I.P)의 한 형태로서 생존기간 중앙값이 약 3-5 년 정도로 보고되는 치명적인 질환으로 알려져 있으나, 최근 분류된 BOOP이나 NSIP등을 제외한 순수한 UIP환자들만을 대상으로 임상경과나 예후에 대한 연구는 외국에서도 극소수아고, 국내에서는 보고된 바가 없다. 이에 연구자들은 수술적 폐생검 검사로 확진된 UIP 환자들의 임상경과와 예후의 예측 지표를 찾고자 본 연구를 시행하였다. 방법 : 1989년 3월부터 1999년 8월까지 서울중앙병원에서 개흉 또는 흉강경을 통한 폐생검검사로 UIP로 진단된 72명(연령 $58.2{\pm}11.6$세, 남 : 여=45:27, 관찰기간 중앙값 : 18.1개월 (0.7-103.6개월))을 대상으로 하였다. 진단시의 임상증상과 흉부고해상도단층촬영, 폐기능검사, 동맥혈 가스검사 등을 증상점수(C) (1-20점), 방사선학점수(R)(봉와양 소견 : 0-5점, 젖빛유리야 소견 : 0-5점) 및 생리적검사점수(P) (FVC 1-12점, $FEV_1$ 0-3점, TLC 1-10점, $D_{LCO)$ 0-5점, $AaDO_2$ 0-10점)로 수치화하여 CRP 접수를 계산하였다. 임상경과와 진단시의 CRP점수, 기관지 폐포세척액검사 등을 비교 분석하여 예후의 예측지표를 검색하였다. 결과 : 1) 대상 UIP환자들의 1년 누적생존률은 78.3%, 3년 누적생존률은 58.1% 였고, 생존기간 중앙값은 42.5개월이었다. 2) 단기(1년)예후 : 단기 사망군(14명)은 생존군(46명)에 비하여 전체CRP점수($28.6{\pm}8.3$ vs. $16.6{\pm}9.7$)가 높았으며, 이는 주로 증상점수($8.4{\pm}2.1$ vs. $5.7{\pm}3.9$)와 FVC, $D_{LCO)$, $AaDO_2$를 포함하는 생리적검사점수 $v{\pm}7.5$ vs. $8.1{\pm}7.3$)의 차이에 기인하였다. 또한 호전 혹은 안정군(61.7%)과 악화군 및 사망군(38.3%) 사이에는 연령과 흡연률, 진단당시의 CRP점수(증상점수, FVC, $AaDO_2$, $D_{LCO)$)가 유의한 차이가 있었다. 그러나 방사선학점수, TLC 및 기관지폐포세척액소견 등은 유의한 차이가 없었다. 2) 장기(3년) 예후 : 장기생존군(14명)은 사망군(22명)에 비하여 역시 진단시 총CRP점수($12.2{\pm}6.7$ vs. $28.7{\pm}7.9$)와 증상점수, FVC, $D_{LCO)$, $AaDO_2$)가 차이가 있었다. 3) 생존기간분석에서도 총 CRP 점수와 증상점수, $AaDO_2$ FVC, $D_{LCO)$가생존기간과 유의한 상관관계를 보였다. 4) Cox 회기분석을 이용한 다변수 분석에서는 $D_{LCO)$ (${\geq}$60% : Hazard ratio : 4.56, 95% CI : 2.30-16.04)만이 UIP환자 사망의 독립적인 예측인자로 나타났다. 결론 : 이상의 결과는 UIP 진단시의 $D_{LCO)$는 UIP환자의 예후를 예측할 수 있는 임상적 지표임을 시사하였다.

  • 목 적: 무정자증이나 심한 희소정자증이 있는 남성 불임 환자에서 Y 염색체 미세 결손을 동반하는 경우 체외 수정 및 세포질내 정자 주입술과 배아 이식의 결과에 미치는 영향을 알아보고자 하였다. 연구방법: 후향적 실험군 -대조군 연구로, Y 염색체 미세 결손이 동반된 무정자증이나 심한 희소정자증이 있는 남성 불임 부부 14 쌍을 실험군으로 하였고 대조군으로는 남성 배우자가 무정자증이나 심한 희소정자증이 있지만 Y 염색체는 정상인 12 쌍의 불임 부부를 대상으로 하였으며, 이 두 군을 다시 정자 채취 방법에 따라 사정된 정자를 이용한 군과 고환 내 정자 추출술을 통해 정자를 채취한 군으로 분류하였다. 연구 결과로는 수정률, 등급이 우수한 배아 수, 착상률, 융모막성 성선자극호르몬 양성률, 초기 임신 소실률, 출생률을 비교하였다. 결 과: 등급이 우수한 배아 수, 착상률, 융모막성 성선자극호르몬 양성률, 초기 임신 소실률, 출생률은 Y 염색체 미세 결손이 있는 군과 정상 Y 염색체를 갖는 군 간에 차이가 없었으나, 수정률은 Y 염색체 미세 결손이 있는 불임 부부 (61.1%) 에서 정상 Y 염색체를 갖는 불임 부부 (79.8%, p=0.003) 보다 낮았다. 정자 채취의 방법에 따라 비교하였을 때, 고환 내 정자 추출술을 통해 정자를 채취한 경우 실험군 (52.9%) 에서 대조군 (79.5%, p=0.008) 보다 유의하게 수정률이 낮게 나타났으나 사정된 정자를 이용한 경우에는 실험군에서 대조군 보다 수정률이 더 낮은 경향을 보였으나 유의한 차이는 없었고, 나머지 결과들은 두 군 간에 차이가 없었다. 결 론: Y 염색체 미세 결손이 동반된 무정자증이나 심한 희소정자증이 있는 불임 부부에서 고환 내 정자 추출술을 통해 정자를 채취할 경우 체외 수정 및 세포질내 정자 주입술 결과 정상 Y 염색체를 갖는 불임 부부에서 보다 수정률이 더 낮게 나타났으나, 사정된 정자를 이용하는 경우나 고환 내 정자 추출술을 통해 정자를 채취하여 수정이 된 후에는 Y 염색체의 미세 결손이 체외 수정 및 세포질내 정자 주입술과 배아 이식의 결과에 영향을 주지 않는 것으로 보인다. 그러나 향후 대규모의 전향적 연구가 더 필요할 것으로 사료된다.

초위성체 마커를 활용한 가축다양성정보시스템(DAD-IS) 등재 재래닭 집단의 유전적 다양성 분석 (Genetic Diversity of Korean Native Chicken Populations in DAD-IS Database Using 25 Microsatellite Markers)

  • 본 연구는 세계식량농업기구(FAO) 가축다양성정보시스템(DAD-IS)에 등재되어 있는 우리나라 재래닭 집단의 유전적 다양성 및 외래품종과의 차별성을 분석하기 위해 25개의 초위성체(MS) 마커를 이용하여 총 18개 집단 548수의 유전자형을 분석하였고, 이를 토대로 기대($H_{\exp}$) 및 관측이형접합도($H_{obs}$), 다형정보지수(PIC), 유전거리, 유전적 균일도 등을 계산하였다. 마커별 다형성 분석 결과, 총 195개의 대립유전자가 나타났으며, $H_{\exp}$와 PIC의 경우 MCW0145에서 각각 0.646, 0.569로 가장 높았으며, $H_{obs}$의 경우 ADL0278에서 0.773으로 가장 높은 수치를 보이고 있었던 반면, MCW0078에서는 $H_{\exp}$, $H_{obs}$, PIC가 각각 0.263, 0.291, 0.217로 가장 낮은 것을 확인할 수 있었다. 집단간 다양성 분석 결과로는 MNA, $H_{\exp}$, $H_{obs}$, PIC 모두 황갈색재래종(KNY) 집단(각각 4.60, 0.627, 0.643, 0.563)에서 가장 높게, 횡성약닭(HYD) 집단(각각 1.84, 0.297, 0.286, 0.236)에서 가장 낮게 나타났다. 대립유전자형의 빈도를 바탕으로 계산된 18개 품종간의 DA 유전거리 분석 결과, 횡성약닭(HYD)와 화이트레그혼F(LGF) 집단 사이에서 0.675로 가장 먼 유전거리를 형성하고 있었으며, 같은 품종인 로드아일랜드레드 두 집단(RRC, RRD) 사이에서 0.027로 가장 가까운 유전거리를 보였다. 한편, 같은 품종임에도 불구하고, 코니시 두 집단(COS, COH)사이에서는 0.313의 비교적 먼 유전거리를 나타내고 있었다. 집단의 실제 구조를 확인하기 위한 집단별 균일도 분석 결과, K=15에서 최적의 K값(${\Delta}K:66.22$)을 얻을 수 있었으며, 18개의 집단 중 14개의 집단에서 90% 이상의 높은 유전적 균일도를 나타내며 독립적인 군락을 형성하고 있었다. 또한, 황갈색재래종(KNY), 현인흑계(HIC), 연산오계(KNO) 집단에서도 각각 88.9%, 83.9%, 76.3%로 독립적인 군락을 형성하고 있는 것을 확인할 수 있었다. 반면, 제주재래닭(JJC)의 경우 독립적인 군락을 형성하지 못하고, 황갈색재래종(KNY) 집단이 속해 있는 2번 군락에서 가장 높은 44.3%의 균일도를 보이고 있었으며, 3번 군락(17.7%)과 8번 군락(19.1%)에도 일부 포함되어 있는 것으로 보아 집단 조성 과정에 있어 타집단과의 교잡이 일어났을 것으로 추정되며, 독립적인 집단으로 구분하는 것이 어렵기 때문에 추후 개체식별을 통한 지속적인 계획교배를 실시하여 유전적 고정화 작업이 이루어질 필요성이 있을 것으로 판단된다. 이상의 결과로 DAD-IS에 등재되어 있는 우리나라 재래닭 집단이 외래 토착종 집단과 확연하게 구분이 되며, 각 재래닭 집단간에도 비교적 뚜렷하게 구분되는 것을 확인함으로써, 고유 종자로서의 과학적인 근거를 확보할 수 있었으며, 추후 재래닭 유전자원에 대한 국가 수준의 관리 및 평가를 통해 다양한 육종 소재로 이용할 수 있는 기초자료로써 활용될 수 있을 것으로 보인다.

시장 환경이 인터넷 경로를 포함한 다중 경로 관리에 미치는 영향에 관한 연구: 게임 이론적 접근방법 (The Impact of Market Environments on Optimal Channel Strategy Involving an Internet Channel: A Game Theoretic Approach)

  • 지난 십년동안 인터넷을 통한 전자상거래는 빠른 속도로 성장해 왔다. 이러한 인터넷의 발달은 기업들의 사업방식에 많은 변화를 유도했으며, 그 중에서도 마케팅경로의 구조와 경로 구성원들 사이의 관계에 중요한 변화를 초래하고 있다. 각 기업이 처한 시장환경은 다양하며 이 다양한 시장 환경은 인터넷 경로가 각 시장에 미치는 효과를 조절하는 역할을 한다. 이러한 시장의 다양성에도 불구하고 지금까지의 선행연구들은 각기 특정한 하나의 시장상황(unique setting)을 상정하여 인터넷경로 도입이 그 시장에 미치는 영향을 분석하는데 그쳐왔다. 이러한 기존 연구의 공백을 채우기 위해 본 연구는 시장의 다양성을 소비자의 지리적 분포, 시장의 인터넷 수용도의 측면에서 살펴보고 이러한 시장 환경이 인터넷 경로 도입 효과에 미치는 영향에 관하여 조사해 보고자 한다. 이를 위해 본 연구는 다양한 소비자들의 지리적 분포, 경쟁강도, 소비자의 인터넷 상거래에 대한 수용도 등을 포함한 다양한 시장 환경을 수요모형에 반영시켜 그 영향력 분석을 가능하도록 하였다. 그러나, 다양한 시장 요소를 모형에 반영하는 과정에서 수요모형이 복잡한 구조를 가지게 되었다. 이 문제를 극복하고 게임이론의 균형해를 도출하기 위해 Newton-Raphson algorithm을 사용한 numerical search 방법을 사용하였다. 분석결과 두 종류의 경로에 대한 소비자선호의 분포에 따라 생산자의 가격차별정도, 생산자와 독립소매상 간의 경로이윤 배분율, 그리고 인터넷경로 도입이 각 경로주체의 이윤 향상에 도움이 되는지의 여부, 소비자잉여 등이 달라질 수 있음을 발견하였다. 끝으로 연구의 학술적, 실무적 시사점과 한계점 및 향후 연구방향도 논의되었다.

가족계획과 모자보건 통합을 위한 조산원의 투입효과 분석 -서산지역의 개입연구 평가보고- (An Intervention Study on Integration of Family Planning and Maternal/Infant Care Services in Rural Korea)

  • This project was a service-cum-research effort with a quasi-experimental study design to examine the health benefits of an integrated Family Planning (FP)/Maternal & Child health (MCH) Service approach that provides crucial factors missing in the present on-going programs. The specific objectives were: 1) To test the effectiveness of trained nurse/midwives (MW) assigned as change agents in the Health Sub-Center (HSC) to bring about the changes in the eight FP/MCH indicators, namely; (i)FP/MCH contacts between field workers and their clients (ii) the use of effective FP methods, (iii) the inter-birth interval and/or open interval, (iv) prenatal care by medically qualified personnel, (v) medically supervised deliveries, (vi) the rate of induced abortion, (vii) maternal and infant morbidity, and (viii) preinatal & infant mortality. 2) To measure the integrative linkage (contacts) between MW & HSC workers and between HSC and clients. 3) To examine the organizational or administrative factors influencing integrative linkage between health workers. Study design; The above objectives called for quasi-experimental design setting up a study and control area with and without a midwife. An active intervention program (FP/MCH minimum 'package' program) was conducted for a 2 year period from June 1982-July 1984 in Seosan County and 'before and after' surveys were conducted to measure the change. Service input; This study was undertaken by the Soonchunhyang University in collaboration with WHO. After a baseline survery in 1981, trained nurses/midwives were introduced into two health sub-centers in a rural setting (Seosan county) for a 2 year period from 1982 to 1984. A major service input was the establishment of midwifery services in the existing health delivery system with emphasis on nurse/midwife's role as the link between health workers (nurse aids) and village health workers, and the referral of risk patients to the private physician (OBGY specialist). An evaluation survey was made in August 1984 to assess the effectiveness of this alternative integrated approach in the study areas in comparison with the control area which had normal government services. Method of evaluation; a. In this study, the primary objective was first to examine to what extent the FP/MCH package program brought about changes in the pre-determined eight indicators (outcome and impact measures) and the following relationship was first analyzed; b. Nevertheless, this project did not automatically accept the assumption that if two or more activities were integrated, the results would automatically be better than a non-integrated or categorical program. There is a need to assess the 'integration process' itself within the package program. The process of integration was measured in terms of interactive linkages, or the quantity & quality of contacts between workers & clients and among workers. Intergrative linkages were hypothesized to be influenced by organizational factors at the HSC clinic level including HSC goals, sltrurture, authority, leadership style, resources, and personal characteristics of HSC staff. The extent or degree of integration, as measured by the intensity of integrative linkages, was in turn presumed to influence programme performance. Thus as indicated diagrammatically below, organizational factors constituted the independent variables, integration as the intervening variable and programme performance with respect to family planning and health services as the dependent variable: Concerning organizational factors, however, due to the limited number of HSCs (2 in the study area and 3 in the control area), they were studied by participatory observation of an anthropologist who was independent of the project. In this observation, we examined whether the assumed integration process actually occurred or not. If not, what were the constraints in producing an effective integration process. Summary of Findings; A) Program effects and impact 1. Effects on FP use: During this 2 year action period, FP acceptance increased from 58% in 1981 to 78% in 1984 in both the study and control areas. This increase in both areas was mainly due to the new family planning campaign driven by the Government for the same study period. Therefore, there was no increment of FP acceptance rate due to additional input of MW to the on-going FP program. But in the study area, quality aspects of FP were somewhat improved, having a better continuation rate of IUDs & pills and more use of effective Contraceptive methods in comparison with the control area. 2. Effects of use of MCH services: Between the study and control areas, however, there was a significant difference in maternal and child health care. For example, the coverage of prenatal care was increased from 53% for 1981 birth cohort to 75% for 1984 birth cohort in the study area. In the control area, the same increased from 41% (1981) to 65% (1984). It is noteworthy that almost two thirds of the recent birth cohort received prenatal care even in the control area, indicating that there is a growing demand of MCH care as the size of family norm becomes smaller 3. There has been a substantive increase in delivery care by medical professions in the study area, with an annual increase rate of 10% due to midwives input in the study areas. The project had about two times greater effect on postnatal care (68% vs. 33%) at delivery care(45.2% vs. 26.1%). 4. The study area had better reproductive efficiency (wanted pregancies with FP practice & healthy live births survived by one year old) than the control area, especially among women under 30 (14.1% vs. 9.6%). The proportion of women who preferred the 1st trimester for their first prenatal care rose significantly in the study area as compared to the control area (24% vs 13%). B) Effects on Interactive Linkage 1. This project made a contribution in making several useful steps in the direction of service integration, namely; i) The health workers have become familiar with procedures on how to work together with each other (especially with a midwife) in carrying out their work in FP/MCH and, ii) The health workers have gotten a feeling of the usefulness of family health records (statistical integration) in identifying targets in their own work and their usefulness in caring for family health. 2. On the other hand, because of a lack of required organizational factors, complete linkage was not obtained as the project intended. i) In regards to the government health worker's activities in terms of home visiting there was not much difference between the study & control areas though the MW did more home visiting than Government health workers. ii) In assessing the service performance of MW & health workers, the midwives balanced their workload between 40% FP, 40% MCH & 20% other activities (mainly immunization). However, $85{\sim}90%$ of the services provided by the health workers were other than FP/MCH, mainly for immunizations such as the encephalitis campaign. In the control area, a similar pattern was observed. Over 75% of their service was other than FP/MCH. Therefore, the pattern shows the health workers are a long way from becoming multipurpose workers even though the government is pushing in this direction. 3. Villagers were much more likely to visit the health sub-center clinic in the study area than in the control area (58% vs.31%) and for more combined care (45% vs.23%). C) Organization factors (admistrative integrative issues) 1. When MW (new workers with higher qualification) were introduced to HSC, it was noted that there were conflicts between the existing HSC workers (Nurse aids with less qualification than MW) and the MW for the beginning period of the project. The cause of the conflict was studied by an anthropologist and it was pointed out that these functional integration problems stemmed from the structural inadequacies of the health subcenter organization as indicated below; i) There is still no general consensus about the objectives and goals of the project between the project staff and the existing health workers. ii) There is no formal linkage between the responsibility of each member's job in the health sub-center. iii) There is still little chance for midwives to play a catalytic role or to establish communicative networks between workers in order to link various knowledge and skills to provide better FP/MCH services in the health sub-center. 2. Based on the above findings the project recommended to the County Chief (who has power to control the administrative staff and the technical staff in his county) the following ; i) In order to solve the conflicts between the individual roles and functions in performing health care activities, there must be goals agreed upon by both. ii) The health sub·center must function as an autonomous organization to undertake the integration health project. In order to do that, it is necessary to support administrative considerations, and to establish a communication system for supervision and to control of the health sub-centers. iii) The administrative organization, tentatively, must be organized to bind the health worker's midwive's and director's jobs by an organic relationship in order to achieve the integrative system under the leadership of health sub-center director. After submitting this observation report, there has been better understanding from frequent meetings & communication between HW/MW in FP/MCH work as the program developed. Lessons learned from the Seosan Project (on issues of FP/MCH integration in Korea); 1) A majority or about 80% of the couples are now practicing FP. As indicated by the study, there is a growing demand from clients for the health system to provide more MCH services than FP in order to maintain the achieved small size of family through FP practice. It is fortunate to see that the government is now formulating a MCH policy for the year 2,000 and revising MCH laws and regulations to emphasize more MCH care for achieving a small size family through family planning practice. 2) Goal consensus in FP/MCH shouBd be made among the health workers It administrators, especially to emphasize the need of care of 'wanted' child. But there is a long way to go to realize the 'real' integration of FP into MCH in Korea, unless there is a structural integration FP/MCH because a categorical FP is still first priority to reduce the rate of population growth for economic reasons but not yet for health/welfare reasons in practice. 3) There should be more financial allocation: (i) a midwife should be made available to help to promote the MCH program and coordinate services, (in) there should be a health sub·center director who can provide leadership training for managing the integrated program. There is a need for 'organizational support', if the decision of integration is made to obtain benefit from both FP & MCH. In other words, costs should be paid equally to both FP/MCH. The integration slogan itself, without the commitment of paying such costs, is powerless to advocate it. 4) Need of management training for middle level health personnel is more acute as the Government has already constructed 90 MCH centers attached to the County Health Center but without adequate manpower, facilities, and guidelines for integrating the work of both FP and MCH. 5) The local government still considers these MCH centers only as delivery centers to take care only of those visiting maternity cases. The MCH center should be a center for the managment of all pregnancies occurring in the community and the promotion of FP with a systematic and effective linkage of resources available in the county such as i.e. Village Health Worker, Community Health Practitioner, Health Sub-center Physicians & Health workers, Doctors and Midwives in MCH center, OBGY Specialists in clinics & hospitals as practiced by the Seosan project at primary health care level.
