식물추출혼합물의 니코틴 분해능에 미치는 효과 (Effects of Phyto-Extract Mixture on the Nicotine Decomposition)

  • 천연식물소재 및 한약재 추출제재인 식물추출 혼합제재가 인체내에서 니코틴 분해능에 미치는 영향을 FRCFR5 세포주, Xenopus oocyte, 임상 실험을 통하여 검토하였다. 본 실험은 체내에 잔존하는 니코틴이 식물추출 홉합 제재에 의해 무독한 대사산물인 코티닌으로 분해량이 증가되고 동시에 NNK, NNN, NNA 등과 같은 폐암 유발물질인 니트로사민 유도체 생성 경로가 억제될 것이라는 가정을 전제로 실험을 수행하였다. 본 실험 결과에서 볼 수 있듯이 식물추출 혼합 제재에는 니코틴에서 코티닌으로 전환시키는 대사 활성물질이 함유되어 있다는 사실을 알 수 있으나, 실제로 어떤 유효성분들이 관여하는지 그리고 정확한 작용 기작을 규명하기 위해서는 더 많은 분석 및 생화학적 연구가 앞으로 수행되어야 할 것이다. 간세포에서 유래된 FRCFR5 세포주 실험 결과, 니코틴과 식물추출 혼합 제재가 첨가된 배지에서 니코틴과 물을 첨가한 배지보다 니코틴에서 코티닌으로 전환능력이 약 2∼3배 높게 나타났으며, 이러한 결과는 Xenopus oocyte에 직접 주사한 경우와 거의 비슷한 양상을 보였다. 임상실험 결과 식물추출 혼합 제재음료를 음용하고 담배를 피운 실험군이 물을 음용하고 담배를 피운 대조군에 비해 약 2배 정도 높은 코티닌 함량을 나타내었다. 이는 실험군의 소변 중에 계속적으로 다량의 코티닌이 배출되는 것을 의미하며, 식물추출 혼합 제재 섭취시 체내에 존재하는 니코틴이 코티닌으로 지속적이면서 효과적으로 전환되는 것을 말한다 이상의 생체 내·외 실험에서 알 수 있듯이 식물추출혼합물은 니코틴에서 코티닌 생성을 약 2배정도 증가 시키는 것을 알 수 있었다.

  • The author studied the action of autonomic drugs on the urinary bladder isolated from Ditrema temmincki Bleaker and the results obtained were summarized as follows: 1) The motility of the urinary bladder of the fish was stimulated by acetylcholine and physostigmine. The stimulating action of these drugs was antagonized by atropine. 2) The motility of the fish bladder was stimulated by epinephrine, nor-epinephrine and phenylephrine, but inhibited by isoproterenol. 3) The inhibitory effects of isoproterenol on the fish bladder was not affected by phenoxybenzamine, but blocked by propranolol. 4) The excitatory effects of phenylephrine on the fish bladder were blocked by phenoxybenzamine, but augmented by propranolol. 5) The excitatory effects of epinephrine and nor-epinephrine were reversed by phenoxybenzamine and augmented by propranolol. 6) The motility of the fish bladder pretreated with phenoxybenzamine and propranolol was not affected by isoproterenol, phenylephrine, epinephrine and nor-epinephrine. 7) It seemed that the bladder muscle of the fish had both alpha excitatory and beta inhibitory receptors. 8) The motility of the fish bladder was stimulated by nicotine and DMPP. The excitatory effects of these drugs were abolished by pretreatment with hexamethonium or atropine. 9) It is, therefore, concluded that there are ganglion cells furnished with cholinergic fiber in the bladder wall of the fish.

A Medicinal Herbal Tea Increases Success Rate and Reduces Withdraw Symptoms of Smoking Cessation in Men

  • The efficacy of a medicinal herb tea (MHT) for assisting smoking cessation and for reducing withdrawal symptoms was evaluated by a placebo controlled subjective test in 100 male smokers. The MHT group experienced less craving for the taste of tobacco, and was more successful at quitting smoking. Among the subjects continuing to the final stage, 23 subjects (38.3 %) who took the MHT and 2 subjects (10.0 %) who took the placebo tea succeeded in quitting smoking. In the group taking MHT, urinary cotinine concentration increased during the first 2 weeks and then greatly decreased from weeks 2 to 4. The urinary cotinine concentration in the MHT group at week 0 was 38.6 mol/L but decreased to 24.5 mol/L by the end of week 4. MHT also reduced withdrawal symptoms, a frequent reason for failure in reducing or quitting smoking. The effects of the herbal tea on smoking cessation and withdrawal symptoms may be attributed to increased metabolism of nicotine and other effects of unidentified components of the herbal ingredients.

  • 최근에는 직접 흡연 뿐만 아니라, 간접 흡연에 의한 인체 유해 영향에 대한 가능성이 공중 보건 문제의 하나로 그 중요성이 증가하고 있다. Brownson등은 1997년 간접 흡연이 폐암의 위험을 증가시킨다는 보고를 하였으며, 간접 흡연은 아동들의 호흡기 질환을 증가시켜 흡연자 가정에 가는 아이들은 비흡연자 가정에 사는 아이들보다 호흡기 감염(기관지염, 폐렴)에 더욱 민감하다고 보고되고 있다(Greenberg, 1988; Zmirou D 등, 1990; Tredaniel J 등, 1989). 따라서 최근에는 간접흡연자들의 환경중 담배연기(ETS) 노출의 잠재적인 건강 영향에 대한 관심이 증가하고 있는 추세이다. 청소년 흡연 자가보고와 요코티닌 검사간의 일치도 (Agreement between Smoking Self-report and Urine Cotinine among Adolescents)

  • Objectives : Cotinine, the major metabolite of nicotine, is a useful marker of exposure to tobacco smoke and self-reporting of smoking status is thought not to be reliable. This study aimed to evaluate the agreement between the smoking self-report among adolescents and the urinary cotinine test. Methods : The study subjects were 1226 middle and high school students in Hanam city, who were selected by stratified random sampling. The self-report about smoking behavior was compared with urine cotinine value measured with PBM $AccuSign^{\circledR}fi$ Nicotine(Princeton BioMeditech Corporation, USA). The percentage agreement, kappa and 95% confidence interval(CI) were calculated. Results : The overall percentage agreement was 88.6%, and those for boys, girls, middle school, general school and vocational school students were 87.3%, 90.1%, 93.7%, 85,5%, 90.7%, and 78.4%, respectively. The overall kappa index was 0.46(95% CI=0.39-0.54)for overall, .and those for boys, girls, middle school, general school and vocational school students were 0.56(95% CI=0.48-0.65), 0.20(95% CI=0.07-0.32), 0.21(95% CI=0.09-0.34), 0.55(95% CI=0.47-0.64), 0.42(95% CI=0.33-0.52), and 0.48(95% CI=0.36-0.60), respectively. Conclusion : The percentage agreement was relatively high but the kappa values very low for girls, and middle school students. Though the prevalence bias can be influenced by these results, the self-report was not a sufficient tool for the evaluation of adolescents' smoking status, especially in girls or middle school students.

Effect on Smoking Quit Rate of Telling Smokers their Health Risk Appraisal in Terms of Health Age: A Randomized Control Trial

  • Background: We evaluated whether providing health risk appraisal for Koreans (KHRA) in terms of 'health age' during smoking cessation program would effectively help smokers quit smoking or not. Materials and Methods: A total of 332 male smokers aged between 30-65 years old, registered for a smoking cessation program in a public health center in a city, were recruited and underwent a baseline survey from January 2010 to February 2011. They were then prospectively randomized to a conventional counseling group (n=165) or a KHRA group (n=167), and received conventional counseling or KHRA-based counseling for six months. Abstinence rates were identified through carbon monoxide measurement (at the $4^{th}$ and $24^{th}$ weeks) or urinary cotinine level (at the $12^th$ week). Results: The abstinence rate confirmed by exhaled carbon monoxide was significantly higher in the KHRA group (61.1%) than the control group (49.1%) at the $4^{th}$ week (absolute difference 12.0%, 95% CI: 1.4%-22.6%). However, there was no difference in abstinence rates between the two groups at the 12th and 24th weeks. The predicting factors of 24 week's smoking cessation success were age, older than 50 years old (OR 2.02, 95% CI: 1.16-3.52), lower Fagerstr$\ddot{o}$m Test for Nicotine Dependence score less than 4 (OR 1.84, 95% CI: 1.03-3.29), and higher Self Efficacy/Temptation score (OR 1.79, 95% CI: 1.05-3.06). Conclusions: Smoking cessation counseling with KHRA could be effective compared to conventional counseling in the short period of smoking cessation. Further study is needed to evaluate the long-term efficacy of KHRA in tobacco dependence treatment and to establish the indication and target population of this tool.
