Backgroud and Purpose ; I intented to be helpful to treat diabetes through a clinical study of effect of soyangin Yangkyuksanwha-tang(凉膈散火湯). Methods : I studied 5 patients with upper diabetes who were diagnosed by Soyangin at department of Sasang constitutional Medicine in Dongeui Oriental Hospital during 1 year from August. 2001 to July. 2002. and then I investigated the clinical symptoms of the diabetics and analyzed level of examination after administration of Yangkyuksanwha-tang. Results and Conclusion ; Yangkyuksanwha-tang was administered to five diabetic patients, So-yang-in for 8 weeks. The symptoms such as frequent urination(P<0.05), thirst with frequent drinking water(P<0.01) have improved after the treatment. As results, level of FBS(P<0.01), total protein(P<0.05) and blood pressure(P<0.05) significantly decreased after administration of Yangkyuksanwha-tang.
Objectives : In this paper, lung atrophy syndrome[肺痿] in 『Huangdineijin(黃帝內經)』 and 『Jinguiyaolue(金匱要略)』 were compared, followed by examining its relation with upper wasting thirst[上消]. Also, ways in which psychological factors that contribute to lung atrophy syndrome could cause upper wasting thirst were studied. Methods : Verses from 『金匱要略·肺痿肺癰咳嗽上氣病脈證治』 and 『素問·痿論』 were analyzed based on various annotators's opinions to determine the cause and mechanism of lung atrophy syndrome and its relationship with upper wasting thirst. Results : In 『Jinguiyaolue(金匱要略)』, lung atrophy syndrome is described as the heat of the upper body entering the lungs to dry it out. The description in 『Suwen(素問)』 differs in that it accompanies atrophy symptoms, but the mechanism is the same. Lung atrophy syndrome in 『Jinguiyaolue』 could come from wasting thirst, while wasting thirst can be accompanied in deficiency caused by chronic lung atrophy syndrome. Heat in the lungs is caused by psychological factors where the person has lost its subject of possession or was unable to attain what was desired. When expanded to include heart atrophy syndrome[心痿] and lung atrophy syndrome[肝痿], the reason for upper wasting thirst could include immense sadness or excessive indulgence in pleasure due to unmet desires. Conclusions : Although diabetes and wasting thirst are not identical, application of wasting thirst pattern differentiation to diabetes treatment and management could lead to tailored treatment of each patient. Moreover, the five zhang pattern differentiation from the 『Suwen(素問)』 could increase treatment efficacy when applied to conditions caused by stress and emotional disorder, which are increasingly playing larger roles in causing wasting thirst, or diabetes.

A 60-year-old woman with a history of diabetes mellitus and chronic renal failure was admitted to the hospital with severe pain in the upper lip, which began 4 days prior to admission, accompanied by a bullous lesion and suspected cellulitis in the upper lip. Immediately after admission, as the patient's general condition worsened, tests revealed a non-ST elevated myocardial infarction, septic embolism of the lung, as well as septic shock. Her upper lip suddenly presented a gangrenous and necrotic change, which the tissue and blood culture confirmed to be a Klebsiella pneumoniae infection. After a quick response, the patient's general condition improved. Subsequently, serial debridement was performed to effectively clear away the purulent discharge. While under general anesthesia, the process confirmed full-layer necrosis of the upper lip including the orbicularis oris muscle. Almost half of the entire upper lip sustained a full-layer skin and soft tissue defect, with scar contracture. Six months later, to correct the drooling and lip sealing following the defects, a scar release and an Abbe flap coverage were performed considering both functional and aesthetic aspects. The follow-up revealed a favorable corrective result of the upper lip drooling, and the patient was satisfied from a functional perspective.
This study was aimed to investigate whether abdominal obesity is associated with non-insulin dependednt diabetes. The distribution of body fat patterns was observed in 181 female patients with diabetes, aged 33 to 83 years, living in the Taegu area, Korea. The following anthropometric measuremetns were made on all participants from October 1 to November 25, 1991 : weight, height ; waist and hip circumferences in standing position. The waist hip circumference ratio was used as an index of abdominal obesity. The results were as follows; 1) The mean fasting blood glucose of diabetic subjects was 145$\pm$50.3mg/dl and the mean duration of diabetes was 4.7$\pm$7.5 years. 2) Obese subjects above the ideal body weight body weight of 120% in the investigation are presently 52%, but 63% of subjects were reported to be obese in the past. The mean BMI of the subjects is 24.57$\pm$3.15 and the past mean BMI was 27.13$\pm$3.26. One year after reaching their highest body weight, 47% of the subjects developed diabetes. Two years after reaching their peak body weight, 74% of diabetic subjects developed diabetes. 3) Using the waist-hip circumference ratio, subjects beloing to the upper body obesity(WHR>0.84) were 65.5%. 4) The average daily energy intake did not differ between the obese and non-obese diabetic subjects, whether they were assessed with BMI or with RBW. 5) The average daily energy intake was higher in the upper body obesity subjects than in the lower body obesity subjects. 6) Diabetics withing the regular exercise group had lower fasting blood glucose levels than the non-regular exercise group. Exercise did not effect the RBW, BMI, and WHR. 7) The waist-to-hip circumference ratio correlated significantly in positive with waist-circumference, but did not correlated with hip-circumference. Therefore, WHR may depended on the increased accumulation of abdominal fat in female diabetics. In conclusion, these findings suggest that caloric intake is more associated with abdominal fat accumulation in diabetic women. Blood glucose concentration is independently effected by exercise, and exercise does not affect the WHR. Therefore, control of caloric intake and development of specific exercises to change the WHR seems important for controling diabetes in female subjects.

Several essential metals are vital for the appropriate performance of various enzymes, transcriptional factors and proteins that are essential in various biochemical paths. Metals like zinc (Zn), magnesium (Mg), and manganese (Mn) are cofactors of hundreds of enzymes. Zn is involved in the synthesis and secretion of insulin from the pancreatic ${\beta}-cells$. Chromium (Cr) increases the insulin receptors activity on target tissues, mainly in muscle cells. Insulin hormone is required to maintain the blood glucose amount in normal range. Continual increase of blood serum glucose level leads to marked chronic hyperglycemia or diabetes mellitus. Deficiency of insulin or its resistance, blood glucose level exceeds the upper limit of the common range of 126 mg/dl. Poor glucose control and diabetes changes the levels of essential trace elements such as Zn, Mg, Mn, Cr, iron etc. by rising urinary excretion and their related decrease in the blood. The aim of this article to discusses the important roles of essential trace elements in particular perspective of type 2 diabetes.
Patients with diabetes mellitus show delayed wound healing and increased susceptibility to infection. Therefore, the effects of diabetes on pulpal and periodontal healing should be taken into consideration when treating diabetic dental traumatized patients. This case presents the treatment for dental traumatized 20 yr old female with uncontrolled type II diabetes. The traumatized upper central incisors had showed pulpal healing in early days. However, 7 mon after the trauma, the teeth had been diagnosed with pulp necrosis with apical abscess. Eventually, non surgical root canal treatment on the teeth had been performed.
Objectives : To prove the channel-tropism theory of herbal medicines on diabetes mellitus as emaciation-thirst disease in Korean Medicine Theory, we investigated the selective therapeutic effects of Mori Cortex Radidus (MCR), Schisandrae Fructus (SF), Anemarrhenae Rhizoma (AR) for the upper emaciation on different organs in high fat and high sucrose (HF/HS) feeding-induced prediabetic mice. Methods : Diabetes in C57BL/6 mice was induced by the administration of high fat (45 kal%) and high sucrose (32 kal%) for 8 weeks, and them treated with each extract at 250 or 500 mg/kg body weight for 4 weeks (once a day). Oral glucose tolerance test and body weight was measured once a week. Insulin, total cholesterol, triglyceride, ${\gamma}GTP$, GOT and GPT were measured in the sera of all mice. Histopathological changes of different organs, lung, heart, pancreas, stomach, liver, and kidney were observed by H&E staining. Results : The results revealed that MCR extract inhibited the impaired glucose tolerance and lung damage, and increased serum insulin levels in HF/HS-induced prediabetic mice. SF extract inhibited the impaired glucose tolerance and lung damage, increased serum insulin levels, and decreased serum triglycerige levels. Meanwhile, AR extract inhibited the impaired glucose tolerance and lung damage, and decreased serum levels of insulin, total cholesterol and triglycerige levels. Conclusions : These results demonstrated that MCR, SF, and AR extract as the upper emaciation herbal medicines were followed their channel-tropism theory like a lung, and may have a selective therapeutic potential for control of diabetic stage.

Diabetes mellitus (DM) leads to a variety of complications and thus we have retrospectively studied to investigate problems of nerve conduction velocity (NCV) study and the heart in the patients with type-II DM. Blood glucose and blood pressure levels were higher in DM group than in Non-DM group. We found that several latencies were delayed in motor conduction study of upper (median and ulnar nerve) and lower extremities (peroneal and tibial nerve), whereas amplitudes and NCVs were decreased in DM group compared with Non-DM group. Latencies of sensory conduction study in upper and lower extremities (sural nerve) were delayed, while amplitudes and NCVs were lower in DM group than in Non-DM group. Abnormal percent of the electrocardiogram was higher in DM group than in Non-DM group. This retrospective study suggests that type-II DM can cause a damage effect on the peripheral nerve and the heart function.
Background: Prior studies have reported that 40%-90% of the patients with celiac plexus-mediated visceral pain benefit from the neurolytic celiac plexus block (NCPB), but the predictive factors of response to NCPB have not been evaluated extensively. This study aimed to identify the factors associated with the immediate analgesic effectiveness of NCPB in patients with intractable upper abdominal cancer-related pain. Methods: A retrospective review was performed of 513 patients who underwent NCPB for upper abdominal cancer-related pain. Response to the procedure was defined as (1) a decrease of ≥ 50% or ≥ 4 points on the numerical rating scale (NRS) in pain intensity from the baseline without an increase in opioid requirement, or (2) a decrease of ≥ 30% or ≥ 2 points on the NRS from the baseline with simultaneously reduced opioid consumption after NCPB. Logistic regression analysis was performed to determine the factors associated with successful responses to NCPB. Results: Among the 513 patients included in the analysis, 255 (49.8%) and 258 (50.2%) patients were in the non-responder and responder group after NCPB, respectively. Multivariable logistic regression analysis showed that diabetes (odds ratio [OR] = 0.644, P = 0.035), history of upper abdominal surgery (OR = 0.691, P = 0.040), and celiac metastasis (OR = 1.496, P = 0.039) were the independent factors associated with response to NCPB. Conclusions: Celiac plexus metastases, absence of diabetes, and absence of prior upper abdominal surgery may be independently associated with better response to NCPB for upper abdominal cancer-related pain.
Objectives: The purpose of the study is to investigate the influencing factors of metabolic syndrome on the periodontal disease in the Korean adults over 40 years old. Methods: The data were extracted from the Fifth National Health Nutrition Survey 2010. The subjects were adults over 40 years old. The periodontal diseases were examined by direct survey. The questionnaire consisted of the general characteristics of the subjects, oral health behavior, metabolic syndrome, and relation to periodontal diseases. CPI(community periodontal index) was measured. CPI indicates periodontal condition and classified into alveolar arch of the upper jaw and the lower jaw. A periodontal disease is defined as $CPI{\geq}3$(over 4 mm). The variables for metabolic diseases included hypertension, diabetes mellitus, dyslipidemia, and obesity measured as BMI. Results: Diabetes mellitus is closely related to periodontal diseases prevalence. Low level of HDL increase the periodontal diseases. Number of toothbrushung, use of dental floss and interdental toothbrush as for the number of toothbrushing, more than twice toothbrushing a day, and use of dental floss and interdental toothbrush reduced periodontal diseases. Conclusions: The risk factors of periodontal disease in Korean adults over 40 years old were sex, age, residence, monthly income, education level, smoking, diabetes and low HDL cholesterol.
