Healthcare is a research field suitable for applying the recent ubiquitous techniques. As a test system, we developed a kind of CDSS (Clinical Decision Support System) running in ubiquitous environment. called as 'uCDSS'. The uCDSS is a core system of the ubiquitous healthcare and is composed of some 'uMLMs(Ubiquitous Medical Logic Modules)'. The uMLMs based on the class in C# programming language could be reused in development of CDSS, or another EHR system running in .NET environment. As a test system, we developed the DM(Diabetes Mellitus knowledge system using ASP.NET. This system shows the potential of C# class-based uMLMs and the extensibility to any .NET development project.

Purpose: We analyzed the need for home care services and awareness of U-healthcare in nurses of the Catholic Parish Home Care Center to link the ubiquitous health care system and the home care. Methods: We recruited 46 nurses from a home care center in the catholic parish of the C medical center from April 4th to June 8th, 2007. Results: The highest needs were 'assessment and diagnosis of the problem', 'management of hypertension & diabetes patients', 'counseling of the patient', and 'counseling of major caregiver and family problems'. Therapeutic nursing showed the highest needs in bedsore care. Nurses want hospital medical records available through the ubiquitous health care system. Conclusion: Home care services are supported from the agency, with high needs in assessing and diagnosing the problem, counseling, and managing of hypertension & diabetes patients. Education and public relations efforts on the U-healthcare system should improve system awareness.

유비쿼터스 컴퓨팅 시대의 도래와 삶의 질 향상을 위한 개개인의 건강 유지와 증진에 대한 욕구는 유비쿼터스 라이프캐어(u-LifeCare)산업의 발전을 촉진시키고 있다. 본 논문에서는 기존 헬스캐어(HealthCare) 시스템에 관련된 연구들의 문제점을 분석하고 이를 해결하기위한 새로운 u-LifeCare 서비스 시스템의 구조를 제안한다. 기존 헬스캐어 시스템의 구조는 대부분 중앙집중형 Client/Server의 구조를 가진다. 이런 구조는 서비스 제공을 위한 모든 처리와 Data가 중앙 서버에 집중되므로 서버의 오버헤드 현상을 발생 시킬 수 있으며 이는 사용자에 대한 서비스 응답시간(Response time)을 증대시키고 심할 경우 서버다운 현상을 초래 할 수 있다. 제안하는 구조에서는 Surrogate를 동적으로 발견하고 Computing resource로 이용함으로써 이러한 문제점들을 해결하고 효율적인 서비스를 제공할 수 있다.

건강과 복지에 대한 사람들의 관심이 증대함에 따라 고령층 인구를 중심으로 병원에서 건강 점검 및 진료가 이루어졌던 기존의 Health산업 패러다임에서 연령층에 관계없이 언제, 어디서나 진료를 받을 수 있는 u-Health산업 패러다임으로 변화하고 있다. 이러한 흐름에 발맞추어 유비쿼터스 컴퓨팅 환경에서 혈압, 맥박, 심전도 등과 같은 의료정보뿐만 아니라 일상생활에서 획득한 정보를 통해 건강증진, 질병예방, 진료를 가능하게 하는 u-LifeCare 의료체계가 필요하다. u-LifeCare 를 지원하는 미들웨어는 유비쿼터스 환경에서 필수적인 기술 요소인 센서 네트워크 미들웨어와 상황인지 미들웨어를 통합하는 미들웨어이며, 라이프케어의 특성을 고려한 '초경량(LightWeight)의, 확장성(Scalability)있는, 보안성(Security)을 갖춘' 미들웨어이다.

Purpose: The purpose of this study was to identify the influence of interpersonal attitude (I+, I-, U+, U-) on communication competence in care workers for frail elderly. Methods: This study was a cross-sectional survey. The data were collected from 153 care workers for frail elderly using a structured questionnaire. The data were analyzed using multiple regression with the SPSS/WIN 20.0 program. Results: The interpersonal attitude style of subjects was I+U+, and the score of communication competence was 3.53. There were significant differences in interpersonal attitude (I+, I-), and communication competence depending on age, education level, experience of personality type test (yes). Factors influencing on communication competence in care workers were interpersonal attitude (I+, I-, U+) and experience of personality type test with $R^2$ value of 48.5% (F=23.47 p<.001). The most influencing factor was I+ (${\beta}$=.36), followed by I- (${\beta}$=-.22), U+ (${\beta}$=.20), and experience of personality type test (yes) (${\beta}$=.16). Conclusion: It is needed to maintain the interpersonal attitude style (I+U+) of care workers. Continuing education program is needed for increasing communication competence especially for those fifties and over, and experienced care workers. Giving an opportunity for personality test is helpful to increase communication competence in care workers.

The Unites States has been plagued with soaring health care costs and an alarmingly large number of uninsured population. The Patient Protection and Affordable Care Act of 2010 ushered in the most sweeping health care reform in the United States since the introduction of Medicare and Medicaid in 1965 to address these issues. The law's requirement for individuals to purchase health insurance (the so-called "individual mandate"), however, not only caused a political stir but also prompted constitutional challenges. Some questioned whether the federal government, lacking general police power, could require its citizens to buy unwanted insurance based on its enumerated powers under the U.S. Constitution. This paper summarizes the decision of the U.S. Supreme Court on the constitutionality of individual mandate, and explores how the decision relates to Korea's own universal health care.

This research is to develop PPG filtering method for respiration measurement in U-Health Care system. Respiration rate was determined by filtering PPG and analyzing its spectrum. Optimal filter of PPG has been selected to get respiration by testing 120 sets of experiment data using 700 filtering cases. As a result, 2nd order Bessel-filter that used band-pass cutoff frequency at 0.175~0.4Hz with second order was good at developing respiration signal. Respiration signal in time domain could be continuously analyzed by converting frequency domain using spectrum analysis. 24 seconds has been found to be optimal time duration of collecting PPG data for determining respiration. Therefore, this study was successful of getting not only heart activity but also respiration by only PPG. Minimal invasive measurement obtaining multi-bio information by one sensor can be expected to apply to U-Health Care and human computer interaction.

u-Health Care 시스템은 장기요양 환자 및 만성질환 보유자에게 의료비 절감 및 수준 높은 의료서비스를 제공 할 수 있는 방안이다. 이러한 의료 서비스를 제공하기 위해 필요한 구성으로 본 논문에선 생체신호 취득 단말기, 신호를 전송하는 스마트폰, 신호를 분석해 환자의 건강 기저선을 분석 할 수 있는 서버로 나뉠 수 있다. 본 논문에서는 이러한 환자에게서 체온, 혈압, 혈당, 산소포화도, 맥박, 심전도, 근전도에 해당하는 생체신호를 수집하는 u-Health Care 시스템을 구성하고 환자의 생체신호를 숫자형 데이터, 심전도, 근전도로 분류해 환자의 생체신호를 분석, 건강이상 상태를 파악하는 자동 분석 시스템을 구현 하였다.

기존의 u-HealthCare 서비스는 환자에 대해서 일정한 공간에서의 센서의 on/off 타입에 대한 모니터링/환자관리의 서비스를 제공하였다. 이러한 환경하에서의 주된 서비스는 현재 환자의 상태에 대한 수동적인 형태이다. 이러한 문제점들을 해결하기 위해 센서 데이터에 대한 연속센서 데이터마이닝 기법을 이용한다. USN의 응용서비스인 u-HealthCare 서비스는 센서데이터로부터 생체정보 및 위치정보를 이용하여 환자/보호자/관련 의료진에게 필요한 정보를 제공한다. 이것은 환자에 대한 관리/모니터링뿐만 아니라 환자의 상태에 따른 센싱(sensing)된 데이터를 이용한 패턴(pattern), 예측-(prediction), 이상치(outlier)를 분석함으로써 보다 나은 서비스를 제공할 수 있다. 본 논문에서는 센서 데이터에 대해 새로운 연속 센서데이터 마이닝 기법을 적용하여 질의를 통해 지식을 추출하고 보다 지능화된 서비스를 제공할 수 있는 응용서비스 기법을 제안한다.

최근 우리 사회의 인구 구성 중, 노인 인구 비율이 크게 늘어남에 따라 실버 세대를 위한 체계적이고 신속한 의료 환경이 요구되고 있다. 이러한 요구에 의해 유비쿼터스 IT 기술과 센싱, 네트워크 인터페이스 등을 통한 새로운 건강 관리 서비스인 U-SilverCare 서비스 개발이 활발하게 이루어지고 있다. 본 논문에서는 가속도 센서(Accelerometer)를 이용하여 실버 세대를 위한 U-SilverCare 응급상황통보 서비스를 구상 및 구현한다.
