교정치료시기에 관한 연구 (ORTHODONTIC TREATMENT TIMING)

  • In the treatment of functional orthodontic problems, timing is not an issue. All orthodontists start as soon as the condition is recognized. However, there is an active dialogue concerning treatment timing for structure problems. The major points in contention center around the operator's ability is to control the growth of the facial bones and to maintain post-treatment tooth position through the maturation period (especially when this position was gained by techniques involving arch expansion or distal driving of posterior segments). Factors taken into account to determine the best time of orthodontic treatment include diagnosis, interception, growth rate, patient cooperation, eruptive state and treatment period. With those exceptions of all functional problems, mild dental discrepancies and skeletal deficiencies with a predictably excellent growth potential (early treatment), the period immediately following the eruption of the permanent second molars is the period during which most orthodontic treatment should be initiated. At this time the full volume of tooth substances is present, the individual growth pattern in well established, there are sufficient teeth to receive nearly any type of appliances and the patient can easily tolerate the wearing of appliances.

Early Decompression in Acute Spinal Cord Injury : Review and Update

  • Spinal cord injury (SCI) has a significant negative effect on the quality of life due to permanent neurologic damage and economic burden by continuous treatment and rehabilitation. However, determining the correct approach to ensure optimal clinical outcomes can be challenging and remains highly controversial. In particular, with the introduction of the concept of early decompression in brain pathology, the discussion of the timing of decompression in SCI has emerged. In addition to that, the concept of "time is spine" has been added recently, and the mortality and complications caused by SCI have been reduced by providing timely and professional treatment to patients. However, there are many difficulties in establishing international clinical guidelines for the timing of early decompression in SCI because policies for each country and medical institution differ according to the circumstances of medical infrastructure and economic conditions in the surgical treatment of SCI. Therefore, we aim to provide a current review of timing of early decompression in patient with SCI.

  • Variable valve timing unit, which is able to decrease environmental load and improve fuel economy is loaded onto many automobiles recently. This unit consists mainly of sprocket, housing and rotor. These parts are requested different properties according to environment. We produce sintered parts for variable valve timing unit by selecting compact, sinter process and special treatment according to demanded properties. In this paper, demanded properties of sintered parts for variable valve timing unit and adopted technique to satisfy them are presented.

Secondary bone grafting for alveolar clefts: surgical timing, graft materials, and evaluation methods

  • Alveolar cleft belongs to the spectrum of cleft lip and/or palate, affecting 75% of cleft lip/palate patients. The goals of alveolar cleft treatment are stabilizing the maxillary arch, separating the nasal and oral cavities, and providing bony support for both erupting teeth and the nasal base via the piriform aperture. Secondary alveolar bone grafting is a well-established treatment option for alveolar cleft. Secondary alveolar bone grafting is performed during the period of mixed dentition using autologous bone from various donor sites. There are several issues relevant to maximizing the success of secondary alveolar bone grafting, including the surgical timing, graft material, and surgical technique. In this study, we reviewed issues related to surgical timing, graft materials, and evaluation methods in secondary alveolar bone grafting.

The difference of castration timing of Korean Hanwoo bulls does not significantly affect the carcass characteristics

  • It is already well known that castration improves marbling quality but exact timing of castration is still highly debated in beef cattle production industry. After castration, blood hormonal changes occur in steer and objective of this study was to investigate the effects of growth hormone (GH) levels on adipocyte differentiation in stromal vascular cells (SVCs) and transdifferentiation into adipocytes in C2C12 myoblasts. Total GH concentrations were measured via enzyme-linked immunosorbent assay (ELISA) in 24 male calves and 4 female calves. Cell proliferation, cellular triglyceride (TG) accumulation, and the cell's lipolytic capability were measured in C2C12 myoblasts and SVCs. Myogenic, adipogenic, and brown adipocyte-specific gene expression was measured via real-time polymerase chain reaction (PCR) using SYBR green. Serum GH levels were the highest in late-castrated calves. Treatment with 5 ng/mL GH resulted in greater TG accumulation as well as increased CCAAT-enhancer-binding protein (C/EBP)α and peroxisome proliferator-activated receptor (PPAR)γ expression compared to that after treatment with 15 ng/mL GH. Treatment with 5 ng/mL GH also resulted in lower myogenin (myo)G and myoD expression compared to that after treatment with 15 ng/mL GH. The expression of bone morphogenetic protein (BMP) 7 after treatment with 5 ng/mL GH was higher than that after treatment with 15 ng/mL GH. But carcass characteristics data showed no significant difference between early and late castrated steers. Therefore, our results indicate that castration timing does not seem to be inevitable determinate of carcass qualities, particularly carcass weight and marbling score in Hanwoo beef cattle.

The Effects of Addition Timing of NaCl and Sodium Tripolyphosphate and Cooking Rate on Pink Color in Cooked Ground Chicken Breasts

  • The current study investigated the effects of timing of NaCl (2%) and sodium tripolyphosphate (STPP, 0.5%) addition and cooking rates on color and pigment properties of ground chicken breasts. Four treatments were tested as follows: treatment 1, no NaCl and STPP added and stored for 7 d; treatment 2, NaCl+STPP added on 0 d and stored for 7 d; treatment 3, NaCl added on 0 d and STPP added on 7 d; and treatment 4, stored for 7 d and NaCl+STPP added. All samples were cooked at a fast (5.67℃/min) or slow cooking rate (2.16℃/min). Regardless of the timing of NaCl and STPP addition, reflectance ratios of nitrosyl hemochrome, cooking yield, pH values, oxidation-reduction potential, and percent myoglobin denaturation were similar (p>0.05) across treatments 2, 3, and 4. The highest CIE a values were observed in treatment 4 (p<0.05), while treatment 2 was effective in reducing the redness in cooked chicken products. The fast cooking rate resulted in lower CIE a values and higher CIE L values and cooking yield in cooked chicken breasts compared to the slow cooking rate. Our results indicate that adding NaCl and STPP to meat, followed by storing and cooking at a fast rate, may result in inhibiting the pink color defect sporadically occurred in cooked ground chicken breasts.

진행성 암환자에서 완화적 항암치료 중단 시점의 결정에 대한 증례보고와 문헌고찰 (Cases and Literature Review of Timing for Withdrawal of Palliative Chemotherapy)

  • 완치가 불가능한 진행성 암환자에게 시행되는 완화적 항암치료의 목표는 생명연장과 삶의 질 향상이다. 그러나 완화적 항암치료는 상당한 독성을 가질 수도 있기 때문에 의사는 항암제 투여의 중단시점에 대해, 특히 생의 마지막 몇 달 내 기간에는, 항상 지속적으로 평가해야 한다. 완화적 항암치료를 중단하는 분명한 권고안은 없지만 진행성 암환자 돌봄의 질 향상을 위해 해결해야 할 쟁점이다. 저자는 완화적 항암치료를 투여 받았던 진행성 대장암, 비소세포 폐암 환자 두 증례를 기술하였다. 두 증례 모두 완화적 항암치료의 중단에 대한 공동 결정 하에 현재까지 최대한의 증상 치료를 시행하고 있다. 두 증례와 최근까지의 문헌고찰을 통해 국내 호스피스 완화의료가 확립 되어가는 시점에서, 항암치료의 중단 시점과 결정에 대해 부각시키는 바이다.

배기가스재순환 적용에 따른 Off-road 엔진의 연료 분사 시기 전략에 관한 연구 (A Study on the Strategy of Fuel Injection Timing according to Application of Exhaust Gas Recirculation for Off-road Engine)

  • The reduction technologies of exhaust gas from both the off-road engine and on-road vehicles are important. It is possible to apply various combustion technologies with engines after the application of a treatment technology to this field. In this study, main injection timing, pilot injection timing, and exhaust gas recirculation (EGR) rate were selected as the experimental parameters whose effects on the emission of exhaust gases and on the fuel consumption characteristics were to be determined. In the experiment, the emission of nitrogen oxide (NOx) and Smoke, and the Torque at the same fuel consumption level, were measured. The experimental data were analyzed using the Taguchi method with an L9 orthogonal array. Additionally, analysis of variation (ANOVA) was used to confirm the influence of each parameter. Consequently, the level of each parameter was selected based on the signal-to-noise ratio data (main injection timing, 3; pilot injection timing, 3; EGR rate, 2), and the results of the Taguchi prediction were verified experimentally (error: NOx, 10.3 %; Smoke, 6.6 %; brake-specific fuel consumption (BSFC), 0.6 %).
