Surgical Treatment of Multiple Lung Abscesses with Adenoid Cystic Carcinoma - A Case Report - (선양 낭포암에 의한 다발성 폐농양의 외과적 치료 - 1례 보고 -)

  • Adenoid cystic carcinoma is a very slowly growing and directly invasive cancer. The treatment of choice is complete surgical resection but if major complications associated with remaining carcinoma occur, aggressive conservative treatment to prevent complication is able to gain long term survival even though remaining carcinoma metastases to other organs. We experienced a case of surgical treatment of uncontrollable fever that caused by multiple lung abscesses due to obstruction of left main bronchus with adenoid cystic carcinoma. The post operative course was uneventful for 4 months to now.

Application of Traditional Acupuncture on Canine Intervertebral Disc Disease (개에서 추간판 질환에 대한 침술치료 증례)

  • A four-year-old female Pekingese dog was presented with intermittent ambulatory hindlimb lameness and severe back pain. The course of the disease was acute and static and 5 days passed by until presentation. In neurological examination, severe back pain and slightly decreased proprioception in right hindlimb were shown. In radiographic examination, radiopacity was increased at T10-11 intervertebral disc space. According to the history taking, neurological examination and radiographic examination, it was tentatively diagnosed as intervertebral disc disease (IVDD). Conservative treatment was performed including cage rest, medication with prednisolone and traditional acupuncture. Acupoints were GV-6, GV-7, BL-18, BL-19. BL-40 and GB-34. Dry needlings rested for 20 minutes. Just after the first acupuncture treatment back pain was significantly decreased. With additional acupuncture treatment, all the clinical signs were not observed. It is considered that conservative therapy including traditional acupuncture, medication and cage rest might be the first choice of therapeutic method for Grade I or II IVDD in dogs.

Surgical versus Vacuum Bell Therapy for the Correction of Pectus Excavatum: A Comparison of 1-Year Treatment Outcomes

  • Background: The purpose of this study was to compare 1-year clinical outcomes between patients who underwent a Nuss operation or vacuum bell therapy and to present vacuum bell therapy as a possible alternative treatment modality for patients who prefer non-surgical correction of pectus excavatum. Methods: We conducted a retrospective review of pectus excavatum patients who had undergone vacuum bell therapy for more than 1 year and examined patients who had undergone Nuss bar removal more than 1 year previously. The treatment outcomes were evaluated by comparing changes in the Haller index before and after treatment in both patient groups. Results: We included 57 patients in this study and divided them into 2 groups according to the type of treatment received. Both groups showed no significant difference in the post-treatment Haller index after 1 year of follow-up, although the Nuss operation group showed a greater change in the Haller index than the vacuum bell group. Conclusion: Although the Nuss operation is a well-established and effective treatment of choice to correct pectus excavatum, vacuum bell therapy showed comparable outcomes and could become an alternative treatment modality for select patients who prefer non-invasive treatment.

FIBROSIS THAT OCURRED AFTER CONSERVATIVE THERAPY OF LARGE ODONTOGENIC KERATOCYST (광범위한 치성각화낭종의 보존적 치료후 발생한 섬유증)

  • Odontogenic keratocysts are developmental odontogenic cysts which derived from rests of dental laminas and often found in posterior area of mandible. Because this lesion has tendency of frequent recurrence, treatment of choice is often radical removal of the lesion. Sometimes in case of large cysts, however, conservative therapy like marsupialization is often selected in treatment plan. A 39-years old woman referred to our department for evaluation of large radiolucent lesion that occupies the areas from mandible angle to upper part of ramus and condyle. In cytology, the lesion was identified as odontogenic keratocyst. Marsupialization was our treatment of choice, and the result was so favorable. 2 years later, there was small radiolucent lesion on upper part of mandibular ramus on panoramic view. It was suspected as recurred lesion, and excisional biopsy was done. On biopsy result, it was not a cystic lesion but fibrosis.

  • The eminent brother doctors Cho-Gaek Hur (Young Hur) and Cho-Sam Hur (Eon Hur) represent the medical tradition of the southeastern region of Mt. Jirisan incluiding Sancheong. This study investigated and examined their herediatary testimonies and medico-historical facts described in the literature tale, genealogy and family collection of works. At an early age after their family run away to the southern region from the harm of political struggle, they gave up taking the highest-level state examination to recruit ranking officials and set their heart on learning medicine. Their choice of medicine was the second best plan for the survival of their family and descendents from the influence of their ancestors who suffered from the party strife during the late Joseon Dynasty. The case is a good example of the deliberate action of the nobility intellectuals since the late Goryeo and early Joseon dynasties to who made attempts to have the minimum elegance and cause while securing livelihood after they have been collapsed politically. The tale of Cho-Gaek and Cho-Sam is a typical story of eminent doctors which combined reality and fiction about the persons who had outstanding medical skills. In the tale, the elder brother was good at herbal prescriptions and the younger brother was good at acupuncture, contrasting the treatment of administrative internal medicine and insertive acupuncture treatment. It shows that the prepositions for the choice of the treatment method for the public during that period were safety and fast efficacy. In their old age, they wrote a book titled Jinyangsinbang. Another Jinwoosinbang is estimated as a different kind of copy, which was a medical writing from experience that derived from the same original book as Jinyangsinbang. written by Cho-Gaek Hur and it is a valuable medical material that possesses the distinct features of the local medicine during the late Joseon Dynasty. Most of the contents are variations in application using the basis of Dongeuibogam, forming the roots of the traditional Korean medicine in Sancheong region together with the medical activities of the Hur brothers.

  • Traumatic bone cyst is a pathologic cavity that is not lined with epithelium. It is, therefore, not a true cyst. It may be a normal variant rather than a disease process. The etiology of the condition is unknown. This condition is occured widely ranging ages(2 to 75years), however, most are ,found during the second decade of life. Radiographically, this condition is radiolucent lesion with well-defined outline, scalloping of superior margins, Cyst enucleation and curettage is the treatment of choice. The authors compared and analyzed the clinicoradiologic features of the five cases of traumatic bone cyst, diagnosed at the Dental college hospital in Chosun University, Kwangju, Korea. The five cases were shown the followed results; 1. 3cases occured in second decade of life & no significant sex differences (M:F, 2:3) All cases occurred in mandible. 2. Two patients complained symptoms, but three cases had no symptom with encountering during routine examination. 3. In 3 of 5cases, teeth vitality existed except one tooth and no checking of teeth vitality in two cases. 4. All cases didn't have any accurate trauma history, but one case was in orthodontic treatment, another case was postextraction site area. 5. Radiologically, 'scalloping appearance' were evident in all cases; in 3 cases, multilocular tendency & only one case seen intact mandibular canal image. 6. Histologically, all section showed bone trabeculae with blastic activity, 2 cases showed no epithelial lining, and other 2 cases were seen innflammatory cell infiltration in edematous tissue. 7. Surgical intervention (curettage) was that treatment of choice.

Critical Pathway for Management of Primary Spontaneous Pneumothorax (원발성 자연기흉의 치료지침)

  • Background: With the advances of video technology, thoracoscopic surgery has been applied to various areas of the thoracic surgical fields including major surgeries. Now a days,-thoracoscopic surgery is Performed as a procedure of choice for primary spontaneous pneumothorax. But the operative indication for the primary spontaneous pneumothorax has not been changed since the last few decades, although the procedure of choice was changed from open thoracotomy to thoracoscopy. Therefore, we thought new treatment strategy will be necessary for the management of primary spontaneous pneumothorax. Material and Method: Between January 1998 and December 1999, 149 primary spontaneous pneumothorax patients were admitted to the Kyungpook National University Hospital. Result: Of these patients, 177 were first attack pneumothoraces and the number of total attacks were 250. Conclusion: Analyzing the amount of pneumothorax, methods of treatment, number of recurrences, recurrence rate and hospital stay, we propose a critical pathway for establishing new treatment strategy for the management of primary spontaneous pneumothorax.

Evisceration and Intrascleral Silicone Ball Prosthesis with Penetrating Keratoplasty on Perforated Corneal Ulcer Secondary to KCS in a Shih Tzu Dog (개의 건성각결막염에 의한 각막천공을 안구내용물적출술과 실리콘볼 삽입 후 각막이식을 적용하여 치료한 증례)

  • When severely large corneal perforation occurs, penetrating keratoplasty is a treatment of choice alternative to enucleation. A twelve-year-old male Shih Tzu was referred with perforated corneal ulcer secondary to keratoconjunctivitis sicca (KCS). Perforated cornea was directly sutured using 10-0 non-absorbable suture material, and rotational conjunctival flap was performed. However, re-perforation of cornea by wound dehiscence was observed at 1 month after operation. The yellowish lens escaped outside the orbit during corneal re-perforation, the diagnosis was re-perforated corneal ulcer, moderate corneal edema, moderate KCS (STT; 6 mm) and endophthalmitis caused by escaped lens outside orbit. Accordingly, penetrating keratoplasty (PK) and evisceration through corneal recipient site and intrascleral silicone ball prosthesis were carried out as the planned treatment, and resulted in good cosmetic improvement compared to enucliation. However, exposure of silicone ball occurred at the 9 months after the surgery due to the irritation of implant, thus enucleation was performed. In perforated large corneal ulcer with severe intraocular damage, evisceration with silicone ball insertion with PK would be alternative treatment choice to improve the cosmetic appearance.

Effect of Soaking and Heat Treatment Conditions on Physicochemical and Organoleptic Quality of Lotus Root (침지처리 및 열처리 조건이 연근의 관능적 특성 및 이화학적 특성에 미치는 영향)

  • To reduce the astringent taste of lotus root, the effects of various treatment methods such as drying, soaking, steaming and roasting on the physicochemical properties and sensory characteristics were investigated. The following process conditions were selected: 1) drying (D), 2) soaking followed by drying ($SK{\rightarrow}D$), 3) steaming followed by drying ($ST{\rightarrow}D$), 4) drying followed by roasting ($D{\rightarrow}R$), 5) soaking and then drying followed by roasting ($SK{\rightarrow}D{\rightarrow}R$), 6) steaming and then drying followed by roasting ($ST{\rightarrow}D{\rightarrow$}. The tannin content of the lotus root was lowest when it was treated by steaming followed by drying ($ST{\rightarrow}D$). The astringent taste of lotus root was reduced by steaming, and the roasted taste was improved by roasting in terms of sensory and flavor characteristics. Consequently, lotus root treated by steaming and then drying followed by roasting ($ST{\rightarrow}D{\rightarrow}R$) showed the highest preference with respect to astringent and roasted taste.

Diagnosis and Treatment of Acute Ankle Sprain (족관절 급성 염좌의 진단과 치료)

  • Acute sprain of the ankle requires comprehensive history taking and physical examination in diagnosing the type of severity and deciding on the plan of treatment. Literature supports functional treatment as the treatment of choice for grade I and II injuries. During the acute phase, the goal of treatment focuses on controlling pain and swelling. PRICE (protection, rest, ice, compression, and elevation) is a well-established protocol at this phase. There is some evidence that application of ice and use of nonsteroidal anti-inflammatory drugs improves healing and speeds recovery. Then the functional treatment (motion restoration and strengthening exercises) is administered to progress the rehabilitation appropriately in order to facilitate healing and restore the mechanical strength and proprioception. Early mobilization has been shown to result in more rapid return to work and daily activities than immobilization. Grade III injuries still generate controversy in terms of the best management available, and more studies on early mobilization, cast immobilization, or surgery are needed. Even the Cochrane reviews published to date are not conclusive.
