Comparison of Depressive Symptoms between Social Anxiety Disorder and Panic Disorder (사회불안장애와 공황장애에 동반된 우울증상의 특성 비교)

  • Objectives The principal aim of the present study was to investigate the characteristic depressive symptoms in patients with social anxiety disorder (SAD) and panic disorder in comparison to patients with depressive disorder. Methods This study included 132 patients with SAD, 128 panic disorder and 64 depressive disorder (major depressive disorder, dysthymia etc.) patients without comorbid psychiatric disorders. The Beck Depressive Inventory (BDI) is used to measure depressive symptoms. We divided BDI into three categories originally described by Shafer AB, including negative attitude toward self, performance impairment, and somatic symptoms. We compared the depressive symptoms of SAD, panic disorder and depressive disorder by using ANOVA. Results Negative attitude toward self was noticeable in SAD (SAD $0.54{\pm}0.23$, panic disorder $0.41{\pm}0.17$, depressive disorder $0.46{\pm}0.11$, p < 0.001). Performance impairment and somatic symptoms were remarkable in panic disorder than in SAD and depressive disorder (performance impairment : SAD $0.39{\pm}0.21$, panic disorder $0.44{\pm}0.14$, depressive disorder $0.40{\pm}0.09$, p = 0.009 ; somatic symptoms : SAD $0.07{\pm}0.10$, panic disorder $0.15{\pm}0.12$, depressive disorder $0.14{\pm}0.08$, p < 0.001). Conclusions The results facilitate an approach to optimal treatment for patients with comorbidity of anxiety disorder and depression.

Effect of creep-fatigue interaction on high temperature low cycle fatigue strength and fracture behavior of STS 316 stainless steels (STS 316鋼 의 高溫低사이클 疲勞强度 와 破壞擧動 에 미치는 크리이프 - 疲勞 相互作용 의 影響)

  • Fully reversed push-pull low cycle fatigue tests under strain control of trapezoid cyclic mode have been conducted in air at temperature of 550.deg. C and with frequency of 0.5 cpm on the domestic stainless steel STS 316 after solution treatment for 1 hour at 1100.deg. C. As an experimental equipment for high temperature fatigue tests, an electric servo-hydraulic fatigue machine(Instron model 1350) was used. This paper presents the effects of creep hold time and plastic strain range on push-pull high temperature low cycle fatigue life and fracture behavior. The fracture surfaces were observed by means of the scanning electron microscope. The results are as follows. (1) The fatigue life decreases with increase of the plastic strain range equal hold time and also decreases as the hold time is getting longer. (2) The frequency modified damage function can predict fatigue life by incorporating a variation of Coffin's frequency modified approach into damage function. (3) The ratios of creep damage and fatigue damage can be calculated by using he linear accumulation damage concept and the ratio of creep damage increases as the hold time is getting longer. (4) At the creep hold time of 5 minutes and the strain range of 2.0%, the fracture mode was intergranular fracture and striations were hardly observed. In this case, the intergranular cracking was originated in void type('.gamma.' type) cracking.

  • Purpose: Operating room management is the serious and complex task for hospital managers and the common approach is to develop relevant standard operational procedures. From patient and staff safety perspective, operating room management should be well-studied and hospital should identify and address any potential risks. Simultaneous usage of different imaging and less-invasive treatment technologies demands strong management control. Materials and Methods: We have formed the multidisciplinary expert panel (surgeons, anesthesiologists, radiologists, healthcare managers etc.) for hybrid theater management standard operational procedure development. On the first stage the general concept of hybrid room design and patient routing was developed. The second stage included the technical details discussion. For patient safety improvement we modified the Surgical Safety Check-list in accordance with potential MRI-related safety challenges and concerns. Results: WHO Surgical Safety Checklist is a simple and easy-to use tool which includes three blocks of question (grouped by the surgery process). We have developed two additional blocks of questions for the intraoperative magnetic resonance investigation. It is very important to have a special detailed routing with a strong control of ferromagnetic devices and anesthesiology care. Conclusion: High-energy MRI (1.5-3.0T) is characterized by potential influence on patient and staff safety in case of hybrid surgery. It is obvious to have a strong managerial control of ferromagnetic devices and anesthesiology care. Surgical Safety Checklist is the validated tool for improving patient safety. Modification and customization of this check-list potentially provides the opportunity for surgery processes improving.

Analysis of Medical Disputes Precedent (의료분쟁 판례분석)

  • Unexpected results caused by medical service are defined as malpractice, and the doctor must bear the responsibilities following the medical treatment. Malpractice disputes are disputes between patients, who are seen as the only victim in this way, and medical personnel who do not admit to the charges. Unfortunately, in reality, an official approach to mutual understanding and communication in the instance of such disputes does not exit. Based on this background, this study was attempted to prevent similar forms of medical disputes from occurrence and provide fundamental data to prepare by analyzing the medical disputes precedent. Results of the study are as follows: First, For type of medical institution, hospitals accounted for the most part as 62.9%. Second, Among total medical disputes, the cases surgery accounted for 27.8%, violation of duty care accounted for 20.6% and that of medical treatments 11.3%. Third, For a mean agreed amount by medical department, it was the highest in obstetrics & gynecology as 38,384,000 won. In conclusion, the most desirable method of dispute resolution is to prevent a cause of dispute to the root.

  • Blood circulation of brain is divided into two major categories; anterior one from carotid artery and posterior one from vertebrobasilar artery. In stroke patients, it is important to diagnose which is involved, because there is many difference in the aspects of clinical menifestations and prognosis, especially in the acute stage. In some cases of vertebrobasilar infarction, such as Wallenberg's syndrome, charicteristic cranial nerve signs, eye movement disorders and cerebellar signs are appeared. And in Dejerine's syndrome, only pure motor or sensory defecits can be appeared without any brainstem signs. So It shoud be differenciated by Brain MRI from those of the cerebral hemisphere lesions. And in the cases that nausea, vomitting and dysphagia are the first menifestations, it is frequently misdiagnosed as internal medical disease, causing appropriate treatment delayed. In this case report, we are to describe the clinical menifestations and progresses of two cases of brainstem infarctions, review previously published case reports about them and compare them to our cases. The first is Dejerine's syndrome i.e. medial medullary infarction, the second is Wallenberg's syndrome i.e. lateral medullary infarction. Simultaneously we are to investigate the oriental medical approach in the bran stem infarctions.

Effective Behavioral Strategies for Pediatric Feeding Problems (섭식 문제 행동에 대한 효과적인 행동전략)

  • It is relatively common to have various feeding problem during infancy and childhood, even for typically developing children. Treatment outcome studies for pediatric feeding disorder consistently reported that applied behavior analaysis (ABA), a type of behavior intervention, is very effective and efficacious. Interdisciplinary team approach is necessary for severe feeding problems, but similar methods could be used through parent training for moderate and mild feeding problems. This article introduced assessment procedure to identify feeding problems as well as specific methods to deal with diver feeding issues. Each strategy can be used effectively only when it is combined with several other strategies including environmental rearrangement and should be developed individually upon each child's feeding issues. In addition, systematic and continuous parent trainings are mandatory. At the end, several treatments related issues were addressed.

  • Aldose reductase (AR), the principal enzyme of the polyol pathway, has been shown to play an important role in the diabetic complications, including diabetic cataract. The inhibitors of AR, therefore, would be potential agents for prevention of diabetic complications. As part of our ongoing project directed toward the discovery of preventive and/or treatment for diabetic complications, 48 Korean herbal medicines have been investigated with an in vitro evaluation system using aldose reductase inhibitory activities. Of these, 12 herbal medicines exhibited a significant inhibitory activity against aldose reductase. Particularly, seven herbal medicines, i.e., Eurya japonica, Chrysanthemum indicum, Akebia quinata, Saururus chinensis, Catalpa bignonioides, Lonicera japonica, Vitex rotundifolia showed two times more potent inhibitory activity than the positive control 3.3-tetramethyleneglutaric acid (TMG). In addition, Catalpa bignonioides showed the retardation of cataract-opacification of the lens of the eye under diabetic condition by xylose. Therefore, this result may provide a potential therapeutic approach for preventing and treating diabetic cataracts.

  • During cleaning steps, ultrafiltration membranes are mechanically and chemically stressed. This may result in membrane degradations and failures. In this paper, polysulfone membranes were used to evaluate membrane deteriorations by commercial detergents in static conditions. Ageing of the membrane was simulated by immersing samples in solutions containing commercial detergents with various concentrations, temperatures and times defined by experimental designs. Indeed, an innovative approach in the chemical membranes ageing researches, based on methodological tools, was used in order to achieve significant ageing experiments without using an accelerated ageing protocol. The macroscopic changes were monitored by permeability measurements and mechanical strength tests coupled with a microscopic characterization by ATR-FTIR and HRSEM. The present work details results obtained for three commercial detergents: an alkaline, an acidic and an enzymatic detergent. It was found that the detergents used in the industrial advised conditions (concentration, temperature and time of contact) were not detrimental for membrane properties (permeability and elongation at break) and so for the quality of the produced water. Over the industrial cumulated time of contact, different ageing effects can be observed and compared with the ones induced by NaOCl.

A Comparison of the Depressive Predisposition between Obese and Non-Obese Elderly (노년기 비만군과 정상 체중군 간의 우울 성향 비교)

  • Objective : Obesity has been considered to be associated with numerous physical, mental and psychological diseases. Depression, which is a major psychological factor affecting occurrence and treatment of obesity, can be a cause of obesity as well as can be triggered by obesity. This study aimed to find out the need of positive consideration to the existence of depressive mood in the management of obesity. Methods : A total of 101 subjects were enrolled among those who were 60 years old and over at one local clinic. Obese group (n=49) was defined as BMI${\geqq}$25 and normal weight group (n=52) as 20$5.2{\pm}2.5$) was significantly higher than that of the normal weight group ($3.8{\pm}1.7$)(p<0.05). In male subjects, there was no significant difference in the mean GDSSF-K score between both groups, but in female subjects, the mean GDSSF-K score of the obese group, $6.4{\pm}2.6$ was significantly higher than that of the normal weight group ($4.0{\pm}1.8$)(p<0.05). Conclusion : The elderly obese group showed higher depressive scale score than elderly normal weight group, especially in female elderly. Since the elderly obese female group had a depressive predisposition, comprehensive management including mental and psychological approach is required in obesity control programs.

Anticancer Activity of Atractylodes lancea (Thunb.) DC in a Hamster Model and Application of PET-CT for Early Detection and Monitoring Progression of Cholangiocarcinoma

  • Opisthorchis viverrini (OV)-induced cholangiocarcinoma (CCA) is an important cancer in the Great Mekong region, particularly in Thailand. Limitations of treatment options and the lack of an effective diagnostic tool for early detection of CCA are major concerns for the control of this type of cancer. The aim of the study was to investigate anti-CCA activity of the ethanolic extract of Atractylodes lancea (Thunb.) DC., and the applicability of positron emission tomography-computed tomography (PET-CT) as a tool for detection and monitoring the progression of CCA in Opisthorchis viverrini (OV)/dimethylnitrosamine (DMN)-induced CCA hamsters. Male Syrian hamsters were used for toxicity tests and anti-CCA activity evaluation. Development of CCA was induced by initial feeding of 50 metacercariae of OV, followed by drinking water containing 12.5 ppm of DMN in hamsters. The ethanolic extract of A. lancea (Thunb.) DC. was administered orally for 30 days. PET-CT was performed every 4 weeks after initiation of CCA using 18F-fluorodeoxyglucose ($^{18}F-FDG$). Results from the present study suggest that the ethanolic extract of A. lancea (Thunb.) DC. rhizome exhibited promising anti-CCA activity and safety profile in the OV/DMN-induced hamster model. To successfully apply PET-CT as a tool for early detection of tumor development and progression, modification of radiolabeling approach is required to improve its specificity for CCA cells.
