모유영양아의 이행유 섭취량과 체중변화 (Changes in Transitional Milk Intakes and Body Weight of Breast-Fed Infants)

  • The longitudinal changes in transitional milk intakes and body weight of infants(18 boys, 10 girls) have been studied in the part of Kangwon Province from 7 days to 15 days postpartum. Milk intakes at 7, 10, 15 days postpartum were 474$\pm$155, 524$\pm$154 and 547$\pm$130g, respectively. The mean milk intakes were 515g from 7 to 15 days postpartum. Milk intakes of boys were significantly higher than those of girls. The transitional milk intakes per weight of infants appeared 147$\pm$41g/kg. The number of feed per day of boys were significantly higher than those of girls. Milk intakes per fed were 56$\pm$20g. The transitional milk intakes were affected by gestational period(p<0.05), maternal height(p<0.05) and mother's transitional milk yields (p<0.001). Infants transitional milk intakes were 82.6$\pm$17.0% of mothers transitional milk yields. Infants gained its weight 24.6, 40.1 and 39.0g/day at, 7, 10, 15 days postpartum, respectively.

분만 첫 15일간 이행유 분비량의 변화 (Changes in the Transitional Milk Yields during the First 15 Days Postpartum)

  • The longitudinal changes in the transitional milk yields of Korean lactating women(14 primiparae, 11 multiparae) have been studied by test-weighing method in the part of Kangwon Province form 7 day to 15 days postpartum. The transitional milk yields at 7, 10, 15 days postpartum were 531$\pm$148,598$\pm$156 and 639$\pm$169g, respectively. The mean milk yield was 589$\pm$162g from 7 to 15 days postpartum. The transitional milk yields between primiparae and multiparae appeared not significantly different but significantly different between mothers of boys and girl(p<0.05). The distribution of individual transitional milk yields were found 550-649g(28.0%), 450-549g(24.0%), 650-749g(13.3%) and 750-849g(13.3%). The transitional milk yields were not affected by mother's age, weight gain during pregnancy, gestational period and infant's weight at birth but affected by maternal height(p<0.05). Maternal weight reduction during the lactation had no correlation with the transitional milk yeilds.

분만후 7일과 60일에 분비된 모유의 Vitamin A 함향 (Vitamin A content of human milk at 7 and 60 days postpartum)

  • The changes of vitamin A contents of transitional (7th day postpartum) and mature (60th day postpartum) milk were comparatively studied from 20 Korean lactating women to provide the basic information for recommended dietary allowances. The mean retinol contents($\mu\textrm{g}$/100$m\ell$) of transitional milk in primiparae and multiiparae were 79.71 (${\pm}$50.61) and 73.43(${\pm}$31.02) and those of mature milk in primiparae and multiparae were 44.73(${\pm}$19.03) and 38.25(${\pm}$17.63) respectively. The mean ${\beta}$-carotene contents ($\mu\textrm{g}$/100$m\ell$) of transitional milk in primiparae and multiparae were 10.44(${\pm}$7.43) and 14.15(${\pm}$11.81) and those of mature milk in primiparae and multiparae were 4.97(${\pm}$2.22) and 5.36(${\pm}$4.23) respectively. The total content of vitamin A($\mu\textrm{g}$/100$m\ell$) in transitional milk was 81.44(${\pm}$ 51.47) in primiparae and 75.79(${\pm}$31.99) in multiparae, and those of mature milk in primiparae and multiparae were 45.56(${\pm}$19.12) and 39.14(${\pm}$17.73) respectively. The vitamin A content decreased with increase of lactation period. The mean content of vitamin A between primiparae and multiparae during lactation was not significantly different (p>0.05) However, there was a significant difference in the total vitamin A contents according to lactation period. The average content($\mu\textrm{g}$/100$m\ell$) of retinol in lactating mothers delivered boys and girls were 74.45 (${\pm}$43.84) and 78.96(${\pm}$35.98) in transitional milk, 40.27(${\pm}$16.61) and 42.51(${\pm}$21.22) in mature milk, respectively. However, no significant difference was found between retinol contents contentscontentsin lactating mothers delivered boys and girls. The average contents($\mu\textrm{g}$/100$m\ell$) of ${\beta}$-carotene in lactating mothers delivered boys and girls were 11.34(${\pm}$4.27) in mature milk. The contents($\mu\textrm{g}$/100$m\ell$) of total vitamin A was significantly different (p<0.05) between transitional and mature milk the difference of total vitamin A content between lactating mothers delivered boys and girls. and primiparae and multiparae was not significant(p>0.05) As a result the contents of vitamin A, retinol and ${\beta}$-carotene in transitional and mature milk decreased according to the increase of lactation period.

수유기간별 이행유중 비타민 E 함량의 변화 (Change of Vitamin E Content in Transitional Milk during Lactation)

  • The longitudinal change of vitamin E($\alpha$-Tocopherol) content and secretion in transitional milk of 32 Korean lactating women and vitamin E intake of infants was studied 7, 10 and 15 days postpartum, Vitamin E content of the milk decreased at 7, 10 and 15 days postpartum with the respective amount of 784, 633 and 539$\mu\textrm{g}$/100ml. The average vitaimin E secretion in the milk of mothers appeared 350$\mu\textrm{g}$/days. The average vitamin E intake in the milk was 3033 and 2424$\mu\textrm{g}$/day in boys and girls respectively and was 174.7% for infants of the recommended lower limit for vitamin E in infant formular of 0.5mg/100㎉. Vitamin E intake per body weight of infants in the milk were 929.731 and 676$\mu\textrm{g}$/kg/day respectively.

모유의 셀레늄과 아연 함량에 관한 연구 (A Study on the Contents of Selenium and Zine in Human Milk)

  • The changes of selenium and zine contents of transitional milk and mature milk at 7 and 60 days postpartum were comparatively studied on 20 Korean lactating women. the selenium and zinc contents of transitional milk and mature milk were analyzed by atomic absorption spectrophotometer after wet-digestion. The mean selenium contents wer 12.2$\mu\textrm{g}$ /ι and 11.5$\mu\textrm{g}$/ι at 7 and 60 days postpartum, respectively. The mean zinc contents decreased significantly from 4.23mg/ι at 7 days to 1.62mg/ι at 60 days postpartum (p<0.05).

  • 한국인 중류층 수유부들로부터 분만 후 70일까지 채취한 유즙의 인 지방질의 자방산 조성이 수유기간의 경과에 어떻게 변화하는지 살펴보았다. 8 : 0 은 이행유 단계에서, 그리고 10 : 0, 12 : 0, 14 : 0 등은 성숙유 I단계에서 각각 유의하게 증가하였다. w6계 대사산물들(20 :2w6, 20 : 4w6, 22 : 4w6)은 수유기간의 경과에 따라 점진적인 감소의 경향을 나타내었다. 24 : 1w9는 이행유 단계에서 유의한 증가의 수준을 유지하다가 그 이후는 초유 수준 이하로 유의하게 감소하였다.

수유 초기 모유 중 철, 구리, 아연의 함량과 영아의 섭취량 추정 (Iron, Copper and Zinc Levels in Human Milk and Estimated Intake of the Minerals by Breast-Fed Infants during the Early Lactation)

  • The objectives of the present study were to measure the content of iron, copper and zinc in human milk and to estimate the intake of iron, copper and zinc of breast-fed infants during the early period of lactation. Twenty-five lactating women who delivered in a hospital in Seoul volunteered for the study. Milk samples were collected at day 2, 3, 4, 5, 6, 7, 15 and 30 postpartum. The contents of iron, copper and zinc were determined using the atomic absorption spectrophotometer after wet digestion. The intakes of iron, copper and zinc of infants were estimated by multiplization with the infant milk intake reported in our laboratory. The content of iron was 0.58 $\mu\textrm{g}$/g in colostrum, 0.48 $\mu\textrm{g}$/g in transitional milk and 0.39 $\mu\textrm{g}$/g in mature milk while the estimated iron intake of infants was 271, 255 and 259 $\mu\textrm{g}$/day, respectively. The content of copper in the milk was 0.45 $\mu\textrm{g}$/g in colostrum, 0.43 $\mu\textrm{g}$/g in transitional milk and 0.33 $\mu\textrm{g}$/g in mature milk while the estimated copper intake of infants was 210, 229 and 220 $\mu\textrm{g}$/day, respectively. The content of zinc in the milk was 5.24 $\mu\textrm{g}$/g in colostrum, 3.70 $\mu\textrm{g}$/g in transitional milk, 2.93 $\mu\textrm{g}$/g in mature milk while the estimated zinc intake of infants was 2452, 1968, and 1949 $\mu\textrm{g}$/day, respectively. These results suggest that copper and zinc intake of infants are met to RDA but iron is not.

수유기간의 경과에 따른 한국인 인유의 칼슘 및 인 함량의 변화 (Longitudinal Changes in Calcium and Phosphorus Concentrations of Korean Human Milk)

  • 수유기간의 경과에 따른 한국인 인유의 수분, 총고형분, 회분, 칼슘, 인, 칼슘/인의 비 등의 변화를 파악하기 위하여 초유(분만후 0∼5일), 이행유(분만 후 6∼10일), 성숙유(분만 후 25∼35일째 및 분만 후 55∼70일째) 단계로 나누어 longitudinal로 추적·조사하였으며, 시유 및 조제분유와의 비교도 행하였다. 수분과 총고형분은 분만 후 22∼35일에서만 초유에 비하석 유의한 차이를 보여주었으며, 회분은 수유기간의 경과에 따라 점차 감소하는 경향이었다. 칼슘 및 인은 초유단계에 비하여 이행유, 성숙유단계 모두 유의하게 높았는데 칼슘은 분만 후 55∼7O일까지, 인은 분만후 25∼35일까지 계속 증가의 경향을 보여준 것이 특이하였다. 칼슘/인의 비는 초유단계에서 2.28, 이행유단계에서 1.76, 분만 후 25∼35일에서 1.68 그리고 분만 후 55∼70일에서 1.95였는데 이행유 이후 단계에서 모두 유의하게 낮았다. 회분, 칼슘, 인, 칼슘/인의 비에서 초산부와 경산부간에는 차이가 없었다. 성숙유의 총고형분 함량을 시판 국산 조제분유의 조유농도로해서 계산하여 본 바 회분과 칼슘/인의 비는 인유의 그것에 비슷하였으나 칼슘과 인은 높은 수준이었다.

한국인 모유의 수유단계별 트랜스지방산 함량 (Trans Fatty Acids of Breast Milk Lipids of Korean Women from Week 1 to 6 Months of Postpartum)

  • This study was done to determine the trans fatty acid (tFA) composition of human milk from postpartum to sixth months after delivery, to investigate the tFA intake of lactating women, and to estimate the intakes of tFA by infants exclusively fed breast milk. A total of 27 lactating Korean women participated to this study voluntarily, gave their breast milk, and responded to an investigation of their diets. The lactating women consumed 2.3-2.8 g/d of tFAs over the period of the first, second, third, and sixth months postpartum, which was 3.4-4.9% of the total fat intake and 0.8%-1.2% of the total energy intake. The proportions of tFAs in the breast milk were 1.89% in colostrum, 1.78% in transitional milk, and 1.78-2.25 in mature milk of the first, second, third, and sixth months postpartum. The tFAs of the breast milk identified in this study were C16:1n9t, C18:1n9t, C18:2n6t12t, C18:2n6t12c, C18:2n6t12t and C18:2n6t11t. Among them, C18:1n9t was predominant, which made up 59.26% of all tFAs in cob strum, 62.36% in transitional milk, and 64.42% in mature milk. The proportion of total tFA was unchanged with time, although some significant differences were noted for individual tFAs. The percentages of C18:2n6t12c and C18:2n6c12t decreased over the study period. Estimated tFA intake of the exclusively breast-fed infants was 0.18 g/d when fed colostrum, 0.29 g/d when fed transitional milk, and 0.53 g/d when fed mature milk until the sixth month of postpartum. Those were 0.5%, 0.8%, and 1.1% of the total energy intake. The results in this study indicate that lactating Korean women consume not a large quantity of tFAs, secrete breast milk not containing much tFA, and the estimated intake of tFAs by infants fed exclusively breast milk is not great.

수유초기 이행유중 비타민 A의 함량, 분비량 및 영야의 섭취량의 변화 (Changs of Vitamin A Concentration, Secretion and Ingake of Infants in Transitional Milk during the Lactation)

  • The longitudinal change of vitamin A concentration and secretion in transitional milk of 32 Korean lactating wormen and vitamin A intake of infants were studied 7, 10 and 15 days postpartum. Retinol concentration of the milk averaged 93.6$\pm$47.1, 80.1$\pm$41.6 and 66.7$\pm$33.5$\mu\textrm{g}$/100ml at 7, 10 and 15 days postpartum respectively and showed decreasing trend in total mothers. $\beta$-carotene concentration showed similar decreasing trend averaged 13.4$\pm$10.6, 8.2$\pm$4.6 and 5.3$\pm$3.4$\mu\textrm{g}$/100ml respectively. Vitamin A concentration decreased with the respective amount of 94.9$\pm$47.1, 80.5$\pm$41.5 and 67.3$\pm$33.0 R.E../100ml during the lactation. The average vitamin A secretion in the milk of all mothers appeared 410.0 R.E/day. Vitamin A secretion of multiparae (450.1 R.E./100ml) appeared not significantly higher than those of primiparae(370.0 R.E./100ml). The average vitamin A intake of infants in the milk averaged 333.7 R.E./day and was 95.3% of RDA for infants. Vitamin A intake per body weight of infants in the milk averaged 110.6, 91.9 and 79.2 R.E./kg/day respectively.
