Effect of Non-contact Korean Medical Treatment for Patients Recovering at Home with Positive Coronavirus Disease 2019 Diagnostic Test Results at a Local Public Health Center: A Retrospective Chart Review (지역 보건소에서 시행한 코로나 바이러스 감염증-19 진단 검사상 양성인 재택치료 환자의 비대면 한의진료 효과: 후향적 차트 리뷰)

  • Due to the coronavirus disease 2019 (COVID-19) pandemic, appropriate management of sequelae and treatment of infectious symptoms became increasingly important healthcare issues. Although the practice guidelines and treatment cases based on the East Asian traditional medicine have been reported, there are rare studies on the use of Korean medicine in Korea. Therefore, this study aimed to present the progress of non-contact Korean medical treatment for infected patients at a local public health center using retrospective chart review. A total of 18 patients were prescribed with 5 days of herbal decoction and medicine extract covered by the national health insurance. With the questionnaire form, the progression and improvement of symptoms before and after treatment were evaluated using the numerical rating scale (NRS), and the treatment satisfaction and opinions were obtained. The symptoms such as cough (5.56±2.23 to 2.89±2.14), sputum (6.11±1,75 to 3.28±2.47), sore throat (6.06±2.70 to 1.47±1.62), anorexia (5.56±2.63 to 1.94±2.21), nausea (3.75±1.71 to 1.17±1.11), diarrhea (3.40±2.63 to 1.50±1.51), chest tightness (4.93±2.46 to 2.29±2.30) and fatigue (6.44±1.79 to 2.67±1.88) all improved according to the NRS, and the satisfaction with herbal medicine treatment on a 5-point Likert scale was 4.24±0.90. No side effects and adverse reactions were reported. Thereupon non-contact Korean medical treatment can be concluded that it effectively reduces the COVID-19 infection mild symptoms in restrictive extent. Since the retrospective data does not include a control group, the more confirmative data is needed by multicenter and large-scale controlled clinical study afterwards.

Current Status of Herb-Drug Interaction Information and Information Database in Korea (국내 한약-양약 상호작용 연구논문 분석을 통한 상호작용 정보 현황파악 및 제공방안에 대한 연구)

  • The objectives of this study are to examine the current status and problems of herb-drug interaction (HDI) information and information database in Korea and suggest the better way to establish useful HDI database. We collected HDI studies that published in Korea and analyzed according to objective, methods, selection criteria of herbs, number of study, correlation between study subject and frequently used herbal medicine (HM). Then we selected representative HM database on the internet made by Korea Food and Drug Administration (KFDA) among the several databases and analyzed its contents related to HDI. Several HDI studies were carried out from laboratory based research to clinical trials and HM databases have been developed for providing information about different aspects of traditional Korean medicine. But the information of HDI and information database are still far from practical applications because there are no coherence to select study subjects and methods among researchers. So, it is necessary to build up HDI database led by the government for providing systematic HDI information. HDI information database is expected to be able to provide useful evidence for health professionals in prescription and consultation to reduce the chance of adverse effects and improve the quality of medical care.

Research of Traditional Herbal Medicines for Anti-aging, Inhibition Effect of Wrinkle and Whitening Effect in the Skin (피부의 노화와 주름 억제, 미백효과를 가지는 전통 한약재의 탐색)

  • The aims of this study are to search herbal medicines that have anti-aging, anti-wrinkle, anti-oxidant and whitening effects. This is preliminary study as a finding for excellent and specific natural agents which inhibit the skin aging and whitening formation. In this study, the articles and the documents, which have been published in domestically or internationally, have been searched and analyzed. Articles were gathered by taking advantage of Society of Cosmetic Scientists of Korea's documents, Pub med database, Korean Medical Database and Korean Studies Information Service System. Among the many species of herbal medicines, we selected the herbal medicines showing superoxide scavenging activities, and 1.1-diphenyl-2-picrylhydrazyl(DPPH) scavenging effects. The herbal medicines were showed anti-wrinkle effects on Metrixmetallo proteinase (MMP-1) inhibition activity and elastase inhibition activity. Also we found herbal medicines that have highly effect of tyrosinase inhibition, L- DOPA inhibition and melanin synthesis inhibition.

Study on the Relationship Between Personality and Ordinary Symptoms from the Viewpoint of Sasang Constitution and Cold-Hot (사상체질과 한열의 관점에서 본 성격과 소증의 연관성 연구)

  • In Traditional Korean Medicine it is one of basic principle that personality reflects physical conditions. In both of personality and ordinary symptoms, there are some criteria used for diagnosis whether the patient is in cold or hot condition. We studied the relationship between the criteria of cold-hot shown in personality pattern and ordinary symptom pattern. And also analyzed the relativeness in four constitution type. 877 patients who had visited oriental medical hospital and had been confirmed their constitution by drug response were included in this study. Patients filled out the questionnaire composed of personality and ordinary symptom questions. We selected questions related to cold-hot patterns. Next we made personality cold-hot index(PCHI) and ordinary symptom cold-hot index(OSCHI). The two indices were compared and analyzed totally and from the viewpoint of 4 constitution types. The coefficient of correlation between two indices was not high. In Soemin the coefficient was 0.257, in three other constitution it was lower than 0.200 or negative. In Soemin both indices were lower (cold-biased), but in three other constitution no significant results were found. In Tae-emin OSCHI was higher (hot-biased) than PCHI, in Tae-yangin PCHI was higher than OSCHI significantly. Personality is not affected by cold-hot less than expected. The criteria used for diagnosis of cold-hot seems to be biased on Soemin's.

Inhibitory Effect of Taraxci Herba Methanol Extract on Pro-inflammatory Mediator in Lipopolysaccharide;Activated Raw 264.7 cells

  • Taraxci Herba (TH; Pogongyoung in Korean) has been used in traditional oriental medicine for the treatment of various ailments. The biological activity of this plant is not yet evaluated systematically. This study was conducted to evaluate the inhibitory effects of TH on the production of nitric oxide (NO) and the expression of inducible nitric oxide synthase (iNOS) in lipopolysaccharide (LPS)-activated Raw264.7 cells. The aim of the present work is to investigate a potential anti-inflammatory activity of TH. The Raw264.7 cells were cultured in DMEM medium for 24 h. After serum starvation for 12 h, the cells were treated with TH for 1 h, followed by stimulating NO production with LPS ($2{\mu}g/ml$). As result of this study, TH inhibited the levels of NO, PGE2, $TNF-{\alpha}$, IL-6 and $IL-1{\beta}$, and the expressions of inducible nitric oxide synthase (iNOS) and cyclooxygenase-2 (COX-2) activated by LPS. These inhibitory effects were mediated though the inhibition of phosphorylation of inhibitory kappa B ($I{\kappa}B$). These findings showed that TH could have some anti-inflammatory effects.

Acute Oral Toxicity of Dangguibohyel-tang in ICR Mice (ICR계 마우스에 대한 당귀보혈탕(當歸補血湯)의 급성 경구 독성시험)

  • The Dangguibohyel-tang (DBT) was used to treat anemia in traditional Korean medicine. Acute oral toxicity of DBT was studied in ICR mice. ICR mice were administered orally with dosages of 100 mg/kg (low dosage group), 200 mg/kg (middle dosage group), and 400 mg/kg (high dosage group) of DBT. We daily examined number of deaths, clinical signs, body weights and gross findings for 14 days. DBT did not show any toxic effect in ICR mice and oral LD50 value was over 400 mg/kg in ICR mice.

Herbal Cocktail Sagunja Protects $H_2O_2$-induced H9c2 Cardiomyoblast Cell Death through the Induction of Heme Oxygenase-1

  • Sagunjatang (Sagunja), containing Radix Astragali, Radix Ginseng, Fructus Schizandrae, Radix Ophiopogonis and Radix Glycyrrhizae, has been used as a prescription for ischemic heart and brain diseases in Korean traditional medicine. This study was designed to investigate the protective mechanisms of Sagunja on $H_2O_2$-induced cytotoxicity of H9c2 cardiomyocytes. Treatment with $H_2O_2$ resulted in death of H9c2 cells, characterized by apparent apoptotic features, including the fragmentation of nucleus and increase in sub-GO/G1fraction of cell cycle. However, Sagunja markedly suppressed the apoptotic characteristics of H9c2 cells induced by $H_2O_2$ with decrease of intracellular peroxide level. In addition, Sagunja suppressed the features of mitochondrial dysfunction, including change of mitochondrial membrane potential, in $H_2O_2$- treated cells. Additionally, Sagunja induced the expression of HO-1 protein in both time-and dose-dependent manner. The role of HO-1 in ROS-scavenging activity of Sagunja is proposed.

Antitrichomonas Activity of Herb-medicine Generally Used in Dong-Eui-Bo-Kham (동의보감의 한약재에 대한 항질편모충 효능에 대한 연구)

  • Eighteen aqueous and two ethanol extracts of herb-medicine used for trichomoniasis therapies in Korea were assessed for their antitrichomonas activities. Two extracts(Sophorae radix, Phellodendri cortex) showed evident antitrichomonas activity at $8mg/m{\ell}$. Two mixture of Sophorae radix and Phellodendri cortex showed low antitrichomonas activity at 15mg/ml. Ethylacetate fraction of Sophorae radix showed the antitrichomonas activity of 400 mg/ml. This indicated that Sophorae radix is potent for a new effective and safe antitrichomonas agent.

Anti-allergic Effects of Socheongyoug-tang on RBL-2H3 Mast Cell and Mice-mediated Allergy Model (RBL-2H3 비만세포와 동물병태모댈에서 소청룡탕(小靑龍湯)의 항알레르기 효과)

  • The discovery of drugs on the treatment of mast cell-mediated allergic disease is a very important subject in human heath. The Socheongyoug-tang(SCYT) has been used for centruries as a traditional medicine in Korea and is known to have an anti-inflammatory effect. However, its specific mechanism of action is still unknown. In this report, we investigated the effect of hot water extract from SCYT on RBL-2H3 mast cell-mediated allergic reaction and studied its possible mechanism of action. SCYT inhibited compound 48/80-induced systemic anaphylaxis and serum histamine release in mice. SCYT decreased the passive cutaneous anaphylaxis reaction activated by Anti-lgE antibody-HSA. SCYT dose-dependently reduced histamine release from mice peritoneal mast cells activated by Anti-lgE antibody-HSA. SCYT increased cAMP and decreased compound 48/80-induced intracellular $Ca^{2+}$ levels. Our findings provide evidence that SCYT inhibits mast-derived allergic reactions, and also demonstrate the involvement of cAMP and intracellular $Ca^{2+}$ in these effects.

Study on Selective Anti-cancer Effects by the Water-extracts of Selaginella Tamariscina in Cancer Cell lines (권백물추출물의 선별적인 항암작용에 대한 연구)

  • Selaginella Tamariscina is widely used in the traditional oriental herbal medicine for its anti-inflammatory, anti-cancer effects. The effects of aqueous extracts of Selaginella Tamariscina (ST) on the cell viability and induction of apoptotic cell death were investigated in A549, Raw 264.7, C6-glioma. Jurkat and HL-60 cells. The cell viability after treating with extract of Selaginella Tamariscina was quantified by MTT assay method. The results showed that ST decreased the cell viability in HL-60 and Jurkat cells not in A549, Raw 264.7 and C6-glioma cells. And we also observed the chromatin condensation and DNA fragmentation in HL-60 and Jurkat cells. The enzyme activity of caspase-3, tightly regulated by an apoptosis activating complex, were markedly increased in HL-60 cells treated with the ST by dose-dependent manner. In conclusion, our results suggest that the extract of Selaginella Tamariscina may induce the selective apoptotic cell death in HL-60 and Jurkat cells via activation of caspase-3.
