Influence of Interferential Current Therapy and Laser Therapy on Functional Recovery after Total Knee Replacement

  • Purpose: The purpose of this study is to investigate the effects of interference current therapy and laser therapy on functional recovery after total knee arthroplasty by measuring the Berg balance scale and range of motion. Methods: Subjects were 30 patients who were admitted to G Hospital after total knee arthroplasty. They were randomly assigned to experimental group I in which interference current therapy was applied (n=10), experimental group II in which laser therapy was applied (n=10), or the control group (n=10). The Berg balance scale and range of motion of the subjects were measured before, after 2 weeks, and after 4 weeks of therapy. Results: There was a statistically significant change (p<0.05) in the Berg balance scale and range of motion before and after therapy intervention among the laser therapy group and the interference current therapy group. There was also a significant change between the groups in the Berg balance scale and range of motion. Tukey's post hoc comparison showed a statistically significant difference between the control group and experimental group I and between the control group and experimental group II (p<0.05). Conclusion: The application of interference current therapy and laser therapy resulted in a significant change in both the Berg balance scale and range of motion among patients with total knee arthroplasty. The findings of this study can be used as preliminary clinical data in evaluating functional recovery in patients with total knee arthroplasty in a post-clinic setting.

족관절 인공관절 치환술 후 족관절 운동범위의 변화 (Change of Tibio-Talar Motion After Total Ankle Replacement)

  • Purpose: Ankle fusion that is operated on severe ankle arthritis has its weakness in that normal walking is impossible, even though the result is pretty good. As a alternative choice, total ankle replacement pursues the longer survivorship with material improvement. However, it is not yet known how much range of motion is possible after the replacement, or how it has changed overtime. Therefore, we need an analyzation for that. Materials and Methods: A retrospective review of sixty-seven patients undergoing STAR total ankle replacement at our institution between 1998 and 2002 was conducted. Of those, twenty-six (39%) had complete sets of full dorsiflexion and plantar flexion lateral radiographs both between "immediate" postop and at a minimum of 2-years follow-up and no revision procedure during that time. The mean age of these patients was 63.2 years when the surgery was done; the etiology of arthrosis was 21(81%) post-traumatic/degenerative, 4 rheumatoid and 1 psoriatic. Results: Average "immediate" ankle range of motion was $15.9^{\circ}$, and total foot (non-ankle) motion was $20.6^{\circ}$. At one, two, and three years the average ankle and total foot ranges of motion were $17.4^{\circ}$, $17.6^{\circ}$, $15.6^{\circ}$ and $21.0^{\circ}$, $22.0^{\circ}$, $21.2^{\circ}$ respectively. Statistically there was no significant difference between "immediate" postop motion and one to three years postop (all p>0.05). Conclusion: The range of motion after the STAR total ankle replacement is maintained from the "immediate" postoperative range of motion, but not increased, in the 1-3 year post replacement period.

슬관절 전치환술 후 수동 및 능동 보조 관절가동범위운동이 슬관절 가동범위 증가에 미치는 영향 (The Effect of PROM and AAROM Exercise After TKA on Increasing the Knee Range of Motion)

  • Background: The purpose of this study was to find out the real truth of the effect of PROM (Passive range of motion) and AAROM (Active assistive range of motion) exercise on increasing the knee flexion. This randomized, controlled trial examined whether or not the incorporation of PROME(passive range of motion exercise) to a postoperative rehabilitation protocol would offer a better clinical outcome after TKA (Total knee arthroplasty) Method: The subject (n=36) measured range of motion (ROM).18 consecutive patients who underwent TKAs at SNUH dept. OS received PROME for knee by a physical therapist during the physiotherapy sessions and not for the other 18 patients who underwent TKAs received No-PROME (AAROME) for knee by herself. Result: There were significant differences in the PROME group, No-PROME group ROM progress width And there were significant differences in the PROME group between No-PROME group ROM improvement width. Conclusion: This study demonstrates that the incorporation of PROME does offer additional clinical benefits to the patients after TKA. Our findings may suggest that encouraging patients to perform PROM exercises would be a better option and that physiotherapy session by a physical therapist holds good even now.

기능적 마사지가 엉덩관절 전치환술 환자의 통증, 관절가동범위, 균형능력, 보행속도에 미치는 즉각적인 영향 (The Immediate Effect of Functional Massage on Pain, Range of Motion, Balance Ability and Gait Speed in Patients with Total Hip Replacement)

  • Background: Functional massage is a therapeutic massage that incorporates joint motion in non-end-range to reduce pain and improve range of motion (ROM) in patients. This study was aimed at investigating the immediate effect of functional massage on pain, range of motion, balance ability and, gait speed of patients having undergone total hip replacement. Methods: Twenty patients were treated by one participating orthopedic manual physical therapist and randomly assigned to the control group (n=10) or the experimental group (n=10). To treat patients of each group, functional massage and range of motion exercises were used. The experimental group received a functional massage and the control group received range of motion exercise for minutes for one session. The visual analog scale was used for pain assessment. Balance ability was measured using a timed up and go test and a one-leg standing test for patients. The 10-meter walk test was used for the measurement of gait speed assessment of patients. Results: Significant improvements were observed in terms of balance ability (p<.05), gait speed (p<.05), and ROM (p<.05) after functional massage. There was no significant inter-group difference (p>.05). Conclusion: Application of the functional massage showed that statistically significant improvements in ROM, gait speed, and balance ability after a single treatment session. This technique may be a useful treatment in patients having undergone total hip replacement.

Changes in pain, swelling, and range of motion according to physical therapy intervention after total knee arthroplasty in elderly patients

  • Objective: This study was carried out to compare changes in pain, swelling and range of motion (ROM) between the two groups according to physical therapy intervention. Design: Randomized controlled trial. Methods: There were a total of 20 elderly subjects ages 65-75 years old who have been admitted to the hospital for a total knee arthroplasty. After surgery, the experimental group (n=10) carried out quadriceps femoris muscle strengthening exercise, hamstring stretching exercises and gastrocnemius stretching exercise with a physical therapist for 30 minutes and additionally, received ice pack therapy for 20 minutes once a day. The control group (n=10) carried out continuous passive motion for 30 minutes and received ice pack therapy for 20 minutes once a day. The experimental group and control group carried out each intervention program for 2 weeks 3 times a week. The Visual Analogue Scale was used to assess pain, tape measurements were taken to assess swelling, and a steel goniometer was used to assess knee joint ROM. Results: As a result, the experimental group showed a statistically significant decrease in the pain and swelling, and a significant increase in knee flexion ROM after the intervention, compared to the control group (p<0.05). There was a significant improvement in pain, edema, and knee flexion and extension ROM in all subjects after intervention (p<0.05). Conclusions: According the results of this study, exercise in the experimental group is effect on the pain, swelling and ROM for total knee arthroplasty.

탄성밴드운동이 슬관절전치환술 환자의 통증, 관절가동범위, 낙상두려움에 미치는 효과 (Effects of Elastic Band Exercise on Pain, Range of Motion, and Fear of Falling in Patients with Total Knee Replacement)

  • Purpose: The objective of this study was to verify the effects of elastic-band exercise on pain, range of motion, and fear of falling in patients with total knee replacement. Methods: The study design was a nonequivalent control group non-synchronized quasi-experimental design. Data were collected from December 10, 2014 to January 10, 2015 in an orthopedic specialty hospital located in the C city. Forty-eight patients participated in the study, and each twenty-four were assigned to the experimental group and the control group. The elastic-band exercise was used with the experimental group for 10 days. The numeric rating scale was used to measure pain, goniometer to measure range of motion, and a questionnaire to measure fear of falling. The data were analyzed using $x^2-test$, Fisher's exact, t-test and paired t-test. Results: Compared to the control group, pain in the experimental group decreased (t=-2.89, p=.006), range of motion increased (t=2.98, p=.005), and fear of falling decreased (t=-4.63 p<.001). Conclusion: The elastic-band exercise for total knee replacement patients is considered to be an effective nursing intervention to decrease pain and fear of falling, and to increase range of motion.

CT기반 3차원 모델을 이용한 고관절 운동범위 예측 (Hip Range of Motion Estimation using CT-derived 3D Models)

  • 본 연구에서는 CT영상기반 3차원 고관절모델을 이용한 컴퓨터시뮬레이션을 통해서 고관절의 운동범위(Range of Motion)를 측정하는 방법을 제시하였다. 본 연구에서는 그 측정방법에 대한 기술적인 사항을 제시하고, 그 기술이 재현성 있게 실현할 수 있도록 대퇴골두 중심점의 결절, 대퇴골 외전(Abduction)/내전(adduction)회전축, 굽힙(flexion)/신전(extension) 회전축을 정의하고 측정하는 명확한 방법을 제시하였다. 외전각은 해부학적인 시상면(Sagittal plane)상의 Anterior-Posterior축에 대해 아래쪽(Inferior)면으로부터 Lateral 쪽으로의 회전각으로 정의된다. 최대외전각은 대퇴골두가 엉덩이뼈(Pelvis)의 절구(Acetabulum)의 테두리와 겹치지 않고 Anterior-Posterior축을 중심으로 회전할 수 있는 최대 외전각으로 결정된다. 굴곡각은 해부학적인 관상면(Coronal plane)상의 Medial-Lateral축에 대해 아래쪽(Inferior)면으로부터 회전각으로 정의된다. 최대굴곡각은 대퇴골이 Medial-Lateral축을 중심으로 엉덩이뼈(Pelvis)의 절구(Acetabulum)의 테두리와 겹치지 않고 회전할 수 있는 최대 굴곡각으로 결정된다. 정상고관절에 비해 인공고관절술을 받은 해당 환자의 경우, 외전에서는 60도 정도, 굽힘에서는 4도 정도 운동범위가 줄어들 수 있다는 예측이 나왔다. 본 연구에서 행한 시뮬레이션을 해보고 외전의 경우 운동범위의 감소가 예측되므로, 대퇴골두를 조금 큰 것을 고르거나 대퇴골목부의 길이 (femoral neck offset)를 길게 시술해야 할 필요가 있음을 의미한다.

  • Human uses sight, tactile sense to evaluate Total Hand Value(FUUAI) of textile fabrics. Tactile sense is important factor which decided the Total Hand Value of a textile fabric. When human feels the FUUAI, physical and physiological phenomena are occurred in finger. We first found out physical variable that is happened in fingertip when human is feeling the FUUAI. Such physical variable means characteristic of action tactual motion of finger such as moving range, tactile time, moved distance, speed of finger and applied force by finger. We study the relationship between action tactual motion and the ability in which the human distinguishes the textile fabric. As a result, we could know the characteristics of the tactual motion of fingertip to get high distinguishable ability. The characteristics were different in men and women respectively. In the case of man, touched time and moving range influenced to distinguish, and moving range, and the moving speed of finger influenced, in woman's case.

외이전기경혈자극과 경피전기신경자극이 슬관절 전 치환슬 환자의 수술 후 통증조절에 미치는 효과 (The Effects of Auricular Electroacustimulation and Transcutaneous Electrical Nerve Stimulation on Postoperative Pain Control in Total Knee Replacement Patients)

  • This study was done to determine differences in effect of postoperative pain control in patients receiving auricular electroacustimulation vs transcutaneous electrical nerve stimualtion following total knee replacement surgery. Thirty-one cases referred to physical therapy department after treated by total knee replacement surgery by orthopedic surgery department at the Pohang St. Mary's Hospital from January 1993 through June 1994. Of 31 total knee replacement cases, 13 cases were auricular electroacustimulation group, 11 cases were transcutaneous electrical nerve stimulation group, and 7 cases were control group. The results of the study summerized are as follows: Thirty-one total knee replacement cases(male in 12 cases, female in 19 cases), ranging in age from 34 to 61 years(mean${\pm}$SD=49.90 7.56) with diagnoses of degenerative arthritis(20 cases), rheumatoid arthritis(9 cases), and other(2 cases). In auricular electroacustimulation group, there was a significant change of pain intensity, unpleasantness, and active range of motion after treatment(p<0.01). In transcutaneous electrical nerve stimulation group, there was a significant change of pain intensity, unpleasantness, and active range of motion after treatment(p<0.01). In control group, did not show significant pre-posttreatment differences in pain intensity, unpleasantness, active range of motion(p>0.05). The mean change in pain intensity and unpleasantness, active range of motion from pretreatment baseline for the 3 groups. Auricular electroacustimulation group showed the large magnitude of increase in pain intensity and unpleasantness, active range of motion when compared to its own pretreatment cycle. Transcutaneous electrical nerve stimulation group showed small magnitude of increase in pain intensity and unpleasantness, active range of motion when compared to its own pretreatment cycle. No significant changes were observed in control group. Highly significant differences in pain intensity, unpleasantness, and active rage of motion were found using an ANOVA measures between treatment groups and control group(p<0.01). The squares correlation coefficients of pain and function measures pretreatment-posttreatment differences for each group. In treatment group, there was significant correlation between pain scale and function(p<0.001). In control group, there was no correlation between the pain scale and function (p>0.05). The continuous study is needd for many interesting issues of auricular electroacustimulation in new future.
