Clinical evaluation of efficacy of transcortical anesthesia for the extraction of impacted mandibular third molars: a randomized controlled trial

  • Background: This study aimed to compare the pain levels during anesthesia and the efficacy of the QuickSleeper intraosseous (IO) injection system and conventional inferior alveolar nerve block (IANB) in impacted mandibular third molar surgery. Methods: This prospective randomized clinical trial included 30 patients (16 women, 14 men) with bilateral symmetrical impacted mandibular third molars. Thirty subjects randomly received either the IO injection or conventional IANB at two successive appointments. A split-mouth design was used in which each patient underwent treatment of a tooth with one of the techniques and treatment of the homologous contralateral tooth with the other technique. The subjects received 1.8 mL of 2% articaine. Subjects' demographic data, pain levels during anesthesia induction, tooth extractions, and mouth opening on postoperative first, third, and seventh days were recorded. Pain assessment ratings were recorded using the 100-mm visual analog scale. The latency and duration of the anesthetic effect, complications, and operation duration were also analyzed in this study. The duration of anesthetic effect was considered using an electric pulp test and by probing the soft tissue with an explorer. Results: Thirty patients aged between 18 and 47 years (mean age, 25 years) were included in this study. The IO injection was significantly less painful with lesser soft tissue numbness and quicker onset of anesthesia and lingual mucosa anesthesia with single needle penetration than conventional IANB. Moreover, 19 out of 30 patients (63%) preferred transcortical anesthesia. Mouth opening on postoperative first day was significantly better with intraosseous injection than with conventional IANB (P = 0.013). Conclusion: The IO anesthetic system is a good alternative to IANB for extraction of the third molar with less pain during anesthesia induction and sufficient depth of anesthesia for the surgical procedure.

만기 잔존된 제 2 유구치 발거에 의한 제 2 소구치의 자발적 맹출 유도 (ERUPTION GUIDANCE OF IMPACTED SECOND PREMOLAR TOOTH BY EXTRACTION OF PROLONGED RETAINED SECOND PRIMARY MOLAR.)

  • 매복은 임상적 또는 방사선학적으로 인식할 만한 맹출로의 장애물이나 잘못된 치배의 위치로 인하여 치아의 맹출에 진전이 없는 것을 말한다. 제 3 대구치와 상악 견치를 제외하고 제 2 소구치에서 가장 흔히 매복이 발생하며, 소구치의 매복은 전체 매복의 0.5%를 차지한다. 소구치 매복의 치료에는 교정적 견인과 외과적 재위치가 필요할 수도 있지만, 만기 잔존된 유구치에 의해 발생한 소구치 매복은 유구치의 발거 및 공간 유지로서 소구치의 맹출을 유도할 수 있다. 본 증례는 제 2 소구치의 미맹출을 주소로 경북대학교병원 소아치과에 내원한 환아에서 만기 잔존된 제 2 유구치를 발거하고 공간 유지를 하여 제 2 소구치의 자발적 맹출을 시도하였는 바 다음과 같은 결과를 얻을 수 있었다. 1. 만기 잔존된 제 2 유구치와 연관된 매복된 제 2 소구치는 유치 발거를 통한 최소한의 치료로 양호한 결과를 얻을 수 있다. 2. 제 2 소구치가 맹출할 때까지 적절한 공간 유지와 정기적인 방사선 검사가 필요하다. 3. 교정적 견인 또는 외과적 재위치를 시도하기 전 치아의 이동을 확인하기 위한 충분한 시간이 허용되어야 한다. 4. 치근이 미완성된 치아에서 좀 더 성공적인 결과를 얻을 수 있다.

수평 매복된 상악 중절치의 교정적 견인: 증례 보고 (ORTHODONTIC TRACTION OF HORIZONTALLY IMPACTED MAXILLARY INCISORS: CASE REPORTS)

  • 본 증례는 수평 변위 매복과 치근 만곡을 가진 상악 중절치를 폐쇄 노출법을 이용한 교정적 견인을 시행하여 영구 전치의 기능과 심미성을 회복하였다. 상악 영구 전치가 수평으로 매복된 경우, 가급적 조기에 발견하는 것이 중요하며, 촉진, 시진, 방사선 사진 등을 이용한 적절한 치료 계획의 수립이 필요하다. 상악 영구 전치의 매복이 조기에 적절히 치료되지 못하면 정중선의 변위, 인접치의 이동에 의한 맹출 공간의 감소, 치조골 높이의 차이 뿐만 아니라, 이로 인한 심미적인 문제를 야기하여 소아 심리 발달에 해로운 영향을 줄 수 있기 때문이다. 매복치의 치료는 매복치의 형태, 위치, 상태에 따라 다양하나, 주기적 관찰 방법, 상부 연조직과 경조직을 단순히 제거하여 맹출을 유도하는 방법, 외과적 노출 후 매복치를 교정적으로 견인하는 방법, 치아 이식술, 발치 등을 시행할 수 있다. 이중 이번 증례에서 시행한 매복치의 교정적 견인은 매복치의 치축이 정상 맹출로와 일치되지 않으며, 주기적인 관찰 후 더 이상의 맹출이 기대되지 않을 때 시행하는 가장 일반적으로 적용되는 방법으로 매복의 심도, 치아의 발육단계, 만곡의 부위와 정도, 수술방법과 견인방향 등을 고려하여 치료의 예후를 향상 시킬 수 있다.

치과용 콘빔 CT를 이용한 상악 정중과잉치의 3차원 분석 (Three dimensional evaluation of impacted mesiodens using dental cone beam CT)

  • Purpose : This study was performed to analyze the position, pattern of impacted mesiodens, and their relationship to the adjacent teeth using Dental cone-beam CT. Materials and Methods : Sixty-two dental cone-beam CT images with 81 impacted mesiodenses were selected from about 2,298 cone-beam CT images at Chonnam National University Dental Hospital from June 2006 to March 2009. The position, pattern, shape of impacted mesiodenses and their complications were analyzed in cone-beam CT including 3D images. Results : The sex ratio (M : F) was 2.9 : 1. Most of the mesiodenses (87.7%) were located at palatal side to the incisors. 79% of the mesiodenses were conical in shape. 60.5% of the mesiodenses were inverted, 21% normal erupting direction, and 18.5% transverse direction. The complications due to the presence of mesiodenses were none in 43.5%, diastema in 19.4%, tooth displacement in 17.7%, delayed eruption or impaction in 12.9%, tooth rotation in 4.8%, and dentigerous cyst in 1.7%. Conclusions : Dental cone-beam CT images with 3D provided 3-dimensional perception of mesiodens to the neighboring teeth. This results would be helpful for management of the impacted mesiodens.

Modified difficult index adding extremely difficult for fully impacted mandibular third molar extraction

  • Objectives: The aim of this study was to evaluate the validity of the existing classification and difficulty index of impacted mandibular third molars in clinical situations and propose a more practical classification system. Materials and Methods: This study included 204 impacted mandibular third molars in 154 patients; panoramic x-ray images were obtained before tooth extraction. Factors including age, sex, and pattern of impaction were investigated. All impacted third molars were classified and scored for spatial relationship (1-5 points), depth (1-4 points), and ramus relationship (1-3 points). All variables were measured twice by the same observer at a minimum interval of one month. Finally, the difficulty index was defined based on the total points scored as slightly difficult (3-4 points), moderately difficult (5-7 points), very difficult (8-10 points), and extremely difficult (11-12 points). Results: The strength of agreement of the total points scored and difficulty index were 0.855 and 0.746, respectively. Most cases were classified as moderately difficult (73.0%). Although only 13 out of 204 cases (6.4%) were classified as extremely difficult, patients classified as extremely difficult were the oldest (P<0.05). Conclusion: For difficulty classification, the authors propose one more difficult category beyond the existing three-step difficulty index: the clinician should consider the patient's age in the difficulty index evaluation.

기저세포모반증후군을 동반하지 않은 다발성 악골낭 두 증례 (Multiple jaw cysts not associated with basal cell nevus syndrome)

  • We present two cases of multiple jaw cysts not associated with basal cell nevus syndrome. Case 1: a nine year-old boy visited CNU Hospital for orthodontic treatment and his radiographs showed cystic lesions surrounding the crowns of teeth #13 and #17 respectively, which were diagnosed as dentigerous cysts. Subsequently, two more cysts were found on his follow-up radiographs in 12 and 15 months. The two cysts were determined to be odontogenic keratocysts. The boy had no skeletal abnormalities and no skin lesions associated with basal cell nevus syndrome. Case 2: a fifty-eight year old man had three impacted third molars with pericoronal radiolucencies, which were diagnosed as dentigerous cysts. He had no additional abnormalities associated with basal cell nevus syndrome. Multiple jaw cysts can occur at any age, and periodic radiographic surveillance may be needed for any cases of impacted tooth.

국소적 원인으로 매복된 구치의 교정치료 (Orthodontic Treatment of Molar Teeth Impacted by Local Factors)

  • 유착, 부적절한 교정치료, 과잉치, 낭종, 비정상적인 맹출경로등의 국소적인 원인으로 인하여 구치부의 매복이 발생할 수가 있다. 매복치는 적절한 치료를 하지 않으면, 교합기능의 상실, 영구치의 상실, 대합치의 정출, 인접치의 치근흡수등을 야기 할 수가 있다. 매복치를 성공적으로 맹출유도하기 위해서는 매복치의 치근형성여부가 중요한 요소가 되며, 매복치의 치근형태, 주위환경등을 유의한 적절한 교정력이 가해져야 한다.

  • This report presents a case of adolescent patient who had multiple impacted teeth and no systemic disease, and was treated with removable and fixed orthodontic appliances. The results obtained through these cases were summarized as follows : 1. Even in the case without systemic disease, there can be the delayed eruption of multiple teeth 2. If the root formation is not completed, root have no excessive curvature, axial inclination of the tooth is not excessive, and there is eruption space, the tooth can be induced to normal occlusion. 3. Since esthetically proper occlusion and dentition was acquired by recovery of the impacted teeth to normal occlusion, the prosthodontic treatment after extraction of the impacted teeth was less needed. 4. This treatment brought psychological stability to pubertal patient who was interested in his facial profile.

Multiple calcifying hyperplastic dental follicles: A case report

  • This report describes a 31-year-old female patient with six impacted teeth. The crowns of the impacted teeth were surrounded with cyst-like lesions with a mixed internal structure and well-defined cortical borders. Microscopic examination of the specimen obtained from the follicle of the left mandibular third molar tooth revealed loose to moderately dense collagenous connective tissue with abundant calcified material and sparse epithelial islands. A diagnosis of multiple calcifying hyperplastic dental follicles was made.

구개측 매복된 상악 견치의 교정적 치험례 (ORTHODONTIC TREATMENT OF THE PALATALLY IMPACTED MAXILLARY CANINE)

  • An impacted tooth is defined pathologically as a tooth that remains under the mucosa of inside bone without eruption of the crown after a specific period of eruption. Clinically, the term includes those teeth, even before eruption period, that are not expected to erupt due to shape, position and alignment of tooth and lack of space. Canine is prone to impaction more than other teeth because it has the longest time to develop and a complex route from the place of formation to the site of eruption. The impaction incidence of maxillary canine is repoted 0.92$\sim$3.3% (Ferguson, 1990). In 1995 Orton reported that the incidence was 0.92$\sim$2.2% and palatal impaction was more frequent than labial impaction(85%:15%). In 1969 Johnston presented it was more common to woman than to man(3:1). The etiology includes systemic disease such as endocrine disorder, cleidocranial dysostosis, irradiation, Crouzon syndrome, ricketts, facial hemihypertrophy and hereditary and local problems such as ectopic position of the tooth, distance of tooth from its place of eruption, malformation of the tooth, presence of supernumerary teeth, trauma of tooth germ, infection of tooth germ, displacement of tooth germ or tooth by a neoplasm, ankylosis, overretention of deciduous predecessor, lack of space for the tooth in the dental arch and mucosal barrier due to gingival fibrosis. The maxillary canine is especially important as it has the longest root, provides guidance for lateral movement of the mandible and masticatory function and assumes an important role esthetically as it is located at mouth angle. If left untreated, it may cause migration and external, internal resorption of adjacent teeth, loss of arch length, formation of dentigerous cyst or tumors, infection and referred pain as well as malposition of the tooth. Therefore, periodic examination of the development and eruption of the maxillary canine is especially important in a growing child. This case study presents the results of treatment of palatally impacted maxillary canine utilizing surgical exposure and orthodontic tooth movement on patients visiting SNUDH dept. of pediatric dentistry.
