Treatment outcomes and prognostic factors in oral tongue cancer: a 20-year retrospective study at the National Cancer Center, South Korea

  • Objectives: This study aimed to analyze the treatment outcomes and to evaluate the clinicopathological prognostic factors of oral tongue cancer. Patients and Methods: We retrospectively analyzed treatment results and prognostic factors in 205 patients with oral tongue squamous cell carcinoma who were admitted to the National Cancer Center, South Korea, between January 2001 and December 2020. The patients were treated with surgery and postoperative, definitive radiotherapy (RT) or chemoradiotherapy (CRT). Results: Eighteen patients (8.8%) were treated with curative RT or CRT, while the rest (91.2%) were treated with surgery with or without postoperative RT or CRT. The median follow-up period was 30 months (range, 0-234 months). The 5-year overall survival (OS) and 5-year disease-free survival (DFS) were 72% and 63%, respectively. Multivariate analysis revealed that a positive neck nodal status (N1, N2-3) was significantly associated with poorer 5-year OS and DFS, while perineural invasion was associated with poorer 5-year DFS. Conclusion: Cervical metastasis and perineural invasion are significant prognostic predictors, and combination treatments are necessary for improving OS and DFS in patients with these factors.

설결손의 재건 후 기능적 예측 인자의 분석 (Analysis of Factors for Satisfying Functional Outcomes in Tongue Reconstruction)

  • Purpose: Tongue cancer is the most common malignant tumor of the oral cavity and the ultimate goal in treatment of the cancer is not only complete excision and meticulous closure of the wound, but also, reconstruction of a demensional and functional tongue. Our study focuses on various factors, such as defect size, extent of tumor, age, application of mandibulectomy or radiotherapy, and their influences on postoperative speech and swallowing function. Methods: Our study was based on 59 patients who underwent tongue cancer operation and reconstruction of the tongue. Speech and swallowing were evaluated according to categories documented by Sultan and Teichgraeber. Patients were classified into 3 groups as partial glossectomy, hemiglossectomy and total glossectomy groups for evaluation. The average age of the patients were 51, and the mean follow-up period was 4 years 2 months. Results: The partial glossectomy group showed statistically relevant results for speech articulation and swallowing abilities compared to the total glossectomy group. In cases of defects involving the mouth floor, the group showed decreased results compared to the group without mouth floor involvement. Increased age showed decreased postoperative results with statistical significance, while mandibulectomy and radiotherapy revealed no statistically significant data. Analysis according to TNM staging resulted in decreased functional result with advanced staging without statistical significance. Conclusion: To summarize the factors influencing the functional outcome in tongue reconstruction, younger patients and early stage cancer with minimal surgical extent revealed more satisfying results while mandibulectomy and radiation did not have influence on our analysis. Addition of various influencing factors and studies with longer follow up periods on our patient groups may provide effective data for more satisfying functional outcomes in the future.

Tongue Cancer 환자에서 PET/CT 검사 시 Open Mouth 촬영법의 유용성 평가 (Usefulness Evaluation of Open Mouth View when PET/CT scan In Tongue Cancer Patients)

  • Tongue cancer는 혀에 생기는 악성종양으로 전체 암의 1.8%, oral cancer의 75%를 차지할 정도로 발생 빈도가 높은 것으로 알려져 있으며 초기 진단, 전이유무, 병기설정 등을 위해 PET/CT 검사를 실시하고 있다. Tongue cancer 환자의 PET/CT 검사 시 close mouth 상태로 Torso를 촬영하고 있으며 그에 따른 겹침이나 인공물의 영향으로 병소부위가 명확히 관찰되지 않는 경우가 발생되었다. 이에 open mouth로 PET/CT 검사 시 발생되는 변화와 그에 따른 유용성을 알아 보고자 한다. 20명의 환자(남 15명, 여 5명)를 대상으로 시행하였으며 먼저 close mouth 상태로 Torso 촬영 후 open mouth 상태로 1bed 추가 촬영하였다. Tumor(T), Normal Tongue(NT), Lymph Node(LN)에 관심영역을 설정하여 표준화섭취계수(standardized uptake value, SUV) $SUV_{mean}$, $SUV_{max}$를 측정하여 Background(Carotid artery) SUV를 빼주었다. Tumor size 변화를 측정하였으며 육안적 영상 해상도 평가는 blind test를 진행하였다. 비모수검정(wilcoxon signed rank test)을 통해 통계 분석하였다. Tumor에서 SUV는 open mouth 검사 시 증가하는 변화를 보였으며, normal tongue과 lymph node에서는 소폭의 변화가 있었으나 유의한 차이는 없었다. tumor size 변화와 blind test를 통해 전체적으로 비슷하거나 높은 영상 해상도를 보여주는 것을 확인할 수 있었다. Open mouth PET/CT 검사가 close mouth시 발생하는 문제점에 대한 완벽한 대안법이 될 수는 없지만 open mouth를 통해 구강 내 다른 구조물과의 겹침이나 tumor의 경계면을 조금 더 명확하게 관찰할 수 있을 것으로 사료된다.

Effects of Tobacco Smoking on the Dorsum of the Tongue and Buccal Epithelium

  • Objective: The aim of this study was to assess the effects of tobacco smoking on the dorsum of the tongue and buccal epithelium. Methodology: This case control cross-sectional study was conducted with 174 smoking and non-smoking volunteers living in the city of Hail, Northern KSA. Cytological Materials were obtained from buccal mucosa and dorsum of the tongue, and assessed using cytopathological methods. Results: In buccal smears, cytological atypia was observed in 17 out of 101 (16.8%) smoker cases but only 3/73(4.1%) of the controls. For cytological atypia in buccal and tongue smears, the adjusted odd ratio (OR) and the 95% confidence interval (CI) were found to be 4.7 (1.3-16.8), P < 0.016)) and 4.3 (0.93- 20.2), P <0.06)), respectively, in the two sites. Conclusion: Tobacco smoking is a major risk factor for occurrence of cytological atypia, which might subsequently develop into oral precancerous and cancerous lesions. Oral exfoliative cytology is an easy and cheap non-invasive procedure which appears highly suitable for screening populations at risk of developing oral cancer.

구강설에 발생한 점액선암종 1예 (A Case Report of Mucinous Adenocarcinoma of the Oral Tongue)

  • Mucinous adenocarcinoma (MAC) is a rare malignant neoplasm that occasionally occurs in the large intestine (colon), followed by the pancreas, ovary, lung, prostate, and breast. It is characterized by large amounts of extracellular epithelial mucin that contains tumor cell nests. We herein present a unique case of MAC originating from minor salivary gland, the second to be reported in literature in South Korea. We report a case of MAC in the tongue considered to be developed from minor salivary gland with a review of literature.

두 경부 암 환자의 방사선치료 시 자체 제작한 고정 기구 유용성의 고찰 (Implementation of Man-made Tongue Immobilization Devices in Treating Head and Neck Cancer Patients)

  • 목 적: 두 경부 암 방사선치료 시 구강 구조물 내에 정확한 자세 재현성을 유지하는 것은 환자에게 편안함을 주는 동시에 최적화된 선량분포를 얻는데 매우 중요하다. 이 연구의 목적은 방사선치료 시 재현성을 유지하고 구강 내부의 공기와 조직계면 사이의 선량을 보정하기 위해 각 환자의 고정 기구를 자체 제작하였으며, 이를 기존의 고정 기구와 비교 분석하는데 그 목적이 있다. 대상 및 방법: 치과 환자의 구강 조형물 제작 시 사용되는 알지네이트(alginate; AROMA FINE DF III normal set, japan)와 퍼티 (putty; aquasil soft putty/regular set, germany)를 이용하여 각 환자의 구강 구조와 동일한 고정 기구를 제작 하였다. 방사선 치료 시 재현성을 평가를 위해 자체 제작한 고정 기구를 이용하여 컴퓨터 단층 촬영 시와 동일한 자세로 5회 Linac-gram을 촬영하고 분석하였다. 자체 제작한 고정 기구의 선량 안정성을 검증하기 위하여 공기, 물, 알지네이트 그리고 퍼티를 삽입할 수 있는 아크릴 팬텀을 제작하였다. 실험은 두 가지 조사면적(3$\times$3 cm, 5$\times$5 cm)에 대해 단일 조사를 통해 이루어졌고 방사선 치료 계획 장비(Pinnacle ver.7.6c, Philips, USA)에 의해 계산된 선량과 EBT 필름을 사용한 측정된 선량을 비교 분석하였다. 결 과: 자체 제작한 고정 기구의 재현성 검증을 위한 조사야 확인 영상 확인 결과 기존의 고정 기구를 사용한 경우에 비해 알지네이트와 퍼티를 이용한 고정 기구는 4배 이상의 우수한 재현성을 보였고, 방사선 치료 시 환자의 불편함을 해소 할 수 있었다. 방사선 치료 계획 장비를 이용한 계산된 계면선량은 공기일 때 3$\times$3 cm, 5$\times$5 cm에서 각각 7.87%, 0.56%로 나타났으며 EBT (international specialty products, USA)필름을 사용한 측정된 계면선량은 36.5%, 11.8%로 EBT 필름을 사용한 측정된 계면선량이 크게 나타났다. 결 론: 두 경부 암 환자의 방사선 치료 시 구강 내 구조물의 자세 재현성을 유지하기 위하여 알지네이트와 퍼티를 이용하여 자체 제작한 고정 기구는 기존의 고정 기구 보다 매우 우수한 재현성을 유지 할 수 있었으며, 소 조사면에 대한 방사선 치료 계획 시 치료 계획 장비의 선량 계산 알고리즘의 한계로 인한 구강 내 계면선량의 감소를 보정해 최적의 선량분포를 얻을 수 있었다.

The effectiveness of elective neck dissection on early (stage I, II) squamous cell carcinoma of the oral tongue

  • Objectives: The purpose of this study was to evaluate and compare the treatment outcomes of partial glossectomy with or without elective neck dissection in patients with tongue squamous cell carcinoma (SCCa). Materials and Methods: A total of 98 patients who were diagnosed with tongue SCCa and underwent partial glossectomy between 2005 and 2014 were evaluated. Only 14 patients received elective neck dissection, and 84 patients received only partial glossectomy. Results: There were 56 men and 42 women with a mean age of 57 years and mean follow-up period of 33.7 months. There were 70 patients graded as T1 and 28 as T2. The total occult metastasis rate was 17.3%. The 5-year overall survival rate was 83.3% with elective neck dissection and 92.4% with observation. The 5-year disease-free survival rate was in 70.7% in the elective neck dissection group and 65.3% in the observation group. Conclusion: We retrospectively reviewed the records of 98 patients with tongue SCCa. These patients were divided into two groups, those who underwent elective neck dissection and those who did not. There was no statistically significant difference between the groups undergoing partial glossectomy with or without elective neck dissection.

초기 구강설암 환자에서 부분 설절제술 후 조음변화 (Articulation Changes after Partial Glossectomy in Patients with Early Tongue Cancer)

  • Backgrounds and Objects: Carcinoma of the tongue is the most common cancer of the oral cavity. A primary treatment strategy includes surgery and/or radiotherapy. Resection of the tongue often results in speech dysfunction, which depends on the site and extent of resection, type of reconstruction, and the mobility of remaining tongue. This study aimed to evaluate the characteristics of articulation errors that were resulted from the partial glossectomy without free flap reconstruction. Materials & Method : Articulation evaluations including speech intelligibility and percent of correct consonants (PCC) were performed for 24 patients who underwent partial glossectomy for their T1 or T2 tongue cancer. Mobility of the tongue, size of the resected tongue, and the history of adjuvant radiotherapy were analyzed for their relationship with the results of articulation evaluation. Results: Speech intelligibility score was $6.4{\pm}0.9$ (on 7-point scale) and overall PCC was 96.9%. There were close relationships between the size of resection and limitations in the tongue mobility, especially in "protrusion and elevation (r=-0.687)" and "retroflexion (r=-0.775)". Errors in "alveolar fricatives" and "palatal affricates" were also closely related with the size of resection (r=-0.537 and -0.538, respectively). PCC for "liquid sound" /r/ was 83.2%, which was closely related with the history of radiation therapy. Conclusion : Overall articulatory function was satisfactory in cases of early tongue cancer after partial glossectomy of a limited volume without flap reconstruction. However, the size of resection and the history of radiation therapy were closely related to the limitations in some types of tongue mobility and the resultant articulation errors.

Comparing Survival of Oral Cancer Patients Before and After Launching of the Universal Coverage Scheme in Thailand

  • Background: As the treatment modalities for oral cancer have been relatively consistent during the last two-decades, this study was conducted to compare survivals of oral cancer patients in Khon Kaen Province before and after the universal coverage scheme (UC) was launched in Thailand. Materials and Methods: The data were retrieved from the population-based cancer registry of Khon Kaen for oral cancer patients diagnosed during 1992-2001 (pre-UC), and 2004-2012 (post-UC). To compare survival of the two cohorts, Kaplan Meier and log rank tests were employed. Results: Of 1,196 patients, 65% were females and the median age was 65 years. The most common primary sites were lip (31.0%), tongue (29.9%), and buccal mucosa (14.6%). The proportion of early stage cancer increased from 20.4 % in pre-UC to 41.3% in post-UC. The overall 5-year survival rate was 36.5% (95% CI =32.6-40.9) for pre-UC and 32.4% (95% CI = 28.8-36.4) for post-UC. The declining survival was mainly due to an increasing proportion of tongue cancer. However, no survival improvement was demonstrated on subgroup analysis of the tongue cancer patients. Conclusions: After the universal coverage scheme had been launched, early diagnosis increased, but no significant gain in survival for oral cancer patients was achieved.

Squamous Cell Carcinoma as the Most Common Lesion of the Tongue in Iranians: a 22-Year Retrospective Study

  • The tongue has been globally considered as an indicator of general health for millennia. This study aimed to determine the prevalence and distribution of tongue lesions in an Iranian population. In this retrospective study, data from 6,435 oral biopsy reports over a 22-year period (1992-2014) were retrieved from archives of Oral and Maxillofacial Pathology Department, Shahid Beheshti Dental School, Tehran, Iran. These reports were analyzed according to age, sex, type of lesion and location. Prevalence of tongue lesions were reported as percentages. Out of total oral lesions, 238 (3.7%) were found in the tongue, with the incidence peak (42%) being between 41-60 years. Men constituted 53% and women 47%of patients. The youngest patient was a 3-year-old girl with pyogenic granuloma and the oldest one was a 93-year-old man with squamous cell carcinoma (SCC). SCC was the most common (25%) lesion generally found in the lateral border of the tongue with a male predilection. The second and third most prevalent lesions of the tongue were benign keratosis (frictional keratosis) (13.4%) and leukoplakia (13%).White-red lesions (38.6%) were the most frequent subgroup followed by neoplastic lesions (28%). Moreover, irritation fibroma, non-specific ulcers, squamous papilloma, and hemangioma were found as the most frequent lesions in their related subgroups.Given the high rate of SCC of the tongue in Iranian patients, this area should be examined more carefully by dental practitioners and physicians.
