Objective : This paper presents data from a retrospective study of endoscopic third ventriculostomy (ETV) in patients with shunt malfunction and proposes a simple and reasonable post-operative protocol that can detect ETV failure. Methods : We enrolled 19 consecutive hydrocephalus patients (11 male and 8 female) who were treated with ETV between April 2001 and July 2010 after failure of previously placed shunts. We evaluated for correlations between the success rate of ETV and the following parameters : age at the time of surgery, etiology of hydrocephalus, number of shunt revisions, interval between the initial diagnosis of hydrocephalus or the last shunt placement and ETV, and the indwelling time of external ventricular drainage. Results : At the time of ETV after shunt failure, 14 of the 19 patients were in the pediatric age group and 5 were adults, with ages ranging from 14 months to 42 years (median age, 12 years). The patients had initially been diagnosed with hydrocephalus between the ages of 1 month 24 days and 32 years (median age, 6 years 3 months). The etiology of hydrocephalus was neoplasm in 7 patients; infection in 5; malformation, such as aqueductal stenosis or megacisterna magna in 3; trauma in 1; and unknown in 3. The overall success rate during the median follow-up duration of 1.4 years (9 days to 8.7 years) after secondary ETV was 68.4%. None of the possible contributing factors for successful ETV, including age (p=0.97) and the etiology of hydrocephalus (p=0.79), were statistically correlated with outcomes in our series. Conclusion: The use of ETV in patients with shunt malfunction resulted in shunt independence in 68.4% of cases. Age, etiology of hydrocephalus, and other contributing factors were not statistically correlated with ETV success. External ventricular drainage management during the immediate post-ETV period is a good means of detecting ETV failure.

The feeder cable assembly is an automotive part used for telecommunication. If it malfunctions, the control and safety of the automobile can be put at risk. ALT (Accelerated Life Testing) is a testing process for products in which they are subjected to conditions (stress, strain, temperatures, etc.) in excess of their normal service parameters in an attempt to uncover faults and potential modes of failure in a short amount of time. Failure is caused by defects in the design, process, quality, or application of the part, and these defects are the underlying causes of failure or which initiate a process leading to failure. Thermal shock occurs when a thermal gradient causes different parts of an object to expand by different amounts. Thermal shock testing is performed to determine the ability of parts and components to withstand sudden changes in temperature. In this research, the main causes of failure of the feeder cable assembly were snapping, shorting and electro-pressure resistance failure. Using the Coffin-Manson model for ALT, the normal conditions were from Tmax = $80^{\circ}C$ to Tmin = $-40^{\circ}C$, the accelerated testing conditions were from Tmax = $120^{\circ}C$ to Tmin = $-60^{\circ}C$, the AF (Acceleration Factor) was 2.25 and the testing time was reduced from 1,000 cycles to 444 cycles. Using the Bxlife test, the number of samples was 5, the required life was B0.04%.10years, in the acceleration condition, 747 cycles were obtained. After the thermal shock test under different conditions, the feeder cable assembly was examined by a network analyzer and compared with the Weibull distribution modulus parameter. The results obtained showed good results in acceleration life test mode. For the same reliability rate, the testing time was decreased by a quarter using ALT.
In this work, a neuro-fuzzy inference system combined with the wavelet denoising, PCA (principal component analysis) and SPRT (sequential probability ratio test) methods is developed to detect the relevant sensor failure using other sensor signals. The wavelet denoising technique is applied to remove noise components in input signals into the neuro-fuzzy system The PCA is used to reduce the dimension of an input space without losing a significant amount of information. The PCA makes easy the selection of the input signals into the neuro-fuzzy system. Also, a lower dimensional input space usually reduces the time necessary to train a neuro-fuzzy system. The parameters of the neuro-fuzzy inference system which estimates the relevant sensor signal are optimized by a genetic algorithm and a least-squares algorithm. The residuals between the estimated signals and the measured signals are used to detect whether the sensors are failed or not. The SPRT is used in this failure detection algorithm. The proposed sensor-monitoring algorithm was verified through applications to the pressurizer water level and the hot-leg flowrate sensors in pressurized water reactors.

Small valves including ball valves, gate valves and butterfly valves have been adopted in the fields of steam power generation, petrochemical industry, carriers, and oil tankers. Butterfly valves have normally been applied to fields where in narrow places installing the existing valves such as gate valves and ball valves have proven difficult due to the surrounding area and the heavier of these valves. Butterfly valves are used to control the mass flow of the piping system under low pressure by rotating the circular disk installed inside. The butterfly valve is benefitted by having simpler structure in which the flow is controlled by rotating the disc circular plate along the center axis, whereas the weight of the valve is light compared to the gate valve and ball valve above-mentioned, as there is no additional bracket supporting the valve body. The manufacturing company needs to acquire the performance and life test equipment, in the case of adopting the improving factors to detect leakage and damage on the seat of the valve disc. However, small companies, which are manufacturing the industrial valves, normally sell their products without the life test, which is the reliability test and environment test, because of financial and manpower problems. Furthermore, the failure mode analysis of the products failed in the field is likewise problematic as there is no system collecting the failure data on sites for analyzing the failures of valves. The analyzing and researching process is not arranged systematically because of the financial problem. Therefore this study firstly tried to obtain information about the failure data from the sites, analyzed the failure mode based on the field data collected from the customers, and then obtained field data using measuring equipment. Secondly, we designed and manufactured the performance and life test equipment which also have the real time monitoring system with the naked eye for the butterfly valves. The concept of this equipment can also be adopted by other valves, such as the ball valve, gate valve, and various others. It can be applied to variously sized valves, ranging from 25 mm to large sized valves exceeding 3000 mm. Finally, this study carries out the life test with square wave pressure, using performance and life test equipment. The performance found out that the failures from the real time monitoring system were good. The results of this study can be expanded to the other valves like ball valves, gate valves, and control valves to find out the failure mode using the real time monitoring system for durability and performance tests.

In 2014, ISO 55000s has been enacted and the power plant asset management is becoming a hot issue for all over the world. The asset management system is being developed as a combination of CBM(Condition Based Maintenance) and RCM(Reliability Centered Maintenance). Therefore, the research on the calculation of the failure rate which is the most basic index of RCM is actively carried out. The failure rate calculation has been going on for a long time, and the most widely used probability distribution is the Weibull distribution. In the Weibull distribution, the failure rate function is determined in three types according to the value of the shape parameter. However, the Weibull distribution has a limitation that it is difficult to apply it when the trend of failure rate changes-such as bathtub curves. In this paper, the failure rate is calculated using the mixture Weibull distribution which can appropriately express the change of the shape of the failure rate. Based on these results, we propose the necessity and validity of applying mixture Weibull distribution.
This study aims to analyze the predict capability of some of the popular software NHPP reliability models(Goel-Okumo model, delayed S-shaped reliability model and Rayleigh distribution model). The predict capability analysis will be on two key factors, one pertaining to the degree of fitment on available failure data and the other for its prediction capability. Estimation of parameters for each model was used maximum likelihood estimation using first 80% of the failure data. Comparison of predict capability of models selected by validating against the last 20% of the available failure data. Through this study, findings can be used as priori information for the administrator to analyze the failure of software.

This study was originated to investigate the imparired parts of the mist and dust blowers and intended to analyze the causes of their failures by the use of the Weibull probability paper. By the use of the paper, the parts which were needed to change the design, the force of the urgency, the mean time between failures of the parts and the basic causes of the troubles could be predicted. The survey showed that the following parts got out of order: (A) flexible rubber hose, (B) blowing fan, (C) lead valve, (D) piston ring. (E) crank main bearing, (F) coil in magneto and (G) needle valve in carburettor. The analysis of the survey indicated that the parts G, C, E and D were belonged to "wear-out failure", and that the mean time between failures became shorter in order as indicated above. To insure longer lives of those parts, it would be necessary to change the design and the material and to upgrade operators mechanical technique of the mist and dust blower. The failure of the parts A, B and F was classified as "randomfailure", and they did not seem to be the "wear-out" at that time. The parts B and F was evaluated as "initial failure". Quality controls and operating tests by the producer. and the operational training for users should be strengthened so as to eliminate these failures. Were the failures only in the parts A, C, D, E, F and G, it could be said that the "reliability" of the mist and dust blower would fall to 10 percent in the second year, and 50 percent of the mist and dust blowers would fail in all parts A, C, D, E, F and G before the blower would have control insect and disease in 58.2 hectares. The use of the Weibull probability paper for analyzing failures of the mist and dust blowers was effective in that it analyzed failure in the relationship between strength of the parts and users actual circumstanoes.

It might be difficult to reduce failure rate for most of the cases are not available for debugging during operational phase, hence, there are some literatures to study that the failure rate is uniform throughout the operational time. The failure rate reduces and the reliability grows with time regardless of debugging. As a result, the products reliability varies with the time duration of these products in point of customer view. It accumulates the products experience, studies the efact operational method, and then finds and takes action against the fault circumstances. I propose the simple model to represent this status in this paper.
Reliability Prediction using MIL-HDBK-217F has some restrictions due to its one modeling basis. One of the restrictions is caused by selecting one operating environment of a system, which is chosen regardless of its detailed conditions, e.g., external impact and vibration. Especially, an equipment, which is installed on a mobile vehicle though its movement is quasi-static, is controversial to designate its environment as ground mobile($G_M$), rather than ground fixed($G_F$). In this paper, failure rates were compared, which are computed using several moving time rates to total operating time. RiAC-HDBK-217Plus was used as the basic calculation model. In addition, $G_F$ conditioned failure rate was evaluated by comparing with that under $G_M$ environment but fixed state.
The reliability prediction and evaluation for general electronic components are required to guarantee in quality and in efficiency. Although many methodologies for predicting the reliability of electronic components have been developed, their reliability might be subjective according to a particular set of circumstances, and therefore it is not easy to quantify their reliability. In this study reliability prediction of electronic components, that is the interface card, which is used in the CNC(Computerized Numerical Controller) of machine tools, was carried out using PRISM reliability prediction specification. Reliability performances such as MTBF(Mean Time Between Failure), failure rate and reliability were obtained, and the variation of failure rate for electronic components according to temperature change was predicted. The results obtained from this study are useful information to consider a counter plan for weak components before they are used.
