보행 시 파킨슨병 환자의 시·공간적 지표의 특성 (Characteristics of Spatio-Temporal Parameters in Parkinson's Disese During Walking)

  • The purpose of this study was to compare spatio-temporal parameters during walking between patients with idiopathic Parkinson's disease and a control group matched for age, height, and weight. Thirty-three subjects were included in this study. Fifteen normal subjects (age, $63.3{\pm}5.8$ yrs; height, $164.1{\pm}8.7$ cm; weight, $60.7{\pm}17.5$ kg) and eighteen patients (age, $64.0{\pm}7.7$ yrs; height, $164.7{\pm}7.3$ cm; weight, $63.6{\pm}7.7$ kg) participated in the study. The Vicon 512 Motion analysis system was used for gait analysis in each group during walking, with and without an obstacle. The measured spatio-temporal parameters were cadence, walking speed, stride time, step time, single limb support time, double limb support time, stride length, and step length. Results in stride length and step length, when walking without an obstacle, showed a significantly greater decrease in the patient group compared to the control group. During walking with an obstacle, the patient group showed a significantly greater decrease in the step length as compared to the control group. For the control group, there were significant decreases in parameters of cadence and walking speed and increases in parameters of stride time, step time, and single limb support time when walking with an obstacle. The patient group had lower cadence and walking speed and higher stride time, step time, and single limb support time during walking with an obstacle than in walking without an obstacle. These results suggest that patients with Parkinson's disease who walk over an obstacle can decrease cadence, stride length, and step length. Further study is needed, performed with more obstacles and combined with other external cues, such as visual or acoustic guides.

요통 여성 노인의 장애물과 이중 과제 보행 시 속도 및 입각기 시간 분석 (Analysis of Gait Velocity, Stance time on Obstacle and Dual Task Gait in Elderly Women with LBP)

  • Purpose : This study was to evaluate gait velocity and stance time on 5 conditions in elderly women and elderly women with LBP. The subjects were 70's generation. Methods : The subjects are 20 divided into 2 groups. They measured gait velocity, stance-time. The 5 conditions were normal gait, 10cm obstacle gait, 25cm obstacle gait, dual 10cm obstacle gait, dual 25cm obstacle gait. The experimental period was between 2008/12 and 2009/2. Statistical analysis was used Repeated measurement for difference between conditions, independent t-test for difference in two groups. Subjects were countdown from 50 during dual task gait. Results : The results were as follow: there were significantly difference 10cm obstacle velocity, dual 10cm obstacle velocity in two group. The others were not significantly differences. Velocity and stance-time were significantly difference in control group. In dual 25cm obstacle gait, velocity was difference of normal gait. Stance-time was difference in 25cm obstacle gait, and dual 25cm obstacle gait. In Experimental group, velocity and stance-time were not significantly difference. But measured value of velocity was gradually decreased and stance time was increased. Conclusion : These results indicate that elderly people with LBP women are reduced gait ability in dual task, and obstacle condition. So they need to prevent falling in dual task, and obstacle gait and to train obstacle/dual tak gait.

Effects of Intensive Neuro Rehabilitation Intervention on the Motor Function Recovery and Balance in Stroke Patients

  • PURPOSE: The purpose of this study was to investigate the effects of differences in exercise time arrangement on the functional recovery of the lower limbs and balance of stroke patients. METHODS: The subjects of this study were 100 patients who were hospitalized in two hospitals located in Gyeonggi-do. Before the experiment, 60 of these patients who met the inclusion criteria and did not meet the exclusion criteria were randomly divided into an experimental group (n = 30), whose exercise time was concentrated, and a control group (n = 30), whose exercise time was diffuse. The two groups underwent six weeks of physical and occupational therapy four times a day for five days a week. One session of therapy took 30 minutes, including three sets of physical therapy and one set of occupational therapy. The rest time between the sessions was different for each group. The experimental group had five minutes of rest between each therapy session, and the control group had two hours of rest time between each session. The Fugl-Meyer assessment (FMA), an assessment of each patient's limit of stability (LOS), and a timed up and go test (TUG) were used as test tools. RESULTS: Both groups showed statistically significant increases in their FMA results, LOS measurements, and TUG results. The FMA results of the experimental group were significantly higher than those of the control group. CONCLUSION: Thus, concentrated exercise time was more effective than diffuse exercise time for the recovery of motor function.

노인의 장애물 보행 시 장애물 높이에 의한 압력중심 이동시간의 차이 (Time Difference of the COP Displacement according Obstacle Height during Obstacle Crossing in Older Adults)

  • Purpose: This study examined the difference in the center of pressure (COP) displacement time in older adults according to the obstacle height during stance at each sub.phase when crossing obstacles. Methods: Fifteen older adults were enrolled in this study (${\geq}65$ years of age). The F-scan was used to measure the COP displacement time when crossing a 0, 10 and 40cm obstacle, and the stance phase was divided into 4 sub-phases according to the foot contact pattern. Results: During the stance phase, the COP displacement time increased with increasing obstacle height. During the mid-stance, terminal stance and pre-swing except for the loading response, there were significant differences in the COP displacement time according to the obstacle height. Conclusion: This study suggests that older adults show differences in the COP displacement time according to the stance sub-phase while crossing obstacles, and they use different mechanisms according the sub-phases to maintain balance during obstacle crossing.

치주 치료후 발생하는 구순포진 (HERPES LABIALIS OCCURING AFTER PERIODONTAL THERAPHY)

  • The purpose of this study is to investigate the relationship between occurrence and inducing factors of herpes labialis developed after periodontal therapy and to suggest prediction model of this lesion. A total of 100 patients were studied. A standard schedule was used for interviews of patients. It included demographic information, patient and familial history of recurrent aphthous ulcer and recurrent herpes labialis, history of systemic disease, religion, and emotional state. In case of female patients, the association of dysmenorrhea and onset of recurrent herpes labialis was also observed. After periodontal therapy, some details about therapy, such as the kind of therapy, location, spending time were recorded. At next appointment, the appearance and location of herpes labialis were examined. The frequency of herpes labialis after periodontal therapy was 8% and the location was predominantly mouth angle. The significant relationship was found between the onset of herpes labialis and the history of recurrent herpes labialis, surgical therapy rather than non-surgical therapy, and spending time. The prediction model of herpes labialis was not apparently established with the results of this study. In conclusion it is suggested that we should minimize traumatic manipulation and treatment time to prevent the onset of herpes labialis after periodontal therapy.

중풍 후유증으로 인한 실어증 환자에 한방치료와 언어치료를 병행한 경험2례 (Two cases of Combination Therapy of Acupuncture, Herbal medication and Speech Therapy for Aphasic Stroke Patients)

  • Aphasia is frequent in stroke patients and most patients with aphasia exhibit spontaneous progressive improvement in language abilities over time, but few recover completely. Neurological variables, especially initial severity of aphasia and time post-onset, appear to have influence on improvement. Effect of speech therapy and pharmacotherapy has been studied and some drugs, like amphetamine are proved to be benefit for recovery of aphasia following stroke. But there has been few evidence to facilitate recovery from aphasia by acupuncture or herbal medication therapy. So we report two cases of aphasic stroke patients who treated by combination therapy of acupuncture, herbal medication(Cheongsinhaeo-tang) and speech therapy over 6 months and improved in language abilities. Further clinical studies will be needed to explore the effects of acupuncture and herbal medication therapy for aphasia. Researchers should examine the long term effect of these treatment, and whether it is more effective than speech therapy and western pharmacotherapy or not..

Effects of kinesio taping on the gait parameters of children with cerebral palsy: a pilot study

  • Objective: Children with spastic diplegic cerebral palsy show weakness especially in the lower-extremity rather than upper-extremity muscles and display characteristics such as asymmetric alignment, deficits in postural control or balance ability, and slow walking speed. Various therapeutic interventions are applied to children with cerebral palsy, of which taping is widely used in the field of rehabilitation, however, there are few studies of the effects of kinesio taping on gait patterns of children with cerebral palsy. The present study investigated the effects of kinesio taping on gait parameters of children with cerebral palsy. Design: Cross-sectional study. Methods: Four children with spastic diplegia participated in this study. The participants' gait parameters while walking 10 m with and without kinesio taping (tibialis anterior, quadriceps femoris, and gluteus maximus) were recorded. Gait parameters including gait velocity, cadence, step length, stride length, single support time, and double support time were evaluated using the GAITRite. Mean values were obtained after having the subjects walk three times in each condition with a 5-minute rest period between each condition. The order of each condition was assigned randomly. Results: There were significant improvements in gait velocity, step length, stride length, and single support time of the right leg with kinesio taping condition compared to the without kinesio taping condition taping (p<0.05). However, there were no significant differences in cadence, single support time of the left leg, or double support time. Conclusions: The results show that kinesio taping may have a positive effect for improving gait parameters of children with spastic diplegia. However, its usefulness in the rehabilitation of children with cerebral palsy needs to be further investigated.

운전 시 브레이크 페달링 속도가 하지 근수축 개시시간에 미치는 영향 (The effects of brake pedaling speed on onset time of muscle contraction in the lower extremity during driving task)

  • Purpose: Driving is essential to maintain independent living status in modern times. Many patients want to know when they can drive again, but it's only possible if they have the ability to control lower extremity muscles. In this study, we compared the effects of velocity on onset time of lower extremity muscles during driving tasks. Methods: Twelve participants (5 male, 7 female) were enrolled. EMGs were used to test the onset time of lower extremity muscles; tibialis anterior, soleus, rectus femoris. To analyze the data, we used two way ANOVA. Results: According to brake pedaling velocity, there was a significant difference in brake response time (p<0.05). Further, when comparing the lower extremity muscles, there was a significant difference in onset time (p<0.05). The order of muscle recruitment was tibialis anterior, rectus femoris, and soleus for achieving maximal velocity, but the order was rectus femoris, tibialis anterior, soleus for achieving submaximal velocity. Conclusion: Brake pedaling velocity has significant effects on onset time of muscle contractions in the lower extremities. We suggested that a future study needs more subjects and more detailed research such as evaluat-ions of visuo-motor coordination and fine motor dexterity.

인지적 시간관리에 필요한 기능적 뇌 활성 영역과 반응시간의 상관관계 (Correlation between Faster Response Time and Functional Activities of Brain Regions during Cognitive Time Management)

  • Purpose: This study was designed to determine the correlation between faster response time and functional activities of brain regions during cognitive time management. Methods: Twelve healthy subjects participated in this experiment. Subjects performed the serial reaction time task (SRTT), which was designed by the Superlab program, during fMRI scanning. When the 'asterisk' appeared in the 4 partition spaces on the monitor, the subject had to press the correct response button as soon as possible. Results: fMRI results showed activation of the left primary sensorimotor cortex, both premotor areas, the supplementary motor area, posterior parietal cortex and cerebellum. There were significant correlations, from moderate to strong, between faster reaction time and BOLD signal intensity in activated areas. Conclusion: These results suggest that motor skill learning to be needed cognitive time management is associated with greater activation of large scale sensorimotor networks.

Outcome Analysis of Cranial Molding Therapy in Nonsynostotic Plagiocephaly

  • Background It is known that nonsynostotic plagiocephaly does not spontaneously improve, and the craniofacial deformities that result from it. This study was conducted to analyze the effectiveness of helmet therapy for the nonsynostotic plagiocephaly patient, and to suggest a new treatment strategy based on this analysis. Methods A total of 108 pediatric patients who had undergone helmet therapy after being diagnosed with nonsynostotic plagiocephaly were included in this study. The patients were classified according to the initiation age of the helmet therapy, severity, and helmet wearing time. The treatment effect was compared using cranial vault asymmetry (CVA) and the cranial vault asymmetry index (CVAI), which were obtained from diagonal measurements before and after therapy. Results The discrepancy of CVA and CVAI of all the patients significantly decreased after helmet therapy. According to the initiation time of helmet therapy, the treatment effect was best at 5 months old or less. The helmet wearing time per day was proportional to the treatment effect up to 20 hours. In addition, the rate of the successful treatment (final CVA ${\leq}$ 5 mm) significantly decreased when the initiation age was 9.1 months or older and the treatment period was less than 7.83 months. Conclusions This study showed the effectiveness of the helmet therapy for nonsynostotic plagiocephaly patients. Based on analysis of this study, helmet therapy should be started at the age of 9 months or younger for 7.83 months or more, and the helmet wearing time should be more than 20 hours a day.
