시스템생물학의 한의학적 응용 (Application of Systems Biology to Traditional Korean Medicine)

  • In Korea and China, traditional medicine's holistic approaches, based on the views of whole-body and whole-person, have been applied to make the solution of health problem. However, these holistic approaches of traditional korea or chinese medicine have been limited in interpreting their theories in a view of modern scientific aspects of medicine. This limitation seems to be mainly due to the reductionism approaches of modern scientific medicine. Traditionally, science has taken a reductionism approach; dissecting biological systems into their constituent parts and studying them in isolation. However, systems biology based on omics technologies is providing a new thought and method for traditional medicine's research and interpretation. Systems biology uses integrity study as the characteristic and bioinformatic technology as the key method for connecting reductionism and holism. Therefore, it has much in common with the theory of traditional medicine. It was reviewed that how systems biology is applied to traditional medicine in Korea and China. Also it was suggested that more future researches on interpretation between traditional medicine and systems biology must be focused on personalized medicine since systems biology will have a major impact on future personalized therapeutic approaches.

탈 의료화로서의 대체의학에 관한 논점 (An Arguing Point on the Alternative Medicine as a Withdrawal Medicine)

  • This study aimes to understand trends in alternative medicine use as new therapeutic ways to overcome the limits of western medicine, and is primarily concerned with function of alternative medicine to influence cancer theraphy in the United States after 1970s. Features of oriental medicine and western medicine through the comparison of Quantum medicine and Newton physics are pointed as the basic theory in alternative medicine. Homeopathic medicine, Chiro practice, Aroma theraphy, Transcendental meditation, Acupuncture is shortly is shortly introduced by alternative medicine use. The advantage of this study is to gain as follows : Firstly, To be recognized alternative function of cancer theraphy by Western nations including United States. Secondly, To understand the integrative program utilized as the alternative ways of cancer theraphy in the field. Thirdly, To understand side effects in alternative medicine use. :astly, To recommand marketing stratigies on alternative medicine services to develop Korean health tourism. In conclusion, it is increasingly emphasized that the importance of alternative medicine in relation to health promotion in the western nations. Western travelers expect that alternative medicine service is attractive and effective as the complementary function on incurable disease. Health tourism to experience alternative medicine services by travel will be expected one of the key industries in the future.

감각통합이론에 근거한 장애와 비장애를 포용하는 놀이터 환경 구성에 관한 연구 (A Study on setting the Environment of Inclusive Playground Including Disabled Children and Children without Disabilities (including at-risk children) based on J. Ayres' Sensory Integration Theory)

  • This study starts at the at-risk children(the students in the margins)' needs for the inclusive education environment who increased lately. It is studied in the limited scope, which is about spacially 'playground' and 'Sensory integrative therapy' in terms of the special education therapy. The 6 environmental setting principles are suggested, which is redefined in terms of suitable for applying to the environment design based on the training way and arbitration principle of J. Ayres' Sensory integrative therapy. And then various environment design methods are drawn from above-mentioned 6 principles, they are categorized in 4 elements for environmental setting - sensory element, behavior element, mental element and design expression element. This trial brings daily practice of therapeutic stimulate to sensory integration dysfunction children, and sensory experiential diversity promoting development to general children. Expecially it has some advantage that at-risk children(the students in the margins) feel a sense of belonging as a member of society.

Cancer stem cell metabolism: target for cancer therapy

  • Increasing evidence suggests that cancer stem cell (CSC) theory represents an important mechanism underlying the observed failure of existing therapeutic modalities to fully eradicate cancers. In addition to their more established role in maintaining minimal residual disease after treatment and forming the new bulk of the tumor, CSCs might also critically contribute to tumor recurrence and metastasis. For this reason, specific elimination of CSCs may thus represent one of the most important treatment strategies. Emerging evidence has shown that CSCs have a different metabolic phenotype to that of differentiated bulk tumor cells, and these specific metabolic activities directly participate in the process of CSC transformation or support the biological processes that enable tumor progression. Exploring the role of CSC metabolism and the mechanism of the metabolic plasticity of CSCs has become a major focus in current cancer research. The targeting of CSC metabolism may provide new effective therapies to reduce the risk of recurrence and metastasis. In this review, we summarize the most significant discoveries regarding the metabolism of CSCs and highlight recent approaches in targeting CSC metabolism.

사상체질의학을 포함한 체질론적 의학이론의 비교 연구 (A Comparative Study for World-wide Constitutional Theories including Sasang Constitutional Medicine)

  • Objectives: This study aimed at comparing Sasang constitutional medicine(SCM) with other constitutional theories of the world. Methods: From PubMed database, we first searched all papers using the term 'constitution', then filtered those having really constitution-focused papers. Then, they were analyzed to compare each constitutional theory and medicinal practice. Results: There were five constitution-related medicines which have been practiced in medical field: Sasang, iridology, somatotype, dermatoglyphics and Ayurveda. There was the largest number of scientific researches for dermatoglyphics while the smallest number of papers was for SCM. The other three constitutional theories were based on anatomic/histologic difference between types in contrast to SCM and Ayurveda, which relied on inherited functional differences of four or three internal organs. Conclusions: This study simultaneously compared five major constitutional theories currently practiced as medicines in the world. We found that even as SCM seems to have the most potential as therapeutic medicine it should be studied more using scientific methodologies to be a world-wide medicine.

사례분석을 통한 『상한론(傷寒論)』 변병진단체계(辨病診斷體系)의 서사의학적 가치의 탐색 (Shanghanlun Diagnostic System : Exploring Value as Narrative Medicine by Analyzing Cases)

  • Objective : The purpose of this study is exploring value of Shanghanlun six diagnostic system as Narrative Medicine by analyzing cases. Methods : In the article, we examined the theory of narrative medicine and current studies. And then key elements of narrative medicine was defined. On the basis of these, two medical cases were analyzed by reordering in accordance with time sequences and causality for confirming narrative factors. Results : The narrative approach to analyzing cases shows us that different diseases could be the result of same pathological patterns based on Shanghanlun. This tells us following four aspects. [1] Shanghanlun is the causes-oriented records and the text includes narrative factors. [2] Shanghanlun six diagnostic system is a process of constructing plot of medical case history through the clinical interpretation of contextual meaning of patient's life. [3] In the process of diagnosis, Schemata and script are engaged in assuming and confirming six patterns diseases. [4] The subjective factors of an individual can be applied universally through the provision of Shanghanlun. Conclusions : Narrative factors of Shanghanlun six diagnostic system suggest the possibility and value of Shanghan Medicine as Narrative Medicine in terms of Medical Humanities, essence of medical diagnosis and therapeutic action.

회상(reminiscence)에 관한 개념 분석 (Concept Analysis : Reminiscence)

  • Concept analysis is a formal linguistic exercise to determine certain defining attributes. The basic purpose of concept analysis is to clarify ambiguous concepts in a theory, and to propose a precise operational definition which reflects its theoretical base (Walker, 1983, 27~28). In later life, elderly people are faced with various psychological problems such as depression, and lowered self-esteem for these psychological problems of the elderly. Reminiscence has been studied as an effective therapeutic measure by gerontologists and psychologists. It is considered to have potential as an effective nursing intervention with geriatric clients. The purpose of this paper was to explore the feasibility of adopting reminiscence as a nursing intervention through analysis of the concept. The concept can be defined as follows : Reminiscence is thinking and talking about one's past with or without a specific purpose. It is a composite mental process which involves psychological, emotional, and social interactional aspects. The defining attributes of reminiscence are : 1. Thinking about past experiences which are meaningful to oneself. 2. It includes the emotional aspect of past experiences. 3. It occurs with or without specific purposes. 4. It is communicated to others by means of language and the subject has the feeling of being heard. In regard to the consequences of reminiscence, it is expected to contribute to psychological adaptation by providing a sense of self-fulfillment and self-achievement, promoting the discovery of the meaning of life, decreasing depression, and overcoming guilt feelings and conflict about one's past. However, if the past is regarded as a total failure, it may be harmful to the psychological well-being of the subject.

통증 생리와 물리치료 원리 (Pain Physiology and Principles of Physical Therapy)

  • The pain is common among individuals with physical disabilities. It can interfere with therapy since patients with pain can become uncooperative and reluctant to move. This paper reviews the natural physiological mechanisms that can reduce pain perception, and considers physiological mechanisms which contribute to clinical pain by describing how the pain system changes its sensitivity depending upon the body's needs. The peripheral and central mechanisms contributing to sensitised nociceptive system are described with reference to the symptoms of clinical pain such as hyperalgesia, allodynia sopntaneous 'on-going'-projected and referred pain. It is suggested that in some chronic pain the nociceptive system maintains a state of sensitivity despite the absence of on-going tissue damage and under such circumstances the nociceptive system itself may have become dysfunctional. Such situations are often initiated by damage to nervous tissue which results in changes in the activity and organization of neuronal circuits within the central nervous system. The ability of the nociceptive system to operate in a suppressed state is also discussed with reference to pain modulation. The physical therapist can help facilitate the activation of these mechanisms through a combination of noninvasive modalities, functional activities, and the therapeutic use of self.

요가 요법이 월경통 증상 완화에 미치는 효과 고찰 (An Effect of Yoga Therapy on Relieving Dysmenorrhea ; RCTs(Randomized controlled trials) Review)

  • Objectives : This study is to explain the effects of Yoga therapy for relieving dysmenorrhea on the aspect of Oriental Medicine and to identify the therapeutic Yoga asanas applicable to clinic medicine. Methods : We searched papers using Pubmed, CNKI and 7 Korean medical databases. We used "Yoga" and "Dysmenorrhea" as keyword. Limitations were as follows ; the last 20 years of publications and search range only including RCTs. Papers without inclusion criteria were excluded. We classified all the searched RCTs studies into Yoga exercise prescriptions. Results : A total of 47 studies were found. And 38 were excluded during extract screening. Finally 3 RCTs were selected and analyzed out of 9 papers. Blood sampling and survey questionnaires were used in the outcome measure. They reported that Yoga exercise could be used for dysmenorrhea patients, which has a curative effect. Conclusions : Yoga therapy can be explained as Meridian muscle theory in the concept of Oriental Medicine. Also, it may be a favorable choice for dysmenorrhea patients. Further studies to describe the effects of Yoga therapy will be needed more.

초음파를 이용한 경피약물수송의 촉진 (Enhancement of Transdermal Drug Delivery Using Ultrasound)

  • Transdermal drug delivery offers various advantages over conventional drug delivery systems, such as avoidance gastrointestinal degradation and hepatic first-pass effect. encourages patient compliance. and possible sustained release of drugs. However, transdermal transport of drugs is low permeability of the stratum corneum, the superficial layer of the skin. Many physicochemical and biological factors influencing transdermal transport is described together with the corresponding experimental and clinical results. Phonophoresis is medical treatment with drugs introduced into the skin by ultrasound energy. Enhanced drug penetration is through to result from the biophysical alterations of skin structure by ultrasound waves. The frequency used for phonophoresis is usually from 20 kHz to 15MHz. Phonophoresis can be categorized in to three ranges: low-frequency range(below 1 MHz). therapeutic frequency range(1 to 3MHz), and high-frequency range(above 3 MHz). The depth of penetration of ultrasound into skin is inversely proportional to the frequency. Cavitation may cause mechanical stress. temperature elevation, or enhanced chemical reactivity causing drug transport. One theory is that ultrasound affects the permeation of the stratum corneum lipid structure as the limiting step in permeating through the skin. The range of indications for phonophoresis is wide. Aspecific classification of the range of indications is obtained by classification of pathological conditions. The continuous research is needed for many interesting issucs of phonophoretic transdermal delivory in new future.
