Radiotherapy Techniques for Breast Cancer (유암의 방사선치료방법에 대한 고찰)

  • Carcinoma of the breast has been treated by surgery followed by irradiation of the chest wall and regional lymphatics treatment planning of the breast cancer is required that lung must be spared as much as possible. However megavoltage irradiation of the internal mammary chain results in high dose to underlying heart, esophagus and spinal cord. Electron beam can be used for the irradiation of the internal mammary chain instead of megavoltage beam. We studied dose distribution of single anterior electron field, compared with traditional treatment methods. 12 and 15MeV electron beam with bolus has good dose distribution to spare underlying lung tissue and other organs.

  • A 70 years old female patient with thyroid cancer received palliative radiation therapy for neck swelling and hemoptysis. She had a cardiac pacemaker under her chest due to complete AV block since 8 years ago. We present clinical detail and review previously reported articles.

  • The use of primary breast irraditation with advantage of improved cosmesis in breast cancer may be the potential risks of radiation for a change in the number of normal breast cancers and lung fibrosis. The magnitude of the scattered dose for a variety of radiation treatment techniques from patient of breast cancer and phantom was measured by adequate dosimeters. We can reduce the dose of the normal breast to treated with radiation by understanding the factors contributing to the unwanted dose and by determining ways to decrease this dose.

  • Authors have measured virtual source distance of electron beam from CL/1800 medical linear accelerator, with newly designed method. Beam scanning was performed with the direction of beam axis in the air. Compared results between this study and well established in phantom measurement shows good agreement with in experimental error. And we have found that build-up cap plays very important role in air measurement because of charge build up. The method of in-air measurement of virtual source distance is very easy to set-up and generate accurate results.

  • The purpose of this study was to investigate the molecular mechanism by which environmental pH alters the radiation-induced expression of p53, the key regulator of cellular responses to radiation. We have already reported that an acidic environment markedly prolongs the radiation-induced expression of p53 and also prolongs the radiation-induced G2/M arrest.(omitted)

  • This paper on the status of radiotherapy machine and related facilities, clinical activities and radiation safety management is based on the statistical data which collected through the questionaire sent to a total number of 37 hospitals holding the Department of Therapeutic Radiology in Korea. It is true that the quality of instruments installed in the hospitals equal to that of the instrument in the industrialized conuntries' hospitals. But the clinical specialists and physicists who can utilize such instruments fall short of the required number, which might be a main factor in hindering the development of therapeutic radiology of Korea. According to the nation-wide cancer statistics, we can estimate the number of annual cancer patients as 45,000 to 50,000. As a result, probably around 25,000 should receive radiation therapy. It is expected that in the future the number of cancer patients to whom radiation therapy should be applied will become twice as much as that of the cancer patients in 1990. Given such a condition, the problem facing the Korean Society of Therapeutic Radiology now is to increase the number of medical doctors and physicists.

The association of rectal equivalent dose in 2 Gy fractions (EQD2) to late rectal toxicity in locally advanced cervical cancer patients who were evaluated by rectosigmoidoscopy in Faculty of Medicine, Chiang Mai University

  • Purpose: To evaluate association between equivalent dose in 2 Gy (EQD2) to rectal point dose and gastrointestinal toxicity from whole pelvic radiotherapy (WPRT) and intracavitary brachytherapy (ICBT) in cervical cancer patients who were evaluated by rectosigmoidoscopy in Faculty of Medicine, Chiang Mai University. Materials and Methods: Retrospective study was designed for the patients with locally advanced cervical cancer, treated by radical radiotherapy from 2004 to 2009 and were evaluated by rectosigmoidoscopy. The cumulative doses of WPRT and ICBT to the maximally rectal point were calculated to the EQD2 and evaluated the association of toxicities. Results: Thirty-nine patients were evaluated for late rectal toxicity. The mean cumulative dose in term of EQD2 to rectum was 64.2 Gy. Grade 1 toxicities were the most common findings. According to endoscopic exam, the most common toxicities were congested mucosa (36 patients) and telangiectasia (32 patients). In evaluation between rectal dose in EQD2 and toxicities, no association of cumulative rectal dose to rectal toxicity, except the association of cumulative rectal dose in EQD2 >65 Gy to late effects of normal tissue (LENT-SOMA) scale ${\geq}$ grade 2 (p = 0.022; odds ratio, 5.312; 95% confidence interval, 1.269-22.244). Conclusion: The cumulative rectal dose in EQD2 >65 Gy have association with ${\geq}$ grade 2 LENT-SOMA scale.
