The Effect of Herbal Formulation KH-305 Mainly consisted of Rubus Coreanus on Benign Prostatic Hyperplasia-Induced Rat (전립선 비대증 유발 백서에서 복분자를 주성분으로 한 생약제제 KH-305가 미치는 영향)

  • Benign prostatic hyperplasia (BPH) is one of the common diseases in elderly men. Recently, the old-aged population has increased, with the interest in the clinical importance of BPH ever growing. This study is designed to investigate the effects of KH-305 on BPH induced rat. The herb formulation KH-305 is consisted of Rubus coreanus, Cornus officinalis and Cuscuta chinensis. An experimental prostatic hyperplasia was induced in male rats by the administration of testosterone propionate, 3 mg/kg SC, for 2 months. The rats were divided into 3 experimental groups: the control, BPH-induced, oral KH-305 ingestion group. After 2 months, the prostates were removed, and analyzed for their prostatic weight and histological examination. The prostate weights were measured in each group, and found to be 820${\pm}$38mg, 3140${\pm}$26mg, 1880${\pm}$21mg in the control, BPH-induced, and oral KH-305 ingestion group, respectively. The BPH induced group showed statistically significant increases in their prostatic weights compared with control group(p<0.05) but oral KH-305 ingestion group showed more significant decreases than BPH-induced group statically(p<0.05). Histologically injected testosterone lead to prostatic hyperplasia in rats, but oral KH-305 ingestion decreased this change. These results suggest that KH-305 may be effective in treatment of BPH, and complementary medicine of BPH.

  • Background: The study was designed to assess the changes in working conditions through a comparative analysis of the characteristics of working conditions in 2006 and 2010. Methods: We performed a comparative analysis of the data related to the first Korean Working Conditions Survey (KWCS) and the second KWCS in the categories of demographic characteristics, quality of labor, exposure to hazards, and health problems. Results: From our analysis of the demographic characteristics, we saw an increase in labor force participation rate of women and elderly people. As a result of the investigation with regards to working hours, the ratio of employees who worked for ${\geq}49$ hours per week was decreased and the ratio of employees who worked for ${\geq}40h/wk$ increased. As for exposure to hazards, exposure to tobacco smoke notably decreased in 2010 compared with 2006. With regards to health problems, there was a sharp increase in the number of people who complained of muscle pain in their arms and legs. Conclusion: KWCS data included many aspects of working conditions as a nationwide sample. In addition, because this is a periodic nationwide survey, the labor force, working hours, harmful factor exposure, and the change in health problems characteristics according to the flow of time could be investigated. The information comparing the main results of the first survey conducted in 2006 and the second survey conducted in 2010 obtained through this study can be used as an important base material for the establishment of the national policy.

Effects of oral health behavior on remaining teeth in Korean adults older than 45 years with hypertension and diabetes (한국성인의 고혈압, 당뇨 환자의 구강건강행태가 잔존치아 수에 미치는 영향)

  • Objectives: The purpose of this study was to investigate the effects of oral health behaviors of patients with hypertension and diabetic patients on the number of remaining teeth in Korean adults over 45 years of age, furthermore, we also aimed to develop oral health care programs and health promotion for patients with hypertension and diabetes. Methods: Altogether, 1,526 subjects with hypertension and diabetes from the 6th Korean National Health and Nutrition Survey (2013-2015) were analyzed. Results: A multiple logistic regression analysis was performed to determine the effects of oral health behavior and periodontal disease on residual teeth in patients with hypertension and diabetes after adjusting for age. We found 1.53 times the risk of having less than 20 remaining teeth in the group of brushing more than 3 times a day (95% CI: 1.00,2.35, p<0.001), 2.27 times (95% CI: 1.23-4.17, p<0.05) in the group not using the dental floss group 0.05), compared to the group undergoing regular dental check-up (95% CI: 0.93-1.85, p>0.05), compared to the non-smoking group 0.77 times (95% CI: 0.51-1.14, p>0.05), 1.33 times (95% CI: 0.98-1.80, p>0.05) in the drinking group compared to the non-drinking group, in the periodontal disease group, the risk of having less than 20 remaining teeth was 2.19 times higher (95% CI: 1.48-3.24, p>0.001) than in the normal group. Conclusions: As a super-aging society, develops in Korea, the prevalence of hypertension and diabetes will increase gradually. Therefore, an oral health policy should be implemented at the national level for the prevention and treatment of periodontal disease as well as the management of oral health the elderly.

  • This study aims at applying Photoshop functions to digital make-up(DM) for aging effect based upon the knowledge of phrenology, comparing the result with that of the real or off-line make-up(OM) and identifying any possible difference between two methods: which contributes to understanding the potential and problems of DM at application level. This study aims at suggesting a method to apply computer graphics(CGs) to special effect make-up for aging effect through a case, contributing to attracting academic concerns and building theoretical frameworks for DM. It used 'Aging Process' in Adobe Photoshop CS4 Extended(CS4E) to change a young girl into a senior lady. In the process of DM, the study applies various tools and methods to making face wrinkled, finds the most effective way among them for each area of face, suggests a method to integrate the ways and shows the result of the method. Compared with OM, DM using CS4E is the less constrained work regarding procedure and time. Specifically, it can save the time tremendously because the reiterative operation of work can be omitted, when the same work is repeated, using 'Action' function which memorizes the history of the work. Once a DM work is produced, since it is durable, reusable and convertible to various images with additional operations, it seems very economical as well as highly promising to market the work through on-line sales. Regarding face features and skin, it reviewed literature in make-up and phrenology while it used Photoshop CS4 Extended tools to demonstrate a case of aging effect by combining and painting pictures of a young lady and a senior citizen.

A Study on Differences in the Caregiving Burden of Primary Caregivers by Type of Caregiving -Focused on Caregivers in Home Stay, Day Care Center, and Nursing Home Situations- (부양형태에 따른 주부양자의 부양부담의 차이 및 영향 변수에 관한 연구 -재가, 주간보호시설, 시설거주 노인의 주부양자를 대상으로-)

  • This study investigates differences by type of caregiving using data on three groups of caregivers, 177 from home stay situations, 189 from day care centers, and 138 from nursing homes. First, the result shows that characteristics of both caregivers and elders differ by type of caregiving. Second, caregivers in the home stay situation have the highest caregiving burden of the three groups. Third, from examination of the related variables, the caregiving burden of home stay caregivers is affected by family income, caregivers' health, type of job, and whether or not the elder has symptoms of dementia. Also, the study reveals that emotional services for elders reduce the aggravation of family relations and economic burden, but that instrumental services highly increase economic burden. It reveals that caregivers of elders in day care centers, especially those who are in bad health, are more likely to experience feelings of constriction, aggravation of family relations and economic burden. On the other hand, caregivers who receive more emotional services have better experiences in family relations, including relations with the elder. In case of the elders of nursing homes, if the main caregiver is a daughter-in-law, aggravation of family relations is higher than if the main caregiver is a spouse. Finally, the caregiver's burden is affected by their own health and income, and by whether the elder has symptoms of dementia or stroke.

Grandmother's Life Satisfaction and Influencing Factors by Grandparenting (손자녀 양육 여부에 따른 조모의 생활만족도 및 영향요인)

  • Purpose: The study was to compare the factors of life satisfaction between grandmothers parenting and not parenting their grandchildren. Methods: A descriptive design with a convenience sampling was used for this study. From communities in S and U city, 85 parenting grandmothers (PG) and 103 non-parenting grandmothers (NPG) were recruited. The PG were those grandmothers who cared for preschooler grandchildren for their daughter or daughter-in-law who was employed. Results: The demographics of the two groups were similar, however the value of grandparenting was significantly higher in the PG than the NPG (t=4.56, p<.001). In the PG, strong and significant correlations were found among the value of grandparenting, health status, quality of relations with adult children, and life satisfaction. However, demographics were more related to health status and life satisfaction in the NPG. The value of grandparenting, health status, and quality of grandmother-adult children relations were found to be significant factors of life satisfaction (F=20.75, p <.001) explaining 42% of the variances. In the NPG, only health status was a significant factor (F=50.66, p <.001) explaining 33% of the variances. Conclusion: This study shows that grandmothers' perceptions of grandparenting and family relationships need to be incorporated into the lifestyle in order to support parenting grandmothers' life satisfaction while grandmothers' health is the common concern of elderly women's life satisfaction.

Quantitative Analysis of Face Color according to Health Status of Four Constitution Types for Korean Elderly Male (고연령 한국 남성의 사상 체질별 건강 수준에 따른 안색의 정량적 분석)

  • In this paper, we performed a quantitative analysis of face color according to the health status of four constitution types. 205 Korean male in age from 65 to 80 were participated in this study and 85 subjects were finally selected for the analysis. Imaging process techniques were employed to extract feature variables associated with face color from a frontal facial image. Using the extracted feature variables, the correlations between face color and health status, face color and health status in each constitution type, and face color and four constitution types in heath status group were investigated. As the result, it was observed that the face color of healthy group contained more red component and less blue component than unhealthy group. For each constitution type, the face parts showing a significant difference according to health status were different. This is the first work which reports the correlation between the face color and health status of four constitution types with a objective method, and the numerical data for the face color according to the health status of four constitution types will be an objective standard to diagnose a patient's health status.

No Evidence of Association of Interleukin 1A (-889) Genetic Polymorphism with Alzheimer's Disease in Koreans

  • To examine whether the IL-1A (-889) polymorphism associates with a risk for Alzheimer's disease (AD) and acts interactively with the apolipoprotein (APOE) $\epsilon$4 in the development of AD, we performed genotype analyses of the IL-1A and the APOE of the 102 Korean AD patients and 200 Korean non-demented controls. We failed to detect a significant difference in genotypic and allelic frequencies of IL-1A between the AD group and control group. No overexpression of the IL-1A C/T genotype and IL-1A T allele was found when we analyzed the late-onset and early-onset patients, separately. There was no significant genetic interaction between IL-1A polymorphism and the APOE polymorphism. I n conclusion, the IL-1A polymorphism did not contribute to the development of AD independently or interactively with the APOE $\epsilon$4 allele in Koreans.

Comparative Interrupted Time Series Analysis of Medical Expenses in Patients with Intertrochanteric Fracture Who Underwent Internal Fixation and Hemiarthroplasty

  • Purpose: The objective of this study was to assess postoperative direct medical expenses and medical utilization of elderly patients who underwent either hemiarthroplasty (HA) or internal fixation (IF) for treatment of a femoral intertrochanteric fracture and to analyze differences according to surgical methods and age groups. Materials and Methods: Data from the 2011 to 2018 Korean National Health Insurance Review & Assessment Service database were used. Risk-set matching was performed for selection of controls representing patients with the same sex, age, and year of surgery. A comparative interrupted time series analysis was performed for evaluation of differences in medical expenses and utilization between the two groups. Results: A total of 10,405 patients who underwent IF surgery and 10,405 control patients who underwent HA surgery were included. Medical expenses were 18% lower in the IF group compared to the HA group during the first year after the fracture (difference-in-difference [DID] estimate ratio 0.82, 95% confidence interval [CI] 0.77-0.87, P<0.001), and 9% lower in the second year (DID estimate ratio 0.91, 95% CI 0.85-0.99, P=0.018). Length of hospital stay was significantly shorter in the IF group compared to the HA group during the first two years after time zero in the age ≥80 group. Conclusion: A noticeable increase in medical expenses was observed for patients who underwent HA for treatment of intertrochanteric fractures compared to those who underwent IF over a two-year period after surgery. Therefore, consideration of such findings is critical when designing healthcare policy support for management of intertrochanteric fractures.

Dorsal Short-Segment Fixation for Unstable Thoracolumbar Junction Fractures

  • Objective : This study is to evaluate the efficacy of dorsal short-segment fixation in unstable thoracolumbar junction fractures. Methods : The cases of 20 patients who underwent dorsal short-segment fixation were reviewed retrospectively. Clinical outcomes were analysed using Sonntag's pain level, work status, and neurological scale according to the modified Frankel classification. Radiological outcomes were analysed using Mumford's anterior body compression[%], canal compromise ratio, and Cobb's kyphotic angle. Results : At the latest clinical follow-up [average=14.6 months]. there were 19 [95.0%] in group I and 1 patient [5.0%] in II in pain level. The postoperative work status were 17 [85.0%] in group I, 2 patients [10.0%] in II, and 1 patient [5.0%] in V. Surgery brought to improve the neurologic status. In success group [19 cases, 95%], the average canal compromise ratio was reduced from 0.57 [${\pm}0.07$] to 0.05 [${\pm}0.08$] [P<005], the average anterior body compression [%] was reduced from 41% [${\pm}17$] to 18% [${\pm}14$] [P<0.05], and the average preoperative kyphotic angle was $20.0^{\circ}$ [${\pm}9.0$], and corrected to $5.7^{\circ}$ [${\pm}7.1$] postoperatively, and progressed to $7.8^{\circ}$ [${\pm}6.2$] at the latest follow-up. There was a case of implantation failure in an elderly osteoporotic patient. Conclusion : Although there are limitations in the patient number and follow-up period, the present study favors dorsal short-segment fixation for selective cases in unstable thoracolumbar junction fractures.
