가족구조의 변화가 성인이행기 발달에 미치는 영향 : 주관적 건강상태, 우울, 교육성취를 중심으로 (Long-term Effects of Change in Family Structure On Achievement During Transition to Adulthood : Focusing on the effect of parental divorce/death on health condition, depression and educational attainment)

  • 본 연구는 한국복지패널 1~2차년도를 활용하여 가족구조의 변화 여부가 성인이행기 신체적 발달, 우울, 교육성취에 미치는 영향을 확인하고 나아가 가족구조의 변화 시기에 따라 성인이행기 발달에 미치는 영향이 상이한지 살펴보았다. 연구 결과는 다음과 같다. 첫째, 0~18세 때 가족구조의 변화 여부는 성인이행기 우울, 교육성취에 부정적 영향을 미치지만 주관적 건강상태에는 유의미한 영향을 미치지 않는다. 둘째, 0~18세 때 가족구조의 변화 여부는 성인이행기 주관적 건강상태에 부정적 영향을 미치지 않지만, 세부 발달단계로 나누어 분석한 결과, 영유아기 때의 가족구조의 변화는 성인이행기 주관적 건강상태에 부정적 영향을 미친다. 셋째, 영유가기 때의 가족구조의 변화는 성인이행기 우울에 영향을 미치지만, 아동기와 청소년기 때의 가족구조의 변화는 유의미한 영향을 미치지 않는다. 넷째, 영유아기, 아동기, 청소년기 때의 가족구조의 변화는 모두 교육성취에 유의미한 부정적 영향을 미치며, 그 영향의 크기는 영유아기, 청소년기, 아동기 순이다. 이를 통해, 아동·청소년기 때의 가족구조의 변화가 성인이행기 발달에도 지속적으로 영향을 미치고 있으며, 가족구조 변화의 시기에 따라 영향을 미치는 발달영역과 강도가 다소 상이함을 알 수 있다. 특히, 주관적 건강상태와 우울과 관련하여 영유아기 때의 가족구조의 변화만 영향을 미쳤지만, 교육성취는 모든 발달단계가 유의미한 영향을 주며 특히, 영유아기, 청소년기, 아동기 순으로 그 영향정도가 큰 것으로 나타났다. 이는 가족구조의 변화가 일어난 시점의 자녀의 발달단계에 따라 영향을 받는 발달영역이 상이하기 때문에, 한부모 가족 자녀를 위한 정책 및 서비스의 종류는 달라야 함을 시사한다.

안면 홍조 증상을 수반한 중년 우울증 여성의 특성 (Characteristics of Middle Aged Depressed Women with Hot Flushes)

  • 연구목적 중년 우울증 여성은 안면 홍조 증상을 자주 경험한다. 우울증과 안면 홍조 간의 연관성은 아직 명확하지 않다. 따라서, 본 연구에서는 안면 홍조가 있는 우울증 여성과 안면 홍조가 없는 우울증 여성을 대상으로 호르몬 농도, 기능적 신체 증상, 폐경에 대한 태도 및 스트레스대처 방식을 비교하여 그 특성에 대해 알아보고자 하였다. 방 법 안면 홍조가 있는 우울증 여성 33명과 안면 홍조가 없는 우울증 여성 33명이 연구에 참여하였다. 안면 홍조 증상은 일주일에 걸쳐 전향적으로 일일 증상 척도를 통해 확인하였다. 연구 대상자의 스트레스 대처 방식은 스트레스 대처 방식 설문지를 통해 평가하였다. 신체 증상은 환자 건강 설문지(Patient Health Questionnaire-15, PHQ-15)로 평가하였다. 성 호르몬 농도는 방사성면역분석법으로 측정하였다. 간편 세계보건기구 삶의 질 평가도구(Brief World Health Organization Quality of Life Assessment Instrument, WHOQOL-BREF)를 이용하여 삶의 질을 평가하였다. 결 과 스트레스 대처 방식에 있어서, 안면 홍조가 있는 우울증 여성이 문제-중심의 대처 방식과($13.15{\pm}3.17$) 사회적 지지에 대한 탐색($11.83{\pm}2.84$) 항목이 안면 홍조 증상이 없는 우울증 여성에 비해 적었다(각각 $15.17{\pm}3.1$, p=0.028 ; $14.25{\pm}3.22$, p=0.009). 안면 홍조가 있는 우울증 여성이 폐경 후에 대하여 상대적으로 부정적 태도를 보였으나, 통계적 유의성은 근사치로 불충분하였다(p=0.059). 본 연구에서는 PHQ-15로 평가한 신체 증상과 성호르몬 농도에서 두 군 간의 차이를 발견하지 못 하였다. WHOQOL-BREF에 따른 사회적 대인 관계의 점수는 안면 홍조가 없는 군보다($9.71{\pm}1.65$ ; p=0.044) 안면 홍조가 있는 군에서($8.62{\pm}2.04$) 유의하게 낮았다. 결 론 우울증이 있는 중년 여성에서, 안면 홍조 증상은 스트레스에 대한 대처 방식의 영향을 받는 것으로 나타났다. 우울증 여성에서 안면 홍조 증상은 삶의 질에 부정적 영향을 주므로, 임상가는 안면 홍조가 있는 우울증 환자에서 적극적으로 안면 홍조에 대하여 치료적인 접근을 해야 할 것이다. 항우울제 혹은 소량의 호르몬 치료에 반응이 없는 우울증 환자에서, 인지 행동 치료 또는 스트레스 관리가 효과적일 수 있겠다. 추후 안면 홍조 및 우울증과 관련된 위험 요인에 대한 인과 관계를 명확하게 하기 위해서는 객관적인 방법을 통한 종적 연구가 필요할 것으로 생각된다.

말기암환자 가족 간병인의 간병 부담과 관련된 요인 (Factors Associated with Care Burden among Family Caregivers of Terminally Ill Cancer Patients)

  • 목적: 말기암환자 가족 간병인의 간병 부담을 줄이는 것은 가족뿐 아니라 환자를 위해서도 중요하다. 본 연구에서는 말기암환자 가족 간병인의 간병 부담과 관련된 요인에 대해 살펴보았다. 방법: 국내 7개 기관의 완화의료병동에서 입원치료를 받는 말기암환자의 가족 간병인 289명의 자료를 분석하였다. 간병인이 느끼는 주관적 간병 부담은 암환자 가족 돌봄 경험 평가도구를 사용하였고, 5가지 차원에서 단계적 변수 선정을 사용한 로지스틱 회귀 모형을 통해 유의한 인자를 확인하였다. 결과: 간병부담의 각 차원에서 다양한 인자들과의 관련성이 확인되었다. 감정적 요인은 가장 폭넓은 영향을 미쳤는데, 감정적 스트레스를 가진 군은 그렇지 않은 군에 비해, 생활패턴이 변할 가능성이 2.54배(95% confidence interval, 1.29~5.02), 가족의 협조가 부족할 가능성이 2.27배(1.04~4.97), 그리고 신체적 부담이 커질 가능성이 5.44배(2.50~11.88)였다. 가족기능은 가족의 협조부족을 매우 잘 반영하였으며, 심한 가족기능 장애를 보일 경우 경제적 부담과도 관련이 있었다. 종교를 가진 군과 동반질환이 없는 군에서 오히려 간병 부담이 더 높은 것으로 나타났으며, 간병기간과 하루 중 간병시간은 생활패턴의 변화와 신체적 부담을 유의하게 예측하였다. 직업을 가지고 있거나, 사회적 지지가 부족하거나, 자주 방문하지 못하는 가족 간병인은 낮은 자아 존중감을 보였다. 결론: 본 연구 결과에 의하면, 가족 간병인의 간병 부담을 파악하기 위해서는 그들의 정서상태와 가족기능을 파악하는 것이 도움이 되며, 사회적 지지체계를 포함하여 경제적 부담을 완화시키는 노력이 필요하겠다.

소아 99mTc-DMSA renal scan에서 방광차폐유무와 방사성동위원소 주입방법에 따른 콩팥섭취율 차이에 관한 연구 (Study on the Difference in Intake Rate by Kidney in Accordance with whether the Bladder is Shielded and Injection method in 99mTc-DMSA Renal Scan for Infants)

  • DMSA 방사성의약품은 몸쪽 세뇨관과 주위 콩팥겉질 세포에 섭취되어 콩팥 겉질의 평가 및 영상화에 유용하게 사용되는 의약품으로 신우신염의 진단에 매우 예민도가 높은 검사여서 소아에게 많이 사용되고 있는 검사이다. 소아에게 투여되는 방사성 동위원소의 양은 미량이 되고 소아의 신체가 Field of View (FOV)에 대부분이 포함이 되는 만큼 방광에 소변이 차있게 된다면 그만큼 콩팥을 영상화하는데 영향을 미치게 됨을 연구를 통해 확인하고자 하였다. 본 연구에서는 총 계수 설정법과 시간 설정법 중에 시간 설정법으로 연구를 진행하였다. 2015년 10월에서 12월까지 요로감염 및 신우신염이 의심되어 본원을 내원 및 입원하여 시행한 생후 1개월부터 12개월까지의 소아 34명을 대상으로 하였으며 환자에게는 동일한 선량 18.5 MBq (0.5 mCi)를 각각의 환자에게 동일한 양을 주입 후 2~3시간 후 검사를 진행하였다. 이때 사용된 장비는 Siemens사의 Symiba E (Siemens Medical solution USA, Inc.) 장비를 사용하였고 영상의 분석하기 위하여 Syngo MI Applications VA60C 소프트웨어를 사용하였다. 통계학적 분석은 IBM SPSS Statistics Ver. 21를 이용하여 분석하였으며 Paired t-test를 이용하여 비교 분석하였다. 검사는 한번의 검사에 7분의 시간으로 후면상을 획득하였으며 이후 자체 제작된 납을 이용하여 방광을 가린 후 추가로 동일한 시간으로 영상을 획득하였다. 영상 분석 시에 동일한 크기의 (가로 55.2 mm ${\times}$ 세로 70.0 mm)의 ROI (Region of Interest)를 설정하여 분석하였다. 콩팥의 계수는 (Lt. Kidney counts + Rt. Kidney counts) / Total counts의 백분율로 나타내어 계산하여 평가하였고. Background 수치는 같은 영상을 비교하기에 배제하고 연구를 진행하였다. 방광을 차폐시킨 후의 콩팥 계수는 $79.40{\pm}5.19%$ 방광을 차폐시키기 전의 콩팥 계수는 $70.87{\pm}3.18%$으로 나타났으며 (차폐시킨 후 - 차폐 전)의 콩팥 계수는 $8.52{\pm}3.29%$로 차폐시킨 후와 차폐시키기 전을 비교 분석하였을 때 유의한 것으로 나타났다. 주사 방법 중 3way stopcock를 이용하여 주사하였을 경우 차폐 후 콩팥 계수는 $78.10{\pm}4.61%$ 차폐 전 콩팥계수는 $68.92{\pm}2.80%$로 (차폐시킨 후 - 차폐 전)의 콩팥 계수는 $9.18{\pm}3.53%$로 나타났으며 Heparin cap을 이용하였을 경우 차폐 후 $79.84{\pm}3.26%$, 차폐 전 $71.33{\pm}5.14%$로 (차폐시킨 후 - 차폐 전)의 콩팥 계수는 $8.51{\pm}2.92%$로 나타났으며 마지막으로 직접 주사했을 경우 차폐 후 콩팥 계수는 $82.07{\pm}2.35%$, 차폐 전 콩팥 계수는 $75.11{\pm}4.30%$로 (차폐시킨 후 - 차폐 전)의 콩팥 계수는 $6.96{\pm}2.78%$로 세 가지 방법 모두 차폐시킨 후와 차폐시키기 전을 비교 분석하였을 때 유의한 것으로 나타났다. 그리고 직접 주사, Heparin cap, 3way stopcock 순의 콩팥 계수율을 보임을 확인 할 수 있었다. 소아의 Renal DMSA scan검사 시에 방광의 방사능을 제거하여 방광을 차폐하였을 때 차폐하지 않았을 때보다 개선된 콩팥섭취율을 보였고 소아의 경우에 혈관 확보에 어려움이 있지만 직접 주입하거나 환자의 몸에 근접하도록 방사성 동위원소를 주입한다면 더 나은 영상 획득에 도움이 될 것이다.

시맨틱 웹 자원의 랭킹을 위한 알고리즘: 클래스중심 접근방법 (A Ranking Algorithm for Semantic Web Resources: A Class-oriented Approach)

  • We frequently use search engines to find relevant information in the Web but still end up with too much information. In order to solve this problem of information overload, ranking algorithms have been applied to various domains. As more information will be available in the future, effectively and efficiently ranking search results will become more critical. In this paper, we propose a ranking algorithm for the Semantic Web resources, specifically RDF resources. Traditionally, the importance of a particular Web page is estimated based on the number of key words found in the page, which is subject to manipulation. In contrast, link analysis methods such as Google's PageRank capitalize on the information which is inherent in the link structure of the Web graph. PageRank considers a certain page highly important if it is referred to by many other pages. The degree of the importance also increases if the importance of the referring pages is high. Kleinberg's algorithm is another link-structure based ranking algorithm for Web pages. Unlike PageRank, Kleinberg's algorithm utilizes two kinds of scores: the authority score and the hub score. If a page has a high authority score, it is an authority on a given topic and many pages refer to it. A page with a high hub score links to many authoritative pages. As mentioned above, the link-structure based ranking method has been playing an essential role in World Wide Web(WWW), and nowadays, many people recognize the effectiveness and efficiency of it. On the other hand, as Resource Description Framework(RDF) data model forms the foundation of the Semantic Web, any information in the Semantic Web can be expressed with RDF graph, making the ranking algorithm for RDF knowledge bases greatly important. The RDF graph consists of nodes and directional links similar to the Web graph. As a result, the link-structure based ranking method seems to be highly applicable to ranking the Semantic Web resources. However, the information space of the Semantic Web is more complex than that of WWW. For instance, WWW can be considered as one huge class, i.e., a collection of Web pages, which has only a recursive property, i.e., a 'refers to' property corresponding to the hyperlinks. However, the Semantic Web encompasses various kinds of classes and properties, and consequently, ranking methods used in WWW should be modified to reflect the complexity of the information space in the Semantic Web. Previous research addressed the ranking problem of query results retrieved from RDF knowledge bases. Mukherjea and Bamba modified Kleinberg's algorithm in order to apply their algorithm to rank the Semantic Web resources. They defined the objectivity score and the subjectivity score of a resource, which correspond to the authority score and the hub score of Kleinberg's, respectively. They concentrated on the diversity of properties and introduced property weights to control the influence of a resource on another resource depending on the characteristic of the property linking the two resources. A node with a high objectivity score becomes the object of many RDF triples, and a node with a high subjectivity score becomes the subject of many RDF triples. They developed several kinds of Semantic Web systems in order to validate their technique and showed some experimental results verifying the applicability of their method to the Semantic Web. Despite their efforts, however, there remained some limitations which they reported in their paper. First, their algorithm is useful only when a Semantic Web system represents most of the knowledge pertaining to a certain domain. In other words, the ratio of links to nodes should be high, or overall resources should be described in detail, to a certain degree for their algorithm to properly work. Second, a Tightly-Knit Community(TKC) effect, the phenomenon that pages which are less important but yet densely connected have higher scores than the ones that are more important but sparsely connected, remains as problematic. Third, a resource may have a high score, not because it is actually important, but simply because it is very common and as a consequence it has many links pointing to it. In this paper, we examine such ranking problems from a novel perspective and propose a new algorithm which can solve the problems under the previous studies. Our proposed method is based on a class-oriented approach. In contrast to the predicate-oriented approach entertained by the previous research, a user, under our approach, determines the weights of a property by comparing its relative significance to the other properties when evaluating the importance of resources in a specific class. This approach stems from the idea that most queries are supposed to find resources belonging to the same class in the Semantic Web, which consists of many heterogeneous classes in RDF Schema. This approach closely reflects the way that people, in the real world, evaluate something, and will turn out to be superior to the predicate-oriented approach for the Semantic Web. Our proposed algorithm can resolve the TKC(Tightly Knit Community) effect, and further can shed lights on other limitations posed by the previous research. In addition, we propose two ways to incorporate data-type properties which have not been employed even in the case when they have some significance on the resource importance. We designed an experiment to show the effectiveness of our proposed algorithm and the validity of ranking results, which was not tried ever in previous research. We also conducted a comprehensive mathematical analysis, which was overlooked in previous research. The mathematical analysis enabled us to simplify the calculation procedure. Finally, we summarize our experimental results and discuss further research issues.

High Resolution HC$_3$N Observations toward the Central Region of Sagittarius B2

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70${\pm}$1.32mmHg/min)compared to CF dialyzers(4.32${\pm}$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. Optimal parameters of electrical stimulation for slow muscles were 20 Hz, 0.2 ms square pulse. For fast muscle, 40 Hz, 0.3 ms square pulse was optimal to produce repeated contraction.
