Genetic factors are important in the pathogenesis of coronary artery disease (CAD). Angiotensin converting enzyme (ACE) gene insertion(I)/deletion(D) polymorphism is one of the genetic factor found to be related with CAD. We investigated the association between I/D polymorphism of the ACE gene and the presence of CAD. Threehundred and seven patients (187 males and 120 females, aged between 35-80, mean $54.3{\pm}9.8$ years) who underwent diagnostic coronary angiography were included in the study. ACE I/D polymorphism was detected by polymerase chain reaction. Of the 307, 176 had CAD. The most frequently observed genotype in all subjects was ID (47.9 %). However, in patients with CAD the frequency of II genotype was lower whereas DD genotype was higher compared to the controls (p < 0.05). The number of D allele carrying subjects were also higher (p < 0.05) in CAD patients. The logistic regression analysis indicated that the ACE D allele is an independent risk factor (odds ratio = 1.48, 95% CI = 1.01-2.18, p < 0.05). In conclusion, the I/D polymorphism of ACE gene (carrying D allele) is an independent risk factor for CAD in the studied Turkish population.
The purposed of this study was to identify cardiocerebrovascular disease risk assessment and the factors that affect cardiocerebrovascular disease risk. The subjects of this study were 1340 middle age women worker in one shipyard. This study was base on worker health check up data. Data were analyzed using SPSS18.0 program. "healthy group" were 1078(80.4%), "low risk group" were 45(3.4%), "moderate risk group" were 150(11.2%), "high risk group" were 67(5.0%). Age, HDL, LDL, smoking, family history of stroke, hypertension were the factors that affected the risk cardiocerebrovascular disease. Results of this study can be used as fundamental data for developing cardiocerebrovascular disease preventive program in middle aged women.
Background: Losing balance during locomotive actions becomes an increasing threat to both the community-dwelling elderly and elderly with Parkinson disease (PD). Those with PD may be at a high risk of fall due to particular characteristics during the turn. Turning around during locomotive actions may be one of problematic factors causing losing balance. Objects: This study is part of a larger study, which in part aims to identify turning strategies, to compare the strategies in the elderly with and without idiopathic PD aged 51 years and older and to distinguish whether the turning strategies can predict the elderly at risk of falls. Methods: A total of 22 community-dwelling elderlies (10 elderlies with idiopathic PD and 12 healthy elderlies) were investigated for the turning strategies during the timed up and go test. Results: There were some significant differences between the two groups during turning (p<.05). The idiopathic PD group had a tendency of challenging on taking more number of steps, more time to accomplish and staggering more for the turn relative to the control group. Conclusion: Taking more number of steps and more time to turn may be useful for distinguishing the characteristics of PD from that of the healthy elderly in turning strategy.
Background/Aims: The aim of this study was to assess the association between the number of existing permanent teeth and chronic kidney disease (CKD) in a representative sample of the elderly Korean population. Methods: A total of 2,519 subjects who participated in the Korean National Health and Nutrition Examination Survey were cross-sectionally examined. The number of existing permanent teeth was evaluated by clinical oral examination. CKD was defined based on definition and classification by Kidney Disease: Improving Global Outcomes (KDIGO) 2012 guidelines. Multivariable logistic regression analyses were performed controlling for age, gender, income, education, tooth-brushing frequency, periodontitis, state of dentition, smoking, alcohol consumption, hypertension, obesity, diabetes mellitus, and hypercholesterolemia. Subgroup analyses by age and gender were also performed. Results: The number of teeth was significantly associated with CKD after controlling for all potential confounders (adjusted odds ratio [AOR], 1.67; 95% confidence interval [CI], 1.04 to 2.70 for lower number of teeth; AOR, 1.59; 95% CI, 1.14 to 2.23 for moderate number of teeth). In the subgroup analyses, the association was highlighted in females aged 75 years over (AOR, 2.55; 95% CI, 1.05 to 6.20 for lower number of teeth; AOR, 1.95; 95% CI, 1.01 to 3.80 for moderate number of teeth). Conclusions: Our findings suggest that the number of existing permanent teeth may be associated with CKD among Korean elderly.

Purpose: The purpose of this study was to investigate factors of sleep disturbance and sleep patterns of elderly hospitalized women patients between young-old (65-74 years), old (76-84 years) and old-old (85 years and above). Methods: The questionnaire included the environmental disturbance factors (Paik, 2000), degree of pain (Wang & Kim,1995), disease symptoms (Paik, 2000), depression (Kee, 1996) and sleep patterns (Oh, Song, & Kim, 1998). Data were analyzed using frequencies, means, ${\chi}^2$-test, t-test, ANOVA, Scheffe, Pearson correlation coefficients and multiple regression (SPSS 14.0). Results: The middle, and oldest group's environmental disturbance factors of degree of pain, disease symptoms, depression and sleep patterns were higher than those of the younger group. The younger, middle, and oldest group's sleep pattern had a significant negative correlation with environmental disturbance factors, degree of pain, disease symptoms and depression. The model including variables related to environmental, physical and psychological disturbance factors, explained the following variances in sleep pattern: 26.8% for the youngest group, 27.6% for the middle group and 40.7% for the oldest group. Conclusion: The result of this study offer basic data for the development of nursing intervention programs to improve sleep patterns for hospitalized women patients according to age differences.
Purpose: The purpose of this study was to determine factors related to regular walking exercise in the elderly using KNHNES data. Method: The study subjects, a total of 1437 adults over age 65, were classified into two groups based on their type of walking exercise: regular walking exercise group or irregular walking exercise group. The study variables were measured using scale physiological function, mental health status, chronic disease and quality of life. Results: The data was analyzed by $x^2$-test, t-test, and logistic regression using the SPSS 18.0 program. The factors related to regular walking exercise were male (OR=0.628, 95%CI=0.503-0.783), subjective health state (OR=0.832, 95%CI=0.751-0.921), thoughts of suicide (OR=0.753, 95%CI=0.586-0.967), hypoHDL-cholesterolemia (OR=0.742, 95%CI=0.591-0.930), and stroke (OR=0.604, 95%CI=0.366-0.997). Conclusion: The findings indicate that regular walking exercise in the elderly is related to physiological function, mental health status, and chronic disease. Therefore, providing for the education and awareness of health promotion for the elderly will may improve the quality of life in old age.
This study was to suggest the roles of social enterprises in maintaining healthy life of the socially vulnerable by creating a customized care environment through chronic disease diet management targeting the elderly at home in need of care in the community. As for the subjects of this study, 102 in-home elderly people aged 65 or older (14 males & 88 females) who needed care in the community were provided with a diet management lunch box delivery service customized for chronic diseases. Theoretical education and cooking class on chronic diseases were provided to 15 volunteers with 10 sessions, and customized lunch boxes were provided to the elderly with chronic diseases at home on that day. In conclusion, it is believed that only by increasing the level of knowledge about chronic diseases and nutritional knowledge of the elderly will it be possible to establish a proper meal plan. Since the role of volunteers is important, it is necessary to systematically seek nutritional education methods for volunteers.
In this study, we investigate the emergence of bullous pemphigoid (BP) after the administration of the Severe Acute Respiratory Syndrome-Coronavirus Disease 2019 (SARSCOVID-19) vaccine. The study presents two cases of women, aged 47 and 53, diagnosed with BP following SARS-COVID-19 vaccination. BP is a common autoimmune blistering disorder prevalent among older populations, with an incidence rate ranging from 2 to 40 cases per million individuals. This condition arises when autoantibodies target adhesive proteins in the skin, resulting in blister formation and mucosal erosion. Drug-induced bullous pemphigoid (DIBP) shares similarities with the classic form of BP but may be influenced by medication usage. Notably, DIBP exhibits distinct characteristics, such as affecting a younger demographic and involving mucosal regions more prominently than classic BP. The growing incidence of BP is linked to factors such as an aging population and the rise of drug-induced cases. This case report provides valuable insights into comprehending DIBP, elucidating post-vaccination discomforts, particularly those related to oral lesions and the exacerbation of existing lesions. By elucidating these aspects, we aim to advance the understanding of DIBP within the medical community.
Purpose: The purpose of this study is to document the experiences of elderly persons with chronic diseases who are under hospital-based home care services and to find out how they describe the experiences using a narrative inquiry method. Methods: The participants are 7 elderly patients over the age of 70. Data collection and analysis were conducted through Jeong Gwang Soon's six-stages: observing phenomena, selecting participants, talking, talking again, writing, and confirming. Results: The results of data analysis were classified into 4 themes of overall experience related to chronic diseases: response to home health care, overcoming chronic disease, life reconstruction, and 22 categories as common denominators extracted from the elderly persons' lives. Conclusion: This research makes the experiences of the elderly living with chronic diseases more understandable, and it can be utilized as a effective nursing praxis to improve the quality of elderly persons with chronic diseases in hospital-based home care services.
Background: Young onset breast cancer (BC) has a worse outcome as compared to in the elderly. However, some studies have shown that BC in the elderly, despite indolent features, does also cause increase in mortality. In an attempt to compare clinic-pathological characteristics, BC subtypes and survival in patients with BC presenting at extremes of age, we performed a retrospective study. Materials and Methods: Patients were either ${\leq}40$ or ${\geq}70$ years old. Subtypes were defined using immunohistochemistry and histological grade. Chi-Square test was used for evaluation of categorical variables, and Kaplan-meier and log-rank for disease-specific survival (DSS) and disease free survival (DFS). Results: We analyzed 256 patients ${\leq}40$ and 366 patients ${\geq}70$. Younger patients presented with more aggressive disease, with less luminal A but more luminal B and triple negative (TN) subtype. With a median follow-up of 57.5 months, DFS at 5 years in younger patients was 72.3% vs 84.6% in the elderly (p=0.007). Luminal A and B disease presented with worse DFS in younger patients. The opposite was seen in the TN subgroup. Although we found no significant differences in DSS, older patients with TN tumors died of BC more frequently. This group also received less chemotherapy. Conclusions: Young patients present with more aggressive disease, this translating into worse DFS. However, elderly patients with TN disease represent a particular subpopulation with worse DFS and DSS, suggesting that chemotherapy should not be withheld only because of age.
