A Seven Cases of Treating Atrophic Acne Scar with Subcision, Microneedle Therapy System and Herbal Peeling Therapy (절개침과 미세피부침, 약초필링을 이용한 위축성 여드름 흉터 치료 7례 임상보고)

  • Objectives : The purpose of this study is to present the effect of subcision, microneedle therapy system and herbal peeling therapy which is treatment of Korean medicine to atrophic acne scar patients. Methods : We administered subcision, microneedle therapy system and herbal peeling therapy to atrophic acne scar patients and tested the results by Qualitative Global Acne Scarring Grading System and Korean Acne Grading System as well. Results : After the treatment, all of the patients showed improvements in Qualitative Global Acne Scarring Grading System and Korean Acne Grading System as well. Qualitative Global Acne Scarring Grading System of patients changed from grade 2.71±0.71 to grade 1.14±0.86 along with the change of Korean Acne Grading System from grade 1.57±0.57 to grade 1.00±0.00. Conclusions : Subcision, microneedle therapy system and herbal peeling therapy may be effective in the treatment of moderate to severe acne scars, and could be applied in clinical practice.

How Many Doctors and Paramedics Does Fire Service Need for Medical Direction in Korea? (소방 구급활동에 필요한 지도의사와 1급응급구조사의 수는?)

  • Purpose : It was to improve medical direction system through presenting need of doctor and paramedic in Korean Fire Service. Methods : This study was conducted by applying demand coefficients(4 for doctor, 3 or 4.5 for paramedic) to some data on medical director, paramedic, ambulance from National Emergency Management Agency. Results : Number of medical director & paramedic were 4 & 1,217. Number of necessary doctor for medical direction was 64 or 28(in case of direct medical direction) & 16 or 7(in case of indirect medical direction). Number of necessary paramedic for direct medical direction was 492(in case of 35% ALS ambulance) & 1,062(in case of 50% ALS ambulance). Conclusions : To improve quality & efficiency of medical direction brought up need of amendment of the Emergency Medical Services Act to apply indirect medical control such as standing orders, protocol, case review.

The Effect of Physical Aspects of Quality Improvement in Medical Services on Premature Infants' Survival Rate (물리적 의료서비스 품질 개선이 미숙아 생존율에 미치는 영향)

  • This paper on an experiment, using System Dynamics, on the affect of increase in number of beds and medical instruments used for the care of premature infants, which constitute the physical requirements in quality of medical services, on changes in the survival rate of premature in ants that leads to demographic changes of Newborn infants. The model has four sectors: take-in capacity, survival rate of premature infants, demographics without newborn infants and demographics with newborn infants. The model simulates the changes in demographics of the newborn infants from 2002 to 2022. The study results show that the survival rate of premature infants can be increased by improving the physical aspects in the quality of medical services. An average of 1,900 premature infants can survive as a result of the physical quality improvements in medical services, adding up to an increase of 37,300 newborn infants by the year 2022.

  • World practice is evidence of that computer systems of an intellectual support of medical activities bound up with examination of patients, their diagnosis, therapy and so on are the most effective means for attainment of a high level of physician\`s qualification. Such systems must contain large knowledge bases consistent with the modern level of science and practice. To from large knowledge bases for such systems it is necessary to have a medical ontology model reflecting contemporary notions of medicine. This paper presents a description of an observation ontology, knowledge base for the physician of general tipe, architecture, functions and implementation of problem independent shell of the system for intellectual supporting patient examination and mathematical model of the dialog. The system can be used by the following specialist: therapeutist, surgeon, gynecologist, urologist, otolaryngologist, ophthalmologist, endocrinologist, neuropathologist and immunologist. The system supports a high level of examination of patients, delivers doctors from routine work upon filling in case records and also automatically forms a computer archives of case records. The archives can be used for any statistical data processing, for producing accounts and also for debugging of knowledge bases of expert systems. Besides that, the system can be used for rise of medical education level of students, doctors in internship, staff physicians and postgraduate students.

Recognition of Medical Doctors Employed to University Hospital on the Cooperative Medical System between Korean Traditional and Western Medicines (의과대학부속병원 의사들의 양.한방 협진에 대한 인식)

  • Objectives : This study was aimed to promote the cooperative system of Korean and Western medicine in the dual health care system through a survey of physicians on recognition, problems and solution of the cooperative system. Methods : The research took place at Dongguk University Hospital from May 25 to 27, 2009 with 44 professors, residents, and interns employed by the hospital. Results : Of total 44 surveyed doctors, positive and moderate responses on the cooperative system between Korean and Western medicines were 40.9% and 43.2%, respectively. They scored it positive (62.5%) and moderate (31.3%) based on their experiences. These results can be supposed to represent the environment for the interdisciplinary medicine. Even in the interdisciplinary hospital of Korean and Western medicine, 68.2% of responders had no experience of the cooperative medical system. Expected interdisciplinary efficient departments were ordered rehabilitation medicine, neurology and orthopedics and associated with musculoskeletal disorders, the most frequent diseases treated by Korean traditional medicine. Conclusions : Korean and Western medical doctors, as medical personnel, intellectual persons as well as specialists, need a recognition and attitude to understand and respect each others' medicine. However, both groups also realize there are many complicated issues in the treatment of patients. It is difficult to require a change of Western doctors' perceptions and attitude toward Korean traditional medicine only with results of a survey. For the efficient cooperative system in the medical field, Korean medical doctors will need to study and consider specific problems mentioned by Western medicine.

A Study on Development of the Sharing System of Foreign Medical Journal in Medical Libraries in Korea (의학도서관 간 외국학술잡지 공동이용의 발전방안)

  • Korean medical libraries have made interlibrary loan active under the korean medical library association since 1969, the sharing acquisition of journals has not been peformed efficiently in the absence of a bibliographic control institute. The medical library of seoul national medical university which is designated to a medical professional information center by korea education academy information developed MEDLIS linking the union catalog and interlibrary loan system and has been operating it in cooperation with the korean medical library association. Two types of questionaries were sent to 350 medical researchers and 160 medical librarians to investigate the realities of domestic and foreign medical information institutes, develop sharing systems and find a solution for sharing systems. Also, some analyses for the recent state of duplicate subscriptions, interlibrary loan, demands for system operations un the basis of the statistics of MEDLIS users were made. The result showed that medical researchers and medical librarians are affirmative for MEDLIS, users need a convenient interlibrary loan system and medical librarians agree to activate interlibrary loan. It also showed that sharing acquisition of journals seemed to be realizable for medical librarians, although there was no intention to improve the centralization of interlibrary loan among them. From the findings, a plan for sharing acquisition of journals established by the sharing acquisition committee under korea medical libraries society and national 8 local centers to play a key role in it was made. Also, a proposal for the establishment of an information center for controlling korean medical bibliographies was suggested. This system will be expected to contribute to make users to acquire materials more effectively and save the national budget.

  • Current Korean medical system is evaluated as inefficient and the government is planning a new medical development plan to provide guaranteed life-long medical service and more efficient medical system Korean medicine also needs to participate as the primary medical provider and strengthen public recognition. The needs for active participation are essential to prevent irrational medical policies and Korean medicine can exercise medical merits. When the doctors of Korean medicine are recognized as the primary medical provider, not only social rights are served but also provide medical service to less privileged sector of the society. This calls for the establishment of system for attending physician and public health doctor from Korean medicine. Another important issue is the establishment of public medical service at the government level. Doctors of Korean medicine should be posted at the national health center and other public hospitals. Revision of the current policies on Korean medicine must be made to yield concrete outcome of the public Korean medicine service as well as cooperation between the conventional allopathic medical community and the Korean medical community

  • Multiple system atrophy (MSA) is a sporadic progressive neurodegenerative disorder characterized clinically by various combinations of parkinsonian, autonomic, cerebellar, or pyramidal signs and pathologically by cell loss, gliosis, and ${\alpha}$-synuclein-positive glial cytoplasmic inclusions in several brain and spinal cord structures. This is a clinical report about a 69-year-old female who had MSA treated by oriental medical treatment and evaluated by Unified Multiple System Atrophy Rating Scale (UMSARS). The patient was treated with herb medicine Chungsimyeonj-aeumgami(淸心蓮子飮加味), acupuncture, moxibustion and cupping. After treatment, the patient's symptoms improved meaningfully and the score decreased in UMSARS Part I, II. This suggests that oriental medical treatment could be effective to improve MSA patients' symptoms. It is necessary to have more observations and many cases of patients with MSA.

  • This paper proposes a smart glass-based emergency tele-medical direction system. This system is designed for hospital specialists to provide remote medical guidance to on-site coast guards or emergency responders. To identify the requirements necessary for system development, relevant technological trends and case studies were analyzed. Based on this analysis, three system requirements were defined: 1) The system must be able to determine the necessity of patient transport, 2) It should assist in providing emergency medical care during transport to the hospital, and 3) It must be capable of transmitting patient information to medical facilities. A prototype that meets these requirements was developed and its usability was evaluated.
