The Clinical Study of Urticaria (두드러기 치료 조사)

  • This study was clinically performed with the 73 outpatients of urticaria who had visited the Dept. of Dermartology, Oriental Medical Hospital, Dong-eui University from January 1st 2000 to June 30th 2003. They have all taken the oriental herb medicine as Sungmagalguntanggamibang(SGT, 升麻葛根湯加味方) more than 1 time. Urticaria is a common disease and a well known reaction pattern of the skin charaterized by erythema, wheals and edema, that is the results of various causes. Urticaria is classified into acute or chronic type. The etilogy of acute uricaria has been determined in many cases, but the cause of chronic urticaria has been determined in 10$\%$ to 20$\%$ of cases. Character of chronic type urticaria is recurrent pruritus and erythematous skin disease by stimulating factor. Through checking up the present patient state, we verified the effect of SGT in various ways. The results were summarized as follows; 1. An increase of taking a medicine(SGT) and attending a hospital in number let down repeated urticaria occurence. 2. SGT is more effective when the symptoms with the severe pruritic wheals are spread on the whole body in daily occurrence. 3. In urticaria treatment method, gradually reducing a western medicine and going side by side SGT, that is good at symptoms improvement. 4. Etiology of urticaria is classified into intestine-stomach accumulated heat by indigestion(食滯腸胃積熱), wind-moisture(風濕), wind-heat(風熱) etc. The SGT is available in the order of frequency of use.

A Clinical Case Report of Oculomotor Nerve Palsy (동안신경마비 사시환자 1례 증례보고)

  • Oculomotor nerve palsy presents itself with sudden onset unilateral ptosis and inability to turn the eye upward, downward, or inward, which causes visual disturbances. Strabismus caused oculomotor nerve palsy refers to muscle imbalance that results in improper alignment of the visual axes of the two eyes It may be divided into paralytic and non paralytic strabismus. paralytic strabismus is primarily a neurologic problem. Characteristic clinical disturbances result from lesions of the third, fourth, and sixth cranial nerves. Lesions of the third nerve result in a paralysis of lateral or outward movement and a crossing of the visual axes. Objective: This study was designed to evaluate the effects of oriental medicine therapy on a peripheral oculomotor nerve palsy. Methods & Result: The clinical data was analyzed on a patient with oculomotor nerve palsy whose main symptoms were right side ptosis and inability to turn the eye inward. The patient was treated by the electroacupucture(4Hz frequency, intensity was adjusted so that localized muscle contractions could be seen). As the result, symptoms are improved remarkably. Conclusion: The patient showed right eye ptosis and unilateral deviation of the right eye ball in neutral position. After acupuncture treatment and electroacupuncture treatment, the ptosis and deviation of the patient's right eye was recovered six weeks after the onset. The study suggests that oriental medicine therapy is significantly effective on the treatment of peripheral oculomotor nerve palsy.

Lonicera Japonioa Suppresses the Mast Cell-Mediated Immediate Allergic Reaction

  • The flowers of Lonicera japonica Thunb. (Caprifoliaceae) have been used as a traditional effective drug in treating bacillary dysentery, purulence. However, the exact role of Lonicera japonica in allergic reaction has not been clarified yet. Immediate hypersensitivity, popularly known as allergy, is a major clinical problem in humans. It has been found that the histamine release from mast cells is an essential step in the pathological process of immediate hypersensitivity. In this study, the effect of aqueous extract of Lonicera japonica (AELJ) on allergic reaction was investigated. AELJ inhibited the compound 48/80-induced anaphylactic reactions and antigen-induced passive cutaneous anaphylaxis (PCA). AELJ in vitro exhibited a dose-dependent inhibition of degranulation in RPMC stimulated by compound 48/80. AELJ also suppressed the morphological changes and the increase of intracellular free calcium level induced by compound 48/80. These results suggest that inhibitory effect of AELJ on allergic reaction may be mediated through the decrease of intracellular free calcium levels, and AELJ importantly contributes to the treatment of anaphylaxis and may be useful for other allergic disease.

  • Backgrounds : sudden sensorineural hearing loss is defined as a disease that has abrupt onset of sensorineural hearing loss without definite cause. In some cases, this disease cause tinnitus, vertigo, headache addition to hearing loss. In traditional oriental medicine, sudden sensorineural hearing loss is could be diagnosed as deficiency of Yang-Gi(陽氣). Objects & Methods : This study is clinical observation about three sudden sensorineural hearing loss cases. For diagnosing in these cases, pure tone audiometry was used. For treatment, acupuncture, herbal-medicine and moxibustion therapy were used. Results & Conclusions : as follows. 1. Generally, in this disease, the shorter the whole period from the time that occurs to the time that starts to treat is, the more the results are better. The research shows that short-term case is more efficient than the other long-term two cases in the aspects of symptoms and pure tone audiometry . 2. In making a diagnosis of sudden sensorineural hearing loss and measuring the effect, subjective symptoms generally correspond to the improvement of pure tone audiometry, but not completely. Therefore, in treating patients, the doctor must consider these both aspects and keep the balance.

The Relationship between Early-onset Androgenetic Alopecia and Metabolic Syndrome: Systematic Review and Meta-Analysis (조발성 탈모증과 대사증후군과의 관계: 체계적 문헌고찰 및 메타분석)

  • Objectives : The study was done to verify the relation between early-onset androgenetic alopecia(AGA) and metabolic syndrome(MetS). Methods : Data were collected through electronic database including KoreaMed, National Assembly Library, KMBASE, NDSL, KCI, KERIS, Google Scholar, Pubmed, Cochrane CENTRAL and EBSCO MEDLINE. A total of 13 case-control studies related to the MetS of early-onset alopecia patients were used for the systematic review and meta-analysis. Risk of bias of included studies were assessed by RoBANS tool. RevMan5.3, CMA3 were used for the meta-analysis. Results : In 13 evaluated articles, most frequent bias was the participant selection bias that was found in 10 articles. Significant association between early-onset AGA and MetS was found in 10(76.9%) out of 13 articles in the systematic review. In meta-analysis, early-onset male AGA was associated with increased risk of metabolic syndrome(OR: 3.73, 95% CI:2.49 -5.61). Conclusions : AGA, particularly early -onset male AGA, is significantly associated with MetS. Therefore all patients with early onset male AGA should be suggested to take preventive treatment to reduce the risk of MetS and various problems associated with it.

A Case of Macular Edema Resulted from Branch Retinal Vein Occlusion (망막분지정맥폐쇄로 유발된 황반부종 치험 1례)

  • Objective : This study was performed to prove the effect of the oriental medical treatments like acupuncture, moxibustion, herbal medicine on macular edema resulted from branch retinal vein occlusion which could lead to terrible result like permanent visual loss at worst. Methods : The patient was treated 62 times, from July 15th, 2009 through May 12th, 2010. We used fundus photography and Dr. Hann's standard test chart for 5m to examinate the patient's eyesight. We used acupuncture, electropuncture, moxibustion and herbal medicine to treat the patient. We considered visual field, scotoma, metamorphopsia and vitreous floater to measure the effect of treatment. Results : After about 10 months of treatment, the eyesight of Rt eye was improved from 0.8 to 0.9, Lt. eye from zero to 0.3. The visual field turbidity were absolutely cleared. The scotomas in Lt. eye were reduced from 5 to 2. The metamorphopsia were improved. The vitreous floaters in Rt. eye were decreased from 5~6 to 1~2. Conclusion : These results show that the oriental medical treatment can be conservative measure to the macular edema resulted from branch retinal vein occlusion.

Clinical Efficacy of External Application Containing Betula platyphyllae Cortex, Viticis Fructus, Agrimoniae herba in Atopic Dermatitis (아토피 피부염에 대한 화피(樺皮), 만형자(蔓荊子), 선학초(仙鶴草) 함유 외용제의 임상 효과)

  • Objectives : The aim of this study is to examine the effect of external application containing herbal extracts (Betula platyphyllae Cortex, Viticis Fructus, Agrimoniae herba) to patients with atopic dermatitis. Methods : Twenty patients (experiment group) were treated with the external application containing herbal extracts (Betula platyphyllae Cortex, Viticis Fructus, Agrimoniae herba) and other twenty patients (control group) were treated with normal external application that doesn't contain herbal extracts for 2weeks. We observed comparisons between experiment and control group after 1week, 2weeks. Degrees of severity of atopic dermatitis were measured by SCORAD index (Scoring of Atopic Dermatitis Index). Statistical significance was achieved if the probability was less than 5% (p<0.05). Results : 1. After 1week of external application treatment, SCORAD Index in experiment group was significantly decreased comparing with control group, which is a marginally significant result between two groups. But after 2weeks the experiment group showed no statistically significant result. 2. After 1week of external application treatment, Intensity Criteria (Erythema, Edema/papulation, Total) in experiment group was significantly decreased comparing with control group, which is a statistically significant result between two groups. But after 2weeks the experiment group showed no statistically significant result. 3. After 1week and 2weeks of external application treatment, comparison of changes in Subjective Symptoms (Pruritus, Insomnia) indicated that the experiment group showed no statistically significant result. 4. After 1week of external application treatment, Total Quality of life through Skindex-29 in experiment group was significantly decreased comparing with control group, which is a marginally significant result between two groups. But after 2weeks the experiment group showed no statistically significant result. Conclusions : As a result of applying the external application containing herbal extracts to patients with atopic dermatitis and watching the progress, we can speculate that the external application containing herbal extracts has especially short-term therapeutical effects in mitigating the symptoms of Atopic Dermatitis. Thus, it is concluded and considered that the external application containing herbal extracts can be used by atopic patients effectively with almost no side-effect.

A Clinical Study of Acupuncture Effect on Children's Myopia (소아근시의 침치료 효과에 대한 임상적 연구)

  • Objectives This study was performed to evaluate the clinical effect of acupuncture on children's myopia. Methods Among the outpatients with myopia who visited to Department of Acupuncture & Moxibustion, National Medical Center from June 2000 to May 2001, we selected 50 cases that did not involve ,any other pathological condition of eyes and patient age was below 17, whose eyes including with glasses or naked examined by monoocular and biocular visual acuity test per each visit and followed up for 6 weeks. Results We investigated 29 female and 21 male patients. The most distribution of age was 12.26. First mean biocular visual acutity was $0.2{\pm}0.074$. Last mean biocular visual acuity after 6 weeks was $0.8{\pm}0.046(p<0.001). Improved cases on biocular test were 38 cases($76.00\%$). Conclusions These results suggest that the acupuncture therapy is effective treatment modality on children's myopia.

The Anti-Inflammatory Effects of NeiShuHuangLianTang In Experi-mental Animals (內疎黃連湯의 消炎效果에 관한 實驗的 硏究)

  • Objective: Under recognition of the similarity between carbuncle in Oriental medicine and inflammatory disease, NeiShuHuangLianJang(N.H.) has been used as an antiphlogistic agent. The present reports shows the anti-inflammatory effects of N.H. Method: Experimental animals made use of 4-5 weeks age(weight 20-25g) ICR(male)mouse. They were farmed individually in a temperature($22{\pm}0.5{\circ}$) and light(06:00 to 18:00 h) controlled room with free access to water and food. The N.H.(1.0g/kg, 3.0g/kg) extracted from NeiShuHuangLianTang were administered intragastrically prior to LPS I.P injection. we measured WBC count, IL-6 level in plasma and TNF-${\alpha}$ level in plasma. Result : 1. N.H. suppressed inflammatory reaction induced by LPS. 2. N.H. suppressed WBC count in inflammatory reaction induced by LPS. 3. N.H. suppressed 1L-6 level in inflammatory reaction induced by LPS. According to above experiments, N.H. was improved its suppression effect against the inflammatory reaction through WBC count and IL-6 level. So N.H. which is used for the inflammatory disease(carbuncle) in Oriental Medicine is assumed to have a curative effects against this disease by controlling WBC count and IL-6 level in plasma.

  • Objectives : The purpose of this study is to consider the definition, history, classification and clinical effect of balneotherapy. Methods : We researched oriental documents and recent sources of balneotherapy. Results and Conclusions : 1. Balneotherapy is medical cure method which uses physical chemistry effects of water. 2. Records of balneotherapy are seen in oriental documents such as Hwangjenaegyeong(黃帝內經), Sinnongbonchogyeong(神農本草經), Jaebyoungwonhuron(諸病源候論), Youmoonsachin(儒門事親), Bonchogangmok(本草綱目) and Donguibogam(東醫寶鑑) etc. 3. Spa can classify eleven types according to ingredient. The types were as following. Simple thermal spirng, Common salt spring, Sodium bicarbonated spring, Sulfate spring, Radioactive spring, Acid spring, Sulfur spring, Carbon dioxide spring, Iron spring, Alum spring & Sulfate-iron spring, Bicarbonatealkaline spring. 4. Physical, chemical and environmental effect of spa therapy have effectiveness on the chronic and we akness disease more than acute disease. And balneotherapy have more effectiveness on digestive disease, pulmonary disease, metabolic disease, circulatory disease, muscle skeletal disease and dermatologic disease than any other diseases.
