A Clinical Study of the Treatment Prescribed Kalkunseunggi-tang for Rectal Tenesmus and Constipation Patient (이급후중(裏急後重)을 동반한 태음인(太陰人) 편비(便秘)환자 치험 1례)

  • 1. Objectives Constipation is a common symptom affecting 16.5% of general population in Korean countries. However, many people are looking for constipation due to the hospital. The clinical studies in herbal medicine to cure rectal tenesmus and constipation caused by the stress of the case. 2. Methods We have tried researching effect of herbal medicines(Chungpyesagan - tang and kalkunseunggi-tang) treatments to Stress caused by Taeumin rectal tenesmus and constipation patient. 3. Results A Taeumin patient diagnosed to rectal tenesmus and constipation was treated with Taeumin herbal medicines (Kalkunseunggi - tang treatments) and change of patient's the feces number of times and abdomen discomfort were improved. 4. Conclusions The cause of constipation variety as was treatments is various. The constitutional treatment with Kalkunseunggi - tang may have an effect on management of constipation. So we report the healing process and results of this patient in this study.

Ganglion Impar Block in the Management of Rectal Tenesmoid Pain (항문 이급후중의 치료에 있어 외톨이 신경절 차단)

  • Rectal tenesmus is a persistent, painful and ineffectual sensation of straining at stool or opening of the bowels. The pain is usually spasmodic in nature and most commonly encountered in patients with carcinoma of the rectum or other pelvic organs. In 1988, Bristowand Foster reported that patients with severe spasmodic painful tenesmus were relieved with chemical sympathectomy. In 1990, Plancarte introduced block of Ganglion impar. This technique is proposed as an alternative means of managing localized perineal pain of sympathetic origin. Ganglion impar block was performed on a 54-year-old female patient when analgesic or psychotropic drugs failed to control the symptoms of post-traumatic severe spasmodic painful tenesmus. Postoperatively, patient was free of tenesmoid pain for only 7 days. We then performed neurotomy by RF lesion generator which provided complete pain relief.

  • Rectal tenesmus is a persistent, painful and ineffectual sensation of straining at stool or opening of the bowels. Lumbar sympathectomy was performed in patient whose main complaint was rectal tenesmoid pain resulting from hemorrhoid operation, and in whom analgesic or psychotropic drugs had failed in controlling the symptom. After chemical lumbar sympathectomy, patient was free from the rectal tenesmoid pain. It is concluded that lumbar sympathectomy is a safe and effective treatment for rectal tenesmus.

Hematocolpos, pyocolpos, and pyocolpometra in intact female dogs with imperforate hymen

  • Imperforate hymen is a rare congenital disorder that may predispose to retention of fluid in the vagina and uterus, thereby resulting in conditions such as hematocolpos, pyocolpos, and pyocolpometra in female dogs. A 7-year-old intact female shih tzu exhibiting abdominal distension, depression, anorexia, dysuria, dyschezia, and tenesmus was diagnosed with pyocolpos; a 9-year-old intact female Yorkshire terrier with abdominal mass, dysuria, and tenesmus was diagnosed with hematocolpos; and a 7-year-old intact female shih tzu with dysuria, dyschezia, anorexia, and vomiting was diagnosed with pyocolpometra. Ovariohysterectomy and partial vaginectomy were performed, and the blind end of the vaginal stump was omentalized. This clinical report provide diagnostic process and surgical treatment option for congenital vaginal obstruction cases.

A Case Report of a Patient with Active Ulcerative Colitis Treated with Do-Che Decoction-based Korean Medicine (도체탕(導滯湯) 위주의 한방치료로 호전된 활동성 궤양성 대장염 환자 치험 1례)

  • Background : Sulfasalazine for treating ulcerative colitis has a clinical limitation due to its adverse effects. This case is about a patient with ulcerative colitis who could not tolerate sulfasalazine and was improved by Do-che decoction-based Korean medicine. Method and Results : Purulent and bloody diarrhea more than 20 times a day, mild fever, chilling and tenesmus were main symptoms of this patient who was diagnosed as ulcerative colitis 10 years ago. Do-che decoction-based Korean medicine which was recommended for purulent and bloody diarrhea in Dong-Ui-Bo-Gam, the classic literature of Traditional Korean Medicine was given to the patient for more than 1 month. After treatment, the frequency of purulent and bloody diarrhea, tenesmus and fever was decreased remarkably, and C-reactive protein, erythrocyte sedimentation rate, white blood cell count and maximum temperature also dropped to the normal range. Conclusion : Do-che decoction-based Korean medicine has a potential effect and may be a alternative therapeutic option in patients with ulcerative colitis who cannot tolerate sulfasalazine due to its adverse effects.

  • 40-day-old mixed male puppy was presented with a 10-day history of tenesmus and extensive ulcerative lesion of perineum. At physical examination, the dog was voiding mainly from an orifice in the perineal fistula. On fistulogram, a urethroperineal fistula was diagnosed. The dog was treated by wet dressing for wound management and antibiotics for 4weeks. Subsequently, the patient improved and no complication or recurrence was presented.

  • A case of severe complications following intrathecal administration of 45 ml of hypertonic saline solution for the treatment of postherpetic neuralgia was presented. Transient immediate complications included were tachycardia, hypertension, neck stiffness and muscle twitch. Pulmonary edema, paralytic intestinal obstruction, and the cauda equina syndrome including sphincter disorder with atonic urinary bladder developed shortly after the injection. Tenesmus and sensory abnormality around perineum and soles were the longlasting complications.

  • The perineum is comprised of diverse anatomic structures with mixed sympathetic and somatic innervation. The coccyx is innervated by the coccygeal nerves and branches of the fifth sacral root. Recently, ganglion impar block has been introduced as an alternative means of managing intractable pain of sympathetic origin, coccygodynia by trauma, tenesmus and perineal hyperhydrosis. We managed a 59-year-old female patient who had suffered from perineal pain by metastasis of cervical cancer. Approach to impar ganglion through the anococcygeal ligament was impossible because her coccyx was hyperflexed anteriorly by old fracture. But we could perform ganglion impar block successfully by approach through the separation of sacrum and coccyx.

Dysentery Caused by Balantidium coli in China

  • Balantidium coli human infection predominantly occurs in tropical and subtropical regions in the world. Human case is extremely rare in China. This report details a case of B. coli infection in a 68-year-old man in China, who presented with history of abdominal pain, tenesmus, diarrhea with blood and was diagnosed as B. coli-caused dysentery. Our case indicates possible occurrence of Balantidium coli-related disease in cooler climates. This case is presented not only because of its rarity but also for future references.

Surgical Correction of Anastomotic Rectal Stricture in a Dog

  • A 5-year-old male Laika dog was referred with chief complaint of dyschezia and tenesmus. The dog had rectal prolapse and it was corrected with rectal resection and anastomosis, a year ago. On rectal examination, the stricture of rectum was identified. The irregular scar tissues on serosal and muscular layers of rectum were noted and they were dispersed with partial thickness incision around rectum. Then, the colopexy and mechanical dilation with balloon catheter were applied. No recurrence of rectal prolapse and other complications were noted during follow up periods of 1 year. This report described a successfully corrected anastomotic rectal stricture in a dog.
