Microvascular autologous submandibular gland transplantation in severe cases of keratoconjunctivitis sicca

  • Dry eye syndrome is a relatively common disease of the tears and ocular surfaces that results in discomfort, visual disturbance, and tear film instability with possible damage to the ocular surfaces. Microvascular submandibular gland (SMG) transfer offers a surgical alternative for a permanent autologous substitution of tears using the basal secretion of a transplanted SMG. Long-term follow-up reveals that this technique is a lasting and effective solution for patients with severe dry eye syndrome. The uncomfortable symptoms were relieved, and the frequency of use of pharmaceutical tear substitutes was reduced. Objective examination showed significant improvement in tear film and some features of ocular surface such as breakup time of tear film and corneal staining. Patients may suffer from obstruction of Wharton's duct or epiphora after surgery. Activation of secretion-related receptors could improve the early hypofunction of the denervated SMG and prevent the duct obstruction. Reduction surgery, partial SMG transplantation, uses of atropine gel or Botulinum toxin A could be the choices of treatment for epiphora.

알코올 섭취에 따른 눈의 생리기능과 안기능의 변화 (Changes in the Physiology of eye and Ocular function due to alcohol intake)

  • 본 연구에서는 알코올 섭취가 눈의 생리기능과 안기능에 영향을 미치는 가를 알아보기 위해 알코올 섭취 전,후의 안압, 누액분비량, 입체시차, AC/A비, 폭주여력, 개산여력을 측정하여 다음과 같은 결과를 얻었다. 전체 대상자의 알코올 섭취 전 평균 안압은 15.14mmHg이고 알코올 섭취 후 평균 안압은 12.8mmHg로 평균 15.44% 감소하는 것으로 나타났다. 누액분비량은 알코올 섭취 전 검사대상자 평균 17.125mm에서 알코올 섭취 후 평균 9.875mm로 42.33%로 감소하는 것으로 나타났다. 입체시차는 알코올 섭취 전 40'에서 알코올 섭취 후 165'로 약 4배가량 증가하였다. AC/A비는 알코올 섭취 전 $5.40{\Delta}/D$, 알코올 섭취 후 $6.19{\Delta}/D$로 14.6% 증가하였고 폭주여력과 개산여력은 원거리, 근거리 모두 유의한 변화가 없는 것으로 나타났다.

백내장수술 안에서 열화상카메라를 이용한 안구표면 온도의 변화 (Thermographic Assessment on Temperature Change of Eye Surface in Cataract Surgery Observation)

  • 목적 : 백내장 환자를 대상으로 수술 전과 후의 안구표면의 온도변화 양상을 열화상카메라의 서모그래피를 이용하여 연구 하고자 하였다. 방법 : 본 연구는 백내장 수술을 받은 환자 50-79세까지 75명 75안의 환자 군을 대상으로 하였다. 과거에 굴절교정수술, 각막관련 수술을 받은 자와 콘택트렌즈를 사용하는 자, 눈물관 이상자, 전신질환 치료 약물을 복용하는 자 등 눈물분비와 눈물막에 영향을 줄 수 있는 자는 연구 대상자에서 제외하였고 눈물막파괴시간 검사(Tear Break Up Time, BUT), 쉬르머 검사(Schirmer's Test), 맥모니테스트(Mcmonnies questionnaire)를 시행한 후 열화상카메라(Cox CX series, Answer., Korea)를 이용하여 안구표면의 온도변화를 실시간으로 측정하였다. 결과 : 전체 대상자의 술 전 안구표면 온도는 $35.20{\pm}0.54^{\circ}C$이었고 술 후에는 $35.30{\pm}0.53^{\circ}C$로 표면온도가 상승하였으나 유의한 차이를 보이지 않았다. 안구표면 온도변화는 술전에서 $-0.12{\pm}0.08{\Delta}$ ($^{\circ}C/sec$)에서 술 후 $-0.18{\pm}0.07{\Delta}$ ($^{\circ}C/sec$)로 통계학적으로 유의한 결과를 나타냈다. 연령 별 비교에서는, 50 대군은 백내장 술 전 대상자의 안구표면 온도변화가 $-0.14{\pm}0.09{\Delta}$ ($^{\circ}C/sec$)에서 $-0.19{\pm}0.05{\Delta}$ ($^{\circ}C/sec$)으로 나타났고 60 대군에서는 $-0.12{\pm}0.08{\Delta}$ ($^{\circ}C/sec$)에서 $-0.15{\pm}0.07{\Delta}$ ($^{\circ}C/sec$)으로 나타났으며 70 대군에서는 술 전 대상자의 안구표면 온도변화는 $-0.12{\pm}0.08{\Delta}$($^{\circ}C/sec$)에서 $-0.18{\pm}0.07{\Delta}$($^{\circ}C/sec$)으로 전 연령에서 모두 유의한 안구표면 온도변화를 보였다. 결론 : 백내장 술 후에는 안구건조증 평가지표가 모두 감소하였고 안구표면 온도변화가 유의함을 보였다. 안구표면의 서모그래피 기술은 비침습적으로 안구건조증을 평가하는데 용이하였고 객관적으로 수치화할 수 있는 장점이 있어 다양한 안구건조증 연구에 활용 될 것으로 기대된다.

일부 대학신입생들의 B형 간염에 대한 지식과 실천 (Knowledge and Practice of College Entrants Toward Hepatitis B)

  • In order to designate a present status necessary for establishment of preventive measures and guidelines of health education against hepatitis B in the course of secondary school education, knowledge and practice toward hepatitis B virus infection was surveyed by a questionnare method on total of 4,855 college entrants in the academic year of 1987 and analyzed the data collected using IBM PC(Trigem 88-II) with SAS package program. About two per cent of college entrants had past history of HBV infections not showing any difference between both sexes and geographical regions. About one third(33.7%) of total students had tested hepatitis B surface antigen(HBsAg), only 4% had tested hepatitis B surface antibody(HBsAb) and vaccination rate amounted to 24.6%, one fourth of total subjects. Both serological tests and vaccination were most commonly performed during adolescence, showing higher rates in female students than in male students. The rates also seemed to be higher in those from urban cities than those from rural cities. Students who had acquired correct knowledge that hepatitis B was infected by virus were amounted to 78.5% of college entrants, and remaining 21.5% had misunderstood that rickettsia, bacteria, fungi or parasites were causal agents. Female students were better aware of the causal agents than male students but there was no difference between places of growth. As for mode of transmission of HBV, 51.5% of male students and 47.7% of female students had correct knowledge. A very few student had known that fact that HBV was transmitted by body fluids such as tear(6.9%), nasal discharge(10.1%) and semen or vaginal secretion(19.2%) and majority(75%) of students had misunderstood that hepatitis B virus would be transmitted per os through food ingestion. Approximately one half(48.9%) of college entrants had knew correctly whom to be vaccinated. Approximately one half of the students knew that hepatr;ma(57.8%) and liver cirrhosis(57.4%) might complicate with hepatitis B virus infection, whereas 12.0% of the students responded that bronchitis was one of the complications of hepatitis B infection. In summary of the above results, we highly recommend that health education program for eradication of hepatitis B virus infection should be introduced in curricula of secondary school education in this country.
