Effects of a Tailored Smoking Cessation Program in High School Girls (여고생을 대상으로 한 개별화 금연 프로그램의 효과)

  • Objectives: This study was conducted to evaluate the effects of a tailored program based on the Transtheoretical Model to smoking cessation in high school girls. Methods: A quasi-experimental research design was used in this study. The participants were 35 industrial high school girls who have been smoking. The girls were allocated to an experimental group(18) and a control group(17) by randomization. For intervention, the experimental group received the group smoking cessation program and individual program which tailored according to the stage of change. A common group smoking cessation program was given to the control group. Data were collected before the program, immediately after and 4 weeks after the program was completed and were analyzed with repeated measure ANOVA. Result: In the experimental group, the daily smoking amount, nicotine dependency and decisional balance(Pros) score were significantly decreased and the self-efficacy score were significantly increased at one month after the intervention in comparison with before and immediately after it. The decisional balance(Cons) and processes of change score of the experimental group were significantly increased at immediately after the intervention and one month after it in comparison with before it, and at one month after it in comparison with immediately after it. A significant difference in the daily smoking amount, urine cotinine, nicotine dependency, decisional balance(Pros, Cons) and processes of change score between the both groups was found after one month of intervention. Conclusions: The tailored smoking cessation program was more effective, compared to the common smoking cessation program on smoking behavior, self-efficacy, decisional balance, processes of change. This program can be used for favorable changes in high school girl's smoking behavior.

Effects of Tailored Supportive Education on Physical, Emotional Status and Quality of Life in Patients with Congestive Heart Failure (맞춤형 지지교육이 심부전증 환자의 신체, 정서적 상태 및 삶의 질에 미치는 효과)

  • Purpose: This study was conducted to examine the effects of tailored supportive education on physical, psychological status and quality of life in the patients with congestive heart failure (CHF) who had not participated previously in cardiac rehabilitation program. Methods: This study was used a non-equivalent control pre-post design, conducted on 64 CHF patients under medical treatment in a hospital. The experimental group (n=31) received the tailored supportive educational program (once 1~2 days before discharge and 6 times after discharge through outpatient visits or telephone contact: once every week for the first 4 weeks after discharge and once every 2 weeks for the remaining 4 weeks). The control group (n=33) received a traditional discharge education. Data were analyzed using descriptive statistics, ${\chi}^2$-test, Fisher's exact test, t-test and repeated measure ANOVA using the SPSS/ WIN 18.0 program. Results: Participants in the experimental group showed the significantly increased scores of the quality of life (F=16.01, p<.001), and the significantly decreased scores of physical function (F=7.27, p=.009), depression (F=8.25, p=.006) and anxiety (F=4.11, p=.047), when compared to those of the control group. Conclusion: The findings indicated tailored supportive education was an effective intervention care in physical, emotional status and quality of life for CHF patients.

Flow Effects on Tailored RF Gradient Echo (TRFGE) Magnetic Resonance Imaging : In-flow and In-Plane Flow Effect (Tailored RF 경자사계방향 (TRFGE} 자기공명영상(MRI)에서 유체에 의한 영상신호 변화 : 유체유입효과와 영상면내를 흐르는 유체의 효과에 대하여)

  • In this paper, we have reported two interesting flow effects arising in the TRFGE sequence using water flow phantom. First, we have shown that the TRFGE sequence is indeed not affected by "in-flow" effect from the unsaturated spins flowing into the imaging slice. Second, the enhancement of "in-plane flow" signal in the readout gradient direction was observed when the TRFGE sequence was used without flow compensation. These two results have many interesting applications in MR imaging other than fMRI. Results obtained were also compared with the results obtained by the conventional gradient echo(CGE) imaging. Experiments were performed at 4.7T MRI/S animal system (Biospec, BRUKER, Switzerland). A cylindrical phantom was made using acryl and a vinyl tube was inserted at the center(Fig. 1). The whole cylinder was filled with water doped with $MnCl_2$ and the center tube was filled with saline which flows in parallel to the main magnetic field along the tube. Tailored RF pulse was designed to have quadratic ($z^2$) phase distribution in slice direction(z). Imaging parameters were TR/TE = 55~85/10msec, flip angle = $30^{\circ}$, slice thickness = 2mm, matrix size = 256${\times}$256, and FOV= 10cm. In-flow effect : Axial images were obtained with and without flow using the CGE and TRFGE sequences, respectively. The flow direction was perpendicular to the image slice. In-plane flow : Sagittal images were obtained with and without flow using the TRGE sequence. The readout gradient was applied in parallel to the flow direction. We have observed that the "in-flow" effect did not affect the TRFGE image, while "in-plane flow" running along the readout gradient direction enhanced the signal in the TRFGE sequence when flow compensation gradient scheme was not used.

Endovascular Treatment of 429 Anterior Communicating Artery Aneurysms Using Bare-Platinum Coils : Clinical and Radiologic Outcomes at the Long-term Follow-up

  • Objective : We reviewed the feasibility, safety and efficacy as well as the clinical outcome and long-term angiographic results of endovascular treatment (EVT) of the anterior communicating artery (ACoA) aneurysms. Methods : A total of 429 ACoA aneurysms in 426 patients were treated using coil embolization between March 1996 and October 2010 in a single institution. Pretreatment aneurysmal features were checked using angiogram. We had usually used tailored steam shaped microcatheter according to individual angiographic architectures. Immediate postembolization outcomes were evaluated using an angiographic outcome scale and clinical evaluation was performed using the Glasgow Outcome Scale (GOS). Results : Postembolization angiograms demonstrated total occlusion of aneurysm in 290 of 429 (67.6%) aneurysms, neck remnant in 80 (18.6%) and body filling in 59 (13.8%). Dome direction and aneurysm angle was not associated with initial angiographic outcomes. The procedure-related morbidity rate was 0.9% (4 of 429). Clinical and imaging follow-up more than 6 months were available in 382 (89.0%) patients with a mean of 26.2 months. Overall rate of major recanalization was 7.9% (30 of 382) and all of them were retreated without complications. At the last follow-up, 233 (99.2%) of 235 patients had GOS of 5 in unruptured group, and 152 (79.5%) of 191 patients showed good clinical outcomes (GOS of 4 or 5) in ruptured group. Conclusion : Tailored steam shaping of the microcatheter is vital to achieve good angiographic outcomes regardless of aneurysmal direction. EVT is feasible and safe for most ACoA aneurysms with acceptable immediate and long-term outcomes.

  • Hydrogels have been developed and used in tissue engineering and regenerative medicine to deliver therapeutics to injured or diseased tissue because of their versatility and properties that can be tailored to match the natural extracellular matrix. Hydrogels can be made with a variety of physical and chemical properties combined with light responsiveness ideal for applications in different fields of medicine that require the spatiotemporal control of therapeutics. Light, as a stimulus, is relatively inexpensive, contact-free, noninvasive with high spatial resolution and temporal control, convenient and easy to use, and allows deep tissue penetration that is relatively harmless. Photoresponsive hydrogels are ideal candidates for on-demand drug delivery systems that are capable of sustained and controlled drug release, minimizing the side effects, and ensuring the activity and efficient delivery of drugs to the target tissue.

  • Objective : To build a basis of a research strategy for development of Korean ginseng as a global competitive medical product in the world market by Korean Oriental medicine group. Methods : Survey of all literature or analysis reports about market patterns related to ginseng or ginseng products in Korea and in other countries. Results : Korean ginseng and it products have rapidly decreased in share of the world market since the 1980s, with Korean ginseng occupying just 3% of the Hong Kong ginseng market in 2005. While Korean ginseng has been satisfied with consumption within the large Korean market itself, it now has only 1% of world market share. Conclusion : A systematic strategy is needed to strengthen global competitiveness of Korean ginseng based upon development of novel customer-tailored ginseng products. In addition, experience and science-based research in the Oriental medicine field should be involved in the process.

A Review on Korean Medicine and Personalized Medicine: Syndrome-based Personalized Medicine on the Basis of Syndrome Differentiation and Treatment (한의학과 개인맞춤의학에 대한 소고; 변증논치에 근거한 '증 기반 개인맞춤의학')

  • Objectives: This study aimed to review the characteristics of personalized medicine and Korean medicine, and the correlation between personalized medicine and Korean medicine. Methods: We investigated various studies in PubMed, Scopus and domestic Korean medicine journals. In addition, we discussed the topic based on literature. Results: Western medicine developed as evidence-based medicine. However, its limitations are being reached, so a new paradigm of medicine is needed. As a result, personalized medicine has appeared. Recently, through the development of human genomics, personalized medicine has been researched on the basis of individual genetic characteristics. Korean medicine has developed with a unique holistic approach and treats not the disease itself but the patient's body. Its characteristic is well expressed through syndrome differentiation and treatment. Syndrome differentiation represents the nature of person-centered medicine and becomes the root of personalized medicine. Conclusions: Compared with genome-based personalized medicine of Western medicine, Korean medicine could be classified as syndrome-based personalized medicine. It would be great to apply this characteristic to clinical practices.

Radiopharmaceuticals of Cardiac SPECT Imaging (심장스펙트 영상에 사용되는 방사성의약품)

  • Ever since it was first introduced as a method of scintigraphically measuring regional myocardial perfusion, myocardial perfusion study has been widely used in patients diagnosed or suspected with coronary artery diseases, and continuously improved upon. In addition to the technological enhancement of nuclear medicine equipments, the innovation of radiopharmaceuticals used in the cardiac exams were important contributors to such improvement. Besides the cardiac perfusion studies, new radiopharmaceuticals that visualize fat metabolism or receptors of the sympathetic nervous system have successfully been applied to clinical practice. More information can be collected before diagnosing coronary vascular disease, evaluating the patient's condition, or assessing therapeutic effects. In this review article, the clinical efficacy and characteristics of radiopharmaceutical products tailored for cardiac SPECT that are commonly used in Korea currently, plus the ones not being used yet but have proven value are briefly described.

  • The management of severe asthma presents a significant challenge in asthma treatment. Over the past few decades, remarkable progress has been made in developing new treatments for severe asthma, primarily in the form of biological agents. These advances have been made possible through a deeper understanding of the underlying pathogenesis of asthma. Most biological agents focus on targeting specific inflammatory pathways known as type 2 inflammation. However, recent developments have introduced a new agent targeting upstream alarmin signaling pathways. This opens up new possibilities, and it is anticipated that additional therapeutic agents targeting various pathways will be developed in the future. Despite this recent progress, the mainstay of asthma treatment has long been inhalers. As a result, the guidelines for the appropriate use of biological agents are not yet firmly established. In this review, we aim to emphasize the current state of biological therapy for severe asthma and provide insights into its future prospects.

Motivating Underserved Vietnamese Americans to Obtain Colorectal Cancer Screening: Evaluation of a Culturally Tailored DVD Intervention

  • Background: Colorectal cancer (CRC) is a leading cause of cancer death among Vietnamese Americans, yet screening remains underutilized. We investigated the effectiveness of a culturally tailored DVD intervention in promoting CRC screening among unscreened Vietnamese Americans age 50 and over. Materials and Methods: Using a community-based participatory research approach, we conducted a trial comparing twenty-eight subjects who received a mailed DVD in Vietnamese, with twenty-eight subjects who received a mailed brochure in Vietnamese. Subjects completed telephone surveys at baseline, One-month, and one-year. The primary outcome was receipt of screening. Secondary measures were participants' knowledge, attitudes, and beliefs about CRC screening. Two focus groups explored the intervention's acceptability and effectiveness. Results: At one year, CRC screening rates of 57.1% and 42.9% were observed in experimental and control group respectively (p=0.42), Subjects in both groups showed increased knowledge about CRC after one month. Focus group findings revealed that the DVD was an effective method of communicating information and would help promote screening. Conclusions: The findings suggest that culturally tailored, linguistically appropriate content is more important than the type of media used. This relatively low intensity, low cost intervention utilizing a DVD can be another useful method for outreach to the often hard-to-reach unscreened population.
