  • Transdermal and topical drug delivery with minimal tissue damage has been an area of vigorous research for years. Our research team has initiated the development of an effective method for delivering drug particles across the skin (transdermal) for systemic circulation, and to localized (topical) areas. The device consists of a laser ablation based micro-particle acceleration system that can be integrated with endoscopic surgical techniques. We have successfully delivered 3μm size cobalt particles into gelatin models that represent soft tissue with remarkable penetration depth.

One-Stage Treatment of Chronic Calcaneal Osteomyelitis with Bone Morphogenetic Protein 2 and Local Antibiotic Delivery in a Cat

  • An age-unknown, 4.8 kg, male, wild, domestic short-hair cat was presented for left hindlimb lameness. A physical examination revealed a draining tract which was suspected of bite on left calcaneal bone. The left tarsal joint was markedly swollen and exudates were observed around the draining tract. Sequestrum at left calcaneus bone, and osteolysis were identified by radiography. The sequestrum and its surrounding exudative tissue were debrided during surgery and the tissue was submitted for bacterial culture and sensitivity test. The debridement caused a bone defect ($1.5cm{\times}0.5cm{\times}0.5cm$) on the medial left calcaneal bone. Plate and screw fixation was performed to the calcaneus bone as buttress plate. Recombinant human bone morphogenetic protein-2 (rhBMP-2) loaded hydroxyapatite was implanted in the bone defect. Furthermore, Amikacin-impregnated collagen sponges were also placed around bone plate to deliver local antibiotics. A systemic antibiotic treatment regimen based on bacterial culture and sensitivity test results was administered for 4 weeks. The wound properly healed without any signs of infection, and the bone healing was confirmed by radiography. The patient showed normal weight bearing ambulation at 18 weeks after surgery. The use of rhBMP-2 and local antibiotic delivery system is a good surgical option for the one-stage treatment of chronic osteomyelitis.

  • Medicine is undergoing a revolution in the understanding of the mechanisms through which disease processes develop. The advent of genetics and molecular biology to oncology not only is providing surrogate predictors of therapy response and survival which are forming the basis for selection among established treatment options, but is providing targets for new directions in therapy as well. Molecular modification of somatic cells for the purposes of protecting the normal cells from the toxicity of cancer chemotherapy, for the sensitization of the tumor cells to therapy and use of conditionally replicating viral vector have been new directions of cancer treatment which have reached the clinical arena. Advances in molecular pharmacology and vector design summarized in this paper may provide solutions to some of the existing problems in the technology of gene transfer therapy. Continued basic research into the biological basis of human disease, systemic studies of the application of these discoveries to therapy and the improvement of vector for gene delivery all combined may result in advances in this important field of therapy over the next few years.

  • To prolong the biological half-life of streptokinase, a thrombolytic agent, streptokinase-bearing liposome with and distearolyphosphatidyl ethanolamine-N-poly (ethylene glycol) 2000 (DSPEPEG 2000) was prepared and evaluated. Streptokinase-bearing liposomes composed of distearolyphosphatidylcholine (DSPC), cholesterol and cholesterol-3-sulfate with DSPE-PEG 2000 was prepared by the freeze-thawing method and administered via femoral vein to rats (15000 IU/kg). The activity of streptokinase in plasma was determined by the method based on the amidolytic activity of streptokinase-plasminogen complex. Pharmacokinetic parameters of streptokinase incorporated in liposomes were compared with those of streptokinase alone. The $T_{1/2}$ and $AUC_\infty$ streptokinase incorporated in DSPC-PEG liposome increased 16.3- and 6.1-fold, respectively, compared with those of streptokinase alone. Streptokinase-bearing long-circulating liposome could increase the circulation time of streptokinase in blood and expect longer thrombolytic activity compared with streptokinase alone.

  • The classification of periodontal disease in 1999 has been widely used for determining a diagnosis, establishing a treatment plan, and evaluating the prognosis of the patient with periodontal disease. However, scientific evidence from many studies indicates the need for a new classification system for periodontal and peri-implant disease. Summary at 2017 world workshop as follows: 1) Periodontal health and peri-implant health was defined; 2) Chronic periodontitis and aggressive periodontitis were unified as periodontitis; 3) Periodontitis was further classified by staging and grading to reflect disease severity and management complexity, rate of disease progression, respectively; 4) Periodontal disease as manifestation of systemic disease is based on the International Statistical Classification of Diseases and Related Health Problems-10 (ICD-10) code; 5) Periodontal biotype and biologic width was replaced to periodontal phenotype and supracrestal tissue attachment, respectively; 6) The excessive occlusal force was replaced by a traumatic occlusal force; 7) ≥3 mm of radiographic bone loss, ≥6 mm of pocket probing depth and bleeding on probing indicates peri-implantitis in the absence of radiograph at final prosthesis delivery.

  • Physical properties of oral adhesive tablets prepared with four kinds of gums for topical or systemic drug delivery were investigated. Oral adhesive tablets containing 5mg of brilliant blue(BB) were prepared from direct compression. Viscosity of 2% gum solutions, water absorption, fracture resistance, stickiness of tablets, and dissolution of BB in pH 6.8 dissolution medium were tested. Acacia showed good stickiness and fracture resistance, and tragacanth showed good retarding effect on the release of BB from tablets. Therefore, tablets with varing ratios of acacia and tragacanth were prepared and their physical properties were examined. In conclusion, it was possible to obtain some adequate properties by compounding acacia and tragacanth.

  • This case study is aimed at introducing a full-mouth rehabilitation of a patient with severe tooth wear using fixed prosthesis. This is a case report of a patient with severe wear dentition with changing vertical dimension. In line with using prosthetic treatment, patient adaptation was verified with provisional restorations followed by diagnostic wax-up. Function, esthetics, and occlusal stability were verified during a 4-week follow up period. Prosthodontic reconstruction based on systemic analysis, diagnosis, and treatment plan led to satisfactory results after delivery of definitive prosthesis. Prosthetic treatment of severe wear dentition was functionally and esthetically successful. Dentists and dental technicians would be able to develop better treatment approaches using fixed dental prosthesis.

  • Corticosteroids have been used most frequently for inflammatory bowel disease. To reduce side effects by the systemic absorption, colon-specific delivery is highly desirable. We expected that conversion of 21-hydroxyl in glucocorticoids into a sulfate ester sodium will greatly increase the hydrophilicity, which consequently restrict the gastrointestinal absorption. Once delivered to the colon, sulfate ester will be hydrolyzed by the sulfatase originated from microbes and release the parent compound, glucocorticoids. In this study, we prepared methylprednisolone 21-sulfate sodium (MPS) and investigated its suitability as a colon-specific prodrug on methylprednisolone (MP). (omitted)
