Purpose: Dysmenorrhea is mostly depending on the causative factor, which usually falls under the categories of Cold and Heat pattern in traditional Oriental medical theories and diagnosis. Thus, we investigated menstruation symptom and sign related to dysmenorrhea and verified the validity of Cold and Heat pattern identification. Methods: We investigated menstruation symptom and sign related to dysmenorrhea in total 14 gynecology-medical books including ${\ll}$Exemplar Of Korean Medicine (Dongui Bogam)${\gg}$ and whole body symptom and sign identifying Cold and Heat pattern at the same time. A survey based on this investigation was carried out targeting women of childbearing age. Results: According to 14 gynecology-medical books, polymenorrhea is relevant to Heat pattern, oligomenorrhea to Cold pattern and darkness of menstrual blood is relevant to Heat pattern. Among the total of 343 womens, 196 subjects suffered from dysmenorrhea. The number of dysmenorrhea with polymenorrhea(Heat pattern) was 6 person, with oligomenorrhea(Cold pattern) was 27 person. And the number of dysmenorrhea with darkness of menstrual blood(Heat pattern) was 39 person. As the result of checking correlations of menstrual symptom scores and whole body symptom scores, there was no significance of Cold and Heat pattern between menstrual symptoms and whole body symptoms. Conclusions: The results suggest that the period of menstrual cycle and the color of menstrual blood provides some informations of Cold and Heat pattern identification. But considering with other whole body symptom and sign is needed for more precise result.
Objectives This research was proposed to present Clinical Practice Guideline(CPG) for Soeumin Disease of Sasang Constitutional Medicine(SCM): Diagnosis and Algorithm. Purpose: Dysmenorrhea mostly depends on the causative factor, which usually falls under the categories of Deficiency and Excess pattern in traditional Oriental medical theories and diagnosis. Thus, we investigated menstruation symptom and sign related to dysmenorrhea and verified the validity of Deficiency and Excess pattern identification. Methods: We investigated menstruation symptom and sign related to dysmenorrhea in total 14 gynecology-medical books including the book ${\ll}$Exemplar Of Korean Medicine (Dongui Bogam)${\gg}$ and whole body symptom and sign identifying Deficiency and Excess pattern at the same time. A survey based on this investigation was carried out targeting women of childbearing age. Results: Total of 14 gynecology-medical books have mostly narrated pre-menstrual and mid & post-menstrual pelvic pain depending on the time of its manifestation for identifying Deficiency and Excess pattern. Dysmenorrhea in pre-menstrual period belonged to Excess pattern and dysmenorrhea in mid & post-menstrual period belonged to Deficiency pattern. Among a total of 343 women, 196 subjects suffered from dysmenorrhea. The number of dysmenorrhea in pre-menstrual period (Excess pattern) was 116 people and in mid & post-menstrual period (Deficiency pattern) was 80 people. Deficiency and Excess pattern of dysmenorrhea in menstrual period significantly correlated to Deficiency and Excess pattern of whole body symptom and sign in the statistics(P-value < 0.05). Conclusion: The results suggest that pre-menstrual and mid & post-menstrual pelvic pain depending on the time of its manifestation is preferentially utilized as symptom and sign related to dysmenorrhea identifying Deficiency and Excess pattern.
The purpose of this study was to determine the effects of loneliness on drinking, smoking, and health perception (symptom pattern & subjective health) in college students. The convenience sample consisted of 417 college students attending four universities. The Revised UCLA Loneliness Scale(RULS) and the Symptom Pattern Scale were used to collect the data. In this study, 84.7% of the subjects used alcohol, with a mean of 5.52 drinks per week, and 32. 9% of the subjects smoked for a mean of 5.08 cigarette packs per week during the previous month. The mean score loneliness measured by the RULS was 40.82, indicating that the subjects were moderately lonely. The majority of the subjects had a low level of symptom pattern and evaluated their health as either very good or good. The level of alcohol drinking and the smoking increased and symptom pattern decreased with age. Female students were lonelier than male students in this study. Also, the female students had a lower level of symptom pattern and evaluated their health worse than the male students. Male students consumed more alcohol and smoked more cigarette than female students. Living arrangement was significantly related to the level of loneliness. The subjects who lived with their parents and siblings were less lonely than those who lived with their friends, or relatives or who lived in a dormitory. Age and religion were not related to the level of loneliness. The level of loneliness influenced drinking, symptom pattern, and subjective health. The study found that subjects who were more lonely consumed alcohol less, had a higher level of symptom pattern, and perceived their health worse than those who were less lonely. Smoking was not influenced by loneliness in this study.
Objectives: This paper aims to investigate the Donguibogam-based pattern diagnosis by applying natural language processing and machine learning. Methods: A database has been constructed by gathering symptoms and pattern diagnosis from Donguibogam. The symptom sentences were tokenized with nouns, verbs, and adjectives with natural language processing tool. To apply symptom sentences into machine learning, Word2Vec model has been established for converting words into numeric vectors. Using the pair of symptom's vector and pattern diagnosis, a pattern prediction model has been trained through Logistic Regression. Results: The Word2Vec model's maximum performance was obtained by optimizing Word2Vec's primary parameters -the number of iterations, the vector's dimensions, and window size. The obtained pattern diagnosis regression model showed 75% (chance level 16.7%) accuracy for the prediction of Six-Qi pattern diagnosis. Conclusions: In this study, we developed pattern diagnosis prediction model based on the symptom and pattern diagnosis from Donguibogam. The prediction accuracy could be increased by the collection of data through future expansions of oriental medicine classics.
Objectives This research was proposed to present Clinical Practice Guideline(CPG) for Taeeumin and Taeyangin Disease of Sasang Constitutional Medicine(SCM): Diagnosis and Algorithm. This CPG was developed by the national-wide experts committee consisting of SCM professors. Methods We searched the literature and articles related to Taeeumin and Taeyangin Symptomatology diagnosis and algorithm. For developing diagnosis and algorithm, we searched the classification, ordinary symptom, present symptom of the Taeeumin and Taeyangin Symptomatology Results and Conclusions We classified the Taeeumin Symptomatology by 4 steps: Exterior-Interior disease, favorable-unfavorable pattern, mild-moderate-severe-critical pattern, initial-intermediate-advanced pattern. And we classified the Taeyangin Symptomatology by 3 steps: Exterior-Interior disease, favorable-unfavorable pattern, mild-moderate-severe-critical pattern. At the unfavorable pattern, ordinary symptom is very important. So doctors are considered to need to focus on the symptom of unfavorable's ordinary symptoms.
Purpose: We intended to observe relation between general symptom and the number of uterine myoma. Methods: We surveyed 948 women from 35 to 45 years old who had received Korean oriental medical questionnaires about general symptom pattern and pelvic ultrasonography at Kyung-Hee university medical center from May, 2007 to October, 2008. After we divided women into three groups (Mono-uterine myoma, Bi-uterine myoma & uterine myoma above 3) by ultrasonography result, we compared general symptom pattern according to the number of uterine myoma. For statistics, we used Kruskal-Wallis Test, SPSS 13.0 for windows. Results: In relation to sleep pattern, severity of dreaming & awaking showed statistical significance according to the number of uterine myoma. In relation to digestion pattern, appetite & heartburn showed statistical significance according to the number of uterine myoma. In relation to evacuation pattern, hardening of stool showed statistical significance according to the number of uterine myoma. In relation to sweating pattern, severity of refreshing after sweating showed statistical significance according to the number of uterine myoma. In relation to the other patterns, hypertonicity of nape neck showed statistical significance according to the number of uterine myoma(p<0.05). Conclusion: The result showed that prevalence of uterine myoma had to be considered in respect of sleep, digestion, evacuation and sweating pattern. we need further study based on this preliminary result.
Objectives: We intended to identify factors related to the severity of hot flush of climacteric women from an East-West medical point of view. Methods: We surveyed 446 climacteric women who had received Korean oriental medical questionnaires about clinical symptom patterns and health medical examinations at Kyung-Hee University Medical Center over 1 year, from June 2007 to May 2008. Then, we compared hot flush with clinical symptom pattern and health medical examination result. Results: As the severity of hot flush increased, hypnagogic disorder in sleep pattern, abdominal gaseous distention in digestion pattern, tenesmus in evacuation pattern, yellow or reddish urine in voiding pattern, spontaneous sweating in sweating pattern, chest oppression in psychologic pattern, not-pulling-bedclothes in cold-heat pattern and mouth dryness in craniocervical symptom increased (p<0.05). In relation to digestion pattern, the severity of hot flush showed statistical significance according to prevalence of gastritis diagnosed by gastroscopy and upper GI series(p<0.01). In relation to voiding pattern, the severity of hot flush showed statistical significance according to prevalence of urine protein diagnosed by urinalysis (p<0.05). In relation to sweating, psychologic & cold-heat pattern, triiodothyronine (T3) increase and thyroid stimulating hormone (TSH) decrease were significantly correlated as the severity of hot flush increased (p<0.01). Conclusions: The result showed that hot flush of climacteric women had to be considered in respect of digestion disorder related to gastritis and sweating psychologic cold-heat disorder related to thyroid hormone.
Purpose: The purpose of this study was to differentiate between women with three perimenstrual symptom severity patterns : premenstrual syndrome(PMS), premenstrual magnification(PMM), and low symptom(LS), and to explore the related dietary factors to premenstrual symptoms. Method: Women were asked to keep a diary record of perimenstrual symptoms and food intake for 50 days. Result: Symptom patterns were defined for 26 among 38 women ; Eight(21.1%) demonstrated a PMS pattern, three(7.9%) demonstrated a PMM pattern, and fifteen(39.5%) exhibited a LS pattern. There were significant differences in symptom scores during the premenstrual phase($x^2=19.30$, p=.000), menstrual phase($x^2=13.32$, p=.001), and post menstrual phase($x^2=9.93$, p=.007) for three groups. Protein, vit E, vit C, niacin, folic acid, and phosphorus in the premenstrual phase, and energy, and vit B6 in the menstrual period were significantly different between the three groups. Among dietary compositions, amino acids, lipids, fatty acids, saturated fatty acids, natrium, vit B6, niacin, and vit E were negatively related to PMS symptoms. Conclusion: Pattern of perimenstrual symptoms should be differentiated for individualized PMS management. As a more efficient diet assessment for PMS women, randomized nutritional analysis during the 3 phases of the menstrual cycle should be done and a replication study is necessary with a larger sample.
