The purpose of this study was to identify symptom clusters. Method: A survey was conducted on 184 women with diagnoses of cervical, ovarian or endometrial cancer. Fifty symptoms were assessed for prevalence, severity and interference, and symptom clusters were identified. Cluster analysis was done using SPSS version 12.0. Results: Fatigue was identified as the most prevalent symptom (81.52%), lack of vaginal lubrication (2.26) as the most severe symptom, and lack of vaginal lubrication as the most interfering one (2.15). Identified six clusters were: Anorexia-pain cluster (loss of appetite, taste change, weight loss, appearance change, alopecia, weakness, pain), Fatigue cluster (lack of concentration, lack of memory, fatigue, dry mouth), Urinary-bowel distress cluster (urinary difficulty, constipation), Abdominal discomfort cluster (lower abdominal pain, abdominal pain, bloating), Emotional distress (sadness, anxiety-worry, nervousness, restlessness), and Menopausal cluster (sweating, hot flush, fever). Conclusion: The result of this study provides fundamental data to health care professionals in developing interventions for effective symptom management for women with gynecologic cancer by understanding identified 6 symptom clusters.

  • Hyperhidrosis is a condition characterized by excessive sweating. It can be generalized or focal. Generalized hyperhidrosis involves the entire body and is usually part of an underlying condition, most often an infectious, endocrine or neurologic disorder. Focal hyperhidrosis is idiopathic, occurring in otherwise healthy people. The condition carries a substantial psychological and social burden, since it interferes with daily activities. We treated two male patients who had hyperhidrosis on the head. In the point of Differentiation of Syndrom(辨證), these subjects were diagnosed as Bi-Wi-Gi-Her (Pi-Wei-Qi-Xu 脾胃氣虛) and was administered revised Bojungikgi-tang (reserved Buzhongyiqi-tang, 補中益氣湯加味方). All of two cases had anorexia, dyspepsia, fatigue. After treatment hyperhidosis and other symptoms improved.

  • Nefopam (NFP) is a non-opioid, non-steroidal, centrally acting analgesic drug that is derivative of the nonsedative benzoxazocine, developed and known in 1960s as fenazocine. Although the mechanisms of analgesic action of NFP are not well understood, they are similar to those of triple neurotransmitter (serotonin, norepinephrine, and dopamine) reuptake inhibitors and anticonvulsants. It has been used mainly as an analgesic drug for nociceptive pain, as well as a treatment for the prevention of postoperative shivering and hiccups. Based on NFP's mechanisms of analgesic action, it is more suitable for the treatment of neuropathic pain. Intravenous administration of NFP should be given in single doses of 20 mg slowly over 15-20 min or with continuous infusion of 60-120 mg/d to minimize adverse effects, such as nausea, cold sweating, dizziness, tachycardia, or drowsiness. The usual dose of oral administration is three to six times per day totaling 90-180 mg. The ceiling effect of its analgesia is uncertain depending on the mechanism of pain relief. In conclusion, the recently discovered dual analgesic mechanisms of action, namely, a) descending pain modulation by triple neurotransmitter reuptake inhibition similar to antidepressants, and b) inhibition of long-term potentiation mediated by NMDA from the inhibition of calcium influx like gabapentinoid anticonvulsants or blockade of voltage-sensitive sodium channels like carbamazepine, enable NFP to be used as a therapeutic agent to treat neuropathic pain.

  • Compensatory hyperhidrosis or reflex hyperhidrosis is the increase in sweating in the postoperative stage of thoracic sympathectomy or lumbar sympathectomy. It shares several features with anxiety disorders and has a negative impact on a patient's quality of life. Oralglycopyrrolate is one of the treatment options available. This study reviewed case notes in a series of 19 patients with compensatory hyperhidrosis. We made a comparison between the Milanez de Campos score of a pre-glycopyrrolate medication group and the Milanez de Campos score of a post-glycopyrrolate medication group. The Beck Depression Inventory (BDI) score, Beck Anxiety Inventory (BAI) score, and autonomic nervous system (ANS) scale score were also compared between the pre-medication and post-medication groups. In the post-glycopyrrolate medication group, there was decrease in the Milanez de Campos score, BAI score, and BDI score (P < 0.05). But no meaningful change was seen in the ANS score in the post-glycopyrrolate medication group (P > 0.05). Glycopyrrolate is an effective medication in the treatment of compensatory hyperhidrosis that, can alleviate anxiety and improve patients' quality of life.

  • Hyperhidrosis of the palms and soles is a disorder characterized by excessive sweating that occurs in the hands and feet and that cause severe psychological, social and/or professional disability. The pathophysiological cause is still unknown but the condition is related to over-activity in the Sympathetic Nervous System. We experienced four young patients with hyperhidrosis of the palms and soles that was thought to be initiated by emotional distress, nervousness, stressful situation and anxiety. In the point of Differentiation of Syndrome, this subject was diagnosed as consumptive fever of the Heart and a weakness of the Spleen and damp heat, and was administrated with Yangshimtang-Gamibang. After treatment of herbal medicine and acupuncture, hyperhidrosis and other symptoms of patients were improved.

  • Objectives : To identify whether and how Korean people understand 'qi deficiency' and related symptoms. Methods : We developed a questionnaire based on the experts' comments and modification asking understanding of 'qi deficiency' and symptoms or situations related with it. Total 128 workers at community healthcare centres across the country completed the questionnaire. They were asked if they know the meaning of the word 'qi deficiency' and to give at least three symptoms or situations that they conceive are related with 'qi deficiency'. The responses were collected and descriptive and frequency analyses were performed. Results : Approximately 79.7% of responders answered that they understand the meaning of the word 'qi deficiency' and of them, 71.6% reported that they experience 'qi deficiency' sometimes or more. Regarding somatic or condition-related responses in qi deficiency, lack of power or energy (26.6%), tiredness or fatigue (22.2%), dizziness (7.2%) or sweating (6.3%) were the most frequently reported symptoms. They related symptoms such as loss of appetite (4.3%) or easily catching colds (2.4%) with qi deficiency. Factors described in traditional pattern recognition of qi deficiency were not exactly matched with the responses from this survey. Conclusions : Korean people have rather a broad concept of qi deficiency covering a range of symptoms and this should be taken into account in developing a deficiency assessment tool.
