Localized Pulmonary Edema in Patient with Severe Mitral Regurgitation (승모판 폐쇄 부전증에서 발생한 국소적 폐부종 1예)

  • An 82-year-old female non-smoker with a history of hypertension presented with increasing dyspnea, cough and some purulent sputum without fever. Upon admission, the patient was in a distressed condition. Auscultation revealed diminished breath sounds with no rales over the right lung. An examination of the heart revealed a regular rhythm and a systolic murmur radiating from the apex of the heart. There was no pitting edema in the lower extremities. The blood tests showed mild leukocytosis and an increased C-reactive protein level. The $O_2$ saturation was 98 % whilst breathing room air. The electrocardiogram demonstrated sinus tachycardia. The chest radiograph showed a moderate cardiomegaly, right lobe infiltrates, and blunting of the both costophrenic sulcus suggesting a small pleural effusion. Three days after admission, the symptoms became slightly aggravated despite being treated with empirical antibiotics for presumed community-acquired pneumonia. Transthoracic color Doppler echocardiography indicated an ejection fraction of 48 %, mild left ventricular enlargement, and moderate left atrial enlargement resulting in severe mitral regurgitation. The clinical symptoms and right pulmonary edema resolved quickly with intravenous furosemide treatment.

A Study on the Original State of the Buildings in NakSunJae Area (낙선제(樂善齊) 일곽(一廓) 건축(建築)의 조영(造營)에 관한 복원적(復元的) 고찰(考察))

  • This paper aims to clarify the historical background and the architectural purpose of the buildings in NakSunJae area to the original state. Through the investigation, the clarified facts are as follows. 1. The relation with the buildings for the Crown Prince in the ChangDok Palace By constructing buildings in the NakSunJae area, the spatial conception of the Royal Palace was changed, that is, buildings for the Crown Prince in the ChangDok Palace disappeared and they became to belong to the ChangGyeong Palace. Moreover, by constructing the NakSunJae and its attached buildings, the area for the Crown Prince shifted to use as a private area for the king. 2. The date and the purpose of the construction The NakSunJae area is consisted of three compounds of building: NakSunJae, SukBokHun and SooKangJae. The NakSunJae was constructed for the king HunChong's private occupancy in 1847 and the SukBokHun for a residence of a royal concubine Kim, in 1847. The SooKangJae was originally built in 1785 in the SooKang Palace where the ex-king took up as his residence after abdicate the throne. Later on, it was repaired as the residence of the Queen SoonWon, king HunChong's grandmother, in commemoration for the 60th anniversary of her birth in 1848. 3. Architectural features of the NakSunJae area at the time of construction First, as the NakSunJae area is surrounded by quite high hills in northeast and by low hills in southwest, it is naturally distinguished from its surroundings and gives a cosy feeling. Second, the NakSunJae area shows very untrammeled style of arrangement in general, because it was constructed as a private occupancy for the king. other buildings are arranged in the latticing orderly, nonetheless, they are free by themselves from a strict form and make themselves an area full of variety. Third, the buildings in the NakSunJae area have a characteristic of naivety of common people's houses, because they were not an official but private place for the king, his concubine and his grandmother. Forth, the ornaments in the NakSunJae area are variously, elegantly and harmoniously patterned with the meaning of good luck. Fifth, back garden in the NakSunJae area was made by taking advantage of sloping hills. The back garden is beautifully configurated by arranging various constituent elements of pavilions, tiered flower beds, chimneys, decorated walls, small doors between walls, stoneworks and trees.

Verification of the Star Tracker Sun Exclusion Angle of GEO-KOMPSAT-2A Through In-Orbit Operation (천리안 2A호 별추적기 태양 차폐각 궤도상 운영 검증)

  • The star tracker detects microscopic star light in space and compares it with a stored list of stars to calculate the satellite's position in the inertial coordinate system. If other light, such as the sun or the earth, enters the optical head, the star cannot be recognized and the star tracker cannot be operated. In particular, strong light such as the sun affects not only operation but also the performance of the star tracker. The sun exclusion angle of the star tracker is one of the important factors determining the performance of the star tracker. This paper performs the verification of the star tracker's sun exclusion angle. In order to verify the sun exclusion angle, we predict the sun exclusion time of the star tracker and compare it to the actual sun exclusion time of the GEO-KOMPSAT-2A star tracker. In addition, the performance of the star tracker is analyzed for normal operations against the sun exclusion in the optical head. It shows that the actual sun exclusion is maintained under the range of 26 degrees, the performance requirement of the star tracker, and the star tracker operates normally in spite of the sun exclusion.

Giant Benign Schwannoma Involving Sacral Bone - A Case Report - (천골부에 발생한 거대 신경초종 - 증 례 보 고 -)

  • Sacral schwannoma is a rare lesion with a tendency to reach large proportions. The benign schwannoma rarely involves the vertebral bodies extensively. The authors report a case of giant intrasacral schwannoma in 30-year-old woman who had intermittent lower back pain during 3 years period. CT and MRI showed a destructive mass lesion within the upper part of sacrum with a large mass extending into the presacral space. The patient underwent combined surgery consisted of anterior transabdominal approach and posterior sacral laminectomy and total removal of tumor. The characteristics of the lesion were discussed with a review of literatures.

  • Schwannomas are uncommon neoplasms arising from Schwann cells of the neural sheath. Gastrointestinal schwannomas are rare, accounting for 1% of all malignant gastrointestinal tumors. Colonoscopic biopsy with immunohistochemical (IHC) staining is useful for confirming this tumor. We report on a patient with schwannoma arising from the ascending colon, which was detected by colonoscopy and endoscopic submucosal dissection was attempted. A 41-year-old man presented with abdominal discomfort. The patient was diagnosed with a subepithelial tumor on colonoscopy. He underwent endoscopic submucosal dissection. Histopathology and IHC staining confirmed that the colonic lesion was a benign schwannoma. However, the resection margin was positive. Therefore, laparoscopic ileocolectomy was performed.

Brain abscess due to odontogenic infection: a case report

  • In this report, we describe a case of brain abscess due to odontogenic infection. A 53-year-old female who had been suffering from headache and trismus for two weeks visited the Department of Oral and Maxillofacial Surgery at the Sun Dental Hospital (Daejeon, Korea). Even after several routine tests, we still could not make a diagnosis. However, after the combined multidisciplinary efforts of oral surgeons and neurosurgeons, the patient was treated for odontogenic infection and made an uneventful recovery. Therefore, patients with infections in the head and neck region showing symptoms such as headache, changes in mental state, nausea, vomiting, seizures, hemiplegia, speech disturbance, and visual disturbance, a brain abscess should be included in the list of differential diagnoses.

  • In this paper, a prototype of implementable Sun tracking algorithm for mobile systems powered by alternative energy is proposed. The proposed system uses 2-axis tilt sensor and 3-axis magnetic sensor to measure orientation and posture of the system according to the horizon coordinates system, which are used to compensate tilt effects. Then through astronomical calculation using the present time and position informations obtained from GPS sensors, the calculated azimuth and altitude of the Sun in that location. The position of the Sun is converted to that of the mobile Sun tracking system coordinates and used to control A-axis and C-axis of the system.

  • Finding sufficient supplies of clean energy for future is one of the society's most important challenges according to technologies. Alternative renewable energy source such as solar energy can be substituted for exceeding human energy need. The main factor affect to solar performance is a collective intensity. To enhance intensity, suitable equipment is a solar tracker. The solar tracker consists of sun point detector module, mechanical mechanism module with actuator and control system module. This paper presents sun point detector for solar tracker of daylight system. To evaluate the detecting accuracy, an experimental device is implemented. In experimental results, the accuracy of development system has under 0.11%/0.5deg.

  • A Sun-Position Sensor using four phototransistors and shadow band device has been designed, and a Sun-Tracking System which tracks varying positions of the sun in elevation and azimuth axes has been built and its performance has been analyzed on the basis of indoor experiments and computer simulations. Two permanent-magnetic Step Motors (1.8.deg./step) for the main actuators and a CRC-800A kit with the Z-80CPU for the main controller have been selected to construct the Sun-Tracking System. It has been shown that the Sun-Position Sensor has about 0.5.deg. resolution and 25msec is required for the response of a single step input to reach its steady state.
